        산풍고삼환(散風苦蔘丸)이 알레르기성 접촉피부염에 미치는 영향

        Objectives: To study the effectiveness of Sanpunggosamhwan(SGH) against Allergic Contact Dermatitis(ACD), the contact hypersensitivity assay, change of cutaneous shape, anti-allergic effect, anti-inflammatory effect, and the effect on skin barrier were observed. Methods: The 200g rats were divided into three groups of 15 rats. The first group is the Normal group which was applied Acetone olive oil only. The second group is the ACD group which has intentionally activated Allergic Contact Dermatitis by DNCB. The third group is the SGH group which was given medication of Sanpunggosamhwan extract after the induction of Allergic Contact Dermatitis by DNCB. Each group of rats was observed after 24, 48 and 72 hours. Results: I. With the result of contact hypersensitivity assay, at 24hours SGH group showed appreciably less ear swelling than ACD group. 2. Regarding general change of skin, SGH group showed less hyperplasia of epidermis, less damage of epidermis than ACD group. 3. Regarding the number of WBC, ACD group showed significantly less than normal and SGH group at 72 hours. 4. Regarding the number of RBC in blood, ACD and SGH group showed significantly more RBC than normal group at 24, 48, 72 hours. 5. Regarding the ratio of neutrophil in WBC, ACD and SGH group showed significantly high percentage than normal group at 24, 48 hours. 6. Regarding the ratio of lymphocyte in WBC, ACD and SGH group showed significantly high percentage than normal group at 48 hours. 7. Regarding the erythema, SGH group showed significantly more erythema than normal and ACD group at 48 hours. 8. Regarding the melanin, SGH group showed significantly less melanin than normal group at 24 hours.9. Regarding the skin hydration, SGH group showed significantly high value than and ACD group at 72 hours. 10. Regarding the skin pH, ACD group showed significantly high value than normal and SGH group af 24 hours. 11. Regarding the number of Total IgE, ACD and SGH group showed more Total IgE than normal. g개up at 24 hours. 12. At Electro microscope-morphologic changes of skin, the damage of epithelium was decreased and regeneration power of skin was increased in the SGH group. Conclusions: The Sanpunggosamhwan extract administration was effective on the mitigation of skin damage in rats with allergic contact dermatitis.

        蔘蘇飮과 蔘蘇飮加味方의 抗알레르기효과에 관한 實驗的 硏究

        Background : Although Samsoeum has been used frequently on allergic rhinitis, but there isn't any experimental research for that. Objective : This study was performed to investigate the anti-allergic effects of Samsoeum and Samsoeumgamibang. Materials and Methods : Katayama's method was used to observe the vascular permeability response induced by serotonin and histamine. Muller's method was used to observe the contact dermatitis response induced by picryl chloride. Miller's method was used to observe the delayed type hypersensitivity response to SRC. Results : 1. In the vascular permeability response to intradermal injection of serotonin, 2,600mg/kg, p.o. group of Samsoeum(蔘蘇飮), 1,300 and 2,600mg/kg, p.o. group of Samsoeumgamibang(蔘蘇飮加味方) showed significant inhibitory effects on the leakage of Evan's blue solution. 2. In the vascular permeability response to intradermal injection of histamine, 1,300 and 2,600mg/kg, p.o. group of Samsoeum, also 1,300 and 2,600mg/kg, p.o. group Samsoeumgamibang showed significant inhibitory effects on the leakage of Evan's blue solution. 3. In the contact dermatitis response induced by picryl chloride, 1,300 and 2,600mg/kg, p.o. group of Samsoeum, 1,300 and 2,600mg/kg, p.o. group of Samsoeumgamibang showed significant inhibitory effects on ear swelling formation. 4. In the delayed type hypersensitivity response to SRC. 2,600mg/kg, p.o. group of Samsoeum and 2,600mg/kg, p.o. group of Samsoeumgamibang showed significant inhibitory effects on foot swelling. Conclusion : This study shows that Samsoeum and Samsoeumgamibang may have anti-allergic effects. So Samsoeum and Samsoeurngamibang can be helpful to treat allergic rhinitis.

        제2형 당뇨병환자에서 인슐린저항성과 뇌혈관 박동성 지수와의 연관성

        -Background: Diabetic patients have a 3-fold risk for cerebrovascular disease compared with nondiabetic controls. The aim of the present study was to investigate the association of insulin resistance with pulsatility index (PI) of cerebral arteries in type 2 diabetic patients. Methods: We compared a group of 90 patients with stroke free, type 2 diabetes and an age- and sex- matched control group of 45 healthy subjects without diabetes. Diabetic patients were divided into 3 groups according to the ISI (insulin sensitivity index). We evaluated PI of the middle cerebral artery (MCA) by transcranial Doppler ultrasonography (TCD) and insulin resistance determined by short insulin tolerance test. Results: The PI was significantly higher in diabetic patients than that in healthy controls (P < 0.05), and also higher in patients with insulin resistance than that in insulin sensitive diabetic patients (P < 0.05). The PI of the MCA was significantly correlated with age (r= 0.465, P < 0.01), duration of diabetes (r = 0.264, P = 0.025), hypertension (r = 0.285, P = 0.015) and inversely correlated with insulin resistance (r = -0.359, P = 0.030).Multiple regression analysis was performed with PI as a dependent variable and insulin resistance as an independent variable along with known clinical risk factors. Age (β = 0.393, P < 0.01) and duration of diabetes (β = 0.274, P = 0.043) exhibited a significant independent contribution to PI. Conclusions: PI might be useful markers of the detection of diabetic cerebrovascular changes and insulin resistance, measured with short insulin tolerance test, showed correlations with PI, but age and duration of diabetes contributed independently to the variability in the PI. (J Kor Diabetes Assoc 30:347~354, 2006) 연구배경: 이번 연구를 통하여 제2형 당뇨병환자를 대상으로 뇌혈관 질환의 지표인 박동성 지수의 증가에 영향을 미치는 인자를 찾고 특히 인슐린저항성과의 연관성을 규명하고자 한다.방법: 뇌경색의 과거 병력이 없는 제2형 당뇨병환자 90명 (남 43명, 여 47명) 및 정상 대조군 45명 (남 24명, 여 21명)을 대상으로 임상적, 생화학적 특성을 조사하였고 단시간 인슐린저항성 검사를 시행하여 인슐린저항성 측정 및 경두개 초음파를 시행하여 박동성 지수를 측정하였다. 결과: 제2형 당뇨병환자의 박동성 지수가 정상 대조군에 비해 컸고 당뇨병환자군에서도 인슐린저항성 그룹이 인슐린감수성 그룹에 비해 박동성 지수가 컸다. 중뇌 동맥의 박동성 지수는 나이, 당뇨병 이환기간, 고혈압의 유무 및 인슐린저항성과 상관관계가 있었다. 다중회귀분석 결과 나이 및 당뇨병 이환기간이 박동성 지수를 결정하는 독립적 위험인자였다.

        당뇨병 신증을 동반한 환자에서 단핵구 내 NF-κB 및 AP-1의 활성화

        Background: We evaluated the role of oxidative stress in diabetic nephropathy by measuring intracellular reactive oxygen species (ROS) and redox-sensitive transcription factors in isolated peripheral mononuclear cells (PBMC). Methods: From 66 diabetic patients with or without diabetic nephropathy (Group III and II, respectively) and 49 normal control subjects (Group I), spontaneous and stimulated ROS levels, activities of nuclear factor-kappa B (NF-κB), activator protein-1 (AP-1), and specificity protein1 (Sp1) in PBMC, urinary and PBMC TGF-β1 (transforming growth factor-β1), and 24-hour urinary albumin excretion (UAE) were measured.Results: Spontaneous ROS was significantly higher in group III and II than group I (60.7 ± 3.3 vs. 60.0 ± 3.0 vs. 41.1 ± 2.4%, respectively), and stimulated ROS were significantly higher in Group III compared to Group II (Increment of H2O2-induced ROS production: 21.8 ± 2.2 vs. 11.1 ± 2.0%, respectively; increment of PMA-induced ROS production 23.5 ± 4.5 vs. 21.6 ± 2.2%, respectively). The activities of NF-κB and AP-1, but not of Sp1, were significantly higher in Group III than in Group II (2.53 vs. 2.0 vs. 1.43-fold, respectively). Both PBMC- and urinary TGF-β1 levels were higher in Group III than Group II (3.23 ± 0.39 vs. 1.99 ± 0.68 ng/mg in PBMCs, 16.88 ± 6.84 vs. 5.61 ± 1.57 ng/mL in urine, both respectively), and they were significantly correlated with activities of NF-κB and AP-1 and 24-hour UAE. Conclusions: Increased intracellular ROS generation in PBMCs of diabetic patients is involved in the pathogenesis of diabetic nephropathy through activation of NF-κB and AP-1, but not Sp1, and increased expression of TGF-β1. (J Kor Diabetes Assoc 31:261~273, 2007) 연구배경: 본 연구는 당뇨병환자에서 세포 내 반응성 산소족 스트레스를 정량화 하고, PBMC 내 NF-κB, AP-1 및 Sp1 등 산화환원반응에 관여하는 여러 전사인자의 활성도를 측정하여 산화 스트레스가 신손상에 미치는 영향을 알아보고자 하였다. 또 산화 스트레스와 연관 있다고 알려진 TGF-β1을 PBMC와 소변에서 측정하여 24시간 UAE와 앞의 전사인자들과 분석하여 당뇨병 신증에서 TGF-β1의 역할에 대해서도 알아보고자 하였다. 방법: 66명의 제2형 당뇨병환자와 연령, 성별에 맞춘 정상 대조군 49명을 24시간 소변 내 알부민 배설량에 따라 세 군으로 나누었다. Dichlorodifluorescein (DCF)와 형광분광광도계를 사용하여 세포 내 반응성 산소족의 활성도를 측정하였고, PBMC 내 NF-κB, AP-1 및 Sp1의 활성도는 EMSA를 이용하여 측정하였으며, PBMC와 소변 내 TGF-β1는 quantitative sandwich enzyme immunoassay로 정량화하였다. 결과: 임의의 반응성 세포족은 당뇨병환자군(II군, III군) 에서 정상군(I군)에 비해 높게 측정되었다(60.7 ± 3.3 vs. 60.0 ± 3.0 vs. 41.1 ± 2.4, %, 순서대로, P < 0.05). H2O2 와 PMA로 자극시킨 반응성 세포족은 III군에서 I군뿐 아니라 II군보다도 의미있게 높게 측정되었다(H202에 의해 자극된 후 증가율: 21.8 ± 2.2 vs. 9.1 ± 4.0 vs. 11.1 ± 2.0, III, II, I군 순서; PMA에 의해 자극된 후 증가율이 23.5 ± 4.5 vs. 12.1 ± 3.0 vs. 21.6 ± 2.2%, III, II, I군 순서). PBMC 내 Sp1을 제외하고 NF-κB, AP-1은 III군에서 II에 비해 의미있게 증가하였다(2.53 vs. 2.0 vs. 1.43배, NF-κB, AP-1, Sp1 순서). 그리고 PBMC 및 소변 내 TGF-β1 역시 III군에서 II에 비해 높게 측정되었고(PBMC에서 3.23 ± 0.39 vs. 1.99 ± 0.68 ng/mg; 소변에서 16.88 ± 6.84 vs. 5.61 ± 1.57 ng/mL, 순서대로), 이들은 NF-κB 및 AP-1의 활성도와 24시간 UAE와 상관관계에 있었다.

        담낭암을 동반한 말단비대증 1예

        Acromegaly is a systemic endocrine disorder due to an excessive release of growth hormone, which increases the serum levels of insulin-like growth factor-1(IGF-1). Elevated levels of these hormones are assumed to increase the incidence of malignant tumors in patients with acromegaly, due to by stimulating the growth and maturation of cells. In particular, IGF-1 is considered to be closely related with the development of colon polyps and colon cancers. Studies suggest that various malignant tumors, including thyroid cancer, brain tumor and renal cell carcinomas, are also more common in patients with acromegaly. Here, a case of gall bladder cancer in a patient with acromegaly, and the possible relationships between these two disorders, is reported 말단 비대증은 증가된 성장호르몬 및 인슐린양 성장인자-1의 성장촉진작용으로 인하여 다양한 심혈관계, 호흡기계, 내분비계 합병증 및 악성 종양 발생의 위험이 증가하는 것으로 여겨지고 있다. 특히, 인슐린양 성장인자-1은 대장용종 및 대장암 발생과 깊은 연관성이 있고, 그 이외에도 말단비대증 환자에서 갑상선암, 뇌종양, 신장암 발생이 증가한다고 보고되어 있는데, 본 저자들은 국내에서 아직까지 보고되지 않은 말단비대증 환자에게서 동반한 담낭암 1예를 경험하고 이 두 질환의 가능한 연관성에 대해 보고하였다.

        일시적인 뇌하수체기능저하증을 동반한 터너 증후군 1예

        Turner's syndrome is characterized by short stature and gonadal dysgenesis, and it is often associated with various systemic manifestations, such as cardiovascular, renal, thyroidal, gastrointestinal, and musculoskeletal disorders. Though very rare, it can also be accompanied by hypopituitarism. It is important to give a meticulous medical attention to short females with gonadal dysgenesis so that neither disease is neglected or gets delayed diagnosis. In this case, Turner's syndrome and hypopituitarism were diagnosed almost simultaneously, but hypopiuitarism was transient, and the normal pituitary function was recovered spontaneously without any treatment. Initial sella MRI showed mild congenital hypoplastic hypopituitarism, and combined pituitary function test was compatible with hypopituitarism, but after 5 years, though growth hormone deficiency was still present, otherwise normal pituitary function was noted without any change in MRI. Herein, we are reporting a case of Turner's syndrome with transient idiopathic hypopituitarism with the review of literature. 터너 증후군은 증후군은 X 염색체의 완전 혹은 불완전 결손으로 인한 저신장증과 성선무발생증을 특징으로 하는 여성에서 가장 흔한 성염색체 질환이다. 그 이외에도 특이적인 근골격계 기형, 심혈관계 질환, 신장기형, 갑상선 기능저하증, 내당능장애, 염증성 장 질환 등 전신적으로 다양한 임상 양상을 보일 수 있다. 드물지만 뇌하수체질환이 동반되기도 하는데, 갑상선 기능저하증과 성선 기능저하증으로 인해 이차적으로 뇌하수체 비후증이나 뇌하수체 선종이 발생한 경우가 있고, 뇌하수체 공터키안 혹은 뇌하수체기능저하증이 동반된 경우가 몇 차례 보고된 적 있다. 저자들은 터너 증후군 환자에서 원인 미상의 일시적인 뇌하수체기능저하증이 특별한 치료 없이 회복된 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.
