        Effect of Hydride Reorientation on Delayed Hydride Cracking In Zr-2.5Nb Tubes

        The objective of this study is to investigate the reorientation of hydrides with applied stress intensity factor, the peak temperature and the time when to apply the stress intensity factor in a Zr-2.5Nb pressure tube during its thermal cycle treatment. Cantilever beam (CB) specimens with a notch of 0.5 mm in depth made from the Zr-2.5Nb tube were subjected to electrolytic hydrogen charging to contain 60 ppm H and then to a thermal cycle involving heating to the peak temperature of either 310 or $380^{\circ}C$, holding there for 50 h and then cooling to the test temperature of $250^{\circ}C$. The stress intensity factor of either 6.13 or $18.4\;MPa\sqrt{m}$ was applied at the beginning of the thermal cycle, at the end of the hold at the peak temperatures and after cooling to the test temperature, respectively. The reorientation of hydrides in the Zr-2.5Nb tube was enhanced with the increased peak temperature and applied stress intensity factor. Furthermore, when the CB specimens were subjected to $18.4\;MPa\sqrt{m}$ from the beginning of the thermal cycle, the reoriented hydrides occurred almost all over the Zr-2.5Nb tube, surprisingly suppressing the growth of a DHC crack. In contrast, when the CB specimens were subjected to the stress intensity factor at the test temperature, little reorientation of hydrides was observed except the notch region, leading the Zr-2.5Nb to grow a large DHC crack. Based on the correlation between the reorientation of hydrides and the DHC crack growth, a governing factor for DHC is discussed along with the feasibility of the Kim's DHC model.

        선행 제왕절개술 후 질식 분만: 성공의 예측인자들

        목적: 선행제왕절개 후 질식분만을 시도한 산모들 중 성공군과 실패군을 비교하여 성공을 예측할 수 있는 인자를 알아보고자 하였다. 방법: 1997년 3월부터 2003년 2월까지 경희대학교 의과대학 산부인과에서 선행제왕절개 후 질식분만을 위하여 내원한 환자중 질식분만의 금기가 아닌 경우 위험요소 등에 대하여 충분히 상의한 후 분만을 시도한 382명의 환자를 대상으로 하였다. 대상군의 의무기록을 통하여 분만당시 산모의 나이, 분만력, 제태연령, 신생아 몸무게, 입원시 자궁경부 상태 및 Bishop score, 분만의 진행과정, 결과 및 합병증을 조사하여 성공군과 실패군의 차이를 비교분석 하였다. 결과: 292(76.5%)명이 질식분만에 성공하였다. 여러 인자 중 태아의 몸무게(odds ratio, 0.2 ; 95% confidence interval (CI), 0.1-0.4), 입원시 Bishop score(odds ratio, 1.8 ; 95% CI, 1.5-2.1), 질식분만의 과거력(odds ratio, 3.6 ; 95% CI, 1.1-11.6)이 VBAC의 성공과 관련이 있었다. 또한 선행제왕절개시 적응증이 태위이상인 경우에 성공률이 높은 것으로 나타났다(odds ratio, 3.2 ; 95% CI, 1.2~8.6). 입원시 Bishop score로 VBAC의 성공을 예측할 수 있는 지 알아보기 위하여 receiver-operating characteristic curve 분석을 시행하였고, 5점을 cut-off로 하였을 때 47.4%의 민감도와 87.6%의 특이도를 보였다(AUC, 0.765 ; p<0.05). 결론: 출생체중, 질식분만의 과거력, 입원시 Bishop score가 선행제왕절개 후 질식분만을 고려하는 경우 성공의 예측인자로서 유용하게 사용되어 질 수 있을 것이다. Objective: The purpose of this report is to determine the predictable factors for successful vaginal birth after cesarean (VBAC). Methods: In this study, 382 women who attempted VBAC at Kyung-Hee university hospital were included. The medical records of them were reviewed retrospectively. Informations was collected about demographics, medical history, obstetric history, neonatal birth weight, complications, treatment, and outcome of the index pregnancy. Results: The overall success rate was 76.5% (292 cases). Among variables, neonatal birth weight (odds ratio, 0.2; 95% confidence interval, 0.1-0.4), Bishop score at admission (odds ratio, 1.8; 95% confidence interval, 1.5-2.1 ; p<0.05), and history of vaginal delivery (odds ratio, 3.6 ; 95% confidence interval, 1.1-11.6 ; p<0.05) were significant. The Bishop score showed most significant relationship with successful VBAC. The Bishop score 5 at admission had a sensitivity of 47.4% and a specificity of 87.6% in the prediction of successful VBAC. Malpresentation as indications of previous cesarean delivery showed more tendency to succeed than others. Conclusion: Several factors such as birth weight, Bishop score at admission, and history of vaginal delivery may be used to predict the success of VBAC.

        불활성 기체 혼합물의 물성에 관한 열역학적 실험식

        대체 소화제로 사용되는 불활성 기체 중 Ar, N₂, CO₂에 대한 혼합물에서의 물성(포화압력, 밀도, 점도)에 관한 실험식을 구하였다. Mixing rule에 의해 계산한 값을 이용하여 다항식 등의 회귀분석에 의해서 실험식을 얻었다. 포화압력은 온도에 대하여 1차 실험식으로 표시하였다. 압축인자와 포화압력을 이용하여 온도에 대한 밀도에 관한 실험식을 제시하였다. 점도는 온도에 대한 지수함수로 표시하였다. Ar, N₂, CO₂ 혼합물의 조성이 40/50/10(mol. %)에서 열역학적 실험식을 구하였다. For the inert gases of Ar, N² and CO², the empirical equations of the gas mixture were correlated in terms of saturated pressure, density and viscosity. They were obtained by regression analysis based on the mixing rule. The empirical equation of saturated pressure was assumed as the first order function of temperature. The empirical form of density was expressed as compressibility factor and saturated pressure while the empirical equation of viscosity was formulated as a power function of temperature. This empirical equations of the physical properties were obtained in the composition of Ar, N² and CO², 40/50/10 (mol. %).

        대체 소화제의 열역학적 물성 비교

        몬트리올 의정서에 의해서 규제받는 CFCs와 Halon의 대체 물질인 HFC-23, HFC-125, HFC-227ea, HFC-236fa와 불활성 화합물 Ar, N₂, CO₂의 열역학적 물성인 포화압력, 밀도, 엔탈피, 점도를 비교하였다. 본 연구에서는 소화제의 물성을 문헌값을 온도의 함수로서 표시하였다. HFC화합물의 열역학적 물성은 Halon-1301과 비슷하게 나타내었다. 불활성 화합물은 주로 혼합물로 이용되지만, 불활성 화합물의 물성은 Halon-13O1에 비하며 바람직하지 않았다. For CFCs and Halons regulated by Montreal Protocol and their alternatives of HFC-23, HFC-125, HFC-227ea, HFC-236fa and the mixtures of inert gases of Ar, N₂ and CO₂, the thermodynamic properties of saturated pressure, density, enthalpy and viscosity were compared. In this study, the data from literature were expressed as a function of temperature. Thermodynamic properties of HFC compounds were similar to those of Halon-1301. Inert gas was mainly used as a mixture, but the physical properties of the inert gas does not have the favorable advantages over those of Halon-1301.

        사망 진단서(시체 검안서) 작성의 문제점

        Background: This study was conducted to analyze the current problems in completing death certificates and to identify the correct method for completing death certificates Methods: We reviewed 262 death certificates in three hospitals from March 1 to April 30, 2000, and 119 death certificates in one hospital from March 1 to 31, 2000. We identified major and minor errors and analyzed and compared them retrospectively. Results: A total of 381 death certificates were reviewed: 59 in Seoul National University Hospital,101 in Ewha Woman's University Hospital, and 102 in Gachon Medical College Hospital, which has no education program for completing death certificates in postgraduate training, and 119 in Samsung Medical Center which has an education program for completing death certificates. 358 certificates(94.0%) had at least one error. There were only 23 death certificates(6.0%) without an error. In 182 cases(47.8%), there was one major error. In 321 death certificates(84.3%), there were more than two errors. A comparison of Samsung Medical Center with the other hospitals showed that the number of total errors was statistically different(p=0.001). Conclusion: There were few death certificates without an error in this study. In a hospital which has postgraduate training in completing death certificates, there are fewer errors than in other hospitals which have no training course. Emergency physicians actually certify many deaths, so they must know the correct method of completing death certificates for statistics on morbidity and mortality.

        불활성 기체에 대한 열역학적 실험식

        Halon 대체 소화제로 쓰이는 불활성 기체 중 Ar, N₂, CO₂의 단일 성분일 때의 물성(포화압력, 밀도, 점도)에 관한 실험식을 구하였다. 문헌에서 얻은 실험값을 이용하여 다항식 등의 회귀분석에 의해서 실험식을 얻었다. 포화압력은 온도에 대하여 각각 2차와 3차의 실험식으로 표시하였다. 밀도는 Ar, N₂는 온도에 대한 1차 함수로 표시하였고, CO₂는 온도에 대하여 각각 2차와 3차의 실험식으로 표시하였다. 점도는 온도에 대한 지수함수로 표시하였다. 이들 실험식을 이용하여 단일 성분일 때의 상태를 알 수 있다. Inert gases, Ar, N₂, CO₂, as a Halon alternative, the empirical equations were correlated in terms of saturated pressure, density and viscosity. They were obtained by regression analysis from the experimental data in the literature. The empirical equations of saturated pressure were expressed as the second and third order function of temperature. The empirical equation for Ar and N₂ of density were expressed as the first order function of temperature. And CO₂ was expressed as the second and third order function of temperature. The empirical equation of viscosity was formulated as a power function with temperature. This empirical equations would allow us to predict pure component state.

        구강 악안면 연조직 결손 재건을 위한 진피 지방 이식의 이용 : 증례보고

        Soft tissue defects in oral & maxillofacial region caused by tumor resection, trauma, congenital deformities have been treated in autogenous soft tissue flap, allogenic material, free dermal graft, fascia graft. Of these methods, autogenous dermis graft had initially been applied in hernia treatment at the beginning of nineteenth century and have been applied in soft tissue augmentation coverage of vital structure, dead space removal and reconstruction of fascia. A fat graft is used in reconstruction of orbit at the enucleation, restoration of facial contours, etc. In this case, patient with chin soft tissue defect in traffic accident was treated in autogenous dermis-fat graft from patient's abdominal and gluteal region. Chin defect was recons ructed favorably. There was no severe atrophy of grafted area 12 months postoperatively. We will report the result that is favorable esthetically with literature review.
