        조현병 환자의 유병기간에 따른 강박증상

        Objectives The aim of this study was to evaluate the psychopathological features such as depression and anxiety in schizophrenics with obsessive-compulsive symptoms (OCS) as well as the severity of OCS according to duration of schizophrenia. Methods We randomly selected sixty four inpatients with schizophrenia. We classified the patients into two groups (OCS group, non-OCS group) according to the Yale-Brown Obsessive-Compulsive Scale (Y-BOCS). Clinical and demographic features were evaluated. To assess OCS, Y-BOCS were performed. The Korean version of the Positive and Negative Syndrome Scale(K-PANSS), the Brief Psychiatric Rating Scale (BPRS), the Korean version of the Calgary Depression Scale for Schizophrenia (K-CDSS), the Beck Anxiety Inventory (BAI) and the Satisfaction With Life Scale (SWLS) were conducted. Independent t-test and chi-square test were conducted to compare the two groups. Pearson correlation analysis was performed to examine the relationship between the duration of schizophrenia and the Y-BOCS score. Results The Y-BOCS, K-CDSS, and BAI scores were higher in the OCS group. There was a significant correlation between the duration of schizophrenia and the Y-BOCS score. Conclusions Anxiety and depression symptoms were severe in the OCS group. In addition, the results of this study indicate that the longer duration of schizophrenia, the more severe the OCS. Therefore, the evaluation of OCS in schizophrenics should be accompanied by treatment intervention.

        만성 폐쇄성 폐질환의 위험요인 선별을 통한 노모그램 구축

        최근 미세먼지 농도가 올라감에 따라 사람들은 호흡기 질환에 큰 관심을 가지고 있다. 본 연구는 인구학적 및 임상적 특징을 통한 만성 폐쇄성 폐질환(chronic obstructive pulmonary disease)의 위험요인을 선별하고 이에 따른 노모그램을 구축하였다. 먼저 국민건강영양조사(KNHANES) 6기 (2013-2015)의 인구학적 및 임상적 특징, 폐기능 검사 결과를 사용하여 로지스틱 회귀분석을 실시 하였고 비전공자들도 분석 결과에 대한 해석을 쉽게 할 수 있도록 만성 폐쇄성폐질환의 위험 요 인을 시각화한 노모그램을 구축하였다. 또한 ROC curve와 Calibration plot을 이용하여 만성 폐쇄 성 폐질환의 노모그램을 검증하였다. The concentration of fine dust has increased in Korea and people have become more concerned with respiratory diseases. This study selected risk factors for chronic obstructive pulmonary disease (COPD) through demographic and clinical features and constructed a nomogram. First, logistic regression analysis was performed using demographic and clinical feature and the pulmonary function test results of the Korean National Health and Nutrition Examination Survey (KNHANES) $6^{th}$ (2013-2015) and the nomogram was constructed to visualize the risk factors of chronic obstructive pulmonary disease in order to facilitate the interpretation of the analysis results. The ROC curve and calibration plot were also used to verify the nomogram of chronic obstructive pulmonary disease.

      • 5㎾급 접시형 태양열 흡수기의 열전달 성능분석

        The thermal performance of the air receiver filled with porous material for 5㎾t dish solar collector installed in Inha University, Korea, is experimentally investigated. The diameter of the parabolic dish is 3.2 m, and its focal length is 2 m. It consists of 10 small pieces of glasses which have their own curvatures, and the effective reflecting area is 5.9 ㎡. The reflectivity of the glass is 0.95, and the thermal capacity of the system is about 5 ㎾ thermal. The aperture diameter of the cylindrical-shape receiver which is made of stainless steel is 100 ㎜, and the height is 210 ㎜. A quartz window is installed at the receiver aperture to minimize the convective heat loss and prevent air leakages. In order to increase the heat transfer area, porous material (nickel-alloy) is inserted into the receiver. Air flows into the upper part of the receiver, which is the opposite side of the aperture. After the air flows through the inside receiver, that goes out of the receiver through 3 exits which are located near the aperture. The volumetric flow rates of air are varied from 600 to 1200 L/min. The thermal efficiency of the receiver ranges from 82% - 92% depending upon the flow rate. The results show that the system efficiency and receiver efficiency increase as the volume flow rate increases as expected. These results from the experiment will be useful for the applications to air heating receivers and solar reactors.

        The thermal performance of air receiver with a change of flow direction for dish solar collector. This system is installed and operated in Incheon, Korea. The thermal capacity of the system is about 5 kW thermal. The aperture diameter of the cylindrical-shape receiver which is made of stainless steel is 100 mm, and the height is 210 mm. Experiments are being carried out to investigate the thermal performance variation of the receivers with several design parameters such as the shape of the receiver, the flow directions and the flow rate of air. First, air flows into the upper part of the receiver, which is the opposite side of the aperture. After the air flows through the inside receiver, that goes out of the receiver through 3 exits which are located near the aperture. Second, air flows into the backside of the receiver, Which is the forward side of the aperture. After the air flows through the inside receiver, that goes out of the receiver through 1 exit. The results show that the system efficiency and receiver efficiency increase as the volume flow rate increases as expected.

        한국의사의 정부신뢰 관련 요인 분석: 의사단체들의 일부 임원의사들을 중심으로

        Objectives: This study aims to investigate the factors related to Korean physicians' trust in the government. Methods: We used structured questionnaires that were composed of multidimensional scales for each of the various categories. Results: The recognition levels of trust of the government by Korean physicians were not high, and they ranged from 3.6 to 4.8 for ten scales. The factors related to trust in the government were categorized into seven factors on the basis of a factor analysis. On the regression analysis, a positive relationship was found between "the individual propensity to trust" and trust in the government, while a negative relationship was found between "the recognition level regarding the government as an authoritarian power" and trust in the government. "Confidence about participation in the policy process" as internal efficacy and "belief in governmental ability and motivation toward public demand" as external efficacy also showed a strong positive relationship with trust in the government. Conclusions: From these results, we can draw the conclusion that making efforts to improve the recognition level of trust in the government among physicians is an important policy task. To increase the trust level, participation of physicians in the policy process in various ways and open communication between the physicians' associations and the government should be facilitated.

        A numerical and experimental studies are carried out to investigate the transient heat transfer characteristics of 5kWth dish type solar air receiver. Measured solar radiation and temperatures at several different locations are used as boundary conditions for numerical simulation. Many parameters' effects (reflectivity of the reflector, the thermal conductivity of the receiver body, transmissivity of the quartz window, etc.) on the thermal performance are investigated. Discrete Transfer Method is used to calculate the radiation heat exchange in the receiver. A transient heat transfer model is developed and the rate of radiation, convection and conduction heat transfer are calculated. Comparing the experimental and numerical results, good agreement is obtained. Using the numerical model, the transient heat transfer characteristics of volumetric air receiver for dish type solar thermal systems are known and the transient thermal performance of the receiver can be estimated.

        암 환자의 치료에 대한 요구도와 관련된 요인분석

        Objectives: Limited research has investigated the specific needs of patients with cancer. This study was performed to explore patients needs and the related factors. Methods: The data were collected by 1 National Cancer Center and 9 regional cancer centers in Korea. An interview survey was performed with using a structured questionnaire for the subjects(2661 patients who gave written informed consent to particiate) survey 4 months after diagnosis and review of medical records. Data were analyzed using t-test, ANOVA and multiple regression analysis. Results: When comparing the relating factors related with patient needs to the sociodemographic characteristics, the female group showed a higher level of recognition for physical symptoms, social support needs. The younger group showed a significantly higher level of recognition for health care staff, psychological problems, information and education, social support, hospital services needs. In addition, the higher educated group showed a higher level of recognition for health care staff, physical symptoms, social support needs. The higher income and office workers group showed a higher level of recognition for hospital services needs. When comparing the relating factors related with patient needs to the cancer, the breast cancer group showed a higher level of recognition for all needs excluding physical symptoms, accessibility and financial support needs. The combined radiotherapy with surgery and chemotherapy group showed a higher level of recognition for psychological problems, information and education, social support needs. Conclusions: This study showed that needs on patient with cancer was significantly influenced by female, higher educagion, lower income, having religion, office worker, liver cancer, breast cancer, colon cancer, chemotherapy, and combined therapy.

        증례 : 신장 ; 우연히 발견된 무증상 기종성 신우신염 1예

        기종성 신우신염은 주로 당뇨병 환자에서 발생하는 심한 괴사성 감염으로 발생빈도는 낮지만 임상경과가 매우 빠르고 예후가 나쁠 수 있기에 즉각적인 처치가 필요한 것으로 알려져 있다. 대개 급성 신우신염과 유사한 증상을 보이나 요로감염을 시사하는 증상 없이 우연히 영상검사 중에 진단된 증례가 국외에서 1예[7] 보고된 바 있다. 저자들은 진단 시점부터 치료 시작 전까지 무증상이었으나 연속적으로 시행한 영상검사상 악화 소견을 보여 항생제 치료를 시작하였고 영상검사상에서 호전을 확인한 증례를 경험하였기에 유사한 증례의 고찰과 함께 보고하는 바이다. Emphysematous pyelonephritis (EPN) is a severe gas-forming infection of the renal parenchyma and surrounding tissues. Patients with EPN commonly present with high fever, chills, and flank pain. These symptoms mimic a simple urinary tract infection, such that diagnosis is often delayed. Because of its life-threatening fulminant course, the early detection of EPN and its prompt treatment with intravenous antibiotics with or without percutaneous drainage are critical. Here we describe a case of a 63-year-old Korean female with diabetes mellitus who had no specific symptoms or signs of EPN. A chest computed tomography (CT) scan to assess a right pleural effusion incidentally detected an abnormal gas shadow in the renal parenchyma. An abdominal CT scan performed 5 days later showed increased gas within the kidney parenchyma, but the patient still had no symptoms of EPN. She was treated with intravenous antibiotics alone. A follow-up abdominal CT scan revealed the complete disappearance of the features of EPN. (Korean J Med 2015,89:567-570)
