        망막정맥폐쇄 환자에서 유리체내 베바시주맙 주입술의 2년 임상 결과

        Purpose: The authors evaluated the 2-year clinical results of intravitreal bevacizumab injection in retinal vein occlusion (RVO). Methods: Thirty-two eyes of 32 patients treated with an intravitreal bevacizumab injection of 1.25 mg (0.05 ml) for RVO (branch RVO: 22 eyes, central RVO: 10 eyes), repeated 3 times at a 6-week interval and were available for a follow-up period of at least 2 years were retrospectively reviewed. Best-corrected visual acuity (BCVA) before treatment and 6, 12, and 24 months after 3 serial injections, was recorded. The optical coherence tomography (OCT) results were analyzed to identify prognostic factors for recurrent macular edema that needed reinjection. Results: Two years after the treatment, mean BCVA was significantly improved (<em>p</em> = 0.000). Out of 32 eyes, 16 (branch RVO: 8 eyes; central RVO: 8 eyes) needed reinjection because of recurrent macular edema. In central RVO, a significantly high reinjection rate was shown in serous retinal detachment (SRD) compared with cystoid macular edema (CME) as identified in OCT findings (<em>p</em> = 0.049). Additionally, in branch RVO, a high reinjection rate was shown in SRD, although statistically not significant (<em>p</em> = 0.375). Conclusions: In patients with RVO, a significant visual improvement was maintained for at least 2 years after intravitreal bevacizumab injection.         군산풍력발전단지의 풍력발전량 단기예측모형 비교에 관한 연구

        최근 신재생에너지와 대체에너지의 필요성이 증가함에 따라 환경오염과 온실효과를 초래하지 않는 풍력에너지 개발에 많은 연구와 투자가 이루어지고 있다. 풍력에너지는 무공해 에너지이며 자원양이 무한대이고 바람이 부는 곳이라면 어디에서든지 전력생산이 가능하다. 그러나 풍력에너지는 바람에 크게 의존하며 불규칙적인 특성이 있어 효율적인 풍력발전이 어렵다는 단점이 있다. 이러한 이유로 풍력발전에 있어서 정확한 풍력발전량 예측은 매우 중요한 요소이다. 본 연구에서는 이러한 풍력발전량의 효율적인 예측을 위해 군산 풍력단지의 자료를 이용해 시계열모형인 ARMA모형과 데이터 마이닝 기법 중 신경망모형을 사용하여 풍력발전량을 예측하고 비교분석 하였다. 그 결과 신경망모형 적합결과가 ARMA모형 적합결과 보다 더 좋은 예측력을 나타내었다. As the needs for alternative energy and renewable energy increase, there has been a lot of investment in developing wind energy, which does not cause air pollution nor the greenhouse gas effect. Wind energy is an environment friendly energy that is unlimited in its resources and is possible to be produced wherever the wind blows. However, since wind energy heavily relies on wind that has unreliable characteristics, it may be difficult to have efficient energy transmissions. For this reason, an important factor in wind energy forecasting is the estimation of available wind power. In this study, Gunsan wind farm data was used to compare ARMA model to neural network model to analyze for more accurate prediction of wind power generation. As a result, the neural network model was better than the ARMA model in the accuracy of the wind power predictions.

        상수도 배관에서 두 지점의 동시 누수에 따른 신호특징 분석

        스마트 워터 그리드와 같은 지하매설 상수도 배관망을 구성할 때 누수 지점에 대한 지능적 탐지는 중요한 요소이다. 이런 지능형 배관망의 구성에는 많은 수의 누수탐지센서를 일정한 간격으로 설치하는 것이 필요하며 이들 센서에서 계측되는 신호를 분석해서 누수 유무 및 누수발생시 그 지점의 추정을 신속히 할 수 있어야 한다. 그래서 본 연구에서는 특히 두 지점에서 동시에 누수가 발생하는 경우에 대해 그 센서가 측정한 신호가 가지는 특성 및 두 누수지점 동시 추정 가능성을 심도있게 분석하였다. 즉, 센서간 거리가 315.6 m 인 100A 사이즈의 주철관 상수도 배관에 대해 두 지점 동시 누수시 각각의 개별 센서신호를 계측하였으며, 그 신호를 기반으로 주파수 특성 및 상호상관함수 등의 분석을 통해 두 지점의 누수탐지에 대해 기술하였다. Intelligent leak detection is an essential component of a underground water supply pipeline network such as a smart water grid system. In this network, numerous leak detection sensors are needed to cover all of the pipelines in a specific area installed at specific regular distances. It is also necessary to determine the existence of any leaks and estimate its location within a short time after it occurs. In this study, the leak signal properties and feasibility of leak location detection were investigated when concurrent leaks occurred at two points in a pipeline. The straight distance between the two leak sensors in the 100A sized cast-iron pipeline was 315.6 m, and their signals were measured with one leak and two concurrent leaks. Each leak location was described after analyzing the frequency properties and cross-correlation of the measured signals.

        TRIP 형 복합조직강판의 기계적 성질에 미치는 2 단 열처리 영향

        N/A Heat treatment conditions and the formation of microstructures were studied for improving the transformation-induced plasticity(TRIP) effect of retained austenite and mechanical properties of Fe-0.2%C-1.5%Si-1.5%Mn sheet steel. An excellent combination of elongation about 30% and high strength over 760 ㎫ was achieved by processing of intercritical annealing and isothermal holding. Intercritical annealing the sheet steel produced fine particles(1∼2㎛) of retained austenite which were stabilized due to C enrichment by subsequent holding in bainite transformation range. Heat treatment conditions were depended on the shape and distribution of second phases as well as the volume fraction and stability of retained austenrte. In this work, the heat treatment condition of optimal strength-elongation balance was obtained by holding the steel at 400℃ for 200sec. after intercritical annealing at 790℃ for 300sec.

        천사의 나팔의 접촉에 의한 편측 동공 산대 1예

        Purpose: To report a case of unilateral mydriasis after accidental exposure to an Angel’s trumpet leaf. Case summary: A 53-year-old woman visited the Eye Department complaining of blurred vision and difficulty in light adaptation in the left eye of 1-week duration. The best corrected visual acuity was 20/20 in both eyes and other ophthalmological findings were considered normal except for dilation of the left pupil and a decrease in light reflex. The patient was initially unaware of any cause of her eye problems, but when asked specifically, the patient remembered an Angel’s trumpet leaf brushing against her left eye when she carried the plants. There were no other previous medical or drug histories, thus the Angel’s trumpet was considered as the cause for the unilateral mydriasis. The patient’s progress was followed for a week, her symptoms improved and the pupil size and reflex returned to normal. Conclusions: The cultivation of Angel’s trumpet has become increasingly popular in Korea in recent years. The present case emphasizes the importance of an accurate and detailed history regarding specific contact history with plants like Angel’s trumpet in otherwise healthy patients affected by unilateral mydriasis.         이상증상 기반 미병 분류도구의 신뢰도 및 타당도 연구

        Objectives : The number of people in Mibyeong state that complain of physical and mental discomfort but without a clear medical diagnosis has rapidly increased, but the conventional medical system is insufficient to care for these people. By establishing an evaluation instrument for Mibyeong state, it will be possible to provide a research base for Mibyeong management system and expand the clinical area of integrative medicine. Methods : Mibyeong Index was designed to measure inconvenience and resilience of subject's complains including four physical symptoms (fatigue, pain, low sleep quality, indigestion) and mental distress include anxiety, anger, depression. A 21-item quality of healthy measure was developed and tested. We used results of a nation-wide stratified sampled adult data in Korea. Results and Conclusions : Mibyeong index had adequate internal consistency, Cronbach's alpha was 0.88 in general population(N=1,110). The correlation between establised quality of life questionnaires (including SF-12 and EQ-D5 VAS) and the Mibyeong index were from 0.468 to 0.493. The national promotion of advanced health for an aging society and original Mibyeong care technology based on traditional Korean medicine can be developed by a self-care system that enhancing health before suffering illness. We expect that this instrument could be contribute to health management of people in Mibyeong state.

        소아 위장관 이물 -강원지역 소아 60례-

        목적: 소아의 위장관 이물은 일상 생활에서 흔히 경험하는 질환으로 대개의 경우는 합병증 없이 자연 배출된다. 저자들은 내시경과 Foley 도관을 이용한 위장관 이물 적출술에 관한 최근 경향을 알아보고자 한다. 방법: 1996년 1월부터 1999년 12월까지 4년간 원주기독병원 소아과에 위장관 이물로 내원한 60례를 대상으로 하였으며 성별, 연령, 임상 증상, 이물의 종류와 위치, 이물의 치료 및 제거 방법, 합병증 등에 대하여 후향적으로 조사하였다. 결과: 위장관 이물 환자의 연령은 7개월에서 13세까지였으며, 5세 이하가 57례(95.0%)를 차지하였으며, 남녀비는 1.07:1이었다. 임상 증상은 무증상이 45례(75.0%)로 가장 많았고 연하곤란 8례(13.3%), 인후통 5례(8.3%), 구토, 복부 불쾌감이 각각 1례(1.7%)였다. 이물의 종류는 동전이 43례(71.7%)로 가장 많았고 구슬, 바둑알, 반지 등이 각각 3례(5.0%), 수은 건전지가 2례(3.3%)였으며 그 외 금속 clip, 나사못, 커튼핀, 머리핀, 오디오 열쇠, 스티커 등이 각각 1례(1.7%)씩 있었다. 이물의 위치는 식도 31례(51.6%), 위 25례(41.7%), 소장 3례(5.0%), 인후 1례(1.7%)였고, 이 중 식도 이물은 경부식도 24례(40.0%), 원위식도 5례(8.3%), 흉부식도 2례(3.3%)에서 관찰되었다. 유연성 내시경적 이물 제거를 시행한 경우가 22례(36.7%)로 가장 많았고, Foley 도관을 이용한 경우가 18례(30.0%), Forcep를 이용한 경우가 1례(1.7%)였다. 18례(30.0%)에서는 자연 배출되었고, 1례(1.7%)에서는 수술을 시행하였다. 결론: 소아에서 상부 위장관 이물은 조기에 제거하여 이환율과 합병증을 감소시켜야 하며, 식도이물의 제거시 X-선 투시하의 Foley 도관과 내시경은 소아에서 안전하고, 효과적인 이물적출방법이다. 얇고, 날카로운 이물은 주위 깊은 관찰을 요하며, 하부 위장관에 위치시 장천공에 대한 주위 깊은 관찰을 요한다. Purpose: Ingested foreign bodies present a common clinical problem. It is well known that most of them pass uninterrupted through the gastrointestinal tract. We evaluated the role of endoscopy and Foley catheter for removal of foreign bodies in the gastrointestinal tract. Methods: We investigated retrospectively 60 cases with foreign bodies in the gastrointestinal tract. They had been treated at Wonju Christian Hospital, Yonsei University of Korea, from January, 1996 through December, 1999. Results: The age of the patients ranged from 7 months to 13 years. Patients under 5 years were 57 cases (97%) and there was no significant difference in sex (M : F=1.07 : 1). 45 cases of the patients had no symptom. The most common foreign bodies were coins (43 cases). The most common location was esophagus (31 cases). The number of foreign body removal using flexible endoscopy and Foley catheter was 22 (36.7%) and 18 (30.0%) cases, respectively. In 18 cases (30.0%), foreign bodies passed spontaneously. Only 1 case (1.7%), curtain pin impaction at ileocecal region, required surgery. Conclusion: Early foreign body removal from esophagus and stomach is recommended to lessen the morbidity and complication. Fluoroscopic foley catheter technique and flexible endoscopy for removal of esophageal foreign bodies in children is safe and effective.
