        Preparation of mechanically enhanced supramolecular carbon nanotube (CNT) film using a water‑soluble π–π bonding linker

        Mechanically enhanced supramolecular carbon nanotube (CNT) films were prepared in water by employing the π-electronrich phenyl, naphthalenyl, and pyrenyl end-functionalized polyethylene oxides (PEOs) as supramolecular linkers, followed by vacuum filtration. Among them, the supramolecular CNT film produced by the pyrenyl end-functionalized PEO (PEOPy) exhibited the highest mechanical strength, which was ~ 1.5–2 times higher than that of the CNT films produced using the typical dispersant, Triton X-100, although the functionality of PEO-Py was lower than that prepared using other linkers, and the content of PEO-Py in the CNT films was lower than that obtained using Triton X-100. Fluorescence and UV–Vis spectroscopy demonstrated that the improved mechanical properties of the supramolecular CNT film result from the formation of π–π interactions between the CNT and the pyrene moieties of the PEO-Py linker. Finally, the supramolecular CNT film exhibited a 40–50 dB electromagnetic shielding efficiency through hybridization with silver nanowires.

      • 골수염의 진단에 있어서의 Tc-99m Hexamethylpropylene amine oxime 백혈구 스캔의 임상적 유용성

        배경 : 골수염의 진단에 있어 Technetium-99m을 이용한 삼상 골스캔(Tc-99m MDP스캔)은 단순방사선 검사상 정상소견을 보이는 경우에 쓰이는 1차적 검사로 매우 높은 민감도를 보인다. 그러나 Tc-99m MDP 스캔은 골절이나, 정형외과적 삽입물, 신경병적 관절병변과 같이 뼈의 재형성이 증가되는 상황에서도 양성소견을 보여 특이도가 낮은 단점이 있다. 이러한 단점을 보완하고자 골수염의 진단에 있어 Tc-99m HMPAO 백혈구 스캔의 유용성을 알아보고자 하였다. 대상 및 방법 : 골수염이 의심되어 Tc-99m HMPAO 백혈구 스캔을 시행했던 15명의 의무기록을 후향적으로 분석하였다. 골수염의 확진은 임상관찰 소견과 배양검사 결과와 병리소견을 기준으로 하였다. 결과 : 골수염의 진단에 있어 Tc-99m HMPAO 백혈구 스캔의 민감도와 특이도는 각각 100%와 78%였고 Tc-99m HMPAO 스캔의 경우는 100%와 18%였다. 가장 흔한 원인균은 methicillin-resistant S. aureus였다. Tc-99m MDP 스캔에서 양성소견을 보였던 환자중 3명의 환자가 Tc-99m HMPAO 백혈구 스캔에서 음성 소견을 보였으며 이 환자들은 추적관찰상 골수염이 없는 것으로 확인되었다. 결론 : Tc-99m HMPAO 백혈구 스캔은 골수염의 존재를 진단하는데 있어 민감도와 특이도가 높으며, 특히 뼈의 재형성이 증가되어 Tc-99m MDP 스캔의 특이도가 낮게 나타나는 상황에서 유용하리라 사료된다. Background Three phase bone scintigraphy, performed with technetium-labeled diphosphonates (Tc-99m MDP scan), is the very sensitive radionuclide procedure of choice for diagnosing osteomyelitis. But, Tc-99m MDP scan is less specific when bone remodeling is increased. To overcome this limitation, we evaluated the diagnostic value of technetium-99m hexamethylpropyleneamine oxime (HMPAO) labeled leukocyte scan in osteomyelitis. Methods : We retrospectively reviewed 15 patients with suspected osteomyelitis who performed Tc-99m HMPAO labeled leukocyte scan. The diagnosis of osteomyelitis was based on clinical course and bacterial culture, and pathologic findings. Results The sensitivity and specificity of Tc-99m HMPAO leukocyte scan was 100% and 78%, and those of Tc-99m MDP scan was 100% and 18%, respectively. The most common organism was Methicilein-Resistant S. aureus. Among the patients that showed persistent accumulation on Tc-99m MDP scan, three patients showed no uptake on Tc-99m HMPAO leukocyte scan and these patients were confirmed that had no osteomyelitis. Conclusion : Tc-99m HMPAO leukocyte scan is a useful test for determining the presence or absence of osteomyelitis, especially under condition that bone remodelin is increased.

        구내 접근법에 의한 하악골 골절 치료에 대한 임상적 고찰

        This is a retrospective study on the transoral approach to open reduction of the Mandibular fractures. Our study was based on a series of 64 patients with mandibular fractures among 99 patients of facial bone fractures who had been treated by transoral approach with or without extraoral approach at Department of Oral and Maxillofacial Surgery, Yonsei Medical Center, Yonsei University from January 1981 to October 1988. We studied favorite sites of open reduction, fixation methods, results and prognosis related to transoral approaches of Mandibular fractures, and which compared with extraoral approaches. The results obtained are as follows : 1. The transoral open reduction was used more frequently in Mandibular fractures(64.6%) than Midfacial bone fractures(35.4%). Among 64 patients of mandibular fractures, 47 patients(73.4%) were treated only by transoral approach and others(26.6%) were treated by both trans and extraoral approach. Among 92 sites of mandibular fractures 75 sites(81.5%) were treated by transoral approach and 17(18.5%) were treated by extraoral approach. 2. The most favorite site for transoral approach compared with extraoral approach was symphysis(100%), and Angle(62.5%) was next in order of frequency on Mandibular fractures. 3. Direct Interosseous Wiring(DIW) was most commonly used for fixation(64.6%) and Miniplate osteosynthesis was used next in 28.1%. 4. Simple(39.1%) and Compound(52.2%) fractures were frequently indicated for transoral approach, however comminuted fractures were rarely indicated. 5. The direction of fracture lines on Angle of the mandible did not influence to determine whether transoral approach should be selected or not. However this area seemed to be more difficult to reduce exactly by transoral procedure than other areas because simultaneous superior and inferior fixation was applied predominantly on this area. 6. The success rate of reduction and fixation analyzed from us was more excellent in Direct Interosseous Wiring(29/53=54.7%) than in Rigid Internal Fixation(9/29=31.0%). But it might be depended upon various factors as like as sugeon's skill. 7. The postoperative complication due to transoral open reduction of mandible was not high(12.0%) and this rate was similar with other published reports.

        기저질환이 없는 사람에서 발생한 Campylobacter fetus에 의한 재발성 심내막염 1예

        C. fetus 심내막염은 드물게 발생하는 질환으로 주로 면역능이 저하된 환자에서 발병하고 대동맥판을 침범하는 경우가 많으며 적절한 치료가 되지 않을 경우 치사율이 높다. 저자들은 기저질환이 없던 69세 남자에서 C. fetus 심내막염을 경험하였기에 문헌고찰과 함께 보고하는 바이다. 환자는 발열로 내원하여 시행한 혈액배양 검사상 C. fetus가 동정되었고 경식도 초음파 검사상 삼첨판에 증식증이 발견되어 심내막염 진단하에 ceftriaxone에 이어 meropenem으로 총 4주간 항생제를 투여받고 호전되었다가 한달 후 다시 재발하였고 판막부전 소견 진행하여 삼첨판 치환술을 시행받고 전신상태 호전되어 퇴원하였다. Campylobacter fetus is a rare cause of endocarditis. To date, only about 30 cases of C. fetus endocarditis have been reported mainly in the elderly with chronic underlying illnesses. Most cases showed tendency to involve aortic valve. This report describes a case of native tricuspid valve infective endocarditis in a 69-year-old man without any underlying disease. Despite adequate antibiotic therapy for 4-weeks, endocarditis recurred and leading to surgical valve replacement.

      • 페니실린 내성 폐렴구균 균혈증의 임상적 특성

        목적: 지역사회 획득 감염의 매우 흔한 원인균인 폐렴구균의 페니실린 및 다른 항균제에 대한 내성의 증가는 세계적으로 문제가 될 뿐 아니라 국내에서 더욱 큰 문제로 대두되고 있다. 본 연구는 항균제 내성 여부에 따른 폐렴구균 균혈증의 임상적 특성 및 위험 요인를 분석하여 항균제 내성의 임상적 의미를 조사하고자 시행되었다. 방법:1989년부터 1994년까지의 폐렴구균 균혈증 41례를 대상으로 하여 의무기록지를 조사하였다. 균주의 항균제 감수성 검사는 oxacillin 디스크 확산법으로 페니실린 내성 여부를 거사한 후, 한천 희석법을 이용하여 페니실린을 비롯한 11개의 항균제의 MIC (minimal inhibitory concentration)를 구하고 감수성 여부를 판정하였다. 결과: 41균주 중 68.3%가 페니실린에 대하여 내성(중등도 12.2%, 고도 56.1%)을 보였으며 다제내성률은 61%였다. 특히 소아에서는 100%(고도 88.2%)의 내성률을 보여 성인보다 내성률이 더 높았다. 페니실린 내성 균주는 대부분 다른 β-lactam 항균제에도 내성을 보였으며 특히 페니실린 고도 내성 균주는 cefotaxime, ceftriaxone에 대하여 100% 내성을 보였고 imipenem에 대해서도 95.5%의 중등도 내성률을 보였다. 혈청형은 19와 23이 많았고 그 외에 6이 있었다. 임상적 특성의 분석상 환자들의 연령분포는 3개월부터 82세까지였으며 기저질환을 가지고 있는 환자가 56%이었으며 지역사회 획득 감염이 71%였다. 균혈증의 일차감염원은 폐렴(22례, 54%)과 수막염(5례, 12%)이 가장 많았다. 페니실린 내성의 위험요인으로는 소아 연령만이 유의한 요인으로 분석되었다. 항균요법에 대한 반응을 볼 때 실패한 경우는 고도 내성 군의 20%에서만 있었으며 병원내 총사망률은 페니실린 감수성 군이 30%, 중등도 내성 군이 20%, 고도 내성 군이 13.6%로서 유의한 차이는 없었다. 결론: 페니실린 내성 폐렴구균은 국내에 만연하고 있으며 높은 다제내성률을 보였다. 이들은 대개 지역 사회 획득 감염을 일으키는데 균혈증의 경우 그 사망률은 내성 여부에 따른 차이를 보이지 않았다. 소아연령만이 페니실린 내성의 위험요인으로 분석되었다. Background: The emergence of penicillin-resistant Streptococcus pneumoniae (PRSP) poses serious therapeutic problem in clinical practice, especially in cases with bacteremia or meningitis. Methods: To investigate clinical characteristics of bacteremic pneumococcal diseases due to PRSP, we retrospectively analyzed 41 cases with cocumented bacteremic diseases seen in a tertiary care hospital between 1989 and 1994. Results: Agar dilution test of 41 strains isolated showed that 68.3% of S. pneumoniae were PRSP [high-level resistance(R) 56.1%, intermediate resistance(I) 12.2%]. High-level resistant strains were not susceptible to other β-lactam agents, whereas isolates of penicillin-susceptible S. pneumoniae(PSSP) were uniformly susceptible to all β-lactam agents. Predominant serogroups of PRSP were 19, 23, and 6. Bacteremic diseases caused by PRSP included pneumonia (22), meningitis (5), peritonitis (3), acute otitis media (2), acute tonsillitis (2), endocarditis (1), pyelonephritis(1), and primary bacteremia(5). Children were more likely to be infected with PRSP with high-level or intermediately resistant strains than were adults(P=0.0001), but no differences were seen between PRSP and PSSP regarding sex. previous antibiotic history, previous hospitalization, and underlying immunocompromised conditions. Most cases were community-acquired (R 78.3%, I 60%, S 61.5%). Fatality rates of patients infected with PRSP were not different significantly from PSSP (22.6% vs. 30%). However, most fatal cases had underlying immunocompromised conditions. Conclusion: PRSP is widely prevalent in Korea and shows resistance to most antibiotics. It causes community-acquired bacteremic diseases with poor outcomes, but there was no difference in mortality between patients infected with PRSP and PSSP. Pediatric age was the only risk factor for penicillin resistance.

        The phenomenon of chatter is a violent relative vibration between workpiece and tool in machining of metals, and is an important limiting factor of production rate and surface quality, and also reduces the life of machine-tool and tool. In this study, for the prevention of chatter, the tendency of vibration of cutting force and horizontal deflection of workpiece at the chatter occurring point was observed, and some methods to determine the chatter occurring point were suggested, and by these methods, the maximum chatter free length of cantilever type workpiece in lathe was determined according to the variation of workpiece diameter and cutting depth.
