cDNA 마이크로어레이와 면역조직화학염색을 이용한 결장직장암에서의 IGFBP3의 발현 연구

Background : Insulin-like growth factor binding protein 3 (IGFBP3), a member of six proteins with a high affinity for insulin-like growth factors (IGFs), seems to modulate the effects of IGFs on cells and to regulate cell proliferation through the IGF-independent pathway. We assessed the role of IGFBP3 in the colorectal carcinoma detected by cDNA microarray. Methods : To identify molecular alterations in the colorectal carcinoma, we analyzed gene expression profiles of the colorectal adenocarcinoma by means of a cDNA microarray representing 7,500 genes. Of the differentially expressed genes, the author assessed the insulinlike growth factor binding protein 3 (IGFBP3) gene at the protein level using immunohistochemistry. Results : The expressions of 21 and 16 genes were noted to have more than fivefold increases or decreases in the colonic adenocarcinoma tissue compared with the noncancerous colonic mucosal tissue. The differentially expressed genes include those associated with cell proliferation/apoptosis, signal transduction/transcription, metabolizing enzymes, cytoskeleton, angiogenesis, ion channel, extracellular matrix and others. Of the total 68 cases of colorectal adenocarcinomas observed, 34 cases (50%) showed positive immunohistochemical stainings for IGFBP3. Conclusion : In this study, it is suggested that IGFBP3 plays a role in colorectal carcinogenesis. And combining an immunohistochemistry with a cDNA microarray can facillitate the rapid characterization of a candidate novel molecular target.

Hyalinizing trabecular adenoma of the thyroid gland is a rare benign neoplasm predominantly diagnosed in middle-aged women. Carney et al. first described this entity that may mimic paraganglioma, medullary carcinoma and papillary carcinoma in 1987. We describe cytologic and histopathologic features of a case of hyalinizing trabecular adenoma combined with occult papillary carcinoma in the opposite lobe. A 55-year-old woman presented with nontender palpable mass of the right neck for 6 months. The aspirate was cellular and contained small clusters and sheets of epithelial cells with abundant filamentous, vacuolated, and ill-defined cytoplasm. The nuclei were slightly pleomorphic and showed nuclear overlapping, nuclear grooves, and intranuclear cytoplasmic inclusions. Histologic examination showed hyalinizing trabecular adenoma in the right lobe and occult papillary carcinoma in the left lobe.
자궁 경부 인유두종 바이러스 감염에 대한 세포검사와 DNA 검사의 비교 연구

Purpose: A newly introduced HPV detection technique in cervical intraepithelial lesion, the HPV DNA Chip test, contains 24 HPV probes and has the advantage of being able to detect 24 HPV types simultaneously. The authors performed HPV genotyping test in cervical specimens from September 2005 to March 2006, and compared their cytologic and histologic diagnoses. Subject and Methods: The HPV DNA Chip test was performed in samples of 263 patients. Among these, 49 patients underwent cervical smear cytology and biopsy. Results: In 160 cases (60.8%) of 263 cases, the HPV DNA was detected. HPV-16 (37.5%), HPV-58 (16.3%), HPV-33 (12.5%), and HPV-18 (10.0%) were frequently detected, including single and multiple infections. The multiple HPV infection was found in 31 cases (11.8%). The sensitivity (91.7%) of the HPV genotyping test using the DNA chip method for detecting HSIL or worse was superior compared with the sensitivity (79.2%) of cytologic diagnosis. Conclusions: Our results indicate that HPV DNA chip test can overcome the low sensitivity of cytologic test and improve the sensitivity of the screening test for cervical intraepithelial lesion.
Carcinosarcoma of the Stomach: A Case Report

Carcinosarcoma is a rare malignant, biphasic tumor comprised of carcinoma and sarcoma components. In the gastrointestinal tract, carcinosarcoma is most frequently seen in the esophagus and rarely in the stomach. We report a 51-year-old female patient with 2-monthhistory of epigastric pain and dyspepsia. Endoscopic finding revealed a huge ulcerative lesion that infiltrated from the antrum to the mid-body. An endoscopically taken biopsy revealed poorly differentiated malignant round cell neoplasm. After the palliative subtotal gastrectomy, immunohistochemical studies showed two positive reactions for the epithelial marker and mesenchymal marker. Based on the above findings, the patient was diagnosed with gastric carcinosarcoma. The immunohistochemical analysis is a critical method in making an accurate diagnosis of carcinosarcoma.

목적: 세포주기의 조절곤란은 종양의 발생과 진행에 영향을 미친다. 몇가지 알려진 연부조직 육종에 대한 면역 조직화학적 예후인자 들이 있지만 상반된 연구도 있고 G1/S phase관계되는 인자에 대한 연구는 거의 없는 상태이다. 따라서 저자는 연부조직 육종의 재발 및 전이와 관계된 G1/S phase 세포조절주기 단백의 면역화학적 예후인자를 알아보고자 하였다. 대상 및 방법: 1998년 1월부터 2005년 12월까지 연부조직 육종으로 진단된 환자 중 최소한 1년이상 추시관찰이 가능하고 파라핀 블록의 보존상태가 비교적 양호한 43예의 환자를 대상으로 연구하였다. 지방육종 15예, 악성 섬유성조직구증 13예, 횡문근육종 5예, 활막육종 5예, 평활근육종 3예, 섬유육종이 2예였다. 모든 환자의 조직은 전 절제술로 제거된 조직을 대상으로 Cyclin D1, Cyclin E, CDK4, CDK, p16, p27, Rb, E2F-1, p53, Ki-67 등의 면역 조직화학적 발현을 조직 microarray 방법을 사용하여 측정하고 환자의 국소 재발 및 전이에 따른 예후를 비교 분석하였다. 결과: 국소 재발은 8예(19%)에서 일어났으며 이것은 Cyclin E(p=0.024), E2F-1(p=0.046)의 발현과 연관이 있었다. 전이는 16예(37%)에서 일어났으며 CDK4의 증가와 연관이 있었다(p=0.031). 결론: Cyclin E와 E2F-1이 연부조직 육종의 국소 재발과 관련있는 예후를 제공해주었고, CDK4가 전이를 예측할 수 있는 독립적인 예후를 제공해주었다. 따라서 조직 검사시 이들 표식자들의 적절한 사용이 환자의 예후를 예측하고 치료의 범위를 결정 짖는데 중요한 도움을 줄 수 있을 것이다.
