MCV와 RDW를 이용한 복합유기용제 취급 근로자의 빈혈 유형에 관한 조사

The hematologic effect by low-concentration and repeated exposure to mixed organic solvent remains obscure. This study was performed to evaluate the hematologic effect by mixed organic solvent exposure, especially on the type of anemia by mean corpuscular volume(MCV) and red blood cell distribution width(RDW). The subject were 64 organic solvent workers(male 4, female 60) and 78 general workers(male 18, female 60) who showed anemic tendency in worker's health examination which carried out by Pusan Paik Hospital from January to December, 1988. The author gained some hematologic findings(hemoglobin, hematocrit, RBC, WBC, MCV, MCHC, platelet count, ESR, RDW) from auto-analysis method by coulter counter S plus IV, and compared the type of anemia by MCV and RDW. In the pilot study for estimating the prevalence of anemia according to the type of b, the prevalence rate was higher in organic solvent workers than in public officials as 10.9% and 4.1% respectively. There were statistical significance in the value of hemoglobin, hematocrit, MCV, MCHC, platelet count, ESR, RDW between the two study groups with more severe anemic tendency in organic solvent workers. The type of anemia in organic solvent workers was microcytic and anisocytic with the mean value of $68.28{\pm}8.3fL$ of MCV and $19.1{\pm}4.0%$ of RDW.

In this study the reactor design procedure and method of solid-solid-gas chemical heat pump system using STELF technology were investigated. For manufacturing IMPEX block which is the kernel of reactor, proper salt pair should be selected, and equilibrium temperature drop and COP should be examined for selected salt pair. Moreover, apparent density, residual porosity, and graphite ratio should be calculated to give minimum block volume and mass, and maximum energy density without causing heat and mass transfer problems. Since heat exchange area can be changed with operating condition, reactor diameter, length, and stainless steel thickness should be decided for desired specifications. These procedure and method were applied to the case study of 6kW cold production and 8 hours storage capacity reactor.

고관절경을 시행하는 집도의사는 관절경을 통해 관찰되는 정상 고관절의 해부학적 구조물에 대한 정확한 지식과 삽입구 위치에 대한 숙지를 통해 환자의 증상과 비정상 관절경 소견을 임상적으로 연관 시키는 것이 중요하며, 적절한 환자의 선택과 고관절경의 술기에 대한 항상성을 유지할 수 있어야 최소 침습적인 술기의 장점을 가지고 있는 고관절경의 만족할 만한 치료 결과를 기대할 수 있다.

To evaluate the effects on health by exposure to mixed organic solvents, the author analyzed some labortory findings of blood (hemoglobin, hematocrit RBC, WBC, SGOT, SGPT) and urine (protein, blood, sugar) that were obtained from 722 workers (male:446, female:276) who worked in plywood, painting and shoes factories in Pusan area from January to December, 1990. The result were as follows ; 1. The mean values of each hematologic findings were all within normal limit but there were significant differences at hematocrit, RBC, WBC, SGOT in male and at WBC, SGPT in female at the comparison of the mean values and the distribution according to the normal criteria by type of work. 2. There were no differences in urinalysis(protein, blood, sugar) by type of work. 3. In stepwise multiple regression, analysis of hematologic findings on age, working duration and type of work, regression coefficients on age and type of work were relatively high in male and female sub1, respectively. As a result of this study, it is considered that the measurement of WBC with morphology is significant in hematologic findings and instead of spot urine and urinstix, 24 hours urine and quantitative analysis is required in urinalysis in screening test for healthy status of workers who deal with the organic solvents.
장애인의 건강관련 삶의 질에 대한 유형화와 영향요인 분석

본 연구는 장애인의 건강관련 삶의 질이 어떤 형태로 유형화되는지 규명하고, 각 집단 유형의 특성을 파악하여 장애인의 건강관련 삶의 질을 증진하기 위한 실증적 기초자료를 제공하기 위해 수행되었다. 이를 위해 한국보건사회연구원과 국민건강보험공단의 한국의료패널에서 장애인 863명을 대상으로 이들의 건강관련 삶의 질 유형화 분석을 실시하였다. 장애인의 건강관련 삶의 질에 대한 유형을 확인하기 위해 잠재계층분석(Latent Class Analysis)을 사용하였으며 분석 결과, 장애인의 건강관련 삶의 질은 전반적 심각형, 통증관련 심각형, 전반적 안정형으로 분류되었다. 도출된 각 건강관련 삶의 질 유형별 특성과 유형화 결정요인을 확인하기 위해 X2검증, 다항로지스틱 회귀분석을 적용한 결과 세 유형 모두 사회경제적, 장애 및 건강행태, 심리적 특성변인별 차이점을 보였다. 연구결과를 토대로 장애인의 건강관련 삶의 질 유형에 관한 실천적 함의와 장애인의 건강관련 삶의 질 증진을 위한 실증적 개입방안에 대해 논의하였다. The purpose of this study is to define types of the disabled’ quality of health life, to find the characteristics of each different group, and thereby to provide the practical basic material for improving their quality of health life. To achieve that, this study analyzed types of the quality of health life that 863 disabled persons had in the Korea Health Panel of National Health Insurance Corporation and Korea Institute for Health and Social Affairs. To find types of the disabled’s quality of health life, this study utilized Latent Class Analysis. As a result, their quality of life was classified into general severeness type, pain severeness type, and general stability type. To find characteristics of each drawn type and determinants of each type, X2 test and multinominal logistic regression analysis was applied. As a result, these three types had differences in terms of social and economic, disability and health pattern, and psychological characteristic variables. Based on the study results, practical implication of types of the disabled’s quality of health life and an empirical intervention plan for the improvement their quality of health life were discussed.
