<P><B>Purpose</B></P><P>This study sought to determine the 1-year clinical effectiveness and safety of a biodegradable, polymer-containing Biolimus A9™-eluting stent (BES) in Korean patients with acute coronary syndrome (ACS).</P><P><B>Materials and Methods</B></P><P>A total of 1000 ACS patients with 1251 lesions who underwent implantation of BESs at 22 centers in Korea were enrolled between May 2011 and July 2013. We assessed major adverse cardiac events (MACE) defined as the composite of cardiac death, non-fatal myocardial infarction (MI), and clinical-driven target vessel revascularization at 12 months.</P><P><B>Results</B></P><P>Patient mean age was 62.6±11.4 years. 72.8% of the patients were male, 28.5% had diabetes, 32.8% had multi-vessel disease (MVD), and 47.9% presented with acute MI (AMI). The mean global registry of acute coronary events risk score of all patients was 103.0±27.6. The number of stents per patient was 1.3±0.6. The incidences of MACE and definite stent thrombosis at 12 months were 3.9% and 0.2%, respectively. On multivariate Cox-regression analysis, age ≥65 years was identified as an independent predictors of 1-year MACE (hazard ratio=2.474; 95% confidence interval=1.202−5.091). Subgroup analyses revealed no significant differences in the incidence of MACE between patients with and without diabetes (4.3% vs. 3.7%, <I>p</I>=0.667), between those who presented with and without AMI (4.4% vs. 3.4%, <I>p</I>=0.403), and between those with and without MVD (4.6% vs. 3.5%, <I>p</I>=0.387).</P><P><B>Conclusion</B></P><P>Our study demonstrated excellent 1-year clinical outcomes of BES implantation in patients at low-risk for ACS.</P>
Long-term postresection prognosis of primary neuroendocrine tumors of the liver

Purpose: Primary hepatic neuroendocrine tumor (PHNET) is a very rare neoplasm, requiring strict exclusion of metastasis from possible extrahepatic primary sites for its diagnosis. Methods: We reviewed our clinical experience of 13 patients with primary hepatic NET who underwent liver resection from January 1997 to December 2015. Results: The mean age of the 13 patients (8 males and 5 females) was 51.1 ± 12.8 years; the most common clinical manifestation was vague, nonspecific abdominal pain (n = 9). Of them, 11 patients underwent preoperative liver biopsy, 7 of which correctly diagnosed as neuroendocrine tumor (NET). Ten patients underwent R0 resection, and 3 underwent R1 resection. Diagnosis of PHNET was confirmed both immunohistochemically and by absence of extrahepatic primary sites. All tumors were single lesions, with a mean size of 9.6 ± 7.6 cm and a median size of 4.3 cm; all showed positive staining for synaptophysin and chromogranin. During a mean follow-up period of 95.1 ± 86.6 months, 7 patients died from tumor recurrence, whereas the other 6 remain alive to date, making the 5-year tumor recurrence rate 56.0% and the 5-year patient survival rate 61.5%. When confined to R0 resection, 5-year recurrence and survival rates were 42.9% and 70.0%, respectively. Univariate analysis showed that Ki-67 proliferative index was the only risk factor for tumor recurrence. Conclusion: PHNET is a very rare tumor with no specific clinical features, and its final diagnosis depends primarily on pathology, immunohistochemistry, and exclusion of metastasis from other sites. Aggressive surgical treatment is highly recommended for PHNET because of acceptably favorable postresection outcomes

Myocardial bridge is characterized by systolic compression of a portion of the coronary artery by a segment of overlying myocardium, commonly involves the middle segment of left anterior descending coronary artery. We present a case report of coronary angiographic evidence of systolic short segmental constriction in the proximal right coronary artery (RCA). A 42 year-old man presented with atypical chest discomfort and palpitation. Electrocardiography showed complete AV block with complete AH block on intracardiac electrocardiogram. Myocardial SPECT imaging demonstrated a mild reversible anterior and inferior wall perfusion defect. Diagnostic coronary angiogram revealed short segmental myocardial sling with wedge-shaped systolic compression in the proximal RCA.
Accounting for Changes in House Prices and Rent in Korea, 2001-2016

We quantitatively evaluate the relative contribution of macroeconomic fundamentals and housing-related policies to the changes in real house prices, rent, and household welfare in Korea. We show that the observed changes in real house prices and rent in 2001-2016 are mainly attributed to a decrease in the real interest rate, an increase in real income, and an increase in the aggregate housing supply. However, housing-related policies turn out to have little impact on the price changes, contrary to the common belief that such policies significantly affected the housing market over this period. We also find that the welfare implications of fundamentals and housing-related policies vary by household income.

<P><B>Abstract</B></P><P>A mutant strain of <I>Streptomyces venezuelae</I> was engineered by deletion of the entire gene cluster related to biosynthesis of the endogenous deoxysugar (TDP-<SMALL>D</SMALL>-desosamine) and replacement with genes required for biosynthesis of an intermediate sugar (TDP-4-keto-6-deoxy-<SMALL>D</SMALL>-glucose) or an exogenous sugar (TDP-<SMALL>D</SMALL>-olivose), from the oleandomycin and urdamycin deoxysugar pathways. The ‘sugar-flexible’ glycosyltransferase (DesVII) was able to attach the intermediate sugar and the new sugar to both 12- and 14-membered macrolactones thus producing quinovose or olivose glycosylated 10-deoxymethynolide and narbonolide, respectively. In addition, hydroxylated analogs of the new metabolites were detected. These results demonstrate a successful attempt of engineering the deoxysugar pathway for generation of novel hybrid macrolide antibiotics.</P>
