A Multi-Center Educational Research Regarding Breastfeeding for Pediatrics Residents in Korea

Purpose: Pediatricians have a significant responsibility to educate mothers about the importance of breastfeeding. However, there have been minimal efforts in the courses of resident training in Korea. The purpose of this study is to evaluate the change in knowledge and attitude before and after a 4-week breastfeeding educational intervention among multicenter residents. Methods: Prospective interventional educational research was designed for residents at eight training hospitals in Korea. Institutional reviews were obtained in each hospital. The education curriculum consisted of 14 courses regarding breastfeeding theory and practice. These materials were used to teach pediatric residents for 4 weeks. Knowledge-based tests were administered before the course, and re-tests were administered after the course using different test items of similar levels. Test scores and survey responses were compared before and after the intervention. Results: A total of 73 residents (1st year 20, 2nd year 23, 3rd year 16, and 4th year residents 14) from eight training hospitals completed the intervention. Their average age was 30.3±2.9 years, 17 (23.3%) were male, 22 (30.1%) were married, and eight had more than one child of their own. The mean pre-test score was 61.8±13.4 and the mean post-test score was 78.3±7.5 (P<0.001). The inter-grade difference in the score was significant in the pre-test (P=0.005), but not significant in the post-test (P=0.155). There were more responses of obtaining confidence after the intervention (P<0.001). Conclusion: In our study, pediatric residents showed improvement in their knowledge and confidence level after 4 weeks of the breastfeeding curriculum. Comparison of outcomes according to the operation for type A esophageal atresia

Purpose: The purpose was to evaluate outcomes according to different operative strategies of type A esophageal atresia (EA). Methods: All patients who underwent surgery for type A EA between 1980 and 2011 were included. Patients were divided into 2 groups: E-E group included patients who received esophageal end-to-end anastomosis, whereas E-G group included patients who received esophago-gastric tube anastomosis. Results: Twenty-two patients were included. The median gestational age was 37.5 weeks. The median birth weight was 2.5 kg. Twenty-one patients underwent gastrostomy as initial procedures, and one patient underwent primary esophageal end-to-end anastomosis. The median gap between both esophageal ends was six vertebral distance (VD). Seven patients underwent primary anastomosis of the esophagus, and 14 patients underwent gastric replacement. Three patients (13.6%) had anastomotic leakage and 10 patients (45.5%) had anastomotic stenosis. Most of the patients (90.9%) had gastroesophageal reflux, but only two patients required antireflux surgery. The median VD was significantly shorter in E-E group than in E-G group (3 VD vs. 6 VD). Stenosis was significantly more often in E-E group, but there was no significant difference in leakage and reflux symptoms. Conclusion: The treatment for type A EA can include E-E anastomosis or E-G anastomosis, depending on the length of the end-to-end interval after performing gastrostomy. Appropriate tension and blood flow in the anastomosis site are essential for preventing postoperative stenosis and leakage, and esophageal replacement with gastric tube is believed to be feasible and safe in cases where excessive tension is present.

Previous phytochemical and pharmacological studies of the fruit of Sageretia theezans (Rhamnaceae) led to the isolation of a small number of anthocyanins and it having antioxidant activity. However, to date, no more study has been carried out to investigate the secondary metabolites from the fruit of S. theezans. In this phytochemical research of S. theezans fruit led to the isolation of a new flavone glycoside, sageretizanin (1) together with four quercetin derivatives (2-5), five kaempferol derivatives (6-10), two myricetin derivatives (11 and 12), and four flavanones (13-16). The structures of the isolated compounds were identified on the basis of their NMR spectroscopic (1H, 13C, DEPT, COSY, HSQC, HMBC, and ESI-MS) data and comparison with literature data. To the best of our knowledge, compounds 7 and 13-16 were obtained from the genus Sageretia for the first time. Moreover, compounds 7 and 13 has not been reported in the family of Rhamnaceae. Among them, (-)-aromadendrin-7-O-glucoside (7) was isolated for the second time from natural sources.
Unprecedented Noncanonical Features in the WS9326A Nonribosomal Peptide Synthetase Assembly Line

A growing number of nonribosomal peptide synthetase (NRPS) assembly lines violating the “co-linearity rule,” where the number and order of NRPS modules indicate to the number and sequence of amino acids in the corresponding product, have been discovered. To understand biosynthetic mechanism of such NRP, NRPS of Streptomyces sp. SNM55 was investigated. Systematic inactivation of NRPS domains and translocation of the thioesterase (TE) domains unveiled several unprecedented nonlinear NRPS assembly processes during the biosynthesis of the cyclodepsipeptide WS9326A in S. sp. SNM55. The WS9326 NRPS assembly line involves two type II TE (TEII)-like enzymes responsible for the transfer of activated amino acids from two stand-alone adenylation (A)-thiolation (T) didomain modules with different specificities to the ‘A-less’ condensation (C)-T module, which operates iteratively, catalyzing two chain elongation steps. This biosynthetic pathway includes the first example of NRPS module skipping, where the interpolated C and T domains are required for chain transfer. Understanding such untapped nonlinear biosynthetic strategies will provide potentially valuable tools for engineered biosynthesis of these complex natural products.
Antioxidant and Anti-Skin Aging Effects of Rutin Treated by Sub- and Super Critical Water

This study was conducted to evaluate the antioxidant and anti-skin aging effects of rutin treated by sub- and super-critical water (SW). The ABTS and DPPH radical scavenging activities of the SW treated rutin were increased to 541.86±33.65 and 728.73±1.34 mg AAE/g at 300°C in 100 bar but decreased to 500.78±12.08 and 694.88±4.69 mg AAE/g at 400°C in 250 bar, respectively. Tyrosinase, elastase and collagenase inhibitory activity were the highest value of 57.76±2.28%, 79.65±0.78% and 79.24% at 300°C in 100 bar, respectively but decreased above condition. Astringent activity was highest value of 57.53±2.17% at 300°C in 100 bar. These results was confirm the high antioxidant activities and anti-aging effect of rutin treated with SW and the SW treatment process can be used as a functional compounds development.
Relationship of serum lymphocyte with outcomes in chronic obstructive pulmonary disease

Background: The role of lymphocyte in the patients with chronic obstructive pulmonary disease (COPD) remains unclear. The purpose of this study was to evaluate the effect of the serum lymphocyte on several clinical parameters, such as lung function, exercise capacity, QOL, exacerbation and mortality in a Korean COPD cohort by repeated and simultaneous measurement of the serum lymphocyte and these clinical parameters. Methods: The study population comprised 451 patients with COPD from the KOLD cohort. Serum lymphocyte percentage (%) were measured every year along with various clinical parameters such as lung function, 6-min walking (6MW) distance, quality of life (CAT and SGRQ score), exacerbations and survival. Serum lymphocyte percentage less than 20% is considered as low lymphocyte. Follow-up data up to 9 years was used for analysis. Results: Among the 451 patients, 409 (90.7%) and 42 (9.3%) were the normal lymphocyte group and low lymphocyte groups at the time of enrollment, respectively. Clustered analysis showed that low lymphocyte group showed lower post-bronchodilator FEV1 (estimated mean=-5.70 % predicted; P=0.001), lower FVC (estimated mean=-5.63 % predicted; P=0.005), shorter 6MW distance (estimated mean=-41.31m; P<0.001), higher CAT score (estimated mean=2.62; P=0.013), and higher SGRQ score (estimated mean=10.10; P<0.001). However, serum lymphocyte % was not associated with frequent acute exacerbation and mortality. Conclusions: Patients with low serum lymphocyte showed low pulmonary function, low 6MW distance, and worse quality of life. Lymphocyte may be related with various conditions in COPD.

Calcium-Collagen-PGA complex consists of poly-gamma-glutamic acid (PGA), fish collagen with low Mw (300 Da), calcium, vitamin D₃, magnesium, zinc, selenium, vitamin B6. The effect of Calcium-Collagen-PGA complex on bone formation was evaluated by measuring calcium deposits with alizarin red S staining in human bone marrow mesenchymal stem cells (hBMSCs). The complex promoted the differentiation and the mineralization of hBMSCs. More specifically, the complex increased the expression of osteocalcin and osteopontin that is biomarker for osteogenic differentiation. The treatment of hBMSCs with calcium complex + 5% fish collagen + PGA (sample 3) increased calcium deposit by 3 times while calcium complex alone (10 mcg per mL, sample 1) and calcium complex + 5% fish collagen (100 mcg per mL, sample 2) increased calcium accumulation by 1.3 and 1.5 times, respectively. In conclusion, calcium complex + 5% fish collagen + PGA stimulated intracellular calcium deposit most effectively, and thus it warrants further in vivo study to develop into a preventive agent for osteoporosis or other bone diseases.
