Clinical Features of Plasmodium Vivax Malaria

목적: 정유공장의 폴리프로필렌(polypropylene) 부서와 복합수지 부서에서 사용되는 첨가제들중 피부첩포검사를 통하여 피부독성을 야기할 수 있는 물질들을 알아보기 위해서 본 조사를 수행하였다. 방법: 일개 정유공장에서 총 44명의 근로자를 대상으로 폴리프로필렌공정에서 첨가제를 취급한 노출군과 그런 작업을 하지 않은 비노출군으로 나누어 피부첩포검사를 했다. 노출군은 27명으로 과거 노출자 10명, 현노출자 17명이었다. 비노출군은 17명이었다. 10개의 첨가물 중 Microstat 16은 0.25%, Amoslip-CP는 0.1%의 농도로 첩포검사를 하고, 나머지 8종은 1%의 농도에서 검사 후 48시간 후 피부에 양성반응을 보이면 0.1%로다시 피부첩포검사를 했으며 음성방응을 보이면 10%로 검사를 시행했다. 결과: 각 물질별 결과로는 Ultranox-626과 Millad 3988이 가장 많은 피부양성반응을 보였다. 피부의 발진을 많이 보이는 (양성 및 의양성) 빈도는 Ultranox-626, Millad 3988, Armoslip-E, Songstab Sc-100 순이었다. 피부에 발진을 보이는 (양성 및 의양성) 대상자는 18명(41%)이었고, 그중 노출군이 14명(51.9%)이고 비노출군이 4명(23.5%)으로누출군에서 더 많은 발진을 야기하였다. 피부첩포검사에서 반응을 보이는 18명 중 자극가증서응로 작용하는 대상자는 7명(38.9%)이었고, 알레으기 가능성으로 작용하는 대상자는 6명(33.3%)이었으며 여러 물질에 강한 반응을 보이는 다중 반응 대상자는 5명(27.8%)이었다. 알레르기 가능성 반응 대상자는 현재의 노출자가 과거노출자와 비노출자 보다 통계적으로 유의하게 많았다(p=0.04). 자극가능성 반응 대상자들 중 현재 노출자, 과거노출자, 비노출자 사이의 통계적 차이는 없었다. 다중 반응 대상자는 과거노출자가 통계적으로 유의하게 많았고(p=0.05), 현재의 노출자는 없었다. 결론: 자극성 반응과 알레르기성 반응을 포함하여 첨가제에 노출된 집단이 비노출군보다 높은 과민상태를 보였다. 결과적으로 이들 물질에 노출되는 경우 잠재적 알레으기 가능성 반응이 유발 될 수 있능 것으로 보인다. 피부첩포검사에서 반응을 보이는 원인물질로 빈도가 높은 Ultranox-626 Millad 3988 Armoslip-E Songstab Sc-100의 4가지 물질에 대한 노출을 우선적으로 관리하는 것이 필요하다고 판단된다. 첨가물의 알레으기 여부를 정확히 판정하는 작업과 피부병 유발 가능성을 향후 좀더 구명할 필요가 있을 것으로 보인다. Objects: To investigate the causative agents for the dermatological problems associated with the polypropylene manufacturing process. Methods: The study subjects consisted of 44 male workers involved in the polypropylene manufacturing process. Twenty-seven of them had been exposed to additives, and the remaining 17 had not. Among the 27 exposed, 10 had been exposed previously, and 17 are currently being exposed through their duties. Patch test were performed with three different concentrations, 0.1%, 1% and 10% of 10 different additives that was diluted with Vaseline. Results: 18 subjects (41%) had positive reactions to the patch tests. Exposed group had a higher reaction rate (51.9%) than the control group (23.5%). When the positive reactions were further classified, 7, 6 and 5 were compatible with probablu irritant, probably allergic, and multiple hypersensitivity reactions respectively. The most frequent reactions were from Ultranox-626 followed by Millad 3988, Armoslip-E and Songstab Sc-100. Conclusions: When considering the significantly higher probability of an allergic reaction rate among the currently exposed group compared to non- or previously-exposed groups, the nature of dermatological problems in a polypropylene plant is probably allergic. Ultranox-626, Millad3988, Armoslip-E and Songstab Sc-100 were the main causatives agents found in the process.
A Case of HELLP Combined with AFLP

HELLP is an acronym that refers to a syndrome characterized by hemolysis with a microangiopathic blood smear, elevated liver enzyme, and a low platelet count. It probably represents a severe form of preeclampsia, but the relationship between the two disorders remains controversial. HELLP may be difficult to distinguish clinically from AFLP since both occur at the same time in gestation and share several clinical features. A 37-year-old female at 25-week gestation, twin pregnancy, presented with labor pain, abdominal pain, spotting vaginal bleeding. She didn’t have any specific medical history. Physical examination revealed alert mental status, acute ill looking appearance. Her blood pressure 124/67mm Hg. Initial laboratory finding revealed mild anemia, no thrombocytopenia, PT 11.0 sec, INR 1.00, aPTT 25.6 sec, AST 34 IU/L, ALT 37 IU/L, protein 6.36g/dl, albumin 3.28g/dl, total bilirubin 0.28mg/dl and no proteinuria. Serology tests like HBsAg, HCV, HIV and HAV were all negative. We tried to suppress premature labor using tocolytics. After fourteen days, liver function test abruptly increased from 71 to 1,680 and from 53 to 840, respectively AST and ALT during 2 days. In the same period, platelet fell from 183,000/uL to 23,000/uL. LDH was 2,192 IU/L. PT was still normal. Ultrasonography revealed IUP around 27 weeks, hepatomegaly with increased periportal echogenicity and secondary GB wall mild swelling, that is, hepatitis finding and increased parenchyme echogenicity of liver, that is, underlying severe fatty liver. Although we did not check hemolysis in PB smear, presumptive diagnosis of HELLP was made. It was decided to perform an emergency lower segment caesarean section performed after about 5 hours. One day later, laboratory finding was aggravated, that is, AST 4,900 IU/L, ALT 1,790 IU/L, protein 4.24g/dl, albumin 2.48g/dl, PT 17.3 sec, INR 1.57, aPTT 33.6 sec, fibrinogen 1.71 g/L, antithrombin 60%, platelet 29,000/uL. Bilirubin increased from 1.19mg/dl to 3.34mg/dl next five days. It revealed AFLP feature. We did supportive care. After six days later, overall finding was improved. There was no hemolysis in PB smear. This report is concerned with a case of 37-year-old female at 25-week gestation with HELLP and AFLP. We guess that this case is the mixed

Clinical Characteristics of Smoking Asthmatics

Background: The smoking prevalence in asthma patients are similar to those in the general population. Asthma and active cigarette smoking can interact to create more severe symptoms, an accelerated decline in lung function and impaired therapeutic responses. Accordingly, asthmatics with a history of smoking were examined to define the clinical characteristics and lung function of smoking asthmatics. Methods: The medical records of 142 asthmatics with a known smoking history were reviewed. The patients were divided into three groups according to their smoking history-current smokers, former smokers and non-smokers. The clinical characteristics, lung function, and annual declines of the forced expiratory volume in one second (FEV1) were compared. Results: Fifty-three of the 142 patients (37%) were current smokers, 24 were former smokers (17%) and 65 were non-smokers (45%). The patients with a hospital admission history during the previous year included 16 current smokers (30%), 4 former smokers (17%) and 7 non-smokers (11%) (p=0.02). The mean FEV1 (% predicted) was 76.8±19.8%, 71.6±21.1% and 87.9±18.7% for current smokers, former smokers and non-smokers, respectively (p<0.001). The FEV1/forced vital capacity (FVC) (ratio, %) values were 63.6±12.6%, 59.3±14.9% and 72.1±11.8% in current smokers, former smokers and non-smokers, respectively (p<0.001). The corresponding mean values for the individual FEV1 slopes were not significant (p=0.33). Conclusion: Asthmatic smokers demonstrated higher hospital admission rates and lower lung function. These findings suggest that the smoking history is an important predictor of a poor clinical outcome in asthma patients.
