        A Simple Dosing Scheme for Intravenous Busulfan Based on Retrospective Population Pharmacokinetic Analysis in Korean Patients

        Busulfan is an antineoplastic agent with a narrow therapeutic window. A post-hoc population pharmacokinetic analysis of a prospective randomized trial for comparison of four-times daily versus once-daily intravenous busulfan was carried out to search for predictive factors of intravenous busulfan (iBu) pharmacokinetics (PK). In this study the population PK of iBu was characterized to provide suitable dosing recommendations. Patients were randomized to receive iBu, either as 0.8 mg/kg every 6 h or 3.2 mg/kg daily over 4 days prior to hematopoietic stem cell transplantation. In total, 295 busulfan concentrations were analyzed with NONMEM. Actual body weight and sex were significant covariates affecting the PK of iBu. Sixty patients were included in the study (all Korean; 23 women, 37 men; mean [SD] age, 36.5 [10.9] years; weight, 66.5 [11.3] kg). Population estimates for a typical patient weighing 65 kg were: clearance (CL) 7.6 l/h and volume of distribution (Vd) 32.2 l for men and 29.1 L for women. Inter-individual random variabilities of CL and Vd were 16% and 9% . Based on a CL estimate from the final PK model, a simple dosage scheme to achieve the target AUC0-inf 0.5 (defined as median AUC0-inf with a once-daily dosage) of 26.18 mg/lㆍ hr, was proposed: 24.79ㆍ ABW mg q24h, where ABW represents the actual body weight in kilograms. The dosing scheme reduced the unexplained interindividual variabilities of CL and Vd of iBu with ABW being a significant covariate affecting clearance of iBU. W e propose a new simple dosing scheme for iBu based only on ABW .

        기본설계 함정무기체계 요구조건 관리방안 연구

        Unlike other weapons systems, a surface vessel weapon's system derives its Required Operational Capability(ROC) through the conceptual and basic design stages in the process of shipbuilding. In the basic design stage, the design requirements include structural requirements such as vessel construction guidelines, operation requirements, and basic design execution plans that are determined based on the ROC for the development of the vessel. The Defense Agency for Technology and Quality performs quality assurance tasks, such as verification of the satisfaction of requirements and adequacy of the outputs of reports and drawings produced at the basic design stage. However, there is a concern about deterioration in quality due to the absence of detailed task performance procedures for requirement management and tracking. Accordingly, a case study was conducted on an efficient design stage quality management plan in the defense and civilian sectors. Hence, a study on a requirement management plan applicable to a vessel weapon system was carried out. It is expected that effective quality assurance work can be performed in the basic design stage through the application of the requirements management(draft) derived from this study.

        Parameter estimation for sigmoid Emax models in exposure-response relationship

        The purpose of this simulation study is to explore the limitation of the population PK/PD analysisusing data from a clinical study and to help to construct an appropriate PK/PD design that enableprecise and unbiased estimation of both fixed and random PD parameters in PK/PD analysis underdifferent doses and Hill coefficients. Seven escalating doses of virtual drugs with equal potency andefficacy but with five different Hill coefficients were used in simulations of single and multiple dosescenarios with dense sampling design. A total of 70 scenarios with 100 subjects were simulated andestimated 100 times applying 1-compartment PK model and sigmoid Emax model. The bias and precisionof the parameter estimates in each scenario were assessed using relative bias and relative rootmean square error. For the single dose scenarios, most PD parameters of sigmoid Emax model wereaccurately and precisely estimated when the Cmax was more than 85% of EC50, except for typicalvalue and inter-individual variability of EC50 which were poorly estimated at low Hill coefficients. For the multiple dose studies, the parameter estimation performance was not good. This simulationstudy demonstrated the effect of the relative range of sampled concentrations to EC50 and sigmoidicityon the parameter estimation performance using dense sampling design.

        In this study, the standard(draft) was prepared through the analysis of the trial evaluation report for the vessels in which the trial operation was performed in the past. Currently, there is a problem with different evaluation procedures and standards as the trial evaluation report is prepared, confirmed and modified for each project without a standardized evaluation. In addition, a number of different evaluation criteria were identified even in similar vessels. Accordingly, the standard(draft) was derived by identifying the contents that require standardization of the test driving evaluation. The effectiveness of applying to the standard draft was analyzed by identifying problems and improvement plans in the existing test operation evaluation report preparation process. As a result of the study, by reducing inefficient work, man-hour reduction and work efficiency increase could be expected, and unnecessary administrative requirements were greatly reduced. Based on the results of this study, it was expected that the procedure of the test operation evaluation report currently in use could be improved and a more objective evaluation could be performed through the application of the standard.

        김은성 분단희곡 연구

        본고는 김은성의 희곡<연변엄마>(2011),<목란언니>(2012),<썬샤인의 전사들>(2016)에 나타나는 분단희곡의 양상을 고찰한 것이다. 구체적인 연구 목표는 두 가지이다.첫째,분단을 바라보는 작가의 시각에 나타난특징적인 지점을 확인하는 것이다.둘째,대상 작품들에 나타나는 극적 형상화가 지닌특징들을 살펴보는 것이다. 논의 결과,이들 작품에 나타나는 공통된 주제의식은 첫째,참혹한 이 땅의 현실과이런 현실의 천민성에 스스로를 가두고 동일화되어가는 인간성의 상실을 비판적으로드러내고자 한다는 점을 확인할 수 있었다.둘째,이런 현실에 맞서 현실과 역사를 넘어서기 위해 필요한 독자/관객의 ‘성찰과 인식적 전환 및 결행의 촉구’라는 점을 확인하였다. 또 이런 주제의식을 극적으로 형상화하기 위한 몇 가지 특징적인 지점들을 보여주고 있다는 점도 확인할 수 있었다.첫째,등장인물들은 일정한 희극성을 드러내고 있다는 점과 함께 그의 희극성을 유발하는 지점들에서 특징적인 지점을 발견할 수 있다는 점을 논증하였다.둘째,서사구도에서 나타나는 특징으로 서사의 시공간이 뒤섞이며,나아가 현실과 초현실이 뒤섞이는 독특한 서사구도와 전개를 보여준다는 점이었다.이런 그의 극양식적 지점들은 양식상의 문제에서 그의 작품들이 거둔 대중적인 성공의 근거가 된다고 생각했다.셋째,오브제가 끌어가는 서사의 힘이다.특히 손수건/ 망치/수첩/거울 등의 오브제들은 각각 독특한 의미를 지니며 일정한 극적 역할을 수행하고 있다는 점도 확인하였다. 그의 이야기들이 ‘안타까움과 연민’이라는 지점에만 머물러 있다는 비판적 견해도있다.하지만,본고에서는 일부 그런 지적들을 수용하면서도 가능하면 분단극복의 가능성을 찾아 현실의 경계를 넘고자 하는 주체의 성찰과 실천을 전경화함으로써 해결의 실마리를 제공하려 했다는 점을 긍정적으로 보고자 하였다.

        Eczematous Vesicular Rruption and Exacerbation of Chronic Spontaneous Urticaria Associated with Secukinumab

        The pathogenesis of chronic spontaneous urticaria (CSU) is complex. Though the involvement of functionalautoantibodies and imbalance of T helper (Th) cell subsets have been reported, the exact mechanism remains elusive. We report a case of eczematous vesicular eruption and exacerbation of CSU in a patient with psoriasis after treatment with secukinumab, an anti-interleukin (IL)-17 monoclonal antibody. The eczematoid eruption subsided after theadministration of systemic steroids, but the exacerbation of CSU was not controlled. After switching the therapy toguselkumab, an anti-IL-23 monoclonal antibody, to supplement the control of the psoriasis lesions, the patient’surticaria flare-ups were well controlled with only H1 antihistamines. We hypothesize that the Th2-skewed immuneresponse induced by anti-IL-17 treatment in predisposed patients might have resulted in the eczematous eruption andexacerbation of CSU.
