      • 자궁경부암의 수술 및 방사선치료 후 국소재발 판정에 대한 MR 및 CT 평가

        The objectives of our study were to analyze the CT and MR findings of local tumor recurrence at vaginal stump in patients of cervical carcinoma treated with hysterectomy and radiotherapy, and to assess the diagnostic value of the MR study. We retrospectively analyzed the CT(n=17) and MR (n=10) findings in 19 patients, who had uterine cervical carcinoma after hysterectomy with radiation therapy (extended-field irradiation and/or brachytherapy). The CT findings of local tumor recurrence were localized mass like lesion(n=5), slightly enhanced infiltrating lesion (n=4) and enhancing mass with central necrosis (n=1). The MR findings wer local iso-signal intensity mass on T1-weighted image and irregular high-signal intensity mass on T2-weighted image. The false positive results were three (two on CT and one on MR), due to inflammation and local fibrosis. On the basis of our study, the CT was the proper screening method for follow up in patients with cervical carcinoma after treatment. But, if any suspicion of tumor recurrence, MR was the useful tool to exclude the local fibrosis and inflammation, and to decide the next studies such as biopsy or observation with MR follow up.

        토끼의 만성 뇌혈관연축 모델에서 혈관확장제에 의한 비가역적 혈관수축의 발현시기와 혈관벽의 전자현미경소견상 이상소견이 발현되는 시기와의 시간관계

        One of the characteristics of the cerebral vasospasm is its irreversibility with the vasodilators.Under the hypothesis that the irreversibility with vasodilators might be caused by the structural change in the arterial wall, authors examined the chronological relationships between the irreversibility and the electron microscopic findings of the arterial wall in the rabbit chronic vasospasm model. The development of the vasospasm and the irreversibility of the vasospasm with the intra-arterial papaverine were defined angiographically. After the second angiography done in one to 30 days after subarachnoid hemorrhage(SAH), eighteen rabbits were sacrificed, and the basilar artery was examined with electron microscope. Arterial narrowing was the severest one day after SAH(54.1% of the pre-SAH diameter), and was maintained up to 30 days afer SAH. The irreversibility of the arterial constriction with the papaverine developed 5 days after SAH, which had a tendency to recover 6 to 9 days after SAH. However the irreversibility was noted again 16 days after SAH. Electron microscopy revealed the endothelial wrinkling, disorganization of muscle fiber, myonecrosis, thickening of smooth muscle fibers, and increase of connective tissue in the tunica media. These structural changes were severest one day after SAH, and gradually diminished up to 30 days after SAH. These data show that there are no chronological relationships between the irreversibility and the structural change per se. However the fact that the irreversibility developed during the reparative phase of the arterial wall injury by SAH suggests that the chronic vasospasm is not a primary event but a secondary phenomenon following an injury to the cerebral arterial wall.

        Nd:YAG Contact Laser를 이용한 새로운 미세혈관문합술의 실험적 연구

        A comparative study was undertaken to evaluate the contact Neodymium : yttrium aluminum garnet(Nd : YAG) laser system for vascular anastomosis of small caliber blood vessels(diameter 0.5-1.2mm) in the animal model. In this study 40 femoral arteries and 40 femoral veins of Sprague-Dawley rats were anastomosed by contact laser assisted microvascular anastomosis(LAMA) utilizing 3 stay sutures which were placed 120 degrees apart and the intervals welded with contact Nd : YAG laser unit, conventionally sutured anastomosis(CSA) served as controls. The time needed for vascular anastomosis, patency rate (immediate postoperative, postoperative 2nd day, postoperative 1 week, postoperative 4 week), gross and microscopic evaluations were compared to conventional microsurgical suture technique. The results are as follows : (1)Post-operative patency rate was 82.5% for femoral artery and 75% for femoral vein by contact LAMA technique compared to 90% and 75% by CSA technique at post operative 4 weeks. (2)Less time-consumed for arterial anastomosis by 6 minutes 23 seconds and venous anastomosis by 8 minutes 55 seconds with contact LAMA technique compared to CSA technique. (3)Grossly almost complete healing had taken place by post-operative 1 week by contact LAMA technique. (4)Aneurysm formation was 5% for femoral artery and 15% for femoral vein by contact LAMA technique compared to 5% and 10% respectively by CSA technique. (5)Microscopically, re-endothelization was complete by post-operative 7th day by contact LAMA technique. There was less medial hypertrophy and hyperplasia and also less inflammatory response compared to CSA.

        "No man's land"에서의 수지 굴근건 손상시 일차 건봉합술의 임상적 고찰

        In past years repair of acute flexor tendon injuries in zone Ⅱ developed a poor reputation because of the high frequency of adhesion. More recently with the development of new techniques, especially early controlled mobilization and microscope, the result have improved and direct repair has again come into favor. In this study, 94 digits with complete laceration of the flexor tendon in zone Ⅱ were treated by primary repair and early controlled mobilization. Using the Strickland formula of total active motion of the interphalangeal joints, 64 digits(68%) were rated "excellent" ; 14 digits(15%) were rated "Good" ; 12 digit(13%) were rated "Fair" ; 4 digits(4%) were rated "Poor"

        Chronic eosinophilic pneumonia(CEP) is a rare disease of unknown etiology characterized by chronic infiltration of the lung with eosinophils. It presents with profound systemic symptoms comprising fever, night sweats, weight loss, dyspnea and blood eosinophilia with nonsegmental air-space consolidation confined to the outer third of the lung, the "photographic negative of pulmonary edema". Histopathologic features of chronic eosinophilic pneumonia are intraalveolar and interstitial infiltrations with eosinophils, histiocytes, giant cells, scattered lymphocytes and plasma cells. Glucocorticoid therapy cause prompt resolution of symptoms as well as disappearans of blood eosinophils, elevated serum Ig E levels and the roentgenographic lesions. Infrequent radiographic findings include nodular infiltrates, consolidations, cavitations, atelectasis and pleural effusions. Deaths from CEP although rare, have been reported, but the majorities of CEP have benign courses and do not need a therapy with ventilator. We report a case of chronic eosinophilic pneumonia, which had a sudden course associated with diffuse pneumonic consolidations on the both lung and bilateral pleural effusion.

        산업재해에 의한 수부손상의 임상적 고찰 (Ⅱ)

        Industrial hand injury causes not only loss of hand function but also many socio-economic problem, because they are low socio-economic class, age of productivity and unemployed status due to functional disability of hand. Mainly, inexpert workers suffer from industrial hand injury (62%) probably due to absence of safe guard, safe-education and occupational training. Therefore, industrial hand injury may be diminished by preventive measure such as adequate safeguard, safe-education and occupational training. Statistical analysis was carried out retrospectively on 495 patients with industrial hand injury at Dept. of plastic and Reconstructive Surgery, Korea University College of Medicine, KURO hospital, from September 1986 to December 1988, and following items are studied. 1. Constitutions of industrial compartments and labors in Kuro and Youngdeungpo region. 2. Educational status of the patients. 3. Area distribution. 4. Monthly distribution. 5. Day of week distribution. 6. Onset time distribution. 7. Duration of occupation in the patients. 8. Causative factor. 9. Underlying factor. 10. Physical treatment and its duration. 11. postoperative grasping power. 12. Aesthetic result. We want this paper attribute to reduction of industrial hand injury.

        There exists a major impediment to learning electricity which may illuminate why direct presentation of scientists' ideas is too aggressive a strategy. This impediment lies in thinking that electricity is used up and in envisaging the light bulb as a consumer. This view is based on a powerful idea which summarises much of our experience; it declares that 'things get used up', 'you cannot get something for nothing', and 'intuitive thinking'. To replace this view with scientific view, we made the analogy model. It is suggested that the analogy model is more effective in teaching electricity circuits than conventional teaching method.
