      • Sustained-release praziquantel tablet : pharmacokinetics and the treatment of clonorchiasis in beagle dogs

        Praziquantel is rapidly absorbed and secreted; and thus fractional doses are recommended for the treatment of cestode and trematode infections. In the present study, we developed a new praziquantel tablet formula allowing sustained-release (SRP). In vitro dissolution of SRP tablets showed that praziquantel at 300 ㎎/tablet combined with hydroxypropyl methylcellulose dissolved completely at a constant rate over 10 h, whereas the conventional praziquantel tablet (PZQ) was only 40% dissolved. Pharmacokinetic studies in dogs confirmed that SRP was absorbed more slowly than PZQ. The mean value of the area under the concentration/time curve from 0 h to the final observation time, the maximum concentration in serum, and the time of maximum concentration in serum for SRP were 3.471.500 ng/min for 0.25 ㎖, 10,300 ng for 0.25 ㎖, and 192 min. while the values for PZQ were 688,600 ng/min for 0.25㎖, 2,500 ng for 0.25㎖, and 135 min. The cure rate in dogs with a heavy infection (500 metacercariae) treated with a single dose of SRP (150 ㎎/tablet) at 50 ㎎/㎏ was 80%, while in dogs treated with a single dose of SRP (300 ㎎/tablet) at 30 mg/kg it was 60%, and the cure rate with PZQ was 20%. In each case. the egg reduction rate was similar (over 90%). No abnormal liver functions or hepatic or renal pathologies were observed in dogs administered with SRP at 30 ㎎/㎏. The SRP tablet showed sustained release and slow absorption; and it had an improved anthelmintic efficacy against Clonorchis sinensis in experimental dogs. compared with conventional praziquantel.

        원인 불명의 실신 환자의 진단에 있어서 Head - up tilt Test 의 유용성과 혈관미주신경성 실신 환자의 임상적 특징

        Background: Syncope, defined as a transient loss of consciousness, is a frequently encountered symptom, but despite thorough clinical and invasive diagnostic investigation, the cause of syncope remains unexplained. In recent years, head-up tilt test has been of increasing interest as a diagnostic aids in patients with unexplained syncope. We studied to define the usefulness of various diagnostic test in the evaluation of patients with recurrent syncope, to assess the usefulness of head-up tilt test in the working of syncope of unknown origin and to evaluate the clinical characteristics of subgroups of patients with syncope induced by head-up tilt test. Methods: Between June, 1991 and November, 1992 at St. Mary`s hospital, 21 patients with history of syncope and 24 control subjects without history of syncope underwent 60° head-up tild test for 60 min. Results: 1) The definite cause for recurrent syncope were diagnosed in 10 of patients (47,6%) after clinical and invasive tests. 2) During head-up tilt test, vasovagal responses were provoked in 5 of 21 patients (23.8%) with recurrent syncope and 1 of 24 patients (4.2%) without syncope (p<0,05). 3) The head-up tilt test induced symptomatic bradycardia or hypotension in 3 of 10 patients (30%.) with negative electrophysiologic results. 4) During tilt induced vasovagal response, a) mean heart rate decreased to 42±24 beat/min from supine control of 70±12 beat/min (p<0. 05), b) mean systolic blood pressure decreased to 95±12 mmHg from supine control of 120±S mmHg (p<0,05), c) mean diastolic blood pressure decreased to 60±21mmHg from supine control of 72±13 mmHg (p<0.05), d) the time interval to onset of vasovagal response was 22±12 min. Conclusions: 1) In a large proportion of patients with recurrent syncope, the diagnosis remains unexplained despite a neurologic and cardiologic investigation including an electrophysiologic study. 2) Vasovagal responses are frequent cause in patients with recurrent unexplained syncope. 3) The head-up tilt test may be a simple, safe and highly yielding provocative test in the investigation of syncope of unknown origin.

      • Fast and Scalable Hydrodynamic Synthesis of MnO<sub>2</sub>/Defect-Free Graphene Nanocomposites with High Rate Capability and Long Cycle Life

        <P>The integration of metal oxides and carbon materials provides a great potential for enhancing the high energy and power densities of supercapacitors, but the rational design and scalable fabrication of such composite materials still remain a challenge. Herein, we report a fast, scalable, and one-pot hydrodynamic synthesis for preparing ion conductive and defect-free graphene from graphite and MnO<SUB>2</SUB>/graphene nanocomposites. The use of this hydrodynamic method using Taylor-Couette flow allows us to efficiently fast shear-exfoliate graphite into large quantities of high-quality graphene sheets. Deposition of MnO<SUB>2</SUB> on graphene is subsequently performed in a fluidic reactor within 10 min. The prepared MnO<SUB>2</SUB>/graphene nanocomposite shows outstanding electrochemical performances, such as a high specific capacitance of 679 F/g at 25 mV/s, a high rate capability of 74.7% retention at an extremely high rate of 1000 mV/s, and an excellent cycling characteristic (∼94.7% retention over 20 000 cycles). An asymmetric supercapacitor device is fabricated by assembling an anode of graphene and a cathode of MnO<SUB>2</SUB>/graphene, which resulted in high energy (35.2 W h/kg) and power (7.4 kW/kg) densities (accounting for the mass of both electrodes and the electrolyte) with a high rate capability and long cycle life.</P> [FIG OMISSION]</BR>

      • BC₃H1 혈관평활근세포의 증식 및 분화과정에 있어서 Polyamine 대사의 역할에 관한 연구

        The influences of spermine (SM) and DFMO. an irreversible ODC-inhibitor, on the polyamine metabolism, CK-activity, [^(3)H]-thymidine DNA synthesis, and isoactin mRNA expression of BC_(3)H1 in culture were studied, with the references of phosphodiesterase(PDE)-inhibitors including isobutyl-methylxanthine (IBMX) and KR30075. BC_(3)Hl cells cultured in 10% fetal bovine serum (FBS)-DMEM for 24 hrs (the first : pre-drug phase) were cultivated in 10% FBS-DMEM added with one of the test drugs for further 96 hrs(the second proliferation phase), and then continue to grow in 2% FBS-DMEM for 48 hrs (the third : differentiation phase). The putrescine content of BC_(3)Hl harvested after the first phase was relatively higher in comparison with those of the other phases and was gradually decreased through the full phases of culture, and the spermine content was little changed. The spermidine (SD) content was. unlike other polyamines, increased by 26.5% at the end of the second phase, but that SD increase was moderately attenuated by PDE-inhibitors, particularly KR30075, and furthermore. the SD content of BC_(3)Hl cultured in the presence of DFMO, unlike other polyamines. showed the marked and gradual decrease throughout the full phases of culture. The [^(3)H]-thymidine DNA synthesis during the third : differentiation phase was not affected by IBMX and slightly inhibited by KR30075, but was significantly inhibited by SM plus DFMO or each of them. And the creatine kinase (CK) activity gradually increased in advancing with culture duration was not changed by PDE-inhibitors. but both SM and DFMO significantly enhanced the increase of CK activity. The synthesis of BC_(3)Hl α-actin and β-/ γ-actin mRNAs was significantly enhaced by DFMO, and the DFMO-induced enhancement was dramatically inhibited by SM. And PDE-inhibitors and SM little affected the synthesis of α-actin mRNA, but PDE-inhibitors attenuated the synthesis of β-/ γ-mRNAs, especially in the second phase. These results suggest that polyamines have a pivotal role in the proliferation and differentiation of BC_(3)Hl cells, that the enhancement induced by DFMO of expression of VSM isoactin gene seems to be an attractive subject to be studied in future, and that PDE-inhibitors may inhibit the synthesis of VSM β-/ γ-actin mRNAs.

        뇌동맥류 파열 환자에서 증후성 뇌혈관연축의 발생 및 회복에 영향을 미치는 인자들

        To determine the factors affecting development and recovery of symptomatic vasospasm. the author analysed the results of patients with ruptured intracranial aneurysms treated by nimodipine and prophylactic “triple0H” therapy. From January 1991 to December 1995. 199 patients underwent surgery for ruptured intracerebral aneurysms. Combined hypervolemic hemodilution was initiated at the time of admission. Induced hypertension was added immediately after surgery(aneurysmal neck clipping). All patients received nimodipine intravenously or orally until the 14th day of SAH Sixty patients underwent surgery on Day 0 through 2 after attacks of subarachnoid hemorrhage(Goup A) 105 patients on Day 3 through 14(Group B). and 35 patients after Day 14(Group C). Of 199 patients. 76(38%) patients suffered from symtomatic vasospasm. Symptomatic vasospasm occured in 34% of Group A patients. 33% of Group B patients. and 5% of Group C patients. Symtomatic vasospasm occured more frequently in the older age group. the Fisher group 3 and Hunt & Hess grades 3, 4 and 5 groups. Of 76 patients who suffered from symptomatic vasospasm. 47(62%) patients recovered completely. The author analysed the relationship between recovery rate and sex, age. Hunt & Hess agrde and Fisher group among these 76 patients. Recovery rates between these groups were not significantly different. Based on this experience the author believes that Hunt-Hess grade. Fisher group and age are important factors affecting symptomatic vasospasm development.

        A mathematical model is formulated for the minimum cost design of reinforced concrete earth structures. The design variables are selected to be the depth and steel ratio in the retaining wall, and the depth, steel ratio and footing sizes in the isolated column footings. An elastic analysis is performed and resulting designs are based on both the ultimate strength theory and the elastic theory of concrete design. Constraint requirements are imposed to insure structural strength and side constraints. The resulting mathematical model is a nonlinear constrained optimization problem. The modified Lagrange Multiplier method is suggested as a satisfactory algorithm for the optimum design of the R.C. earth structures.

      • Prognostic Value of Multidetector Coronary Computed Tomography Angiography in Relation to Exercise Electrocardiogram in Patients With Suspected Coronary Artery Disease

        Objectives: This study was designed to determine the prognostic value of multidetector coronary computed tomography angiography (CTA) in relation to exercise electrocardiography (XECG) findings. Background: The prognostic usefulness of coronary CTA findings of coronary artery disease in relation to XECG findings has not been explored systematically. Methods: Patients with suspected coronary artery disease who had undergone both coronary CTA and XECG (<90 days between tests) from 2003 through 2009 were enrolled retrospectively. Coronary CTA results were classified according to the severity of maximal stenosis (normal, mild: <40% of luminal stenosis, moderate: 40% to 69%, severe: ≥70%), XECG results were categorized as positive and negative, and Duke XECG score was calculated. Clinical follow-up data were collected for major adverse cardiac events (MACE): cardiac death, nonfatal myocardial infarction, unstable angina requiring hospitalization, and revascularization after 90 days from index coronary CTA. C-statistics were calculated to compare discriminatory values of each test. Results: Among the 2,977 (58 +/- 10 years) study patients, 12% demonstrated positive XECG results. By coronary CTA, patients were categorized as normal (56%) or having mild (26%), moderate (13%), or severe (5%) disease. During a median follow-up of 3.3 years (interquartile range: 2.3 to 4.6), 97 MACE were observed and the 5-year cumulative event rate was 3.6% (95% confidence interval: 3.0 to 4.3). Although both XECG (C-statistic: 0.790) and coronary CTA (C-statistic: 0.908) improved risk stratification beyond clinical risk factors (C-statistic: 0.746, p < 0.05 for all), XECG in addition to coronary CTA (C-statistic: 0.907) did not provide better discrimination than coronary CTA alone (p = 0.389). In subgroup analyses, coronary CTA stratified risk of MACE in groups with both positive and negative XECG results (all p < 0.001 for trend). However, positive XECG results predicted risk of MACE on coronary CTA only in the moderate stenosis group (hazard ratio: 2.58, 95% confidence interval: 1.29 to 5.19, p = 0.008) and severe stenosis group (hazard ratio: 2.28, 95% confidence interval: 1.19 to 4.38, p = 0.013). Conclusions: In patients with suspected coronary artery disease, coronary CTA discriminates future risk of MACE in patients independent of XECG results. Compared with coronary CTA, XECG has an additive prognostic value only in patients with moderate to severe stenosis on coronary CTA.

        This paper is an attempt to propose our study that draw up a plan about faculty and major course to the Daegu Polytechnic College authorities. Our approach is a mixture of methods. It is systematic, in that it attempts to treat a college as a system. Through the second chapter, we examine the periodic, social, environmental, governmental, and economic context within which the College's developing effort must progress. Chapter three examines how the college systems actually design about faculty and major course. Thus, chapter four discusses how new faculty and major course participants attempt to compose a curriculum; chapter five accesses the institutions available in the College to distribute these designs. In conclusion, this paper presents that the College system would be better nine faculty; education and nurse·health field only be in administration unit, an information field be better divided into two parts: internet computer and information communication, and business and computer design faculty is established as intimate information. After all, the system will be characteristic in its information traits.

        Endoscopic submucosal tunnel dissection salvage technique for ulcerative early gastric cancer.

        <P>Endoscopic submucosal dissection is an effective treatment modality for early gastric cancer (EGC), though the submucosal fibrosis found in ulcerative EGC is an obstacle for successful treatment. This report presents two cases of ulcerative EGC in two males, 73- and 80-year-old, with severe fibrosis. As endoscopic ultrasonography suggested that the EGCs had invaded the submucosal layer, the endoscopic submucosal tunnel dissection salvage technique was utilized for complete resection of the lesions. Although surgical gastrectomy was originally scheduled, the two patients had severe coronary heart disease, and surgeries were refused because of the risks associated with their heart conditions. The endoscopic submucosal tunnel dissection salvage technique procedures described in these cases were performed under conscious sedation, and were completed within 30 min. The complete en bloc resection of EGC using endoscopic submucosal tunnel dissection salvage technique was possible with a free resection margin, and no other complications were noted during the procedure. This is the first known report concerning the use of the endoscopic submucosal tunnel dissection salvage technique salvage technique for treatment of ulcerative EGC. We demonstrate that endoscopic submucosal tunnel dissection salvage technique it is a feasible method showing several advantages over endoscopic submucosal dissection for cases of EGC with fibrosis.
