Purpose: To report the results according to anterior elevation changes following corneal crosslinking (CXL) treatment for keratoconus. Methods: The present retrospective study included 14 patients (15 eyes) with progressive keratoconus who underwent CXL with a follow-up of 12 months. Patients were classified into 2 groups according to pre and postoperative anterior elevation difference maps. On the preoperative anterior elevation map, distances from maximum anterior elevation to pupil center were compared between the 2 groups. The outcome of best correct visual acuity (BCVA), maximum keratometry and parameters of corneal topography were compared between the 2 groups before CXL as well as 6 and 12 months after CXL. Results: The anterior elevation changes were classified as group 1 (-7.88 ± 10.53 μm) or group 2 (8.71 ± 5.99 μm) (p = 0.001). The preoperative corneal topography of eyes observed in group 1 (0.19 ± 0.13 mm) had shorter mean distances from maximum anterior elevation to pupil center than eyes in group 2 (0.47 ± 0.23 mm) (p = 0.018). BCVA (log MAR) improved from 0.68 ± 0.78 to 0.57 ± 0.81 (p = 0.115) 12 months after CXL in group 1 and decreased from 0.51 ± 0.34 to 0.56 ± 0.38 (p = 0.109) 12 months after CXL in group 2. The maximum keratometry decreased from 63.01 ± 19.07D to 58.95 ± 16.32D (p = 0.017) in group 1 and increased from 60.70 ± 9.46D to 61.29 ± 7.51D (p = 0.674) in group 2. Conclusions: Clinical and optical effects improved postoperatively in group 1, and were stabilized in group 2. The preoperative distance from maximum anterior elevation to pupil center and the anterior elevation changes after CXL were factors in predicting the CXL outcome.

We present two cases of deep cerebral venous thrombosis(DCVT) with the totally occluded straight sinus. A 42-year-old female received with altered mentality . She has taken antihistamine for six years to treat the paranasal sinusitis. Another 34-year-old female who used the oral contraceptive for 11 months presented with acute behavior change . Both of these patients were diagnosed by computed tomography(CT), magnetic resonance(MR) imaging, and cerebral angiography. They were fully recovered with systemic urokinase thrombolysis followed by heparin therapy. We report that the intravenous thrombolysis was potentially effective management strategy in our cases of DCVT with the totally occluded straight sinus.

Purpose: To evaluate the effects of macular ischemia and early treatment on the visual outcomes of patients with branch retinal vein occlusion (BRVO). Methods: This study retrospectively reviewed the records of 42 patients who were treated with an intravitreal bevacizumab injection for BRVO, repeated 3 times at 6-week intervals and were whose data available for a follow-up period of at least 4 years. Best-corrected visual acuity (BCVA) and optical coherence tomography (OCT) results before treatment and at 6, 12, 24, 36 and 48 months after the 3 serial injections, were measured. We assessed macular ischemia, time to the first treatment, and the relationship of these with BCVA. Results: Mean BCVA (log MAR) was significantly improved from 0.67 ± 0.43 at baseline to 0.30 ± 0.30 at 48 months (p < 0.001). Four years after treatment, mean BCVA in the macular non-ischemic group was better than in the ischemic group, but this relationship did not hold for mean change in BCVA. There was a statistically significant (p < 0.05) difference between the early (≤6 weeks) and late (>6 weeks) treatment groups in BCVA and mean change in BCVA after 48 months. There was statistically significant (p < 0.05) difference between the early (≤6 weeks) and late (>6 weeks) treatment groups with macular ischemia in BCVA after 48 months. Conclusions: In patients with BRVO, a significant visual improvement was maintained after intravitreal bevacizumab injections, despite the presence of macular ischemia. Early treatment (within 6 weeks) is more effective for maintaining and improving visual acuity. Similar results in the macular ischemia group confirmed the importance of early treatment. 공중 전화 통신망에서의 가입자 선로에 대한 고장진단을 전문적인 지식을 이용하여 수행하는 전문가 시스팀 (expert system)을 LAMBDA 2x2 /PLUS에서 ZetaLISP를 이용하여 개발하였다. 본고에서는 이를 위한 현장 지식의 습득과 모델화, 지식을 표현하기 위한 소프트웨어의 개발과 지식의 표현 방법, 그리고 시험 환경의 구성을 통한 시험에 대해 기술하였다. 본 시스팀에는 추론을 위해 룰과 프레임을 혼합하여 사용하였으며 사용이 용이한 사용자 인터페이스를 구성하였다.

We report a 38-year-old Korean female diagnosed with (tubulointerstitial nephritis and uveitis syndrome. The patient was hospitalized because of an elevated level of serum creatinine with persistant nausea. Blood tests revealed renal insufficiency and a renal biopsy revealed acute tubulointerstitial nephritis. Four days after admission, the patient developed a conjunctival injection in the left eye and slit-lamp examination revealed anterior uveitis. The patient was diagnosed with tubulointerstitial nephritis and uveitis syndrome and was treated with systemic and topical steroids.

<b>Purpose:</b> We compared the measurements of anterior chamber depth (ACD) and anterior chamber angle (ACA) using ultrasound biomicroscopy (UBM) in the sitting position compared with IOL Master<sup>Ⓡ, Pentacam<sup>Ⓡ, and Spectralis optical coherence tomography (OCT) to evaluate the clinical usefulness of UBM in the sitting position. <b>Methods</b>: We evaluated 92 eyes in 47 healthy adults. ACD was measured by IOL Master<sup>Ⓡ, Pentacam<sup>Ⓡ, and UBM. ACA was measured using Pentacam<sup>Ⓡ, UBM, and Spectralis OCT. UBM was performed in the sitting position using bag/balloon technology. Measured values were compared statistically. <b>Results</b>: ACD measured by IOL Master<sup>Ⓡ, Pentacam<sup>Ⓡ, and UBM was 3.57 ± 0.32 μm, 3.64 ± 0.33 μm and 3.51 ± 0.32 μm, respectively. UBM measurements of ACD were significantly shallower than with the other methods (<em>p</em> < 0.001). The results among the 3 methods were strongly correlated (<em>r</em> > 0.8, <em>p</em> < 0.05 in all groups). Mean ACA of 4 directions was not significantly different among the 3 methods (<em>p</em> > 0.05). There was strong correlation between UBM and Spectralis OCT (<em>r</em> = 0.957) but moderate correlation between Pentacam<sup>Ⓡ and UBM and Pentacam<sup>Ⓡ and Spectralis OCT (<em>r </em>= 0.557, 0.571, respectively, <em>p</em> < 0.05). Specifically, ACA of the superior quadrant showed a low correlation between Pentacam<sup>Ⓡ and UBM and Pentacam<sup>Ⓡ and Spectralis OCT (<em>r</em> = 0.257, 0.295, respectively). <b>Conclusions:</b> ACD measured by UBM in the sitting position was shallower compared to the other methods; however, ACD measured by IOL Master<sup>Ⓡ, Pentacam<sup>Ⓡ, and UBM showed significant correlations among the methods. The mean ACA measured by Pentacam<sup>Ⓡ, UBM, and Spectralis OCT showed no significant differences. Due to the high correlation of ACA measurements between UBM and Spectralis OCT in the present study, UBM is expected to be a good tool for measuring anterior segment parameters. The authors present a case of brain abscess in a patient with primary mucociliary transport failure. Primary mucociliary transport failure is unfamiliar term to neurosurgeon. It encompasses three hereditary disorders, that is, primary ciliary dyskinesia, cystic fibrosis and Young's syndrome. Clinical manifestations in these disorders appear to overlap each other, e.g., male infertility and chronic sinopulmonary infections. These are characterized by ciliary dysfunction or abnormality of mucus secretion therefore recurrent infection occurs in organs containing the mucociliary transport system. Major causes of non-traumatic brain abscess are sinusitis and pulmonary infection. So the possibility of brain abscess is much higher if mucociliary transport failure exists. Especially, young patients who have brain abscess coexisting with chronic sinopulmonary infection should be considered primary mucociliary transport failure.

Ependymomas are rare neuroepithelial tumors that arise from the ependymal cells of the cerebral ventricles, the central canal of the spinal cord and cortical rests. Some tumors may extend to the gray matter reaching the pial surface, but pure cortical ependymomas are uncommon. We report a case of ependymoma arising from extraventricular system, especially supratentorial falx. Sixty five-year-old woman presented memory impairment and apraxia was admitted to the our clinic. Magnetic resonance imaging (MRI) of the brain showed a 4.5×5.5 cm sized mass in the left occipital lobe, attached to the falx. The patient underwent the total tumor removal and the final pathology showed grade II cellular ependymoma. Because of the supratentorial and pure cortical ependymomas are rare, proper studies and pathologic confirm are needed to distinguish different tumors.
