        Frontoparietal Cortical Thinning in Respiratory-Type Panic Disorder: A Preliminary Report

        Objective-Many evidences raise the possibility that the panic disorder (PD) patients with respiratory subtype (RS) may have characteristic structural abnormalities. We aimed to explore the structural differences between PD patients with and without the respiratory symptoms. Methods-Patients with PD were recruited from the Department of Psychiatry at Korea University Anam Hospital. Respiratory subtype (RS) was diagnosed when at least 4 out of 5 of the following respiratory symptoms were present during the panic attack: fear of dying, chest pain/discomfort, shortness of breath, paresthesias, and a choking sensation. We acquired high-resolution MRI scans and used FreeSurfer to obtain a measure of cortical thickness for each patient. Results-Cluster based analysis revealed significantly decreased cortical thickness in the left hemisphere in the caudal-middle-frontal, superior frontal, and posterior parietal areas in the RS group. No significant difference was observed in any of the limbic areas. Conclusion-Respiratory symptoms of panic disorder were associated with a reduction in cortical thickness in the left frontal and parietal areas. This finding leads to the assumption that the frontoparietal network is the crucial component in a larger cortical network underlying the perception of dyspnea in RS.

      • The Effects of Glehnia littoralis on the Inflammatory mediators in Mouse Macrophage Cells

        Glennie littoralis (Umbelliferae) is the medicinal plant used traditionally for treatment of immune-related diseases. Prostaglandins and nitric oxide (NO) have been implicated as important mediators in the processes of inflammation and carcinogenesis. For understanding the mechanisms for pharmacological activities of Glehnia littoralis, we evaluated the inhibitory activity of Glehnia littoralis on lipopolysaccharide (LPS)-induced prostaglandin E₂ (PGE2) and NO production in mouse macrophage RAW264.7 cells. The results showed that the extract of Glehnia littoralis inhibited LPS-induced PGE₂ production effectively, but not NO. Additional study revealed that the extract of Glehnia littoralis suppressed cyclooxygenase-2 (COX-2) expression in a dose-dependent manner. Present study suggests that Clehnia Iittorslis may have anti-inflammatory and/or cancer chemopreventive activity through the inhibition of PGE₂ production by the suppression of COX-2 activity.

      • Pharmacological activities of Glehnia littoralis

        Glehnia littoralis (Umbelliterae), a perennial herb distributed along the coastline of northern Pacific countries, is the medicinal plant used traditionally for treatment of various diseases. This review focuses on the various pharmacological activities of Glehnia littoralis for understanding about its traditional medicinal applications, medicinal uses in the modem society, and potentials for drug development. Glehnia littoralis was reported to have anti-oxidant, anti-tumor, anti-amnesic, blood circulation-promoting, immunomodulatory, anti-microbial, and allelopathic activities. However, their mechanisms remain to be clarified. Because Glehnia littoralis has been prescribed in traditional Oriental medicine as a tonic herb, Glehnia littoralis can be better than other chemical drugs and medicines which exert the equal pharmacological activities. Although the activities of Glehnia littoralis are not specifically high-potent with unique mode of action, it may turn out that it can be beneficial to exert multiple pharmacological activities. In view of low toxicity, relative cheapness, presence in the diet, and occurrence in various herbal remedies of Glehnia littoralis, it needs to be prudent to evaluate its properties and applications further.

      • 한의학 고문헌의 단방용례 분석을 통한 천문동의 기대효능 연구

        We referred to twenty-two Korean medicinal literatures for application of a single-medicine prescription using Asparagi Tuber(Asparagus cochinchinensis) to form the groundwork for scientific modernization of Korean medicine. Our study revealed the following. 1. Principle expectant effects of Asparagi Tuber were prolong life; elevation of stamina or activity; improvement of asthenia; remedy of epilepsy, mental disease or convulsions; relief of xerosis; treatment of tumor, abscess or intumescence; extermination of endoparasite; solution of numbness, etc. 2. The records show that 95% of directions are per oral. 3. 38% of the cases, Asparagi Tuber was used without its jacket or core. 4. Alcohol and honey were generally used as solvent and additive. We suggest to perform the further studies for the scientific verification of the expectant effects of Asparagi Tuber and its different efficacy by processing, solvent and additives.

        Hibiscus syriacus is rarely used herb. For finding up new applications of this herb, the medicinal parts and expected efficacys that recorded in 24 classics of oriental medicine were inquired. As a result of the literature review, medicinal parts and expected efficacys of Hibiscus syriacus were as follows: The medicinal parts were mainly flower and bark But the root, branch, leaf and fruit were mentioned too. Some unidentified medicinal parts seemed to be branch or bark. The mentioned symptoms that prescribed Hibiscus syriacus are intestinal 'wind', red-white dysentery, thirst with fever after dysentery, insomnia, 'wind' syndrom, leukorrhea, regurgitation, etc. And this herb was mentioned that applied externally for scabies, hemorrhoids, prolapse of the rectum, headache, extracted tooth, etc. Therefore, bleeding intestinal diseases, insomnia, vaginal or uterine hemorrhage(for internal use), scabies or mycosis, hemorrhoids, anal fistula, prolapse of the rectum, headache(for external use) were presented as expected efficacys of Hibiscus syriacus. In future, studies for verifying the efficacys are needed.

        Characteristics of Residual Symptoms in Korean Patients with Major Depressive Disorder: A Validation Study for the Korean Version of Depression Residual Symptom Scale

        Objective-Residual symptoms of depression are related to more severe and chronic course of functional impairment with higher risk of relapse. The objective of this study was to validate, and determine psychometric properties of the Korean version of Depression Residual Symptom Scale (KDRSS). Methods-A total of 203 outpatients with recent episode of major depression based on DSM-IV criteria were enrolled in this study. They had been treated with antidepressants and assessed by KDRSS, Hamilton Depression Rating Scale-24 (HDRS-24), and Montgomery-Åsberg Depression Rating Scale (MARDS). The validity and reliability of KDRSS were assessed, including internal consistency reliability, concurrent validity, temporal stability, factorial validity, and discriminative validity. Results-Internal consistency (Cronbach’s alpha=0.961), concurrent validity (MADRS: r=0.731, p<0.01, HDRS-24: r=0.663, p<0.01), and temporal stability (r=0.726, p<0.01) of KDRSS were all excellent. KDRSS showed good discriminative validity based on MARDS. KDRSS consisted of one-factor structure accounting for 63.8% of total variance. All subjects except two in full remission group had one or more residual symptoms. In 7 subscales of KDRSS consisting of similar items respectively, ‘lack of energy’ was the most commonly reported, followed by ‘increased emotionalism’ in this group. Conclusion-KDRSS is a useful and sensitive instrument for measuring residual depressive symptoms. Since some depressive symptoms including ‘lack of energy’ and ‘increased emotionalism’ in patients with full remission might be persistent during psychiatric intervention, these symptoms need to be focused on in clinical practice.

        A Genome-Wide Association Study Identifies UTRN Gene Polymorphism for Restless Legs Syndrome in a Korean Population

        Objective-Restless legs syndrome (RLS) is a highly heritable and common neurological sensorimotor disease disturbing sleep. The objective of study was to investigate significant gene for RLS by performing GWA and replication study in a Korean population. Methods-We performed a GWA study for RLS symptom group (n=325) and non-RLS group (n=2,603) from the Korea Genome Epidemiology Study. We subsequently performed a replication study in RLS and normal controls (227 RLS and 229 controls) to confirm the present GWA study findings as well as previous GWA study results. Results-In the initial GWA study of RLS, we observed an association of rs11645604 (OR=1.531, p=1.18×10-6) in MPHOSPH6 on chromosome 16q23.3, rs1918752 (OR=0.6582, p=1.93×10-6) and rs9390170 (OR=0.6778, p=7.67×10-6) in UTRN on chromosome 6q24. From the replication samples, we found rs9390170 in UTRN (p=0.036) and rs3923809 and rs9296249 in BTBD9 (p=0.045, p=0.046, respectively) were significantly associated with RLS. Moreover, we found the haplotype polymorphisms of rs9357271, rs3923809, and rs9296249 (overall p=5.69×10-18) in BTBD9 was associated with RLS. Conclusion-From our sequential GWA and replication study, we could hypothesize rs9390170 polymorphism in UTRN is a novel genetic marker for susceptibility to RLS. Regarding with utrophin, which is encoded by UTRN, is preferentially expressed in the neuromuscular synapse and myotendinous junctions, we speculate that utrophin is involved in RLS, particularly related to the neuromuscular aspects.

        스마트폰 사용량의 급격한 증가와 함께 모바일 게임의 게임 시장점유율이 증가하고 있다. 이러한 추세에 따라 게임 업계에서는 PC와 모바일에서 동시접속 가능한 inter-networking 게임을 개발하고 있으나 (one source multi platform 기반게임) 게이밍 UI의 한계로 인하여 모바일 게임 플레이어는 PC 기반 게임 플레이어와 동등한 수준에서 경쟁을 할 수 없다. 본 논문에서는 모바일 게이밍 UI와 PC 게이밍 UI의 성능을 비교·?분석하여 새로운 모바일 게이밍 UI를 제안하였다. 이를 위해서 인터랙션 속도를 예측하기 위한 전통적인 인터랙티브 시스템 평가 도구인 Keystroke-Level Model(KLM)을 모바일 환경에 적합한 형태로 수정한 Fingerstroke-Level Model(FLM)을 개발하였다. 이를 적용하여 현재 모바일 게이밍 UI의 문제점을 파악하고 PC기반의 게임방식과 경쟁이 가능한 새로운 모바일 게이밍 UI를 제안하였다. Market share of the mobile games are increasing in the game market with radical incremental of demands of smartphone. Following this trend, game companies are trying to develop the inter-networking games which provide concurrent access between PC and mobile(one source multi platform-based game). However, current mobile gaming UI has fundamental disadvantages comparing to PC gaming UI. It makes mobile users cannot compete with PC users on an equal basis. Therefore, this paper proposes a novel mobile gaming UI after comparing and analyzing performance of current mobile gaming UI with PC gaming UI. We modified the Keystroke-Level Model(KLM), a traditional interactive systems evaluation tool, then made the Fingerstroke-Level Model(FLM) which can be applied as a mobile system evaluation tool. By using the FLM, we identified drawbacks of current mobile gaming UI, improved it and then proposed a new mobile gaming UI competing with PC-based game style.

        A Comparison of Personality Characteristics and Psychiatric Symptomatology between Upper Airway Resistance Syndrome and Obstructive Sleep Apnea Syndrome

        Objective-To investigate the personality characteristics of patients with upper airway resistance syndrome (UARS) and those of patients with obstructive sleep apnea syndrome (OSAS). Methods-Eighty-eight patients with UARS and 365 patients with OSAS participated. All patients had a diagnostic full-night attended polysomnography (PSG) and completed the Athens Insomnia Scale (AIS), Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), Symptom Checklist-90-Revision (SCL-90-R) and Eysenck Personality Questionnaire (EPQ). Results-The UARS group scored significantly higher than the OSAS group on the ESS, AIS, and PSQI (p<0.001). The scores of all SCL-90-R subscales in the UARS group were significantly higher than those in the OSA group (all p<0.001, except for somatization, p=0.016). Patients with UARS scored lower on EPQ-E (extroversion/introversion) (p=0.006) and EPQ-L (lie) (p<0.001) than those with OSA. UARS patients also showed higher scores on EPQ-P (psychoticism) (p=0.002) and EPQ-N (neuroticism) (p<0.001) than OSAS patients. Conclusion-Our results suggest that patients with UARS have worse subjective sleep quality than OSAS patients in spite of their better PSG findings. UARS patients tend to have more neurotic and sensitive personalities than patients with OSAS, which may be a cause of the clinical features of UARS.

        Usability Evaluation of an Image-based Pill Identification Application

        Objective. To measure the accuracy and usability of an the image-processing based pill identifier application that we have developed. Methods. The subjects selected were medical residents and nurses. Five nurses and five physicians were randomly assigned to use either an the image-processing based pill identifier application (n=10), or the conventional pill identifier application (n=10). They were instructed to examine 10 pills using the application assigned to them, and searches that took <3 minutes to find candidate drugs were recognized as successes. Among these successful searches, the accuracy was defined to identify the correct names of the drugs and the times needed in the correctly identifications were also measured. After using one application the subjects were instructed to use the other one and repeat the same process. Finally, they answered a questionnaire on the usability of the applications. Results. The average proportion searches completed within 3 minutes was 91% for the the image-processing based pill identifier application, slightly, but not significantly, higher than that for the conventional pill identifier application (85%). The accuracies of the the image-processing based and conventional pill identifier applications were similar, 89% and 83%, respectively. In the usability examination, the the image-processing based pill identifier application yielded higher scores for the desirable, usable, findable and useful qualities than the conventional pill identifier application. Conclusion. The the image-processing based pill identifier application application has a similar accuracy to the existing conventional pill identifier application, and its usability was also found to be good.
