      • 실험적 치은염에서 dextranase 함유 구강 세정액의 양치 효과

        A novel glucanhydrolase from a mutant of Lipomyces starkeyi(KSM 22) has been shown effective in hydrolysis of mutan, reduction of mutan formation by Streptococcus mutans and removla per-formed sucrose-dependent adherent microbial film and Lipomyces starkeyi KSM 22 dextranase has been strongly bound to hydroxyapatitie. These in vitro properties of Lipomyces starkeyi KSM 22 dextranase are desirable for its application as a dental plaque control agent. This study was performed to determine oral hygiene benefits and safety of dextranase(Lipomyces starkeyi KSM 22 dextranase)-containing mouthwash in human experimental gingivitis. This 3-week clinical trial was placebo-controlled double-blind design evaluating 1U/㎖ dextranase mouthwash and 0.12% chlorhexidine mouthwash. A total 39 systemically healthy subjects, who had moderate levels of plaque and gingivitis were included. At baseline, 1, 2 and 3 weeks, subjects were scored for plaque(Silness and Lo¨e plaque index and plaque severity index), gingivitis (Lo¨e and Silness gingival index), and at baseline and 3 weeks of experiment, subjects were scored for plaque(Turesky-Quigley-Hein's plaque index and plaque severity index), tooth stain(Area and severity index system by Lang et al). Additionally, oral mucosal examinations were performed and subjects questioned for adverse symptoms. Two weeks after pre-experiment examinations and a professional prophylaxis, the subjects provided with allocated mouthwash and instructed to use 20-ml volumes for 30s twice daily without toothbrushing. All the groups showed significant increase in plaque accumulation since 1 week of experiment. During 3 weeks' period, the dextranase group showed the least increase in plaque accumulation of Silness and Lo¨e plaque index, compared to the chlorhexidine and placebo groups, but chlorhexidine group showed the least increase in plaque accumulation by Turesky-Quingley-Hein's plaque index. As for gingival inflammation, all the groups showed significant increase during 3 weeks of experiment. The groups showed significant increase during 3 weeks of experiment. The dextranase group also showed the least increase in gingival index score, compared to the chlorhexidine as well as the placebo groups. Whereas the tooth stain was increased significantly in the chlorhexidine group, compared to the baseline score and the placebo group since 3 weeks of mouthrinsing. It was significantly increased after 3 weeks in the dextranase group, still less severe than the chlorhexidine group. As for the oral side effect, the dextranase group showed less tongue accumulation, bad taste, compared to the chlorhexidine group. From these results, mouthrinsing with Lipomyces starkeyi KSM 22 dextranase was comparable to 0.12% chlorhexidine mouthwash in inhibition of plaque accumulation and gingival inflammation and local side effects were if anything less frequent and less intense than chlorhexidine in human experimental gingivitis. All data had provided positive evidence for Lipomyces starkeyi KSM 22 dextranase as antiplaque agent and suggested that further development of dextranase formulations for plaque control are warranted.

      • 수면 무호흡증 환자의 진단방사선학적 고찰

        Our purpose was to determine whether there are statistical differences in upper airway size and compliance when patients with obstructive sleep apnea are compared with healthy volunteers using ultrafast CT, and to evaluate the diagnostic accuracy of ultrafast CT in obstructive sleep apnea. 29 patients clinically suspected of obstructive sleep apnea and ten healthy volunteers underwent examination of the upper airway with ultrafast computed tomography. All patients had operations; uvulopalatopharyngoplasty (UPPP,n = 19), UPPP with tosillectomy (n = 7) and UPPP with septoplasty (n = 3). Eight axial images every 0.7 sec in 8 ㎜ sections were obtained using flow mode. CT was performed twice while the subjects were awake with quiet breathing and were asleep with apnea. Smallest cross-sectional area in the nasopharynx, oropharynx and hypopharynx and compliances were assessed in patients and control subjects. Among the 29 patients, 22 patients showed narrowing at oropharyngeal level and six patients showed narrowing at two or more levels. Patients with obstructive sleep apnea had significantly smaller mean cross-sectional area than obstructive sleep apnea had significantly smaller mean cross-sectional area than control subjects in the oropharynx while sleeping (p<0.05). But there was no significant difference in mean smallest cross-sectional area of the nasopharynx (p = 0.47). In addition, the patients with obstructive sleep apnea had greater compliance of the oropharynx (79.8% vs 39.3%) and hypopharynx (58.9% vs 19.2%) (p<0.01). Patients with obstructive sleep apnea are characterized by a small oropharyngeal airway and collapsible oropharyngeal, hypopharyngeal airways. Ultrafast CT scanning allows rapid and noninvasive assessment of airway variables.

      • ‘한글’ 프로젝트를 통한 유아의 읽기 문식성 변화

        The purpose of this study is to explore the aspect of change in literacy level of preschoolers through project ‘Hangeul’. While the project <Hangeul> was implemented from October 2019 to November 2020 with the research participants, a three-year-old girl and a five-year-old boy, the data were collected through participant observation, in-depth interviews, and other references during the course of the project, and was analyzed and interpreted through a qualitative method regarding the change in literacy level of the preschoolers. The result of this research proved that the learning activities of the project ‘Hangeul’ changed the reading literacy level of the preschoolers. The project ‘Hangeul’ changed the reading behaviour of the preschoolers. Their inner motivation to read was enhanced by experiencing diverse environmental print in their daily surroundings. Also, their ability to read increased from reading a variety of kinds of literature including encyclopedias, comic books, and dictionariy. In conclusion, a positive change in the literacy level, in other words, the reading behaviors of preschoolers could be found through the project ‘Hangeul’. 본 연구의 목적은 유아들의 일상과 놀이의 장면에서 ‘한글’을 주제로 함께 탐색하고 표상하 고 문제를 해결해가는 프로젝트 활동을 통하여 유아의 읽기 문식성 변화가 어떠한지 심층적으 로 알아보는 것이다. 본 연구는 2019년 10월부터 2020년 11월까지 연구자가 만 3세 여아와 만 5세 남아와 함께 '한글' 프로젝트 활동을 실행 및 전개하면서 Spradley의 12단계 참여관찰과 심층 면접을 통해 일상에서 이루어지는 유아들의 놀이와 상호작용을 깊이 있게 관찰 및 기록 함으로써 다양한 자료를 수집하여 유아의 읽기 문식성 변화를 중심으로 분석하고 해석하였다. 연구 결과, 유아들은 ‘한글’ 프로젝트를 통하여 일상의 주변 환경에서 다양한 환경 인쇄물을 경 험함으로써 읽기에 대한 내적 동기가 향상되었으며, 이러한 유아의 내적 동기를 따라 백과사 전, 만화책, 국어사전 등 다양한 종류와 장르의 책들을 찾아보며 함께 대화를 나누고, 책 읽기 에 대한 즐거운 경험을 통해 유아의 읽기 능력이 향상되었다.

        With the widespread use of fertility medications and techniques of in vitro fertilization, multiple gestation has become epidemic in modern obstetric practice. Delivery of the initial fetus in a multiple gestation usually is followed by delivery of the subsequent fetus or fetuses shortly thereafter. However in rare circumstances, the delivery is delayed for days due to disappearance of uterine contraction after delivery of the first fetus. When uterine activity ceases after the delivery of the first neonate, a policy of nonintervention may be considered in case of an immature or very premature delivery, in order to achieve a gestational age for the remaining fetus(es) more compatible with neonatal survival. We report a case of twin gestation with prolongation of the delivery interval between the twins for 46 days.

      • 간외담관 폐쇄에서의 초음파소견 : 담관벽 경계소실의 중요성 Significance of Decreased Echotexture of Bile Duct Wall-Interface

        Ultrasound is very useful in evaluation of obstructive jaundice. We retrospectively evaluated the useful findings of ultrasonography in differentiating benign extrahepatic duct obstruction from malignant diseases. Among the 36 patients, 25 patients were benign extrahepatic obstruction and 11 patients were malignant obstruction. The shapes of extrahepatic dilatation were classified into fustiform, meniscus, and abrupt cut off by bowel gas. The diameter, length, and bile duct wall-interface were compared between benign and malignant groups and serum bilirubin and alkaline phosphatase were also compared between two groups retrospectively. The results were as follows. The most common form of extrahepatic dilatation was meniscus(10 of 26 patients, 38%) in benign obstruction, and fusiform(8 of 11 patients, 73%) in malignant obstruction. No significant difference was noted in diameter, length, serum bilirubin, and alkaline phosphatase between benign and malignant groups. Only one case showed decreased bile duct wall-interface in benign obstruction, but loss or decrease of bile duct wall-interface were 8 patients in malignant obstruction. The combined findings of fusiform dilatation and loss of bile duct wall-interface in ultrasonography will be helpful in differential diagnosis of malignant extrahepatic bile obstruction from benign conditions.

This study was performed to determine the adjunctive oral hygiene benefits and safety of dextranase (Lipomyces starkeyi KSM 22 DXAMase)-containing mouthwash when used alongside normal toothbrushing. This 6-month clinical trial was placebo-controlled double-blind design evaluating 1U/㎖ dextranase mouthwash and 0.12% chlorhexidine mouthwash. A total 39 systemically healthy subjects, who had moderate levels of plaque and gingivitis were included. At baseline, 1, 3 and 6 months, subjects were scored for plaque accumulation(Turesky modification of Quigley-Hein's plaque index), gingivitis status (Lo¨e and Silness gingival index), and tooth stain(Area and severity index system by Lang et al). Additionally, oral mucosal examinations were performed and subjects questioned for adverse symptoms. Two weeks after pre-experiment examinations and a professional prophylaxis, the subjects were provided with allocated mousewash and instructed to use 20-ml volumes for 30s twice daily after toothbrushing. All the groups showed significant increase in plaque accumulation since 1 month of experiment. During 6 month's period, the Dextranase mouthwash group showed the least increase in plaque accumulation, compared to the Chlorhexidine mouthwash and placebo groups. As for gingival inflammation, all the groups showed significant increase during 6 months of experiment. The Experimental group (Dextranase mouthwash) also showed the least increase in gingival index score, compared to the Positive control (Chlorhexidine mouthwash) as well as the Negative control (placebo) groups. Whereas the tooth stain was increased significantly in the Positive control group, compared to the baseline score and the Negative control group since 3 months of mouthrinsing. It was significantly increased after 6 months in the Experimental group, still less severe than the Positive control group. As for the oral side effect, the Experimental group showed less tongue accumulation, bad taste, compared to the Positive control group. From these results, mouthrinsing with Lipomyces starkeyi KSM 22 dextranase provided adjunctive benefits to toothbrushing, comparable to 0.12% chlorhexidine mouthwash in inhibition of plaque accumulation and gingival inflammation and local side effects were if anything less frequent and less intense than chlorhexidine, with long-term use of the mouthwash. All data had provided positive evidence for Lipomyces starkeyi KSM 22 dextranase as an antiplaque agent and suggested that further development of dextranase formulations for plaque control are warranted.

      • 국내 주요호수의 육수학적 조사 (1) : 옥정호

        The limnological survey of Lake Okjong was conducted for one year from June 1993 to May 1994 on the monthly basis. The loading of phosphorus, nitrogen and organic carbon from the watershed into the lake were monitored at the main in flowing sites. Secchi disc transparency , epilimnetic chlorophyll a, total nitrogen, total phosphorus concentration and primary production were in the range of 1.3~4.H m, 2.4~ 18.7 mg Chl/m³. 1.25~2.87 mg N/l, 7~65 mg P/m³, 325~2,113 mg C/m²/day, respectively. TN/Tl atomic ratio varled from 129 to 443. N/P ratio decreased in summer because phosphorus concentration was higher than in winter, while nitrogen did not vary much. The seasonal succession of phytoplankton was distinct. In winter and spring, diatoms, Aulacoseira granulata var. angustissima and Aulacoseira italica were dominant while cyanobacteria, Microcystis sp.,M. ichthyovlabe, Phormidium sp. and P.valderianum var. tenuis were dominant in warm seasons. The dominant zooplankton species were Thertmocyclops taihokuensis in warm seasons while Boosmina longirostris were dominant in cold seasons. The organic carbon, nirtogen and phosphorus content of lake sediment were 8.0~14.8, 0.59~0.71, 1.14~1.87 ng N/g, respectively. The sediment of Lake Okjong can be classified as oligohumic based on C/N ratio. The total phosphorus loading from the watershed and fishfarm were estimated to be 2.7g P/m²/yr, which far exceeded the critical loading for eutrophication. The organic carbon loading from the watershed and primary production were determined to be 998t C/yr, 6,348t C/yr, respectively.       • 뇌기저동맥 첨단부 폐쇄증후군 후 지연성 올리브 비대에 동반된 눈떨림

        눈입천장떨림(Oculopalatal tremor)은 치아핵, 적핵 및 올리브핵을 연결하는 신경로(dentatorubro-olivary pathway, Guillain-Mollaret triangle)의 손상 이후에 발생하는 비대성올리브핵 변성(hypertrophic inferior olivary degeneration)에 의한 지연성(delayed) 합병증이다. 다리 뇌출혈(pontine hemorrhage) 또는 뇌줄기경색(brainstem infarction)후 아래 올리브핵비대(inferior olivary hypertrophy)가 발생한 환자에서 입천장떨림(palatal tremor) 없이 눈떨림이 보고된 경우가 있으나, 뇌바닥동맥끝증후군(top of the basilar syndrome) 후 아래올리브핵비대가 발생한 환자에서 눈떨림이 보인 경우는 드물다. 뇌바닥동맥끝증후군 후에 아래올리브핵비대가 발생한 환자에서 지연성 수평 눈떨림 보고하고자 한다. 이것은 양측 소뇌다리의 손상 때문에 눈떨림의 수직 성분이 동반되지 않은 수평눈떨림을 보인 것으로 유추할 수 있다. Oculopalatal tremor(OPT) is a delayed complication of hypertrophic inferior olivary degeneration after the lesion in the dentato-rubro-olivary pathway, the Guillain-Mollaret triangle. Some cases of ocular tremor without palatal tremor associated with inferior olivary hypertrophy after pontine hemorrhage or brainstem infarction have been reported, but ocular tremor with inferior olivary hypertrophy after top of the basilar syndrome has rarely been described. We describe a patient showing delayed-onset horizontal ocular tremor with inferior olivary hypertrophy after top of the basilar syndrome. It can be postulated that the vertical component of ocular tremor could have not developed because of the lesions in bilateral cerebral peduncles.

      • 국내 주요 호수의 육수학적 조사(4): 주암호

        In this study limnological characteristics of Lake Juam was surveyed from June 1993 to May 1994 in order to provides important information regarding water resources. Seschi disc transparency, epilimnetic chlorophyll a(chl-a), tatal nitrogen(TN), total phosphorus(TP) concentration and primary productivity were in the range of 2.0~4.5 m, 0.9~13.6 mgChl/㎥, 0.78~2.32N/l, 11~56 mgP/㎥, 270~2,160mgCm^(-2)day^(-1), respectively. On the basis of TP, Chl-a and Secchi disc depth, the trophic state of Lake Juam can be classified as mesotrophic lake. The phosphorus inputs from non-point sources are concentrated in heavy rain episodes during the monsoon season. As a result, phosphorus concentration are higher in summer than in winter. TP loading from the watershed were estimated to be 0.9 gPm^(-2)yr(-1), which correspond to a boundary of the critical loading (1.0 gPm^(-2)yr(-1), which correspond to a boundary of the critical loading (1.0 gPm^(-2)yr(-1)) for eutrophication. From the results of the algal assay, both phosphous and nitrogen act as limiting nutrients in algal growth. The seasonal succession of phytoplankton community structure in Lake Juam was similar to that observed in other temperate lakes. Diatoms(Asterionella formosa and Aulacoseira granulata var. angustissima) were dominant in spring and winter, cyanobacteria (Microcystis aeruginosa, M. sp. and M. viridis) were dominant in warm season. The organic carbon, nitrogen and phosphorus content of lake sediment were 9.5~14.0 mgC/g, 1.01~1.82 mgN/g, and 0.51~0.65 mgP/g, respectively. The allochthonous organic carbon loading from the watershed and autochthonous organic carbon loading by primary production of phytoplankton were determined to be 1,122 tC/yr and 6,718 tC/yr, respectively. To prevent eutrophication of Lake Juam, nutrient management of watershed should be focus on reduction of fertilizer application, proper treatment of manure, and conservation of topsoil as well as point source.
