        Inguinal hernia is the most common disease treated by the pediatric surgeon. There are several controversial aspects of management 1)the optimal timing of surgical repair, especially for preterm babies, 2)contralateral groin exploration during repair of a clinically unilateral hernia, 3)use of laparoscope in contralateral groin exploration, 4)timing of surgical repair of cord hydrocele, 5)perioperative pain control, 6)perioperative management of anemia. In this survey, we attempted to determine the approach of members of KAPS to these aspects of hernia treatment. A questionnaire by e-mail or FAX was sent to all members. The content of the questionnaire were adapted from the "American Academy of Pediatrics (AAP) Section on Surgery hernia survey revisited (J Pediatr Surg 40, 1009-1014, 2005)". For full-term male baby, most surgeons (85.7 %) perform an elective operation as soon as diagnosis was made. For reducible hernia found in ex-preterm infants already discharged from the neonatal intensive care unit (NICU), 76.2 % of surgeons performed an elective repair under general anesthesia (85.8 %). 42.9 % of the surgeons performed the repair just before discharge. For same-day surgery for the ex-premature baby, the opinion was evenly divided. For an inguinal hernia with a contralateral undescended testis in a preterm baby, 61.9 % of surgeons choose to 'wait and see' until 12 month of age. The most important consideration in deciding the timing of surgery of inguinal hernia in preterm baby was the existence of bronchopulmonary dysplasia (82.4 %), episode of apnea/bradycardia on home monitoring (70.6 %). Most surgeons do not explore the contralateral groin during unilateral hernia repair. Laparoscope has not been tried. Most surgeons do not give perioperative analgesics or blood transfusion.

        Fatigue tests and analyses were carried out to investigate fatigue strength and crack initiation point of load-carrying asymmetric double bevel cruciform welded joints. Mesh-insensitive structural stress approach was adopted to estimate high precise fatigue life and crack initiation point. Two different case material and weld shape were considered in this study. Results of fatigue tests and analyses were compared and discussed in consideration of applicability of structural stress approach as the reliable fatigue assessment method of cruciform welded joints.

        Synthesis and Application of the Novel Azomethine Metal Complexes for the Organic Electroluminescent Devices

        New azomethine metal complexes were synthesized systematically and characterized. Beryllium, magnesium, or zinc ions were used as a central metal cation and aromatic azomethines (L1-L4) were employed as a chelating anionic ligand. Emission peaks o f the complexes in both solution and solid states were observed mostly at the region of 400-500 nm in the luminescence spectra, where blue light was emitted. Three of them (BeL1 (Ⅰ), ZnL2 (Ⅱ), and ZnL3 (Ⅲ)) were sublimable and thus were applied to the organic light-emitting devices (OLED) as an emitting layer, respectively. The device including the emitting layer of Ⅰ exhibited white emission with the broad luminescence spectral range. The device with the emitting layer of Ⅱ showed blue luminescence with the maximum emission peak at 460 nm. Their ionization potentials, electron affinities, and electrochemical band gaps were investigated with cyclic voltammetry. The electrochemical gaps of 2.98 for I, 2.70 for Ⅱ, and 2.63 eV for Ⅲ were found to be consistent with their respective optical band gaps of 3.01, 2.95 and 2.61 eV within an experimental error. The structure of OLED manufactured in this study reveals that these complexes can work as electron transporting materials as well.

        Superior mesenteric artery (SMA) syndrome is a rare disorder caused by extrinsic compression of the third portion of the duodenum by the SMA. The operative treatment of choice is bypassing the obstructed duodenal segment by duodenojejunostomy. We report one case of SMA syndrome treated by laparoscopic duodenojejunostomy and followed up by 3D-reconstructive CT scan. A fifteen-year-old boy with intermittent vomiting and weight loss was admitted. Ultrasonography showed narrowing of the distance between the SMA and aorta. Hypotonic duodenography showed dilatation of duodenal third portion and barium stasis. On 3Dreconstructive CT scan, the angle between SMA and aorta was $37^{\circ}$. The postoperative course was uneventful. Three months later, he had gained 3 kg of weight and the angle between SMA and aorta increased to $38-39^{\circ}$ on 3D reconstructive CT scan. Laparoscopic duodenojejunostomy for bypassing the obstructive duodenum in SMA syndrome is a feasible and safe method.

      • 잔존 총배설강 기형 수술 후 발생한 요도-질 누공에 대한 전방 시상 경직장적 접근 술식 (Anterior Sagittal Transrectal Approach, ASTRA) 1 례보고

        The authors applied anterior sagittal transrectal apporach (ASTRA) for the repair of urethrovaginal fistula which developed after total repair of persistent cloaca. The patient had been diagnosed to have persistent cloaca, double uterus and double vagina, and received PSARP, excision of right-side uterus and vagina, and left vaginal switch operation at 22 months old. After operation, the patient admitted several times due to frequent urinary tract infection and ectopic stone formation in bladder and neovagina. Urethro-neovaginal fistula was confirmed by cystoscopy and corrected with ASTRA. Postoperative voiding cystourethrogram showed no fistula tract. ASTRA showed improved surgical field, minimized ureterocystic damage, and preserved perirectal nerve due to limited incision of rectum.

        TPMS 양방향 통신을 위한 유효 데이터 판정기법

        TPMS(Tire Pressure Monitoring System)는 차량에서 사용되는 무선통신기기의 일종으로 운전자가 타이어의 상태를 보다 효율적으로 파악하고 관리하기위한 안전보조 시스템으로 정의할 수 있다. TPMS는 무선통신을 사용하므로 데이터 송/수신 시 각종 잡음이나 간섭에 의한 통신장애가 발생하게 된다. 운전자의 안전과 직결되는 타이어 사고를 사전에 예방하고 무선통신으로 인한 데이터 전송 시 보다 정확한 데이터 신뢰성 확보를 위해 본 논문에서는 신호처리수신부에서 수신신호의 SINR(signal-to-interference and noise ratio, 신호 대 간섭 및 잡음 비)을 측정하여 데이터 신뢰도를 판정하는 신뢰도 판정기법을 제안한다. 또한, 이를 수행하기 위한 효과적인 데이터 신뢰성 판정 알고리즘 구조를 제안한다. 본 논문에서 제안된 데이터 신뢰성 판정기법은 TPMS 뿐만 아니라 센서를 이용한 양방향통신 시스템에서 폭넓게 사용될 수 있을 것으로 판단되다. 컴퓨터 시뮬레이션을 통해 95%, 97%, 99%의 데이터 신뢰도를 만족하는 임계 SINR 값을 확인한다. TPMS(Tire Pressure Monitoring System) using the wireless communication technique is defined as the safety aid system to efficiently realize and manage the condition of tires in the vehicle. The wireless communication system of TPMS should suffers from various noise and interferences such as signals of each tire sensor or outside electrical equipments. In order to retain the data reliability of TPMS, we propose an assessment method of the data reliability based on signal-to-interference and noise ratio (SINR) of the received signal. The proposed technique can be widely applied to wireless duplex communication systems based on various sensors. We verify critical SINR values to satisfy data reliabilities of 95%, 97%, and 99% through computer simulation.

        미취학 아동 어머니의 양육특성과 건강 특성이 경제활동 참가에 미치는 융합적 요인 분석

        본 연구는 제7차 여성 가족 패널 조사자료를 2차 분석하여, 직장군과 비직장군의 양육 특성과 건강 특성을 비교하기 위한 서술적 조사 연구이다. 만19세 이상 64세 이하의 여성 중에서 미취학 아동 어머니를 대상 총 697명을 대상으로 교차분석과 로지스틱 회귀분석을 실시하였다. 아이돌보미가 있는 경우, 이용하는 보육 시설개수가 많을수록, 시부모나 친정부모 돌봄을 받는 경우, 신체활동을 주 3회 미만으로 하는 경우가 경제활동을 많이 하였다. 반면, 미취학 자녀 수가 많을수록 경제활동을 안 하는 것으로 나타났다. 미취학 아동 어머니의 신체활동 확보와 일과 가정 양립을 뒷받침할 수 있는 양육 친화적 가족 및 직장문화 조성이 필요하다. This study is a second analysis of the 7th Korean Longitudinal Survey of Women and Families. And, it is a descriptive survey research for comparing parental and health characteristics of 2 groups of workers and non-workers. For this, it has used crossover analysis and logistic regression analysis with the subjects of mothers of preschoolers - of 697 females in total, aged between 19 and 64. Based on the result of the logistic regression analysis, we have discovered a few factors inducing the females to participate in more economic activities. The factors are as follows : baby sitters available, the more number of child care centers, the more helps of baby sitting from the females' parents and in-law parents and physical activities less than 3 times a week. In contrast, however, the more preschoolers they have, the less likely they are to take an active part in the economy. Against the backdrop, the government should develop social service systems to encourage various child care services and to foster 'child raising mothers-friendly' environments not only in families but in workplaces. All this is to improve the health characteristics of preschoolers' mothers in a more healthy way' and to strike a balance between work and family.

        간섭제거를 위한 TPMS 빔형성기들의 복잡도 비교

        타이어 파손으로 인한 사고를 사전에 방지하기 위해 개발된 TPMS(Tire Pressure Monitoring System)는 타이어에 장착된 센서부로부터 전송된 데이터를 기반으로 타이어의 이상 유무를 디스플레이 장치에 표시하고 운전자가 실시간으로 타이어의 온도, 압력 등의 정보를 수시로 파악하여 최적의 주행조건을 유지시켜주기 위한 안전보조시스템이다. 무선통신을 사용하는 TPMS는 측정된 데이터의 송수신 시 간섭이나 잡음으로 인해 데이터 전송 시 수신 오류가 발생할 수 있다. 이러한 문제점을 해결하기 위해 스위칭(Switching), MVDR(Minimum-Variance Distortionless-Response), GSC(Generalized Sidelobe Canceler) 빔형성기(Beamformer) 등과 같은 간섭제거에 효율적인 대표적인 빔형성 기술들을 사용한다. 본 논문에서는 이 세 가지 빔형성기의 복잡도를 계산하여, 복잡도 성능 비교에 대한 수학적 근거를 제시한다. 컴퓨터 시뮬레이션을 통하여 본 논문에서 고려된 세 가지 빔형성기의 복잡도 성능을 확인한다. TPMS (Tire Pressure Monitoring System) is a safety assistant system to prevent the serious accident due to the damaged tire by the abnormal tire pressure. It is designed to transmit the measured data for pressure and temperature of tires from the sensor unit installed in each tire to signal processing unit installed in a vehicle. Based on the received information, a driver monitors the condition of tires using a display device, to maintain the optimum travelling condition. Since TPMS should employ the wireless communication technique, it may suffer from various interferences from external electrical or electronics devices. In order to suppress them, the beamforming techniques such as switching, minimum-variance distortionless-response (MVDR), and generalized sidelobe canceler (GSC) have been considered for TPMS. In this paper, we calculate computational complexities of three beamformers and suggest mathematical basis to compare their performance of the complexity.

      • 소아의 췌장의 고형성 가유두상 종양

        Solid pseudopapillary tumor (SPT) of the pancreas occurs most frequently in the second or third decades of life, and is prevalent in females. Unlike other pancreatic malignancy, SPT usually has a low malignancy potential. This study reviews our clinical experience and surgical treatment of pancreatic SPT. Admission records and follow-up data were analyzed retrospectively for the period between January 1996 and January 2003. Five patients with a pancreatic mass were operated upon and SPT was confirmed by pathology in each case. The male to female ratio was 1: 4. The median age was 13.8 years. Findings were vague upper abdominal pain (n=5, 100 %) and an abdominal palpable mass (n=3, 60 %). The median tumor diameter was 6.8cm and the locations were 2 in the pancreatic head (40 %) and 3 in the pancreatic tail (60 %). Extra-pancreatic invasion or distant metastasis was not found at the initial operation in all five cases. A pyloruspreserving pancreaticoduodenectomy (n=1) and a mass enucleation (n=1) were performed in two patients of pancreatic head tumors. For three cases of tumors in pancreatic tail, distal pancreatectomy (n=2) and combined distal pancreatectomy and splenectomy (n=1) were performed. The median follow-up period was 60 months(12-117month). During the follow-up period, there was no local recurrence, nor distant metastasis. Postoperative adjuvant chemotherapy or radiotherapy was not carried out. All five children were alive during the follow up period without any evidence of disease relapse. SPT of the pancreas in childhood has good prognosis and surgical resection of the tumor is usually curative.
