        임상검체에서 분리된 그람 음성 세균들에 대한 Isepamicin의 시험관내 항균력

        Background : The increasing incidence of multidrug-resistant gram-negative bacteria causing nosocomial infections is an important clinical problem. Isepamicin is a recently developed aminoglycoside which has been known to have potent activity against gram-negative organisms. We evaluated the in vitro activities of isepamicin and other aminoglycosides against a large number of gram-negative organisms. Materials and Methods : We tested the in vitro antimicrobial activities of isepamicin, amikacin, gentamicin, and tobramycin against 566 gram-negative organisms collected between January 2006 and June 2006 in Asan Medical Center. Minimal inhibitory concentrations (MICs) were determined and interpreted according to the recommendations of Clinical and Laboratory Standard Institute (CLSI). The breakpoint MIC used for interpretation of isepamicin was MIC≤16 ㎍/mL as susceptible, 32 ㎍/mL as intermediate, and 64 ㎍/mL as resistant. Results : The MIC_(50)/MIC_(90) of isepamicln for Escherichia coli, Klebsiella pneumon/ae, Pseudomonas aeruginosa, Acinetobacter baumannii, and Enterobacter cloacae were 1/2, 0.5/>128, 4/16, 16/>128, and 1/2 ㎍/mL, respectively. The susceptibilities for E. coli, K. pneumoniae, P. aeruginosa, A. baumannii, and E. cloacae were 100%, 86.4%, 89.7%, 50.0%, and 96.6%, respectively. For E. coli, K. pneumoniae, P. aeruginosa, and E. cloacae, isepamicln had better in vitro activities than gentamicin and tobromycin, and had similar activities with amlkacin. For A. baumanii, all four tested aminoglycosides had similar in vitro activities. Conclusion : Isepamicin had excellent in vitro activities against gram-negative organisms, except A. baumanii. The overall in vitro activities were similar with amikacin. 목적 : 그람 음성 세균은 다양한 지역사회 및 병원 획득 감염의 원인균이다. 최근에는 병원 감염을 일으키는 그람 음성 세균에서 항균제 내성균의 출현과 확산이 큰 문제가 되고 있다. Isepamicin은 최근에 소개된 gentamicin B의 유도체로 외국에서의 연구에 의하면 그람 음성 세균들에 대한 항균력이 우수함이 보고 된 바 있다. 본 연구에서는 우리나라의 임상 균주들을 대상으로 그람 음성세균에서 isepamicin의 시험관내 항균력을 기존의 아미노배당체 제재들과 비교하여 평가하고자 하였다. 재료 및 방법 : 2006년 1월에서 6월 사이에 서울아산병원 환자에서 분리된 Escherichia coli 113 균주, Klebsiella pneumoniae 110 균주, Pseudomonas aeruginosa 117 균주, Acinetobacter baumannii 108 균주, 그리고 Enterobacter cloacae 118 균주를 대상으로 isepaimcin, amikacin, gentamicin, tobramycin에 대한 최소억제농도(minimal inhibitory concentration, MIC)를 한천 희석법(Mueller-Hinton Agar)을 이용하여 측정하였다. 감수성여부는 CLSI 기준에 따랐으며, isepamicin은 MIC≤16㎍/mL를 감수성, MIC=32 ㎍/mL를 중등도 내성, MIC≥64 ㎍/mL를 내성으로 판정하였다. 결과 : Isepamicm의 E. coli, K. pneumoniae, P. aeruginosa, A baumannii, E. cloacae에 대한 MIC_(50)/MIC_(90)은 각각 1/2, 0.5/>128, 4/16, 16/>128, 1/2 ㎍/mL였고, 항균제 감수성률은 차례대로 100%, 86.4%, 89.7%, 50.0%, 96.6%였다. 기존 아미노배당체 제재들과 비교하면 E. coli, K. pneumoniae, P. aeruginosa, E. cloacae에서는 amikacin과 비슷한 감수성률을 보였고, gentamicin과 tobramycin보다 높은 감수성률을 보였다. A baumannii의 경우 isepamicin의 감수성률이 50.0%로 비교적 낮았지만 amikacin, gentamicin, tobramycin도 각각 50.0%, 46.3%, 47.2%의 감수성률을 나타내어 4가지 항균제가 모두 비슷한 항균력을 보였다. 결론 : Isepamicin은 그람 음성 세균들에 있어 기존의 아미노배당체 제재들과 비교했을 때 amikacin과는 동등한 시험관내 항균력을 보이고 gentamicin, tobramycin보다 뛰어난 시험관내 항균력을 보여 사용이 유망한 아미노배당체 중 하나라 생각된다.

        BNT-ST 세라믹스의 저온 소결과 강유전 및 압전 특성

        0.75(Bi1/2Na1/2)TiO3-0.25SrTiO3 (BNT-25ST) ceramics with high densities were successfully prepared at a sintering temperature of 1,000℃ by adding a mixture of 1 mol% CuO and 0.5 mol% Na2CO3 or 0.5 mol% CuO and 0.25 mol% Na2CO3. Double polarization-electric field (P-E) hysteresis curves and sprout-shaped bipolar strain-electric field (S-E) hysteresis curves with small negative strains were observed in the pristine and CuO-added BNT-25ST ceramics whereas the Na2CO3-added sample showed similar P-E and S-E curves to a typical ferroelectric. The pristine BNT-25ST ceramics showed an extremely large strain and a large-signal piezoelectric strain constant (d33*): 0.287 % at 80 kV/cm and 850 pm/V at 20 kV/cm. Similar values, 0.248 % at 80 kV/cm and 655 pm/V at 20 kV/cm, were obtained in the CuO-added sample. However, the pristine and CuO-added samples showed large hysteresis in unipolar S-E curves at an electric field of less than 20 kV/cm. The Na2CO3-added sample showed smaller values of the strain and d33* but displayed a linear change and small hysteresis in the unipolar S-E curve. The co-added sample with CuO and Na2CO3 displayed intermediate P-E and S-E hysteresis curves.

        Toxocariasis: A Rare Cause of Multiple Cerebral Infarction

        Toxocariasis is a parasitic infection caused by the roundworms Toxocara canis or Toxocara cati, mostly due to accidental ingestion of embryonated eggs. Clinical manifestations vary and are classified as visceral larva migrans or ocular larva migrans according to the organs affected. Central nervous system involvement is an unusual complication. Here, we report a case of multiple cerebral infarction and concurrent multi-organ involvement due to T. canis infestation of a previous healthy 39-yearold male who was admitted for right leg weakness. After treatment with albendazole, the patient’s clinical and laboratory results improved markedly.

        안견의 몽유도원도(夢遊挑源圖)에 표현된 구도와 공간개념의 창신(創新)에 관한 연구

        Before the Joseon Dynasty era, common landscape paintings drawn with in black and white were hugely affected from landscape paintings from Buk-song, China. Because of that, its forms and contents of the landscape paintings before the Joseon Dynasty period were very similar with Chinese forms of landscape paintings. However, An-Kyon's style created a unique landscape painting style from those in traditional forms of painting, which means that he created a new pattern of landscape painting that is suited in our culture. Especially, An-Kyon's style of landscape painting showed unique form in composition. Before An-Kyon's style of painting, all of the landscape paintings appeared as two-dimensional. However, An-Kyon shedded those old way of paintings but tried to draw landscape with using diagonal composition. In this way, An-Kyon could draw landscape paintings in three dimensions. Through An-Kyon's style of painting, we can estimate position of new landscape painting pattern, which means creations of new style of painting is related to existing form. Especially, An-Kyon's painting style was affected from Buk-song, Nam-song, Kum, and Won-duck, which means that he learned from each style of painting what he needed to learn. Because An-Kyon made his style of painting as he learned merits from each painting style, it can be said that An-Kyon's painting style is reinvention of landscape painting. Since An-Kyon's painting style appeared, the pattern of the landscape drawing changed in many times. Like this, An-Kyon's painting form was unique and creative and led to many new patterns of landscape drawing as it was existed as revolutionary change in landscape painting's history. Especially, we can see that he was an artist who was always looking for a new way to draw paintings to express that era, so he could create a new form of painting of landscape.

        Candida albicans 에 의한 칸디다 안구내염에 병발한 감염성 흉부 대동맥류 1예

        Candida species cause various invasive fungal diseases, including candidemia, endocarditis, endophthalmitis, peritonitis, osteomyelitis and arthritis, but infected (mycotic) aortic aneurysms caused by Candida species are very rare. So, we report a case of infected thoracic aortic aneurysm concurrent with endophthalmitis by Candida albicans in a 42-year-old man. The patient initially was diagnosed with candidial endophthalmitis and hospitalized for vitrectomy. On admission, he had chest CT taken and infected thoracic aortic aneurysm was detected. He treated with antifungal agent and resection and patch repair of aortic aneurysm. Two months later, a new aneurysm on the patch repair site was detected and thoracic endovascular aortic repair (TEVAR) was performed. After TEVAR and long-term antifungal therapy, his infected aortic aneurysm has been successfully treated.

        < 몽유도원도 >의 심미(審美)와 창신(創新)에 관한 도교 미학적 고찰

        본 논문은 조선 초기 안견의 <몽유도원도>에 내재된 유가, 도가의 심미(審美)의식의 창신(暢神)에 의해 구도의 재창조와 동시에 무한 초월된 우주공간을 재현하여 공간개념에서 양식창신(創新)한 도가 미학적 관점을 고찰한다. <몽유도원도>는 도가 미학적 관점의 허와 무의 관념수용에 의해 유가의 삼원 관념의 해체와 탈피, 그리고 장자미학의 소요유에 의한 무한 초월된 우주공간의 확대에 의해 구도와 공간개념에서 양식 창신된 것이다. 현실과 이상세계, 도원입구, 도원의 네무더기의 경물들은 흩어져 마치 분리된 듯, 서로 시각적 조화와 이동 시점의 변화를 이루어 대각선 또는 사선구도와 한국적 심미특색이 있는 넓고 시원하게 확대된 공간개념을 강조한다. 이것은 <몽유도원도>가 장자의 심미 관념인 허정과 자연무위정신, 그리고 심재와 좌망을 통해 유의 자유로운 정신과 정신수양을 추구하였다는 사실을 드러낸다. 이것은 또한 도가 미학적 요소인 허와 실과 유와 무의 대비, 그리고 무한과 무궁개념이 구도와 공간개념의 재창조에도 결정적으로 기여한 것이라는 사실 역시 표출한 것이다. 본 연구는 <몽유도원도>가 도가의 이상과 유가의 현실이라는 대립적인 자연관을 배태하고 있음에도 불구하고, 상호조화를 이루고 있다는 사실, 그리고 특히 도가적 무의 심미의식은 구도 뿐 아니라 공간개념의 확대에도 중요한 영향을 주었다는 사실을 고찰한다. <몽유도원도>는 도가적 심미 관점의 수용결과로서 첫째, 장자 미학의 핵심인 허정과 도의 산수 관념을 함축한다. 유가미학의 도덕적 규범으로서의 자연관을 바탕으로 하고 있음에도 불구하고, 유가의 규범에서 벗어난 도가적 도의 체득과 정신수양이 강조된 것이다. 둘째, 노장의 은일과 와유사상, 특히 무위자연의 심미의식을 내포한다. 셋째, 장자미학의 심재와 좌망을 통한 유의 자유정신과 소요유의 정신적 자유의 심미의식과 무궁한 공간관념을 드러낸다. 이러한 요소들은 <몽유도원도>가 도가 미학적 요소의 커다란 영향 하에 완성된 작품이라는 사실의 예시가 된다. The purpose of this study is to examine how aesthetics based on the ideas of ‘ri’, ‘do’ and ‘eui’, which originated from the philosophies of Confucianism, Taoism, Buddhism and the Zen sect, influenced ‘jeonsin’ and ‘euigyeong’ aesthetics found in <Mongyu Dowondo> painted by Ahn Gyeon in the early Joseon period, and artistic backgrounds behind which the influencing factors were formed. < Mongyu Dowondo > has the aesthetic characteristics of ‘jeonsin’ and ‘euigyeong’, respectively based on the idea of ‘ri’ from the circles of yuantihua in the North Song period and that of ‘eui’ from the circles of literary painting. In other words, the traditional landscape painting shows an aesthetic fusion of the ideas of ‘ri’ and ‘do’ from Confucianism, Taoism, Buddhism and the Zen sect. The Confucian idea of ‘ri’ was the starting point of the vision of nature that oriented unity between heaven, earth, nature and man. In the North Song Period, the vision of the universe or universal ontology from neo-Confucianism and the artistic spirit of ‘ri’ fused with each other, forming the aesthetics of ‘jeonsin’. The aesthetics of the Zen-sect, or that of ‘sim’ often implied in literary painting greatly contributed to the aesthetic spirit of ‘saeui’. This study first clarifies the Confucian visions of nature and the universe both of which made the biggest contribution to the aesthetics of < Mongyu Dowondo >. Next, the study considers the aesthetic characteristics of universal ontology from neo-Confucianism of the North Song period, Taoistic vision of nature in the South Song period and artistic spirits reflected in a later variation of that ontology. Furthermore, this study investigates ‘saeui’ aesthetics based on Kwak Hee``s ideas of ‘won’ and ‘ri’, which were the key to aesthetics dominant in the North Song period, and ‘saeui’ aesthetics of literary painting, to which the Zen sect of the Win period contributed. This study conducts a few analyses in the following. Noting that Confucian aesthetics created ethical ontology and the visions of nature and the universe, all of which were based on the idea of unity between heaven, earth, nature and man, and that universal ontology from neo-Confucianism since the period of North Song influenced the idea of enormous space, suggested by the Geobi school of the same period, and then landscape painting, above all, the study analyzes the visions of nature and the universe from Confucianism and neo-Confucianism. By the way, the vision of the universe was changed by the period of South Song when neo-Confucianism of North Song gave way to Taoistic aesthetics whose vision of nature and spirit of art, both of which were based on the idea of the above mentioned unity, removed the idea of enormous space. In addition, the study analyzes Taoism-based aesthetic characteristics and art spirits since contrasts between truth and falsehood and between existence and nonexistence, found in the vision of the universe, had lots of effects on the creation of paintings. Finally, this study investigates the aesthetic characteristics of ‘jeonsin’ and ‘saeui’ since < Mongyu Dowondo > has aspects of ‘euigyeong’ aesthetics, associated with the aesthetic sense of ‘won’ as the key to North Song’s art theory-based aesthetics and has aspects of ‘saeui’ aesthetics in which the idea of ‘eui’ from the theory of literary painting, influenced by the Zen sect-based aesthetics of the Win period, reflected.
