최근 항생제의 남용에 의한 항생제에 대한 내성균주가 증가하고 있다. 특히 vancomycin 내성 장구균이 중환자실 환자 등에서 급속히 증가하고 있으나, 내성균에 대한 확실한 치료방법이 없어 내성균주의 전파에 주의하여야 한다. 저자들은 국내에서 골수이식과 관련된 VRE 패혈증에 의한 사망한 1예를 경험하여 문헌고찰과 함께 보고하는 바이다. Enterococci are inhabitant of the intestinal tract of human. Vancomycin-resistant enterococci (VRE) were first recognized in the min-1980s. VRE infection have been increasingly reported in immunosuppressed patients over the past decade and have became one of major nososcomial pathogens. The mortality rate for the patients with VRE bacteremia are extremely high because the optimal therapy for these infection is unknown. Moreover VRE have a potential to transfer their resistance genes to pathogenic gram-positive bacteria. We report a case of vancomycin-resistant E. faecium sepsis in a patient undergoing allogeneic BMT. She had multiple cytotoxic chemotherapies, and received prolonged course of multiple broad-spectrum antibiotics including vancomycin during complicated transplantation course.

Self-efficacy is important for adolescents to cope with current and future difficulties. Therefore, in this study, the effect of school-system factors (friendship, teacher relation, and school happiness) on self-efficacy was investigated for students attending high schools located in Daejeon Metropolitan City. As a result of this study, it was found that friendship, teacher relationship, and school happiness have a significant correlation with self-efficacy, respectively. And it was found that class grade, health status, friendship, and school happiness were factors that significantly influenced self-efficacy. The implications and limitations of this study were also presented

Ultrasonic Vibrator is designed to achieve the maximum vibration amplitude at 30 kHz by in-cluding a horn (diameter, 40 mm), mechanical vibration amplifier at the top of the ultrasonic vibrator in the system and making the complete system resonate. In addition, it is experimentally visualized by particle imaging velocimetry (PIV) that the acoustic streaming velocity in the gap is at maximum when the gap between the ultrasonic vibrator and stationary plate agrees with the multiples of half-wavelength of the ultrasonic wave. This fact results from the resonance of the sound wave and the theoretical analysis of that is also accomplished and verified by experiment. It is observed that the magnitude of the acoustic streaming dependent upon the gap between the ultrasonic vibrator and stationary plate possibly changes due to the measurement of the average velocity fields of the acoustic streaming induced by the ultrasonic vibration at resonance and non-resonance. There exists extremely small average velocity at non-resonant gaps while the relatively large average velocity exists at resonant gaps compared with non-resonant gaps. It also reveals that there should be larger axial turbulent intensity at the hub region of the vibrator and at the edge of it in the resonant gap where the air streaming velocity is maximized and the flow phenomena is conspicuous than that at the other region. Because the variation of the acoustic streaming velocity at resonant gap is more distinctive than that at non-resonant gap, shear stress increases more in the resonant gap and is also maximized at the center region of the vibrator except the local position of center (r〓0). At the non-resonant gap there should be low values of vorticity distribution, but in contrast to the non-resonant gap, high and negative values of it exist at the center region of the vibrator with respect to the radial direction and in the vicinity of the middle region with respect to the axial direction. Acoustic streaming is noise-free due to the ultrasonic vibration and maintenance-free because of the absence of moving parts. Moreover, the proposed method by acoustic streaming can be utilized to the nano and micro-electro mechanical systems as a driving mechanism in addition to the augmentation of the streaming velocity.

School social work has been a professional area interested in helping students make satisfactory adjustments to school, and its prospect seems bright. Yet school social work is expected to meet some challenges in the future. Under such circumstances, American experience and researches can be a good guide for future directions of Korea's school social work, because the United States has belonged to the countries where school social work has well developed so far. Hence on the basis of the cases in American contexts, this study suggests some future directions for school social work at the exploratory level. The results of this study indicate that the future school social work needs to devote particular attention to the scientific and macro/comprehensive approaches.
사질성 지반에서 이수식 쉴드 TBM 적용시 굴진면으로의 이수 침투특성에 대한 해석적 고찰

본 논문은 이수식 쉴드 TBM 굴착시 굴착면 안정에 큰 영향을 미치는 굴착면의 이수 침투에 따른 이막형성여부에 대하여 해석적 고찰을 수행하였다. 이러한 해석적 접근은 이수식 쉴드 TBM의 계획 및 설계시 최적 장비 형식 선정 및 시공계획 수립에 참고가 될 수 있으나, 이제까지 이에 대한 효과적인 해석적 방법에 대하여 제안된 것이 없는 상황이다. 따라서 본 논문에서는 사질토 입자의 모델링에 효과적인 $PFC^{2D}$의 Fluid Coupling simulation을 이용하여 해석을 수행하였으며, 해석결과, 이수침투 특성은 굴착 대상지반의 투수성과 이수특성(비중, 점성계수등)에 따라 변화됨을 파악할 수 있었다. 본 해석적 접근방법을 통하여 이수식 쉴드 TBM의 계획 설계단계에서 대상지반 특성을 고려한 이수 점도관리 방안 수립과 함께 시공중의 초기굴진단계에서의 최적 이수점도의 재검증을 통하여 안전한 터널시공에 도움이 될 것이라 판단된다. This paper presents numerical analysis of the mud cake infiltration behaviour which is influenced tunnel face stability during excavation by slurry shield TBM. This analysis method can make useful data to select proper shield TBM type and to set up the construction plan. But effective analysis did not proposed until now. In this paper, we carried out numerical analysis using by $PFC^{2D}$ fluid coupling simulation which is suitable for sandy soil modelling. As a analysis result, we checked that the slurry infiltration behaviour varied with soil permeability and slurry characteristic(specific weight, viscosity etc). This analysis method is helpful safety excavation through anticipating the proper slurry viscosity at the design stage and verifying the slurry quality at initial excavation stage.
정량적 위험도 분석을 이용한 복선철도터널에서의 적정 대피통로 간격 산정을 위한 연구

This paper describes a study to determine proper escape way interval for the design phase of single bored double track tunnel. Among many methods which determine escape way interval, we choose a QRA(Quantitative Risk Analysis) method. But a different method must be chosen differ from other country because of special design situation of Korea. So, it is necessary to develop a method which considers a special design condition of Korea. Because fire accidents of railway tunnel are a rare event, simulated situation can be produced by CFD simulation and evacuation analysis simulation. However, it is generally difficult to estimate of fatalities from these methods, so a concept of FED is introduced to estimate of fatalities. Quantification process provides effective results for practical design stage and the result were employed in design.
