축구선수의 Detraining과 Retraining이 혈청지질 및 호르몬농도에 미치는 영향

The purpose of this study was to investigate the effects of detraining and retraining on serum lipid and hormones in soccer players. Subjects were seven male high-school soccer players. V˙O_2max was determined for each subjects by administering a treadmill test(initial speed: 90m/min, grade: 5%, increasing speed per 3 min: 30m/min). Serum lipid(T-C, TG, HDL-C LDL-C) and hormones(epinephrine, norepinephrine, growth hormones, cortisol) were assayed pre and post detraining in 10, 20, 30 days after retraining. The repeated ANOVA was used to determine significant differences. The 0.05 level of significance was as critical level for the study. The results of the study were as follows: 1. V˙O_2max(ml/min) were 3576.3±204.2ml/min pre detraining, 3234.1±198.9 ml/min post detraining. There are significant(p<.05) difference between pre and post detraining. In 10, 20, 30 days after retraining, V˙O_2max(ml/min) were 3601.4±170.9 ml/min. There were significantly(p<.05) increased in retraining periods. 2. V˙O_2max(ml/kg/min) were significantly(p<.05) decreased from 62.3±2.9 ml/kg/min to 55.9±4.7 ml/kg/min in detraining. In 10, 20, 30 days after retraining, V˙O_2max(ml/kg/min) were 62.4±3.4ml/kg/min, 62.7±2.3ml.kg/min, 67.3±7.2ml/kg/min respectively. There were significantly(p<.05) increased in retraining periods. 3. T-C were significantly (p<.05) increased from 166.6±8.5mg/dl to 175.3±10.3 mg/dl in detraining. In 10, 20, 30 days after retraining, T-C were 160.1± 3.2mg/dl, 156.7±3.7mg/dl, 140.3±9.0mg/dl. There were significantly(p<.05) decreased in retraining periods. 4. HDL-C were 61.4±6.6mg/di pre detraining, 5.3±6.6mg/dl post detraining. There are significant(p<.05) difference between pre and post detraining. In 10, 20, 30 days after retraining, HDL-C were 56.9±7.1mg/dl, 56.4±9.2mg/dl, 57.7±9.1mg/dl respectively. There were no significant difference in retraining periods. 5. The hormones(epinephrine. norepinephrine, growth hormone, cortisol) were changed as same patterns. Epinephrine were 26.0±7.0[g/ml pre detraining, 24.6±3.2pg/ml post detraining. In 10, 20, 30 days after retraining, epinephrine were 26.9±5.6pg/ml, 30.6±6.2pg/ml, 29.4±5.6pg/ml respectively. There were no significant difference in retraining periods. In conclusion, HDL-C, epinephrine, norepinephrine, growth hormone and cortisol were decreased, T-C, LDL-C and TG were increased in detraining. But HDL-C, epinephrine, norepinephrine, growth hormone and cortisol were increased, T-C, LDL-C and TG were decreased in retraining.

The unicornuate uterus with a rudimentary horn is a very rare congenital anomaly of the female genital system. We have experienced a case of an unicornuate uterus with a left non-communicating functional rudimentary horn associated with agenesis of an ipsilateral kidney, progressive dysmenorrhea, hematometra, hematosalpinx and extensive endometriosis in a 13-year-old girl and treated by laparoscopy. We report this case with relevant literatures.

<P>House dust mites (HDMs) are a common cause of allergic asthma. The group 2 allergen from Dermatophagoides farinae, Der f 2, is one of the major HDM allergens. Elevated Der f 2 immunoglobulin E (IgE) levels are observed in most of the allergic patients. Interleukin-13 (IL-13), a gene associated with asthma pathology, was induced by Der f 2 in BEAS-2B human airway epithelial cells; however, the signaling pathways associated with Der f 2 are not fully understood. In this study, we identified a role of the phosphatidylinositol-3-kinase (PI3K)/Akt pathway, a well-known potential target for anti-asthma drugs, in the IL-13 induction by Der f 2. First, Der f 2 activated the PI3K/Akt pathway, which subsequently activated the nuclear factor-kappa B (NF-κB) pathway and induced IL-13 expression in BEAS-2B cells. Treatment with the PI3K inhibitor LY294002 abolished Der f 2-induced activation of Akt and NF-κB and the expression of IL-13. Furthermore, Der f 2-induced activation of the PI3K/Akt and NF-κB pathways, expression of IL-13, and the blockade of these effects with a PI3K inhibitor were confirmed in the lungs of mice that were intranasally exposed to Der f 2. Taken together, these results indicate that the PI3K/Akt pathway regulates Der f 2-induced IL-13 expression via activation of the NF-κB pathway.</P>

Atopic dermatitis (AD), which is known as the most common pruritic skin disease, is caused by epidermal barrier dysfunction, allergies, microwave radiation, histamine intolerance, and genetic defects. To investigate the therapeutic effects of fermented soycrud (FSC) on AD pathology, alteration of AD phenotypes induced by phthalic anhydride (PA) treatment was assessed by ear thickness analysis, measurement of immunerelated organ weights, ELISA, and histological and pathological analyses of ICR mice after FSC treatment for 2 weeks. Except for water content, the concentrations of most major components were lower in FSC compared to common tofu (CMT). Thymus and lymph node weights were significantly reduced in ICR mice treated with PA+CMT or PA+FSC, whereas spleen and body weights were maintained. Elevation of ear thickness induced by PA treatment was rapidly diminished in the CMT- and FSC-treated groups, although there was no significant difference between the two groups. Furthermore, significant reduction of epidermal thickness was detected in both the PA CMT- and PA+FSC-treated groups. However, IgE concentration and dermal thickness were reduced only by PA+FSC treatment, whereas PA+CMT treatment maintained levels comparable to PA+vehicle treatment. The number of infiltrated mast cells was higher in the PA+vehicle-treated group compared to the untreated control. Following CMT or FSC treatment, mast cell infiltration was slightly reduced, although the CMT-treated group showed greater cell numbers. These results indicate that FSC may significantly relieve the phenotypes of AD induced by PA treatment and should be considered as a potential candidate for AD therapy.

Dermatomyositis (DM) is an idiopathic inflammatory process characterized by proximal muscle weakness and cutaneous lesions, such as the Gottron`s sign, heliotrope rash, and erythematous photosensitive rash. Administration of systemic agents for the treatment of underlying systemic diseases leads to remission of the cutaneous lesions in many cases. However, cutaneous lesions may remain refractory to treatment. Pimecrolimus is a calcineurin inhibitor with combined anti-inflammatory and immunomodulatory activity. It has high affinity to the skin and low permeation potential, even in patients with acute skin inflammation and in those undergoing post-topical corticosteroid therapy. We herein report two DM patients whose cutaneous lesions were refractory to conventional treatment but showed dramatic response to topical pimecrolimus. The clinical outcomes suggest that topical pimecrolimus may be a good therapeutic alternative for the management of the cutaneous lesions of DM.
다양한 환경에서 적용 가능한 유니버설 물병 보조기기 프로토콜 개발연구 - 경수손상 장애인을 중심으로 -

Objective: 본 연구에서는 다양한 환경에서의 식음행위를 보조하기 위한 ‘유니버설 물병 보조기기 프로토콜개발’을 하고자 한다. Background: 식음행위는 일상생활에서 수시로 반복하여 일어나는 행위이다. 다양한 환경에서 다양한 물병, 컵, 캔 등으로 식음행위를 한다. 하지만 손과 팔의 기능이 제한되는 경수손상 장애인은 스스로 하기에 어려움이 있어 주변의 도움을 받을 수밖에 없는 실정이다. 보조기기를 통해 식음행위를 스스로 할 수 있도록 지원한다면 경수손상 장애인의 자립 생활에 많은 도움이 될 수 있을 것으로 사료되어 본 연구를 진행하였다. Method: 본 연구에서는 HAAT (Human – Activity - Assistive Technology-Context) 모델을 기반으로 어떤 요소들을 고려하여 프로토콜을 개발해야하는지 분석하였다. 또한 기성품을 활용하여 해소할 수 있는 부분과 개발이 필요한 부분을 구분하였다. 개발이 필요한 부분에서는 어포던스(affordance) 디자인을 적용해 유니버설(universal)하게 사용할 수 있도록 고려하였다. Results: 장애인의 상황, 특성에 맞추어 필요에 맞게 각 요소(Human, Activity, Assistive Technology, Context)별로 선택하여 보조기기를 사용할 수 있는 ‘유니버설 물병 보조기기 프로토콜’의 초안을 개발하였다. 프로토콜에는 다양한 상황에서의 식음행위를 위해 개발이 필요한 부분과 기존 상용제품을 활용할 수 있는 부분 등을 구분하여 수록하였다. 또한 경수손상 장애인의 잡기와 식음 행위를 보조하는 ‘유니버설 물병 보조기기’를 개발하였다. Conclusion: 경수손상 장애인의 삶의 질 향상과 자립을 위해 다양한 환경에서 식음행위를 스스로 할 수 있도록 지원하는 프로토콜과 보조기기를 개발하였다. Application: 개발된 물병 보조기기의 손잡이는 여러 행위에서 공통적으로 필요한 보조기기이다. 이에 식음행위 뿐만 아니라 다른 행위(식사, 머리빗기 등) 시에도 활용할 수 있는 확대 연구로 적용 가능하다. 또한 경수손상 장애인의 다양한 일상생활 지원을 위해 여러 행위를 지원하는 다양한 프로토콜 개발연구도 필요하다.

Cornelia de Lange syndrome (CdLS) is a relatively uncommon, multiple malformation syndrome involving neurodevelopmental, craniofacial, cardiac, musculoskeletal and gastrointestinal systems. Anesthetic management of a patient with CdLS may pose a serious problem mainly from difficult tracheal intubation, aspiration complications and cardiac malformation. We report our successful experience in anesthetic management of a 15-month-old male patient with CdLS who underwent orchiopexy for bilateral cryptochidism. (Korean J Anesthesiol 2008;55:639~43)
