Background: Nephrotoxicity is a common side effect of medications. Panax ginseng is one of the best-known herbal medicines, and its individual constituents enhance renal function. Identification of its efficacy and mechanisms of action against drug-induced nephrotoxicity, as well as the specific constituents mediating this effect, have recently emerged as an interesting research area focusing on the kidney protective efficacy of P. ginseng. Methods: The present study investigated the kidney protective effect of fermented black ginseng (FBG) and its active component ginsenoside 20(S)-Rg3 against cisplatin (chemotherapy drug)-induced damage in pig kidney (LLC-PK1) cells. It focused on assessing the role of mitogen-activated protein kinases as important mechanistic elements in kidney protection. Results: The reduced cell viability induced by cisplatin was significantly recovered with FBG extract and ginsenoside 20(S)-Rg3 dose-dependently. The cisplatin-induced elevated protein levels of phosphorylated c-Jun N-terminal kinase (JNK), p53, and cleaved caspase-3 were decreased after cotreatment with FBG extract or ginsenoside 20(S)-Rg3. The elevated percentage of apoptotic LLC-PK1 cells induced by cisplatin treatment was significantly abrogated by cotreatment with FBG and the ginsenoside 20(S)-Rg3. Conclusion: FBG and its major ginsenoside 20(S)-Rg3, ameliorated cisplatin-induced nephrotoxicity in LLC-PK1 cells by blocking the JNKep53ecaspase-3 signaling cascade.
Predictive factors for success of chronic atrial fibrillation after DC cardioversion

Background: Few data are available concerning head up tilt table test (HUT) outcomes in unselected populations in the elderly. The aim of this study was to compare the elderly with younger patients in a large number of consecutive patients with unexplained syncope who underwent nitrate-potentiated head up tilt test. Methods: Consecutive outpatients with history of recurrent unexplained syncope underwent HUT by being tilted to 60 degree after supine position for five minuntes. The test was potentiated by the administration of isosorbid dinitrate spray 20 minutes after that. The frequency of syncope occurrence and hemodynamic responses during the passive and nitrate phases of HUT were evaluated; symptoms preceding HUT-induced syncope were recorded with heart rate and arterial blood pressure values. Results: 311 (46.9%) of 633 patients were positive. The positive results showed 39.5% in those aged ≥60 years (n=215), 45.2% in the age of 36-64 years (n=272) and 58.5% in the age of ≤35years (n=176). The patients experienced HUT positive results almost after using isosorbid dinitrate spray(95.1%), and average time to syncope was 33 minutes after HUT started. HUT positive patients had increased pulse rate more than older age groups. Conclusion: The rate of nitrate-induced syncope was more frequent in younger patients. In younger patient with HUT positive, heart rate was more increased in the passive and the active phases than the one in older patients.

Background: Rate control and rhythm control was recommended strategy for patients with atrial fibrillation(AF). Cardioversion, pharmacologic agent and catheter ablation was done to achieve sinus rhythm. Cardioversion and pharmacological conversion are appropriate option. We evaluate predictive factor for success of atrial fibrillation after DC cardioversion Methods: 137 patients with chronic AF to undergo cardioversion reveived serial monophasic (100, 200, 300 up to 360J)shock or serial biphasic(100, 150, up to 200J) shock. Results: 105 of 137 patients was restoration to sinus rhythm. Restoration group used more beta-blocker (29.8 vs 6.3%) and more amiodarone(32.7 vs 12.5). And enlarged left atrial dimension(4.70 vs 4.48 mm), more elderly(58.7 vs 53.9), higher body mass index(BMI, 25.3 vs 23.8) was associated to sinus conversion. Age(p=0.01), BMI (p=0.03), beta-blocker(p=0.009) were independently associated with outcome by multivariate analysis. Conclusion: DC conversion to sinus rhythm for patients with AF is associated with more elderly, higher BMI and used beta-blocker patients.

Fibrillation was attempted to determine the suitability of KOCC for the production of nanocellulose in order to utilize it as a high value added material, and the possibility of using fibrillated KOCC obtained therefrom as a papermaking additive was evaluated. A grinder was used for the fibrillation of KOCC. In order to investigate the fibrillation characteristics of KOCC, the changes of SEM images, Brookfield viscosity and degree of polymerization were measured. Also, the handsheets were prepared and the changes of major physical properties were investigated in order to evaluate the performance and applicability of fibrillated KOCC as an additive for linerboard manufacture. The fibrillation of KOCC was significantly delayed compared to the bleached chemical pulp, and the complete fibrillation was not achieved, probably due to the residual lignin content (kappa no. 19.36). It is difficult to produce nanocellulose without proper pretreatment. The average fiber length decreased to the level of fines at 20 times of grinding because KOCC was severely aged. As the fibrillation of KOCC proceeded, the degree of Brookfield viscosity continued to increase and the degree of polymerization showed a gradual decrease. Bulk, tear index, and burst index of paper were not significantly affected by the addition of fibrillated KOCC. However, it was found that the tensile index and stiffness could be improved by the combination of retention aid and fibrillated KOCC.
