특발성 양측성 안면신경 마비 환자에 대한 한의치료 증례보고 1례

Objectives: This case report presents the effects of traditional Korean medicine on a patient with idiopathic bilateral facial nerve palsy. Methods: Two evaluation scales, the House-Brackmann (H-B) scale and the Yanagihara scale, were used to evaluate the symptoms of bilateral facial palsy. Herbal medicine and acupuncture treatment were administered to improve the patient’s symptoms in parallel with Western oral medication (Valvirus Tab 500 mg, Solondo 5 mg, Lipitor 20 mg, Almagel 15 ml, Lanston 15 mg). Results: Before treatment, the patient had severe facial paralysis with an H-B scale of 5/5 and a Yanagihara scale of 3/2 on both sides. However, after treatment, the symptoms improved significantly with an H-B scale of 2/3 and a Yanagihara scale of 31/26. Conclusions: This case report shows that traditional Korean medicine in parallel with Western oral medication can be effective in the treatment of idiopathic bilateral facial nerve palsy.
중대뇌동맥 뇌경색 이후 발생된 뇌출혈에 동반된 브로카실어증(Broca aphasia) 환자에 대한 한의치료 증례보고 1례

Objective: The aim of this case report was to present the effects of traditional Korean medicine on a patient with Broca's aphasia associated with cerebral hemorrhage after middle cerebral artery infarction. Methods: The Korean version of the Western Aphasia Battery (K-WAB) and the Evaluation of Articulator Performance were used to evaluate the language functions of the patient. Herbal medication and acupuncture were administered to improve the patient's symptoms. Results: The inpatient and outpatient treatments improved the patient’s symptoms. The aphasia quotient (AQ) score of the K-WAB test before treatment was 6.4, but it increased to 21.4 after treatment. The Evaluation of Articulator Performance score improved by 0.0 points before treatment, by 1.0 points after 9 days of treatment, and by 1.5 points after 42 days of treatment. Conclusions: This case report suggest that Korean medical therapy can be effective in improving the language functions of patients with Broca's aphasia.
만성 경막하 혈종(Chronic Subdural Hematoma)의 한의 치료에 대한 최신 임상연구 동향 - CNKI 검색을 중심으로 -

Objectives: This study investigates the latest clinical studies on Korean medicine treatment of chronic subdural hematoma (CSDH) in China. Methods: A CNKI search investigated the last 10 years of clinical studies of Korean medicine treatments in Chinese medical journals for CSDH. The search focused on authors, publication years, types of studies, purposes of studies, methods and duration of treatment, evaluation criteria, and results of treatment. Results: In 19 studies, there were 14 randomized controlled trials (RCTs), 4 case series, and 1 case-control study. In all 19 studies, Korean medical treatment and Western treatment were combined. Herbal medicine was used in 19 studies and acupuncture treatment in 2 studies. The most frequently used prescription was Hyeolbuchukeo-tang-gagambang, and the most frequently used acupoint was PC6. In all 19 studies, the effectiveness of Korean medicine treatment performed in parallel with Western treatment was confirmed. Conclusions: In this study, Korean medicine combined with Western treatment for CSDH was effective. More clinical studies and case reports are required to develop a guided treatment for CSDH.

The phosphorylated 43-kDa transactive response DNA-binding protein (TDP-43) was identified as a major disease protein in sporadic amyotrophic lateral sclerosis (ALS) and frontotemporal lobar degeneration. We present a case with progressive muscle weakness who was diagnosed with sporadic ALS. On postmortem examination, TDP-43 immunoreactive neuronal cytoplasmic inclusions were noted in motor cortex, hippocampus and anterior horns of spinal cord, which was compatible with ALS-TDP, stage 4. This is the first documented autopsy-confirmed ALS case with ALS-TDP pathology in Korea.

Elsberg syndrome is a rare disease of lumbosacral myeloradiculitis, mostly caused by herpes simplex virus type 2 (HSV-2). It frequently has concomitant myelitis, and immunocompromised patients can be fatal with ascending myelitis. Since anti-viral agents might lead to good recovery, clinicians need to be suspicious for conditions of cauda equina syndrome. Detection of viral DNA from cerebrospinal fluid ensures the diagnosis. We report a case of Elsberg syndrome caused by HSV-2 and are to delineate the clinical and radiologic spectrum.

Background: Enlarged perivascular space (ePVS) is recently reported to be associated with cerebral small vessel disease (SVD) and Alzheimer’s disease (AD). The topographical location of ePVS may relate to the underlying pathology; basal ganglia (BG)-ePVS has been associated with cerebral vascular diseases and centrum semi-ovale (CSO)-ePVS associated with cerebral amyloid angiopathy (CAA). However, the effects of ePVS on various neurological conditions remain still controversial. To investigate the clinical relevance of ePVS in neurodegenerative diseases, we tested relationships between ePVS and cognition, markers of SVD, vascular risk factors, or amyloid pathology. Methods: We retrospectively reviewed 292 patients (133 AD dementia, 106 mild cognitive impairment, 39 other neurodegenerative diseases, 14 subjective cognitive decline) who underwent both amyloid positron emission tomography and brain magnetic resonance imaging. Vascular risk factors and cognitive tests results were collected. The ePVS in the BG and CSO, SVD markers and the volume of white matter hyperintensities were measured. Results: There were no significant differences in the severity and distribution of ePVS among clinical syndromes. Both BG- and CSO-ePVS were not related to cognitive function. Patients with lacunes were more likely to have high-degree BG-ePVS. High degree CSO-ePVS had an odds ratio (OR) for amyloid positive of 2.351, while BG-ePVS was a negative predictor for amyloid pathology (OR, 0.336). Conclusions: Our findings support that ePVS has different underlying pathologies according to the cerebral topography. BG-ePVS would be attributed to hypertensive angiopathy considering the relation with SVD markers, whereas and CSO-ePVS would be attributed to CAA considering the association with amyloid pathology.
기능성 소화불량에 대한 사군자탕의 치료효과 - 체계적 문헌고찰과 메타분석

Objectives: This study examines the effect of Sagunja-tang on functional dyspepsia (FD) through a systematic review and meta-analysis of a randomized controlled trial (RCT). Methods: A search for RCTs that tested the effect of Sagunja-tang on functional dyspepsia was conducted in Medline, Embase, PubMed, CENTRAL, CiNii, CNKI, NDSL, RISS, OASIS, and KISS databases on November 8, 2020, with no limit on the year of publication. A meta-analysis was performed by synthesizing the findings, including total efficiency, clinical symptom score, myosin light-chain kinase (MLCK) level (pg/mL, and gastric half-emptying time (min). RevMan 5.4.1 software was used for data analysis. The quality of the literature was evaluated using Cochrane's risk of bias (RoB) tool. Results: A total of 14 RCTs met the selection criteria. As a result of the meta-analysis, the treatment group had higher total efficacy and MLCK levels (gastric antrum, jejunum) than the control group, and the clinical symptom score and gastric half-emptying time were lower. However, due to the low quality of the included RCT and the small sample size, the results may be slightly biased.
