        방사선치료 Patterns of Care Study 개발을 위한 2002년 한국 방사선종양학과 전문의들의 직장암 방사선치료 원칙 조사연구

        목 적 : 한국에서 발생빈도가 높은 암의 하나인 직장암의 치료에 있어 전국의 각 병원 치료방사선과(방사선종양학과)에서 시행하고 있는 치료 형태를 세부 항목별로 파악하여 방사선종양학 전문의 간의 치료원칙의 차이점을 분석하고 이를 바탕으로 향후 Patterns of Care Study를 위한 연구의 기본 자료로 제시하고자 한다. 대상 및 방법 : 경인지역 소재 18개 대학병원 및 방사선종양학과가 있는 병원들의 방사선종양학과 전문의들이 모여 직장암의 방사선치료 원칙 분석을 위한 consensus committee를 구성하고, 회합 및 설문조사를 통하여 각 의사들의 방사선치료 원칙을 파악하기 위한 세부 항목 설정과 이를 토대로 직장암 환자 개개인의 치료 내용을 측정하는 구체적인 항목들을 개발하였다. 이 항목들을 이용하여 직장암의 방사선치료를 담당하고 있는 전국의 방사선종양학과 전문의들을 대상으로 설문조사를 시행한 후 각 전문의들의 의견이 일치하는 부분과 상이한 부분들을 분석하였다. 결 과 : 전문의 별 치료원칙의 측정에 사용된 항목들은 1) 치료대상환자의 선정기준 8개 항목, 2) 수술 전후 병기 및 예후 인자 검사 20개 항목, 3) 수술 및 항암화학요법과의 병용기준 7개 항목, 4) 환자의 setup 및 처치 9개 항목, 5) 모의치료시 조사야 결정 19개 항목, 6) 치료계획 5개 항목, 7) 치료 중 상태 확인 위한 이학적/병리학적 검사 4개 항목, 8) 치료 후 추적 관찰시 검사할 10개 항목 등 총 82개의 항목이 개발되었다. 이 항목들은 전국 각 병원들의 방사선종양학과 전문의를 대상으로 설문조사 되어 치료원칙에 대한 분석이 항목별로 이루어졌다. 설문조사 결과 전국의 각 병원 방사선종양학과에서 시행되고 있는 직장암의 방사선치료 형태는 일정 범위를 유지하고 있으나, 일부 항목에 있어서는 치료자 간 의견차이가 큰 것으로 나타났으며 이들 중 전체 응답자의 50% 이상에서 합의가 되지 못한 항목들은, 1) 치료환자의 선정 기준 중 수술 전 방사선치료 대상, 2) 병기 결정 검사 중 직장내 초음파(EUS, endorectal ultrasound), 흉부 CT, 뼈 스캔 등의 시행 여부, 3) 항암제와의 병용 원칙, 4) 모의치료시 소장 위치 확인을 위한 조영제 사용, 5) 조사야의 경계선 결정, 6) 치료 후 추적 관찰 시 CEA와 대장내시경의 시행여부 등이었다. 결 론 : 이견이 많은 항목들에 대해선는 향후 담당 전문의들을 대상으로 consensus meeting등을 통한 체계적인 분석 및 합의의 도출이 필요한 것으로 보이며, 본 연구에서 밝혀진 각 병원들의 치료 내용을 토대로 향후 표준화된 치료 모델 개발을 위한 Patterns of Care Study가 진행될 예정이다. Purpose : To conduct a nationwide survery on the principles in radiotherapy for rectal cancer, and develop the framework of a database of Korean Patterns of Care. Study. Materials and Methods : A consensus committee was established to develop a tool for measuring the patterns in radiotherapy protocols for rectal cancer. The panel was composed of radiation oncologists from 18 hospitals in Seoul Metropolitan area. The committee developed a survey format to analyze radiation oncologist's treatment principles for rectal cancer. The survey items developed for measuring the treatment principles were composed of 1) 8 eligibility criteria, 2) 20 items for staging work-ups and prognostic factor, 3) 7 items for principles of combined surgery and chemotherapy, 4) 9 patient set-ups, 5) 19 determining radiation fields, 6) 5 radiotherapy treatment plans, 7) 4 physical/laboratory examination to monitor a patient's condition during treatment, and 8) 10 follow-up evaluations. These items were sent to radiation oncologists in charge of gastrointestinal malignancies in all hospitals (48 hospitals) in Korea to which 30 replies were received (63%). Results : Most of the survey items were replied to without no major differences between the repliers, but with the following items only 50% of repliers were in agreement : 1) indications of preoperative radiation, 2) use of endorectal ultrasound, CT scan, and bone scan for staging work-ups, 3) principles of combining chemotherapy with radiotherapy, 4) use of contrast material for small bowel delineation during simulation, 5) determination of field margins, and 6) use of CEA and colonoscopy for follow-up evaluations. Conclusion : The items where considerable disagreement was shown among the radiation oncologists seemed to make no serious difference in the treatment outcome, but a practical and reasonable consensus should be reached by the committee, with logical processes of agreement. These items can be used for a basic database for the Patterns of Care Study, which will survey the practical radiotherapy patterns for rectal cancer in Korea.

        순차 주소 접근 ROM의 효율적인 설계 방법

        ROM은 디지털 시스템에서 전력 소모가 크고 속도의 병목현상을 갖는 블록이다. 점증적인 시스템의 고속화에 따라 ROM 설계시 전력소모 감소와 동작 속도 향상이 요구 된다. FFT 및 FIR 필터에 적용되는 ROM은 주소를 순차적으로 접근하는 방식의 ROM이 필요하며, 본 논문에서는 순차적으로 주소를 접근하는 ROM을 설계할 때 기존의 ROM과 같은 값을 출력 하면서 저장되는 셀을 줄일 수 있는 방법을 제안하였다. 이러한 방법을 이용하면 비트라인에 연결된 저장 셀 개수가 감소되며 따라서 비트라인의 커패시턴스 값이 감소된다. 비트라인의 커패시턴스 값이 감소하면서 지연시간 및 파워가 감소한다. 논문에서 예제로 사용한 FIR 계수 저장용 ROM의 경우 제안한 알고리즘을 적용하였을 때 저장 셀 '1'이 최대 86.3% 감소함을 알 수 있다. In the digital system, ROM has a large power-consumption and a speed-bottleneck. According to gradual growth of system speed, ROM is demanded to have low-power consumption and high-speed operation design. The ROM adapted in FFT or FIR filter needs method of sequential accessed addressing. We proposed a reduction method for the number of storage cells in this paper. The number of storage cells which is connected with bit-line is reduced by the proposed method so that the capacitance value of bit-line is reduced. In this case, delay time, and power consumption are reduced. Design result of ROM in this paper using the proposed method could reduce up to 86.3% of storage cell '1' compare with conventional method.

        교육대학교 실과교육 프로그램 개발 요구분석

        The purpose of this study was to assess need for practical arts education program development at National Mniversity of education. The population for this study consisted of all the 3th, 4th grade trainee-teachers, all practical arts professors currently being at nationwide National University of Education, all teachers at nationwide elementary school. A random sample of 1,363 cases was chosen for this study. A survey instrument was developed from the review of literature to collect data. The result of this study were as follow: 1) It was proposed that standard plan of practical arts lesson applied at nationwide colleges of education should be developed for an effective teacher education. 2) Choosing contents of practical arts subject, first of all, an emphasis should be given to connection with practical arts of elementary school and relationship with real life. 3) The degree of educational need in practical arts education part was most high among four parts. So these results should be reflected at developing an instructing materials for compulsory course. 4) The degree of educational need in home economics part was relatively high at consumers’ economy education, consumers’ right and consumption environmental problem, relationship with nutrition and health. 5) The degree of educational need in life science part was relatively high at an understanding of agriculture and life engineering, a practice using garden plant, an understanding of agricultural environment preservation. 6) The degree of educational need in life technology part was relatively high at understanding and manipulation of a robot for education, understanding of machine and equipment in life.

        노인 우울증에 영향을 미치는 요인들에 대한 연구

        Objective: The purpose of this study was to investigate the related factors of elderly depression among Korean adults aged 60 years and above. Methods: From the database of Korea National Health and Nutrition Examination Survey conducted in 2018, data of 2,288 adults over age 60 who responded to the questionnaires of health interview were included in this study. We analyzed the data using logistic regression analysis. Results: According to our results, the number of chronic diseases seemed to be associated with depression in elderly people, especially when they have more than 4 chronic diseases. Poorer self-rated health was identified as a significant factor and the odd ratio was the highest among all independent variables in our study. Walking over 10 minutes more than 4 times a week was considered as a protective factor for elderly depression. The association between education level and depression was inconsistent. Variables such as female gender, single status, smoking showed statistically insignificant results. Older age itself was not identified as a risk factor as odd ratios of all groups (over age 65) compared to reference group (age 60-64) were below 1. Conclusion: This study confirms several related factors for elderly depression. Having more than 4 chronic diseases, poorer self-rated health were identified as a significant risk factor for depression. Regular walking was considered as a protective factor for elderly depression. The knowledge of these factors might help to manage risks of elderly depression and devise early intervention approaches.

        Quantum Mechanical Study of van der Waals Complex. Ⅰ.The $H^2$ Dimer Using the DFT and the Multi-Coefficient G2/G3 Methods

        Molecular hydrogen dimer, ($H_2)_2$ is a weakly bound van der Waals complex. The configuration of two hydrogen molecules and the potential well structure of the dimer have been the subjects of various studies among chemists and astrophysicists. In this study, we used DFT, MCG2, and MCG3 methods to determine the structure and energy of the molecular hydrogen dimer. We compared the results with previously reported ab initio method results. The ab initio results were also recalculated for comparison. All optimized geometries obtained from the MP2 and DFT methods are T-shaped. The H-H bond lengths for the dimer are almost the same as those of monomer. The center-to-center distance depeds on the levels of theory and the size of the basis sets. The bond lengths of the $H_2$ molecule from the MCG2 and MCG3 methods are shown to be in excellent agreement with the experimental value. The geometry of optimized dimer is T-shaped, and the well depths for the dimerization potential are very small, being 23 $cm-^1$ and 27 $cm-^1$ at the MCG2 and MCG3 levels, respectively. In general the MP2 level of theory predicts stronger van der Waals than the DFT, and agrees better with the MCG2 and MCG3 theories.

        3/8″유압 퀵 커플러의 피스톤형상을 고려한 최적설계에 관한 연구

        A hydraulic quick coupler is a component used to easily connect or disconnect pipes or hoses that transfer high pressure fluid without leakage in various mechanical devices. In this study, to obtain an optimal design of a 3/8" hydraulic quick coupler, the effect of different shapes of the internal piston on the internal flow characteristics of the coupler was analyzed and evaluated through numerical analysis based on computational fluid dynamics. Subsequently, the optimal shape design of the internal piston was obtained by comparing the flow characteristics results such as velocity distribution, temperature distribution, and the pressure drop of the hydraulic quick coupler.
