        This study was attempted to identify trends and determinants of the profitability of Korean Provincial Hospitals different. Data used in this study were chosen from provincial hospitals that passed the standardization audit undertaken by the Korean Hospital Association from 1998 to 2001 for the purpose of accrediting training hospitals. There were collected from 19 provincial hospitals with complete general data of present conditions as well as financial statements(balance sheets, income statements). The dependent variables in this study were used net profit to total assets and operating profit to gross revenue as a proxy indicator for profitability. The independent variables were classified by general factors(location, teaching status, number of beds, period of establishment), operating and financial factors(current ratio, fixed ratio, liabilities to total assets, total asset turnover, average length of stay, bed occupancy rate, admission ratio of outpatients, patients per specialist, personnel costs per adjusted inpatient, material costs per adjusted inpatient, administrative costs per adjusted inpatient), in addition time leg effect(fiscal years). As a result of hierarchical multiple regression analyzation of net profit to total assets and operating profit to gross revenue, with general factors, operating and financial factors, adjusted R² of operating and financial factors is very powerful. The factors had significant positive effect on net profit to total assets were period of establishment, current ratio, admission ratio of outpatients. The factors had significant negative effect on net profit to total assets were average length of stay, personnel costs per adjusted inpatient, fiscal years. The factors had significant positive effect on operating profit to gross revenue were teaching status, fixed ratio, total asset turnover. The factors had significant negative effect on operating profit to gross revenue were location, liabilities to total assets, personnel costs per adjusted inpatient.

      • Sheehan 증후군 발생한 Torsades de Pointes

        재발하는 심실 빈맥으로 내원한 환자에서 Sheehan 증후군을 진단하였고, 적절한 호르몬 보충 치료 후 심전도 이상이 정상으로 회복된 예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Torsades de Pointes is a form of polymorphic ventricular tachycardia that is associated with prolongation of QT intervals, characterized by QRS complexes of progressively changing ampulitude and cyclic length, giving the appearance of oscillations around the baseline. A 77 years-old woman was presented with hypotention and arrhythmia. Twenty four-hour ECG showed typical episodes of torsades de pointes with QT prolongation. This arrhythmia was controlled by isoproterenol and magnesium infusion, but it was not disappeared. She was diagnosed as Sheehan's syndrome by past history, anterior pituitary stimulation test and empty sella on CT scan. After hormone replacement treatment, Prolonged QT interval was normalized and polymorphic ventricular tachycardia has not recurred. We report a case of torsades de pointes associated with Sheehan's syndrome

      • 석청(Mad-honey) 복용 후 유발된 부정맥 1예

        석청에 포함된 grayanotoxin에 의해 독성 작용이 나올 수 있으며, 이러한 독성 작용은 일반적으로 24시간 이내 저절로 회복된다. 최근에 본 저자들은 석청 복용 후 발생한 심실빈맥을 경험하였기에 보고하는 바이다. 49세 남자가 호흡곤란으로 내원하였으며, 내원당시 분당 40회 정도의 동성 서맥이 관찰되었고, 수액을 투여하던 중 심실 빈맥이 발생하였다. 항부정맥제를 투여 후 정상 동율동으로 전환되었으며, 특별한 이상 없이 4일 후 퇴원하였다. Mad-honey intoxication caused by the consumption of honey producted from the nectar of rhododendrons. The grayanotoxins cause the intoxication. The toxic effects of mad-honey poisoning are rarely fatal and generally last for no more than 24 hours. We experienced one case, a 49 years-old man who presented with dyspnea after ingestion of mad-honey. He showed marked sinus bradycardia with < 40 beats per minute on admission. The cardiac rhythm was changed to ventricular tachycardia immediately. These features resolved completely in 24 hours with continuous infusion of amiodarone(600 mg per day) and fluids. We report the case of intoxication of mad-honey as a presentation of fatal cardiac arrhythmia.

        A Comparative Study of Behavioral and immunohistological Changes after Spinal Cord Injury between Young and Adult Rats

        목적: 어린 쥐와 성숙 쥐의 척수 손상에서 나타나는 행동상과 면역 조직 화학상의 변화의 차이를 비교함으로써 척수 손상에서 나타나는 변화의 기전 차이를 설명하고자 하였다. 대상 및 방법: 생후 5주된 수컷 쥐 25마리와 16주된 수컷 쥐(Spraque-Dawley rat) 25마리가 사용되었다. 쥐들을 페노바비탈로 마취시킨 후 제 11번, 12번 흉추 위치에서 추궁절제술을 시행하였으며 개량된 뉴욕 대학 충격기(modified New York University impactor)를 이용하여 20 ㎜ 높이에서 10 gm의 충격을 줘서 척수 손상을 유발하였다. 이에 따른 배뇨장애를 방지하기 위해 매일 2회씩 손으로 눌러 방광을 비워주었으며, 수술을 받지 않은 쥐를 정상 대조군으로 사용하였다. 척수손상을 입은 쥐들에 대한 행동 시험을 Basso-Beatti-Bresnahan (BBB) 가산표를 이용하여 시행하였고, 행동 시험은 척수손상 후 제 1, 7일에 시행하였다. 쥐들의 손상된 척수를 수술 후 제 1, 7일에 절개하였으며, 척수 절편에 H-E 염색과 c-Jun과 GFAP유전자에 대한 면역조직화학 검사를 시행하였다. 면역 반응은 상온에서 1시간 동안 말초 혈액 도말 내의 avidinbiotin-peroxidase 복합체 내에서 그리고 0.05% 3,3 -diaminobenzidine과 0.01% 과산화 수소에서 5-10분간 반응 시켰다. 결과: 생후 5주된 쥐들의 뒷다리는 수술 1일 이후 마비되었고 수술 7일후에 부분적으로 회복되었으나, 생후 16주된 쥐들의 경우 수술 후 7일째에도 마비된 상태로 남아있었다. 생후 16주된 쥐들에서는 손상 받은 척수의 회백질에서 c-Jun유전자의 발현이 7일째까지 증가했으며, 생후 5주의 쥐들에서는 c-Jun유전자의 발현이 수술 1일째 상당히 증가하였으나, 회백질의 c-Jun유전자의 발현은 수상 후 7일째부터 감소하였다. 회백질의 GFAP유전자의 발현은 생후 16주의 성인 쥐들에서는 수상 후 제 1일째부터 증가되기 시작하였으나 수상 제 7일째부터 괴사부분에서 감소되기 시작하였고, 생후 5주의 어린 쥐들이 회백질에서는 GFAP유전자 발현이 수상 후 7일째까지 점진적으로 증가하였다. 결론: 나이가 많은 쥐들에서 척수 손상이 회복되지 않은 것은 척수 손상 후 c-Jun유전자 발현의 지속적인 상향조절 또는 GFAP유전자의 하향조절과 연관되어 있을 가능성을 제시한다. 반대로, GFAP유전자의 상향조절 또는 c-Jun유전자의 하향 조절은 동일한 손상을 받은 어린 쥐들에서 척수 손상의 부분 회복과 관련되어 있는 것으로 사료된다. Purpose: The aim of this study was to compare the behavioral and immunohistochemical changes in a spinal cord injury (SCI) between young and adult rats and to clarify the differences in the mechanism underlying the changes in a SCI between young and adult rats. Materials and Methods: A total of 25 young and 25 adult male Sprague-Dawley rats (5 weeks and 16 weeks old) were used. The rats were anesthetized with pentobarbital and laminectomies were carried out at the level of the 11th and 12th thoracic vertebra. Using a modified New York University Impactor, a SCI was induced by dropping a 10 gm weight at a height of 20 mm. The bladders were emptied manually twice a day to prevent urinary problems. The animals that received no surgery were used as the normal controls. Behavior tests were performed using the Basso-Beatti-Bresnahan (BBB) scoring system, 1 and 7 days after the weight drop injury. The difference in the BBB score between the young and adult rats were analyzed by a paired t-test with a p value <0.05 considered significant. The injured spinal cords were dissected at 1 and 7 days after surgery. H-E stain and immunohistochemistry for c-Jun and GFAP were performed in the spinal cord sections. The immunoreactions were visualized by incubation for 1 hour at RT in an avidin-biotin-peroxidase complex in PBS and 5-10 min in 0.05% 3,3 -diaminobenzidine and 0.01% H₂O₂ in 0.1 M PBS. Results: The hind limbs of young rats were paralyzed 1 day after surgery, but had recovered partially 7 days after surgery. However, the adult rats remained in the paralyzed status 7 days after surgery. The c-Jun expression level increased in the gray matter up to 7 days after the weight-drop injury in adult rats. The c-Jun expression level increased significantly in the gray matter 1 day after the injury in the young rats. However, the c-Jun expression level decreased significantly in the gray matter 7 days after the injury in the young rats compared with that of the 1 day post-injury. The GFAP expression level in the gray matter increased 1 day after the weigh-drop injury in the adult rats. However, it decreased in the necrotic region 7 days after the injury in the adult rats. GFAP expression in the gray matter increased gradually up to 7 days after the injury in the young rats. Conclusion: These results suggest that lack of recovery from a SCI in adult rats may be related to the continuous upregulation of c-Jun expression and/or the downregulation of GFAP after the weight-drop injury. In contrast, the upregulation of GFAP expression and/or the downregulation of c-Jun expression in the spinal cord might be related to a partial recovery in young rats after a weight-drop injury.

        변형된 ZSM - 5 촉매상에서 n - 헥산의 방향족화 반응에 관한 연구 : 1 . HZSM - 5 촉매의 Si/Al 비의 영향 1. Effect of the si / Al Ratio of HZSM - 5 Catalyst

        형상 선택성이 우수한 ZSM-5 촉매를 Si/Al 비를 달리하여 제조한 다음 n-헥산의 방향족화 반응에 적용하였다. HZSM-5 촉매상에서 n-헥산은 일단 C₁-C_5 범위의 탄화수소로 크래킹된 후 이 중에서 프로필렌과 C₄ 탄화수소가 2차적으로 반응을 일으켜 방향족 화합물을 생성시키는 것으로 관찰되었다. 방향족 화합물은 Si/Al 비가 100 이하인 HZSM-5 촉매상에서만 생성되었으며 반응온도가 상승할수록 방향족 화합물의 선택도가 높았는데 BTX중에서 톨루엔이 가장 많이 생성되었다. 크래킹 반응에 의한 저급 탄화수소의 생성과정과 방향족화 과정을 Si/Al 비 및 반응온도와 연관지어 n-헥산의 반응경로를 유추하였으며 이를 바탕으로 HZSM-5 촉매상에서 방향족화 반응에 유리한 조건을 규명하였다. ZSM-5 catalyst was synthesized with various Si/Al ratios and applied for the aromatization of n-hexane. Over HZSM-5 catalyst, n-hexane was first cracked to yield C₁-C_5 hydrocarbons, of which propylene and C₄ were converted to aromatics via secondary reactions. Aromatics were observed over HZSM-5 catalysts of Si/Al ratios below 100 and the selectivity to aromatics increased with the temperature. Among aromatics, toluene was the major product. The reaction pathways were deduced by analyzing each reaction step of cracking and aromatization in connection with the Si/Al ratio and the reaction temperature. Based on these results, favorable conditions for the aromatization of n-hexane over HZSM-5 catalyst were investigated.

        개방성 골절의 새로운 분류 및 치료

        Purpose: To make new classification system of open fractures including the status of skin because of high rates of nonunion and complication in closed fractures with extensive skin damage. Materials and methods: One hundred-twelve one patients with fixation for open tibial fractures or closed tibial fractures with skin damage were included in this study from March 1991 to January 1997. The follow up period was at least 1 years. We classified the patients according to the Gustilo &Anderson classification, AO/ASIF classification and our new classification, and we compared the bony union time, bony union rate and complication rate among these classification. Result: The bony union time was 18.6 weeks in open fractures, 14.5 weeks in closed fractures with skin damage, and 18.1 weeks in combined new classification. The rate of bony union was 81% in open fractures, 83% in closed fractures with skin damage, and 77% in combined new classification. The complication was 40% in open fractures, 27% in closed fractures with skin damage, and 42% in combined new classification. There was close matched correlation in bony union time, bony union rate and complication rate among the Gustilo & Anderson classification, AO/ASIF classification, and our new classification. In closed fractures with skin damage above IC3, the rates of bony union and complication were similar with the open fractures above grade II. Conclusion: We propose simple new classification system of open fractures into 4 classes. The class I has minimal soft tissue damage and contamination with exposed wound less than 1cm or skin contussion, the class II has moderate soft tissue damage and contamination with exposed wound from 1 to 10cm or circumferential deglobing wound, the class III has severe soft tissue damage or contamination with exposed wound more than 10cm or extensive deglobing wound, and the class IV has class III injury with vascular involvement. And in the treatment of open fractures, we propose internal fixation in class I or II, and external fixation in class III or IV.

        <P>This study examines the performance of adhesive and cohesive interfacial crack model in assessing the brittle shear behavior of reinforced concrete bridge columns. The quarter-scale reinforced concrete columns tested at the University of California at San Diego by Xiao et al. (1993) to explore the ductility of reinforced concrete columns under cyclic loading. The three columns R-1, R-3 and R-5 with different transverse reinforcement are considered for finite element failure analysis.</P>
