        암 관련 망막증 1 예

        The cancer-associated retinopathy (CAR) is a very rare paraneoplastic syndrome. Most of the cases are associated with the lung cancer, especially small cell lung cancer. The triad of CAR is described as photosensitivity, scotomatous peripheral visual field loss, and attenuated retinal arterial caliver. The pathogenesis is suggested that anti-retinal antibody induced by malignat tumors causes the degeneration of photoreceptors in the retina through the cross reaction. The titer of these antibodies can be used as an index of disease activity and treatment response. The CAR antigen is not clearly determined yet, but 26 kD proteins located in cone and rod photoreceptor cells are considered as that antigen. Consequently, the destructioon of the photoreceptors by immune mechanism results in visual changes even complete blindness. If no treatment is done, the visual loss progresses to complete blindness. Many authors recommanded prednisone as a treatment. This can reduce the antibody titer even to normal range, and stop the disease progress through the immunosuppression. With a brief review of literature, we report a case of cancer-associated retinopathy in 54-year old female patient who has a small cell lung cancer in the stage of limited disease, Her initial symptomes were peritpheral visual loss and sparkling in front of her eys. We treated her with the combination chemotherapy of cisplatin and etoposide (VP-16) and radiation therapy. The gastrointestinal symptom made her stop prednisone medication. The outcome of teatments was complete remission and her visual symptoms were not progressed. We have a good result in this patient. So we report this case.

        This research deals with medical institution’s inventory control to develop Heuristic Algorithm; such Algorithm’s flow can classify inventory asset, and it can vary its form depending on the request. In other words, all of inventory properties need to assemble a targeted inventory specification. Independent demanding materials are applied by a short term probability model. On the other side, Dependent demanding materials are applied by Discrete-time period EOQ model, Periodic order quantity model, Part-period Balancing model, and Silver-Meal Heuristic model. Moreover, Dependent demanding materials estimated a reorder point and an optimum inventory amount ; the models that have an application of this Heuristic Algorithm are approved after computational study of hospital’s inventory items. In conclusion, this research proved that a proposed Heuristic model can be applied depending on the hospital’s inventory type or condition.

        위암에서 에스트로젠 수용체 알파의 소실과 프로모터 과메틸레이션

        목적 : 위암에서 에스트로겐 수용체 알파의 프로모터 부위의 메틸레이션으로 인한 위암 조직 내 발현소실이나 불활성화에 대해서는 아직 보고된 바가 없다. 본 연구에서는 위암 세포주와 위암으로 진단 받은 환자들의 조직에서 에스트로젠 수용체 알파의 발현을 평가한 후 위암에서 에스트로젠 수용체 알파의 소실이 5' 프로모터 부위의 메틸레이션과 관련이 있는지를 알고자 하였다. 방법 : 위암 세포주에 대하여 에스트로젠 수용체 알파의 발현을 보기 위하여 웨스턴블롯(12개 세포주)과 RT-PCR(13개 세포주)을 시행하였다. DNA에 대하여 bisulfite modification을 하여 메틸레이션 특이적 중합효소 연쇄반응과 TA 클로닝 후에 염기서열 분석을 시행하였다. 위내시경 조직검사에서 위암으로 진단 받은 42명의 위암 환자의 조직에 대하여 에스트로젠 수용체 알파에 대한 면역조직 화학검사를 시행하였다. 결과 : 위스턴블롯과 RT-PCR결과 KatoⅢ 세포주의 mRNA만 약하게 발현되었으며 모두 음성 반응을 보였다. 메틸레이션 특이적 중합효소 연쇄반응에서 모두 양성이었다. 염기서열 분석에서 ATG 시작 코돈 주변의 CpG island의 methylation을 확인할 수 있었다. 결론 : 위암에서 에스트로젠 수용체 알파의 ATG 시작 코돈주변의 프로모터 부위의 CpG 부위에서 관찰된 메틸레이션은 ER-α유전자의 소실을 일으킬 수 있는 원인의 하나가 될 수 있을 것으로 생각된다. Background : The significance of ER expression and hormone manipulation in gastric cancer is not established. There have been several reports supporting the role of the ER gene as tumor suppressor gene in carcinogenesis. The ER-α gene is located on chromosome 6q25.1. Deletions of the long arm of chromosome 6 are common in gastric carcinoma, suggesting the pressence of tumor suppressor genes in this region. The proportion of ER-positive gastric cancers ranges between 0% and 67% depending on the method of detection. Epigenetic inactivation might explain the loss of ER-α gene expression in gastric cancer. There is no information available regarding the methylation status of the ER-α gene promotor region in gastric cancer so far. The aim of this study was to assess the expression of ER-α in gastric cancer cell lines and determine whether methylation of the 5' promotor region is associated with loss of ER-α expression in gastric cancer. Methods : We investigated such methylation in 13 gastric cancer cell lines. Western blot analysis, reverse transcription-polymerase chain reaction (PCR), methylation-specific PCR (MS-PCR) and bisulfite sequencing analyses were used. Immunohistochemical staining for the ER-α gene was dome for forty-two paraffin embedded tissues from gastric cancer patients. Results : ER-α protein was not detected in any cell line, ER-α mRNA was expressed in only Kato Ⅲline. MS-PCR and bisulfite sequencing showed all thirteen gastric cancer cell lines had methylated CpG regions in their ER-α gene promoters. Immunohistochemical staining of ER-α showed no positivity in any of examined samples. Conclusion : Inactivation of ER-α gene expression in gastric cancer cell lines appears associated with CpG island methylation near the TGA initiation codon of the ER-α gene.

        맞춤형화장품 비즈니스 생태계 재구성을 위한 제언

        연구목적 본 연구는 현재 맞춤형화장품 비즈니스 생태계를 진단하고, 동반성장을 위한 생태계를 재구성하고자 한다. 연구방법 연구는 문헌연구와 사례 분석으로 진행되었다. 결과 세계 최초로 맞춤형화장품 제도를 법제화한 한국의 맞춤형화장품 생태계는 첫째, 국가 주도형 산업 육성 시스템, 둘째, 대기업 등 일부 소수 기업이 독점하는 구조, 셋째, 선형적 가치사슬의 폐쇄적 생태계라는 특징을 가지고 있다. 이와 같은 구조로는 혁신이 어려우며, 상호의존, 개방성, 다양성, 혁신성을 바탕으로 함께 성장할 수 있는 생태계로 재구성되어야 한다. 결론 맞춤형화장품의 대중화를 위해서는 가격 적정화, 품질 안정성, 소비자 접점 확대가 필수적이며, 디지털 플랫폼을 통해 모든 구성원이 유기적으로 협력·경쟁할 수 있는 건강한 생태계가 조성되어야 한다. PURPOSE This study aims to diagnose the current customized cosmetics business ecosystem and reconstruct the ecosystem for co-evolution. METHOD The study was conducted with literature research and case analysis. RESULT Korea's customized cosmetics ecosystem, which legislated the world's first customized cosmetics system, is characterized by First, a state-led industry promotion system, Second, a structure monopolized by some small companies such as large corporations, and, Third, a closed ecosystem with a linear value chain. Innovation is difficult with such a structure, and it should be reorganized into an ecosystem that can grow together based on interdependence, openness, diversity, and innovation. CONCLUSION For the popularization of customized cosmetics, price optimization, quality stability, and expansion of consumer contact are essential, and a healthy ecosystem should be created where all members can organically cooperate and compete through a digital platform.

        N/A Some studies suggest that patients with diabetes mellitus have an increased incidence of osteoporosis, but others have disputed it. The mechanisms of reduced bone density in diabetes are unclear but excessive calcium loss in the urine is generally accepted as one of the factors which contribute to bone loss in diabetes. Whole body bone densitometry makes possible to estimate the total body calcium concentration which might be influenced by urinary calcium loss and other humoral factors. To elucidate the possible direct relationship between total body calcium and reduced bone density, we examined the total body calcium, total body fat and the bone mineral density of the lumbar and femur with densitometry (Dual Energy X-ray Absorptiometry) in 93patients with type II diabetes mellitus end 316nondiabetic normal controls, Serum osteocalcin levels also measured by radioimmunoassay in all the patients. The results are summarized as follows: l) The bone mineral density declined with age in both diabetics and the control group but had no significnat differences statistically. 2) Thirty-six patients with diabetes (37.9%) had reduced bone mineral density below one standard deviation (1SD) of the normal controls. 3) The total body calcium concentration correlated highly to the total body bone density. 4) The femur bone density correlated more significantly than the lumbar bone density to the whole body bone density. 5) There were no correlations between the whole body bone density and total body fat or serum osteocalcin levels.

        Chemotherapy induced peripheral neuropathy (CIPN) could debilitate the quality of life in the patients with cancer. According tothe severity of CIPN, the modification of dosage of chemotherapeutic agents and switch to other drugs can be unavoidable. Platinum such as cisplatin and oxalipatin, vinka alkaloids, bortezomib, and taxane can cause CIPN. The characteristics and severityof CIPN depends on the dosages, duration of exposure of chemotherapeutic agents, comcomittant illness or other drugs affecting onperipheral nervous system and the methods of assessment for CIPN. The symptoms may last for several months or permanentlyeven after quitting chemotherapy. Typically it distributed bilaterally and starts from the distal part of extremities and is presentedprogressively in stocking and glove pattern. Sensory nerve is more involved rather than motor nerve and amplitude of sensorynerve conduction is observed in CIPN. Prevention for CIPN is not effective at present. Tricyclic antidepressant including amitriptylineor nortriptyline and gabapentine have been tried in the practice for the management of CIPN despite of the lack of significantevidence through clinical trials. Recently duloxetine has been reported to decrease pain in the patients with CIPN compared with thepatients with placebo (p = 0.03).

        성인의 급성 림프구성 백혈병에서 L - 17 M 복합화학요법의 치료효과 ; 최종 보고

        N/A Objectives: When treatment schedules for childhood ALL (Acute Lymphocytic Leukemia) were used in adults with ALL, remission rates were lower and disease free survival was inferior. L17M chemotherapy is characterized by intensive chemotherapy regimen calling for 2.5~3 years of treatment that included prophylactic intrathecal methotrexate without cranial irradiation. MSKCC (Memorial Sloan-Kettering Cancer Center) reported complete remission rate 82% and median survival duration 30.7 months in L17M chemotherapy for ALL in adults. Methods: Twenty-six previously untreated patients with acute lymphocytic leukemia were treated with L-17M chemotherapy. L-17M chemotherapy contained multiple drugs including vincristine, prednisolone, adriamycin, cyclophosphamide, daunorubicin, Ara-C, 6-thioguanine, methotrexate, L-asparaginase, dactinomycin, and BCNU which were employed in various combinations, doses and schedules during the induction, consolidation and maintenance phases. Results: Twenty-two out 26 (88%) cases achieved complete remission, median duration of remission was 80 weeks (1+ - 338+ weeks), and median duration of survival was 79 weeks (3 - 340+ weeks). Ten out of 22 (45%) patients relapsed in bone marrow (8 cases) and CNS (2 cases) relapse. Median period of granulocytopeia was 5 days (0 - 31 days) during the induction phase and 8 days (0 - 26 days) in consolidation phase. Nonhematologic toxicities were nausea, vomitig, alopecia, stomatitis, diarrhea, neurotoxicity. Conclusion: L-17M chemotherapy was considered as an effective therapy in adult ALL.
