        Poorer dynamic postural stability in patients with anterior cruciate ligament rupture combined with lateral meniscus tear than in those with medial meniscus tear

        Background: Only limited data are available regarding postural stability between anterior cruciate ligament (ACL)-injured patients with medial meniscus (MM) tear and those with lateral meniscus (LM) tear. The purpose of this study was to compare preoperative postural stability for both involved and uninvolved knees in ACL rupture combined with MM and LM tears. It was hypothesized that there would be a significant difference in postural stability between these two groups. Methods: Ninety-three ACL-injured patients (53 combined with MM tears vs. 40 combined with LM tears) were included. Static and dynamic postural stability were evaluated with the overall stability index (OSI), anterior–posterior stability index (APSI), and medial–lateral stability index (MLSI) using stabilometry. Knee muscle strength was evaluated using an isokinetic testing device. Results: In the static postural stability test, none of the stability indices showed significant differences between the two groups for both knees (p > 0.05). In the dynamic postural stability test for involved side knees, the OSI and APSI were significantly higher in the LM tear group compared to the MM tear group (OSI: 2.0 ± 0.8 vs. 1.6 ± 0.5, p = 0.001; APSI: 1.5 ± 0.6 vs. 1.3 ± 0.5, p = 0.023), but not the MLSI (p > 0.05). In the static and dynamic postural stability tests in each group, there were no significant differences between the involved and uninvolved side knees (p > 0.05). There was no significant difference in the knee muscle strength between the two groups (p > 0.05). All postural stability showed no significant correlation with knee muscle strength (p > 0.05). Conclusion: Dynamic postural stability was poorer in patients with ACL rupture combined with LM tear than in those with MM tear. Therefore, close monitoring for postural stability would be necessary during preoperative and postoperative rehabilitation, especially for patients with ACL rupture combined with LM tear.

        장애인 근로자 고용의 질에 관한 종단연구 : 경증과 중증장애의 차이를 중심으로

        본 연구는 경증 및 중증장애인 고용의 질에 종단적으로 영향을 미치는 요인을 살펴보고 비교하는 것을 목적으로 한다. 이를 위해 한국장애인고용패널조사 2차(2017년) ~ 5차(2020년) 자료를 활용하여 경증장애인 근로자 897명과 중증장애인 근로자 213명의 응답 자료를 패널 확률효과 모형으로 분석하였다. 연구결과, 첫째, 중증장애인은 경증장애인보다 고용의 질이 낮은 것으로 나타났다. 둘째, 경증 및 중증장애인 고용의 질에 주관적 사회경제적 지위, 학력, 컴퓨터 활용능력, 장애인 근로자 수, 직장 유형이 공통적으로 종단적 영향을 미치는 것으로 나타났다. 셋째, 중증과 다르게 경증장애인 고용의 질에만 성별, 배우자 유무, 영어 활용능력, 만성질환 보유 여부 변수가 종단적 영향을 미치는 것으로 나타났다. 그리고, 경증과 다르게 중증장애인 고용의 질에 대한 상대적 영향력 크기는 직장 유형, 컴퓨터 활용능력, 주관적 사회경제적 지위 등 순이었다. 본 연구결과를 토대로 장애인 고용의 질을 향상하기 위해 경증과 중증장애인 맞춤형 실천적⋅정책적 방안이 제공되어야 함을 제언하였다. This study aims to examine and compare the factors that longitudinally affect the quality of employment for people with mild and severe disabilities. For this purpose, we used data from the 2nd (2017) to 5th (2020) waves of the Korea Panel Survey of Employment for the Disabled(PSED) to analyze the responses of 897 workers with mild disabilities and 213 workers with severe disabilities, using a panel random effects model. The results found that, first, people with severe disabilities have lower quality of employment than people with mild disabilities. Second, subjective socioeconomic status, education, computer literacy, number of workers with disabilities, and type of workplace have common longitudinal effects on quality of employment for people with mild and severe disabilities. Third, the longitudinal effects of gender, spouse, English language proficiency, and chronic disease status on quality of employment for people with mild disabilities were different from people with mild disabilities. In addition, the relative magnitudes of the effects on employment quality for people with severe disabilities were different from those for people with mild disabilities, in the order of workplace type, computer literacy, and subjective socioeconomic status. Based on the results of this study, it is recommended that practical and policy measures customized for mild and severe disabilities should be provided to improve the quality of employment of people with disabilities.

        Helicobacter pylori의 CagA 단백질에서 SHP-2 (Scr homology 2 domain-containing tyrosine phosphatase)에 대한 결합 부위의 다양성과 위선 위축 및 위암과의 연관성

        이 연구는 Helicobacter pylori CagA (cytotoxinassociatedgene A)의 다양성과 임상 양상 사이의 관계를 보기 위해 시행되었다. 총 115개의 Helicobacter pylori (H.pylori) 균주에서 cagA 유전자의 염기서열을 분석하였으며 SHP-2에 대한 CagA의 결합력을 in vitro로 측정하였다.CagA의 아형은 염기서열의 분석을 통해 동아시아형과 서구형으로 분류하였다. 동아시아형 CagA 양성 H. pylori에 감염된 위염 환자의 경우, CagA가 음성이거나 서구형 CagA 양성인 경우에 비해 환자의 위에서 관찰된 만성 염증의 등급, 위염의 활성도, 그리고 위축의 등급이 더 높았다. 위암 환자에서 분리된 H. pylori는 모두 동아시아형 CagA양성이었다. SHP-2에 대한 결합력은 서구형 CagA에 비해 동아시아형 CagA가 더 강하였다. 이 소견들은 동아시아형 CagA 양성 H. pylori의 감염이 위축성 위염 및 위암과 연관되어 있고, 동아시아형 CagA 양성 균주에 의해 초래된 지속적인 활동성 염증이 주된 발병 기전일 수 있음을 시사한다.

        장애 유형별 가구 내 자살생각 군집화 영향요인 분석

        The occurrence of disability makes people with disabilities more vulnerable to suicide clustering, in which suicidal ideation among household members is influenced by each other. This study analyzed the clustering of suicidal thoughts in households by disability type and its influencing factors by applying the SUR bivariate probit model using pairwise data from the '2018 Disability Life Panel Survey' of people with disabilities and their family members. In the final analysis, 1,798 pairs of externally disabled households, 826 pairs of internally disabled households, and 215 pairs of mentally disabled households were selected. The results showed that the risk of suicidal ideation clustering was greater in the order of mentally disabled households, internally disabled households, and externally disabled households, and the variables differed by disability type: in externally disabled households, the risk of suicidal ideation clustering increased with lower disability acceptance of the person with a disability, more spouses in the family compared to children or other family members, and less frequent professional contact for family members. Among households with an internal disability, the risk of clustering suicidal ideation in the household increased with distance to health care facilities. In households with mental disabilities, the risk of clustering suicidal ideation in the household with a person with a disability increased as the family health of family members decreased. Based on the findings, we recommend individualized practical and policy measures that take into account the type of disability to prevent suicide clustering in households with disabilities.

        폴리머 굽힘센서를 이용한 손의 형상 추정과 로봇 팔 제어 연구

        초음파 로봇 검사시스템은 발전설비 등 상시감시가 요구되는 구조물의 감시를 위해 연구와 개발이 활발히 진행되고 있다. 그러나 이러한 설비의 구조물은 형상과 구조가 매우 다양하고 복잡하여 단순한 패턴으로 이동과 검사가 가능한 로봇으로는 상시감시에 한계를 가지게 된다. 복잡한 구조물의 정밀검사는 정교한 행동이 가능한 로봇 시스템이 필요하다. 로봇의 제어를 초음파검사 작업자의 손의 형상에 따라 직관적으로 움직이도록 한다면 보다 정밀한 검사 로봇의 개발이 가능할 것으로 판단된다. 폴리머 굽힘센서는 대변형의 측정이 가능하고 곡면에 적용성이 높아 최근 인체 형상 측정 및 재활보조 등 다양한 활용을 위한 연구가 활발히 진행되고 있다. 본 연구에서는 구조물의 상시감시를 위한 초음파검사 로봇이 작업자의 손동작에 따라 동작이 가능한 직관적 제어를 위해 폴리머 굽힘센서를 이용하여 사람 손의 형상 감시 연구를 수행하였다. 다수의 센서를 적용하여 손의 다양한 움직임을 추정하고 실험적 검증을 통해 사람 손의 형상에 따른 직관적 로봇의 제어가 가능함을 확인하였다. Ultrasonic inspection robot systems have been widely researched and developed for the real-time monitoring of structures such as power plants. However, an inspection robot that is operated in a simple pattern has limitations in its application to various structures in a plant facility because of the diverse and complicated shapes of the inspection objects. Therefore, accurate control of the robot is required to inspect complicated objects with high-precision results. This paper presents the idea that the shape and movement information of an ultrasonic inspector's hand could be profitably utilized for the accurate control of robot. In this study, a polymer flex sensor was applied to monitor the shape of a human hand. This application was designed to intuitively control an ultrasonic inspection robot. The movement and shape of the hand were estimated by applying multiple sensors. Moreover, it was successfully shown that a test robot could be intuitively controlled based on the shape of a human hand estimated using polymer flex sensors.

        일본 도시형 유료노인홈의 건축계획특성에 관한 연구

        This study aims to analyse the planning characteristic of fee charging residential facility for the elderly focused on the actual state in Tokyo metropolitan area. For doing this, 18 facilities have been chosen and the analyse is divided into 4 main categories: general factor, mass of facility, actual usage, and planning characteristic. Results are as fallow:Firstly, the facility has been mostly built up in residential area and running a business by jointed-stock company. Secondly, the average plottage is 500~600(pyung) with single building which mainly have 2~5 stories. The number of rooms are mainly less than 50 or so which means most business are operated on a small scale. Thirdly, most popular size of room is less than 20㎡ and then 20~30㎡ which are comparatively small and of practical use. For using facility, user's need to pay in advance average 1,000万円~1,700万円/㎡ for rent, 16万円~22万円/month for management and food expenses and assistance expenses is extra outlay which are mostly covered by user's pension and insurance. Lastly, most facilities' function link systems are vertical in single building and more than half of facilities have pavilion shape and middle-corridor that seem to be caused for efficiency usage of management and land.

        Two-module robotic pipe inspection system with EMATs

        This work introduces a two-module robotic pipe inspection system with ultrasonic NDE device to evaluate the integrity of pipe structures. The proposed robotic platform has high mobility. The two module mobile robot platform overcomes pipe obstacle structures such as elbow, or T-branch joints by cooperative maneuvers. Also, it can climb up the straight pipeline at a fast speed due to the wheel driven mechanism. For inspection of pipe structure, SH-waves generated by EMAT are applied with additional signal processing methods. A wavelet transform is implemented to extract a meaningful and specific signal from the superposed SH-wave signals. Intensity ratio which is normalized the defect signals intensity by the maximum intensity of directly transmitted signals in the wavelet transforms spectrum is applied to evaluate defects quantitatively. It is experimentally verified that the robotic ultrasonic inspection system with EMAT is capable of non-destructive inspection and evaluation of defects in pipe structure successfully by applying signal processing method based on wavelet transform.

        Integrity Evaluation of Pipe Welding Zones using Wavelet Transforms, and Specific Sensitivities based on SH-EMAT Pulse-Echo Method

        Pipes in facilities contain lots of welding zones and requires automated ultrasonic inspection. An electromagnetic acoustic transducer(EMAT) can be usefully applied in an automated ultrasonic inspection system. In this study, shear horizontal waves (SH-waves)generated by EMAT were implemented for the inspection of pipe welding zones. In analyzing of SH-EMAT signals, wavelet transformswere useful to extract key information. The intensity of the directly transmitted signal was used as a reference signal. A ratio of thereflected signal to the reference was proposed as the specific sensitivity. The sizes of defects were evaluated quantitatively using thespecific sensitivity.
