        제2형 당뇨병 환자에서 Voglibose와 Acarbose의 비교임상연구

        연구배경:아카보스와 보글리보스는 ­glucosidase inhibitors로써 비록 약리학적 작용이나 부작용에 있어서 두 약물간에 차이가 있다는 것은 잘 알려져 있으나 당뇨병 환자를 대상으로 아직 이에 대한 두 약물간에 직접적인 비교에 대해 연구된 바는 없었다. 이에 저자등은 국내 2형 당뇨병 환자에 대해 유효성과 부작용 발현에 대해 두 약제를 비교하고자 무작위법에 의한 위약 대조군의 이중 맹검법 연구를 시행하였다. 방법:시험 약제 투여 4주간의 관찰기를 설정하여 공복혈당의 변화가 30㎎/dL 이하이고, 식후 혈당이 200㎎/dL 이상인 환자로 기타 제외 기준에 해당하지 않은 환자 53명을 대상으로 하여 보글리보스 군(24명)과 아카보스 군(29명)으로 무작위로 나누었다. 치료기간은 총 8주로 하였으며, 4주간 간격으로 혈청학적 검사와 부작용을 분석하여 치료 효과가 부작용을 평가하였다. 결과:1)혈당 변화:보글리보스군은 식후 1시간 혈당이 치료 후 4주, 8주째 의미 있게 감소하였도, 아카보스군은 식후 1시간과 2시간 혈당이 치료 후 4주, 8주째 의미있게 감소하였다. 또한 관찰기 혈당에 대한 치료 4주째 감소량은 아카보스군에서 더 큰 경향을 보였으나, 치료 8주째에는 두 군 간의 강하정도에 의미있는 차이가 없었다(p=0.569). 2)인슐린 치의 변화:보글리스 군은 식후 1시간 인슐린 치가 치료 전에 비해 치료 4주, 8주째 감소하는 경향을 보였고, 공복 인슐린이나 식후 2시간 인슐린치는 치료전 후에 의미 있는 차이가 없었다. 아카보스군에서는 치료 전후로 공복 인슐린, 식후 1시간과 2시간 인슐린치에 의미 있는 차이를 보이지 않았다. 두약제 간에 치료 전과 치료 8주사이의 식후 2시간 인슐린의 감소량이 보글리보스 군에서 의미있게 높았다(p=0.040). 3)당화혈색소:보글리보스 군은 치료 전에 비해 치료후 당화혈색소가 감소하는 경향을 보였고, 아카보스군은 치료 전에 비해 의미있게 감소하였다. 당화혈색소 변화량은 두 군간에 의미 있는 차이는 없었다(p=0.412). 4)지질대사의 변화:중성지방, 콜레스테롤, 고밀도 진단백 콜레스테롤에 대해 두 군간에 의미 있는 차이는 없었다. 5)부작용:소화기계 부작용의 빈도는 치료 4주째 보글리보스 군에서 의미 있게 낮았으나(p=0.028), 치료 8주째 부작용의 빈도는 두 군간에 의미 있는 차이가 없었다(p=0.215). 결론:2형 당뇨병 환자에서 보글리보스와 아카보스의 두 약제의 임상적 유효성과 부작용발현에 대해 비교한 결과 치료 후 8 주 후 혈당강화효과는 두 약제간에 유사한 효과를 보였으나 보글리보스군에서 4주째의 초기 위장관 부작용이 적었다. Background : Acarbose and voglibose are alpha-glucosidase inhibitors. Although different pharmacological effects and adverse abdominal events associated with the two drugs have been reported, no study directly compared acarbose and voglibose in diabetes has been undertaken. To compare the pharmacological effects and gastrointestinal adverse events between two drugs, a randomized, placebo-controlled, double-bind study was performed in type 2 diabetes patients. Methods : The period of study was 12 weeks(observation period: 4 weeks; treatment period: 8 weeks). Fifty-three patients were randomized into two groups(the acarbose group: 24 patients; the voglibose group: 29 patients). The serum glucose, insulin, fructosamine, HbA_1c, cholesterol, triglyceride and the incidence of adverse events were measured. Results : 1) The reduction of glucose from before treatment to 4 weeks after treatment was significantly higher in the acarbose group, but the change before treatment and 8 weeks after treatment in the two groups was similar(p=0.569). 2) The insulin significantly decreased after voglibose treatment(p=0.040). 3) HbAa_1c level tended to decrease in voglibose group, and there was a significant decrease after acarbose treatment. However, the change in HbA_1c level before and after treatment was similar between the two groups(p=0.412). 4) The two drugs did not cause any other changes in the total, HDL-cholesterol and triglyceride. 5) The number of patients with gastrointestinal adverse events was significantly low 4 weeks after voglibose treatment (p=0.049), but the incidence in the two groups was similar after 8 weeks(p=0.215). Conclusions : Acarbose and voglibose significantly improved postprandial hyperglycemia in diabetes. The incidence of gastrointestinal adverse events was low 4 weeks after voglibose treatment(J Kor Diabetes 26:134~145, 2002).

      • 자동차용 실리콘 가속도센서의 개발(Ⅱ)

        자동차의 air-bag 장치에 실용될 수 있는 압저항형 단결정 실리콘 가속도센서 칩을 개발하기 위하여 결정 실리콘 미세구조의 제조방법을 확립하고, 단위공정의 검증을 통하여 일괄공정에 의한 PROTO-TYPE 칩을 만드는 기술을 연구하였다. 단결정 실리콘 미세구조는 선택확산법을 이용하여 정확히 선택된 영역에만 air-gap을 형성하여 미세구조의 측면식각을 방지하는 선택확산법에 의한 실리콘 마이크로머시닝 기술로 제조하였다. 일괄공정을 위한 단위공정확립을 위하여 PROTY-TYPE 8빔 브릿지형 가속도 센서를 제조하였다. 제조된 칩의 가속도에 따른 출력전압은 선형성을 나타내고 있으며, 감도는 약 50 ㎶/V·g로 나타났다. 이 감도는 50G용 가속도센서의 사양을 만족하지 못했다. 이는 공정에 의한 문제라기 보다는 가속도센서의 시뮬레이션에 의해 설계한 구조가 이미 원하는 감도에 못 미친다는 것으로 생각된다. 따라서 2차 공정으로 제조될 가속도센서의 파라미터를 SuperSAP 유한요소 패키지를 이용하여 실리콘 미세구조부의 파라미터에 따른 특성을 시뮬레이션하였다. 설계된 50G용 가속도센서의 mass Pad의 반경 및 빔 길이, 빔 폭, 빔 두께, 그리고 mass의 각 파라미터 값은 700 ㎛, 120 ㎛, 5 ㎛, 1.0 ㎎ 이었다. 반도체 공정기술, 관성질량 제조법 및 선택확산을 이용한 마이크로머시닝을 사용하여 일괄공정으로 8빔 브린지형 가속도센서를 제조하였다. We researched the establishment of the silicon microstructure fabrication technique to develop a piezoresistive type silicon acceleration sensor chip and the technique to make a proto-type chip by the verification of the unit-process. Silicon microstructure is fabricated silicon micro-machining by selective diffusion method. This method prevent a side-etching of microstructure because selective diffused region is only formed an air-gap. We fabricated a proto-type 8-beam bridge-type acceleration sensor to establish the unit-process for the batch-process. The output voltage of the chip represented linearity with acceleration, and the sensitivity was about 50 ㎶/V·g. But this sensitivity dosen't satisfy the requirements of a practical acceleration sensor. The cause of this result is assumed not process problem, but the structure designed by simulation isn't suitable already. Threfore, the characteristics of parameters of the acceleration sensor that will be fabricated by 2nd-process is simulated by SuperSAP finite-element package. The determined parameter values of beam length, beam width, beam thickness, mass, and mass radius are 120 ㎛, 5 ㎛, 1.0 ㎎, and 700 ㎛, respectively. We fabricated 8-beam bridge-type acceleration sensor by batch-process using a semiconductor process technique, proof-mass fabrication method, and micromachinig using selective diffusion.

      • 소라, Turbo cornutus SOLANDER의 棲息場 環境과 集團成長에 關하여

        1977년 6月부터 12月까지 西歸浦 沿岸에 位置한 숲섬,甫木里,法還里의 3個 養成場을 對象으로 海洋基礎生態學的 要因 및 標識放流한 소라의 成長과 移動등을 調査하였다. 1. 各 養成場의 水溫 및 比重의 垂直的 變化는 보이지 않았고 숲섬 養成場이 多少 低溫을 나타냈다. 2. 숲섬 養成場의 海低地型은 B,C형태이고 甫木里 養成場은 A形態,甫木里 養成場은 C, D形態가 主로 構成되어 있다. 3. 各 養成場의 流速은 숲섬 養成場이 가장 빨라 4∼20 ㎝/sec였고, 法還里 養成場은 1∼10㎝/sec로 緩慢한 流速을 보였다. 4. 西歸浦 沿岸의 吸收係數는 0.218∼0.329(평균 0.283)이였고 숲섬, 法還里, 甫木里 順으로 작았다. 5. 調査된 海藻相 中 各 養成場別 優點種은 共히 모자반, 감태, 석회조, 클라도포라 등이였으며 특히 法還里 養成場은 모자반이 優勢하게 分布했다. 6. 標識放流한 소라의 成長度는 6月부터 10月까지 빠른 傾向을 보였고,養成場別 成長速度는 甫木里,숲섬,法還里의 順이었다. 7. 標識放流期間 동안 具殼 新生部에 形成된 棘數는 2∼3個가 가장 많았다. 8. 소라의 移動거리는 各 養成場 共히 5∼25m의 範圍가 가장 높은 頻度를 보였다. Studies on the growth of topshell which had tagging liberated and ecological survey were taken from the three culturing topshell forms of the Seogwipo coast form June to December, 1977. Three culturing topshell farms are Supseom, Bomkri and Beophwanri coat. The culturing topahell farms are shallow with the mean depth 5∼8m, and vertical changes of water temperature and specific gravity in each arears were not appeared. The water temperaure of Supseom area was lower than the other areas. The compositions of submarine geological type in Supseom, Bomokri and Beophwanri area were B (Hole type) and C (crevice type) type, A (Rock type) type, and C and D (Sand type) type respectively. The Current speeds of each culturing farm area were in the range of 1∼20㎝/sec, and the current of Beophwanri culturing farm area was slower than the other areas. The absorption coefficient of Seogwipo coastal water in the range of 0.218~0.329 and the mean was 0.283. Fifty species of marine algae were identified. Major algal group of each culturing farms were Sargassum sp.,Ecklonia cava, Chladophora sp., calcareous algae. Particularly, Beophwanri culturing farm was abundant in Saragassum sp. Of the three culturing topshell farms increment in growth of topshell were showed rapid growth from June to October and noted that increment in growth of Bomokri was the highest. During the tagging liberated period, the number of spines on newly formed parts of shell was in the range of 0∼5 spines. The creeping distance was 5∼25m in each culturing topshell in all.

        성견에서 수직적 골결손부에 골이식후 교정력 적용시기에 따른 치주조직의 반응

        본 연구는 수직적 골결손부에 골이식을 시행한 후 교정력 적용시기에 따른 치주조직의 반응을 알아보고자 시행하였다. 체중 15 kg 내외의 성견 9마리에서 상악 좌우측 제3절치의 원심 치근면에 수직적 골결손을 형성하고 치주질환을 유발시킨2주 후 치주판막술을 시행하고 골결손 기저부 수준의 치근면에 기준 흠을 형성하였다. 골이식 후 치유기간에 따라 2주군, 4주군, 6주군으로 각각 3마리씩 분류하고 우측 제3절치는 치주판막술만 하는 비이식측으로, 좌측 제3절치는 치주판막술과 골이식을 병용한 이식측으로 설정하여 양측 모두 술후 2주, 4주, 또는 6주부터 120 gm의 교정력을 적용하여8주 동안 치체이동 시킨 후2주간 보정하였다. 이후 동물을 희생시키고 조직표본을 만든 후 치주조직의 반응을 비교 관찰하여 다음과 같은 결과를 얻었다. 1. 모든 실험동물에서 비이식측에 비해 이식측의 골재생양이 많이 나타났다. 2. 6주군에서는 홈에서 백악-법랑 경계부까지 거리의 1/2이상 치조골과 백악질이 재생되었고 상피의 하방증식없이 결합조직대가 치근면과 부착을 이루었다. 3. 4주군에서는6주군과 유사한 치주조직 재생소견을 보였으나 그 양은 더 적게 나타났다. 4. 2주군의 경우 골재생이 양측 모두 홈에 한정되어 나타났으며 상피의 하방증식이 홈의 기저부까지 연장되고 백악질과 결합조직의 재생은 관찰되지 않았다. 이상의 결과는 치주질환에 의한 수직적 골결손 환자에서 치주판막술과 골이식을 시행하고 교정적 치아이동을 할 경우 교정력 적용시기에 따른 치주조직의 반응이 차이가 있음을 시사하였다. (주요 단어 : 수직적 골결손, 골이식, 교정력 적용시기, 치주반응) The aim of this study was to investigate the periodontal response according to the timing of orthodntic force application after bone graft into the angular bony defect. Nine dogs were divided into three groups, 2, 4, and 6 weeks, according to the timing of orthodontic force application after bone graft. Periodontal angular bony defects were created surgically at the distal aspect of both maxillary third incisors. Two weeks later, flap operation was performed to eliminate inflammation and reference notch was made on the root surface at the level of the bottom of each defect. Demineralized freeze-dried bone was implanted on the left side whereas only debridement was done on the other side. Experimental tooth movement was executed during 8 weeks on both graft and non-graft sides. After 2 weeks of retention period, animals were sacrificed for histologic specimens. The results were obtained as follows : 1.New bone formation was more pronounced in the graft side than in the non-graft side in all experimental animals. 2.In the 6-week group, new bone and cementum formation was observed in more than half from the notch to the cemento-enamel junction, and the zone of connective tissue attachment was found without apical migration of junctional epithelium. 3.In the 4-week group, the amount of new bone formation was smaller than in the 6-week group whereas the overall remodeling pattern was similar. 4.New bone formation was confined to around the notch and the junctional epithelium migrated apically to the level of the notch with no connective tissue attachment and cementum formation in the 2-week group. The results of the present study suggest that periodontal response may be influenced by the timing of orthodontic force application after bone graft into angular bony defect.       • 교사가 지각한 유치원 원장의 변혁적 리더십이 교사의 직무만족도, 직무 스트레스 및 직무효과성에 미치는 영향

        This study aims to find out the effect of the transformational leadership of the principal of a kindergarten recognized by a teacher on the job satisfaction, stress and effectiveness of a teacher, To do that, the author circulated questionnaires to kindergarten teachers of Daegue · Gyeongbuk, and the collected materials was analyzed by regression analys is using SPSS 120 program. As a analysis result, it was found that the transformational leadership of a kindergarten principal recog-nized by a kindergarten teacher had a positive effect on the job satisfaction and effectiveness of a teacher and a negative effect on job stress of a teacher. That is, the transformational leadership of a kindergarten principal was found to increase the job satisfaction and effectiveness of a teacher and to lower the job stress of a teacher and to lower the job stress of a teacher.

        정서 지능 검사의 타당화 연구

        This study is intended to verify an appropriateness of a test tool for elementary school students, which was made by Oh Suk-Yeong(1966) and modified by Jang Jeong-Ae (1998), in order to examine the test tool of the emotional intelligence for elementary school students, as a part of providing the basic data for a development of a new test tool. The survey was conducted for 250 students in the fifth grade and 150 students in the sixth grade, except dishonestly-responded 12 students among total 412 students of 'P' and 'S' Elementary School located in Dalseo-gu, Daegu. The following is the result of looking into a reliance of the Emotional Intelligence test through statistics of the emotional intelligence test. Firstly, the overall average answer rates were similar. However, there was a high variation of a minimum value and maximum value. Secondly, students in the fifth grade had higher score of the emotional intelligence than students in the sixth grade for all areas except the emotional control, as the result of examining the difference o the emotional intelligence score by grades. In terms of the difference of the emotional intelligence score by gender, female students had higher average score than male students for all areas except a repletion. Thirdly, there was the different reliance on the test tool of the emotional intelligence between male and female students. However, the overall reliance by grade was similar. Next, the survey indicated that there was a low relation in terms of a relationship between the whole and items. However, the appropriateness of individual item was low. Finally, there was the relatively low relation between sub-areas, as the result of looking into the appropriateness of the emotional intelligence test. However, it showed the relatively high relation between sub-area and the whole.

Therefore, the development of detector with high performance is very important in pulling up the resolution of SEM This study presents the secondary electron detector for use in scanning electron microscope, electric circuit and I/V conversion circuit for driving that detector.

      • 반응중심 문학활동이 유아의 조망수용능력에 미치는 영향

        The purpose of this study was to identify the effect of response centered litera-ture activity on children's perspective taking, To demonstrate the effect of response centered literature activity, 65 children at the age 5 were subjected to this study, and they were to 2 groups and were each located to experiment group who were to go through 24 response centered literature activity and comparison group who were read 8 picture books. This study surveyed the children's per-spective taking, the pre-test, post-test to examine the effect of response cen-tered literature activity. The data collected from the pre-test and post-test. ANCOVA procedure was conducted to analyze the effects of the experimental activities compared to the comparison group. The test results demonstrated that children in the experiment group made significant improvement on perspective taking. In conclusion, these activities were designed to correspond to children's experience, interest and demands by using of picture books, it is highly expected to be able to used in the field as useful social and emotional education.

      • 피부 적용 겔제제의 첨가제에 의한 수분 보유력 평가

        The aim of this study was to improve the moisturizing and water retaining effects of a gel containing poloxamer 407, glycerin, sod hyaluronate, sorbo(sorbitol 70%). Topical preparations were formulated as a gel containing different concentration poloxamer 407, glycerin, sod. hyaluronate, sorbo(sorbito1 70%), lipid(ceramide. sphingosine ratio 1:l). We tested a various combination of different additives for the moisturizing effect The water holding capacity of the mixture was measured using a moisture analyzer. In vivo test was carried out on human skin using Skicon-200 and water retaining effect was increased by addition of lipid. Water evaporation test was performed by various concentration of additives and water evaporation characteristics of the preparations were determined using the moisture analyzer. Moisturizing and water retaining effects was increased by adequate combination of various additives with topical gel formulations.
