        실험적 급성 허혈성 신부전에서 Verapamil 이 혈중 Polyamine 동태에 미치는 영향

        N/A It has been proposd that calcium entry from an external medium increases intracellular free calcium to toxic levels during ischemic acute renal failure, and verapamil (ARF) has been suggested as the agentblocking calicium entry into renal cells and protecting renal function during ischemic injury. Polyamines, anorganic cations that play various roles in normal cellular proliferation and differentiation, accumulate in renal failare. Alsa it has been suggested tht the synthesis and metabolism of polyamine are influence by acute changes of the general condition, such as acute renal failure, and are mediated by a calcium influx into the cells. The study was designed to study the protective effects of systemic verapamil pretreatment on renal function and the influence on polyamine metabolism in experimental ischemic ARF in cats. For these purpose, the experimental animals were divided into 2 groups. While Group I (n=5) was an ischemic ARF model by renal artery clamping for 60 minutes, Group II (n=5) was ischemic ARF with systemic verapamil (5 ml/min/kg) pretreatment. The creatinine clearance and plasma and urinary polyamine were measured in each group before and after the renal artery clamp. The results were as follows: 1) Creatinine clearance before and after the renal artery clamp were 10.64±7.18 ml/min/kg and 2.09±1. 70 ml/min/kg in Group I, 4.47±3.38 ml min/kg and 0.60±0.79ml/min/kg in Group II, respectively, So creatinine clearance decreased more significantly in Group II campared with Group I. 2) Plasma polyamine increased after ischemia in Group I. In group I, plasma levels of putrescine, spermidine, and spermine before ischemia were 4.75±0.40 nmol/ml, 0.69±0.09 nmol/ml, and 0.83±0.63 nmol/ml, were elevated to 7.17±2.91 nmol/ml, 9.83±1.46 nmol/ ml, and 2.64±1.14nmol/ml after ischemia. But in Group II, the plasma level of polyamine was not changed, and especially, spermine decreased significantly from 0.83±0.27 before ischemia to 0.49±0.23 nmol/ml after ischenmine (p=0.033). 3) Urine polyamine excretion decreased after ischemia in Group I and Group II. In Group II, urinary excretion of spermidine and spermine before ischemia, 0.13±0.10 nmol/min and 0.17±0.13nmol/min, decreased after ischemia to 0.01±0.01nmol/min (p=0.019) and 0.032±0.26 nmol/min (p=0.0257). 4) In renal tissue, spermine content vas highest. In Group II, preischemic spermine were 397.20 nmol/g and increased to 646.66nmol/g after ischema, But there were no significant changes in the polyamine contents in Group II. From these data, it was suggested that systemic verapamil pretreatment exerts no protective effect on ischemic ARF. Plasma polyamines are elevated in ischemic ARF, and verapamil may protect these elevations.

      • 허혈성 심질환에 있어서 아데노신 급속 정맥투여후 관동맥압의 변화에 관한 연구

        Background: Adenosine interacts with A1 receptors present on the extracellular surface of cardiac cells, activating K+channels in a fashion. It produces coronary vasodilatation and AV nodal block. The half-time is one to six second and has ultra-short action. In ischemic heart disease, fractional flow reserve(FFR) can be achieved safely with intravenous adenosine infusions at a rate of 150㎍/kg/min which cause maximal coronary hyperemia. Purpose: We investigated the patients who have had ischemic heart disease(n=8). After adenosine intravenous administration(150㎍/kg/min), the effect cause maximal hyperemia of coronary artery. During peak maximal hyperemia, we reported clinical findings, coronary hemodynamics and electrocardiogram findings. Methods: After diagnostic coronary angiography and left ventriculography, catheter was advanced into the ostium of coronary artery. We investigated coronary hemodynamics such as systolic coronary artery pressure, diastolic coronary artery pressure, mean coronary artery pressure and heart rate. To assess the use of adenosine for determination of fractional flow reserve, hemodynamics and electrocardiogram were measured at basal state and during peak maximal hyperemia. Results: Symptoms developed 1.5-2 minute after adenosine intravenous administration and each symptom disappeared 2-2.5 minute after adenosine administration. Two patients represented asymptomatic appearance and six patients complained of flushing, chest pain, palpitation and headache. In eight patients with ischemic heart disease, 1.5 minute after venous administration of 150㎍/kg/min of adenosine, coronary artery pressure decreased and heart rate increased significantly. Conduction abnormalities after administration of 150㎍/kg/min of adenosine were transient 2nd degree AV block 2 cases(25%). Conclusions: Adenosine is a potent ultra short-acting vasodilator. Although coronary artery pressure decreased and heart rate increased and mild cardiovascular symptoms occurred after continuous infusion of 150㎍/kg/min of adenosine, it is useful as an agent for determination of fractional flow reserve.

        C -11 및 F - 18 표지 콜린의 합성과 체내동태에 관한 연구

        Objectives: Recently, [methyl-(11)^C]-(β-Hydroxyethyl)trimethylammonium ([(11)^C]choline) has been discovered to be a very effective tracer in imaging various human tumors using positron emission tomography. Because of the short half-life of C-11, it is very difficult to use in a routine imaging procedure and needs a frequent synthesis of [(11)^]choline. This can be supplemented by the substitution of [(11)^Ccholine with [methyS-18]fluorocholine. Here, we would like to report cell uptake and biodistribution of [(11)^Ccholine and [(18)^F]fluorocholine as a basic study. Methods [(11)^C]Choline was prepared by the treatment of [(11)^C]CHzI with N,N-dimethylaminoethanol and [18F]fluorocholine was synthesized from reaction of CHzBr[18F]F with N,N-dimethylaminoethanol. The radiochemical purity was checked by high performance liquid chromatography (HPLC). The biodistribution of [(11)^C]choline and [(18)^F]fluorocholine was determined in balb/c mouse at 5 min, 20 min, 40 min and 80 min. The cell uptake wa measured using glioma (9L) and colon adeocarcinoma (SW620). Results: The radiochemical purity was more than 98% after purification. In the liver, uptake did not change over time the uptake was 20/ID/g for [C]choline and 13%ID/g for [(18)^F]fluorocholine. In the kidney, radioactivity decreased over tirne the uptake was 15%1D/g for [(11)^Ccholine and 20%ID/g for [(18)^F]fluorocholine, 80 min post-injection. The cell uptake of [(11)^Ccholine was 4.93% for glioma (9L) and 18.69F for colon adenocarcinoma (SW620). For [(18)^F]fluorocholine, 1.77% for glioma (9L) and 2.77% for colon adenocarcinoma (SW620). Conclusion: [(11)^CCholine and [(18)^F]fluorocholine showed a different cell uptake tendency, depending on cancer cell line. (Korean J Nucl Med 200135:185-191)

        항정신병약물 사용 중인 정신분열병 환자에서 올란자판으로의 교체 방법에 관한 연구(II) : Comparison of Safety 안전성 비교

        연구목적: 이 다기관 공동임상연구는 사용 중인 항정신병약물을 ’직접 교체 방법’또는 ’시작-감량 교체 방법’중 한 가지 방법으로 올란자핀으로 교체한 후, 안정성 측면에서 두 교체 방법 간의 비교와 교체후의 변화를 관찰하기 위한 것이다. 방법: 국내 13개 병원의 입원 및 외래에 내원한 환자들 중 ICD-10 지단기준으로 정신분열병에 해당되며, 임상적으로 항정신병약물 교체가 필요한 환자를 대상으로 하였다. 두 가지 교체 방법 중 한 가지를 무작위로 피험자에 적용하였으며, ’직접 교체 방법’에 배정된 경우에는 사용중인 항정신병약물을 일시에 중단하고 10㎎의 올란자핀을 바로 투여하였고, ’시작-감량 교체 방법’에 배정된 경우는 10㎎의 올란자핀 투여하고 2주에 걸쳐서 기존 약물을 감량하여 중단하였다. 올란자핀 사용기간은 총 6주이며, 용량은 5∼20㎎ 범위로 제한하였다. 한정성 평가를 위해서 체중, 생명징후, 자발적인 이상반응 복, 실험실 검사 그리고 Simpson-Angus Scale(SAS), Barnes akathisia rating scale(BARS), Abnormal involuntary movement scale(AIMS). Liverpool University neuroleptic side effect rating scale(LUNSERS)등을 이용하였다. 결과: 총 103명의 정신분열병 환자를 대상으로 하였다. 사용한 올란자핀의 용량, 벤조디아제핀의 병용률, 탈락률과 탈락 사유, 자발적인 이상반응 보고, 생명징후, 실험실 검사 그리고 대부분의 부작용 척도 상에서 임상적으로 의미 있는 차이를 두 교체 방법간에 발견하지 못하였다. 다만 AIMS의 감소는 ’직접 교체 방법’군에서 보다 적었고, 항콜린제의 병용률은 ’시작-감량 교체 방법’군에서 보다 많았다. 기저 상태에서 전체 피험자의 SAS와 BARS 점수는 각각 3.5점과 1.8점이었으며 70% 이상의 피험자가 고프로락틴 혈증을 보였다. 올란자핀으로 교체한 후, SAS, BARS, AIMS 점수의 유의한 감소가 있었으며 고프로락틴 혈증을 보인 피험자 분율도 약 30%이하로 감소하였다. 그러나 교체 방법과 상관없이 올란자핀 교체 후 유의한 체중 증가가 있었다. 결론: 이 연구를 통해 교체 방법에 관계없이 비교적 안전하고 용이하게 올란자핀으로 교체 할 수 있음을 알 수 있었다. 그리고 기존 항정신병약물을 올란자핀으로 교체함으로써 일부 부작용들을 줄일 수 있음을 간접적으로 관찰할 수 있었다. 하지만 이 연구는 여러 제한점과 문제점을 지니고 있기 때문에 보다 체계적인 연구를 통해 검정이 필요하리라 생각된다. Objectives: This multicenter clinical trial involving 13 hospital sites compared the safely of switching to olanzapine between ’direct switching method’ and ’start-tapering switching method’. Method: This study included both inpatients and outpatients who fulfilled the criteria for schizophrenia as defined in the ICD-10, and were in need to be appropriate for switching antipsychotics. Subjects were randomly assigned to one of the two switching methods. For ’direct switching method’group, previous antipsychotics were abruptly discontinued and 10㎎ of olanzapine was administered, and previous antipsychotics was gradually tapered for 2 weeks. Olanzapine was used for 6 weeks and the dose was adjusted within the range of 5-20㎎. The safety of switching to olanzapine was measured with vital sings including body weight, adverse events reported spontaneously, laboratory tests, and various scales such as Simpson-Angus Scale(SAS), Barnes Akathisia Rating Scale(BARS). Abnormal Involuntary Movement Scale(AIMS), and Liverpool University Neuroleptic Side Effect Rating Scale(LUNSERS). Results: 103 patients were switched to olanzapine in this study. The comparison between two switching methods did not show any significant difference in the dosage of olanzapine used, the concomitant use of benzodiazepine, the rate and reasons of drop-out, the adverse events, vital signs, laboratory tests, and most scales for measuring side-effects. However, the decrease in AIMS scores was significantly lower in ’direct switching method’ group, and the concomitant use of anticholinergics was comparatively greater in ’start-tapering switching method’ group. At baseline, SAS and BARS scores were 3.5 and 1.8 points respectively, and more than 70% of the subjects showed hyperprolactinemia. After switching to olanzapine, SAS, BARS, and AIMS scores were significantly decreased and the proportion of the patients with hyperprolactinemia was also decreased to less than 30%. However significant weight gain after the treatment of olanzapine was observed regardless of switching method. Conclusion: This study may suggest that switching to olanzapine can be done with relatively high safety regardless of switching methods and olanzapine can significantly decrease some side-effects induced by other antipsychotics.

        Recently, in vitro reconstruction of anterior cruciate ligament(ACL) has been tried using tissue engineering technique, especially mesenchymal stem cell(MSC) is considered as a good cell source. In this study, we tried to find the effect of mechanical stimulus on the differentiation of bone marrow-derived MSC into ACL fibroblastlike cell using a cell training bioreactor imposing cyclic mechanical tension whose parameters were 120 min/day, 1 Hz, and 5% strain. After 2 weeks of culture, MSCs under cyclic tension showed more regularly oriented alignment and lower expression of CD73 and CD105 than control by microscopy and FACS analysis. In transcriptional level, type I collagen, type II collagen, fibronectin, elastin, α-smooth muscle actin mRNA expressions of MSCs under cyclic tension were higher than control and similar to native ACL. In conclusion, it is thought that mechanical cyclic tension induced the loss of stemness of MSC, therefore improved the differentiation into ACL fibroblast-like cell.

        Effects of Mechanical Stimulation on the Proliferation of Bone Marrow-derived Human Mesenchymal Stem Cells

        To support and enhance the in vitro growth and activity of mesenchymal stem cells (MSCs), the cell culture medium may be supplemented with various proteins and factors to mimic the physiological environment in which the cells optimally proliferate and differentiate. In this study, the effects of mechanical factors on cellular metabolic responses were investigated experimentally using a bioreactor. The effects of various chemical factors, such as growth factors, cytokines, and hormones, were also investigated. Based on previous reports demonstrating the important roles of mechanical factors in the growth and activity of MSCs, we sought to evaluate the effects of mechanical stimuli on the proliferation of bone marrow-derived MSCs using a cell training bioreactor that imposed cyclic mechanical stretch, with parameters of 240 min/day, 0.03 Hz, and $5{\sim}15%$ strain. The application of cyclic stretch $(5{\sim}15%\;strain)$ to the MSCs enhanced their proliferation during the early stage(3 days), but not the late stage (14 days), of batch culture. Mechanical stretch did not increase the release of lactate dehydrogenase (LDH) from the MSCs during culture. Appropriate levels of mechanical stretch $(5{\sim}10%\;strain)$ increased collagen synthesis, but did not alter MSC stretch was able to serve as a potent positive modulator of MSC proliferation during the initial stages of culture.

        악교정 환자의 악교정 수술전후 발음양상에 대한 비교연구

        The purpose of this study was undertaken to determine the effects of orthognathic surgery on speech. The hyposis stated herein is that functional behaviors of the dentofacial complex, such as speech production, may be adversely affected by deviations of a structural nature(especially, Class III malocclusion) . Twenty adults with Class III malocclusion(13 female and 7 male) were studied preoperative, immediate postoperative and either 6 or 12 months postoperative lateral cephalograms. They had mandibular prognathism and had undergone mandible setback operation. The position of tongue, soft palate(Uvula), hyoid bone, respiratory track width, and pharyngeal depth were assessed on lateral cephalograms with 23 cephalometric variables, ANOVA, Paired t-tests and Pearson's product-moment correlation coefficient tests were used to evalute the operative changes in all cephalometric parameters. A experienced speech and language pathologists performed narrow phonetic transcriptions of tape-recorded words and sentences produced by each of the ninth patients and the recording tapes were analyzed by phonetic computer program(Computerized Speech Lab(CSL) Model 4300BI(U.S.A.)) These judges also recorded their ratings of each patient's overall consonants, hypernasality, hyponasality, and articulation proficiency. The results obtained are as follows ; 1.There were significant changes in distance of posterior pharyngeal wall to tongue (TI-TW2, TS-TW3) after the surgery at 6 months postoperatively(each p<0.01 p<0.05) 2.The posterior tongue point(TI, TS, PPT) moved posteriorly after surgery and remained to its changed position at 6 months postoperatively(p<0.05) . The displacement of tongue was correlated with the movement of mandibular setback amount(p<0.05). The hyoid bone moved posteriorly superiorly after immediate postoperative period. There was significant changes in hyoid bone movement after immediated post-operative period(p<0.05), but returned to its original position during the follow-up period(p>0.05) 3.The soft palate was displaced posteriorly superiorly after immediated operative period and remained to its changed position at 6 months postoperatively(p<0.05) ANS-PNS-SPT angle increasing, PPU-PPPo distance narrowing was showed after surgery, and remained its appearance 6 months postoperactively (p<0.05) 4.There were significant changes in formant value and squre diagram of vowel sound after the orthognathic surgery and the follow-up period. There were significant changes in /ㅅ/sound and posterior tongue sound. 5.The posterior movement of tongue and the posteriosuperior movement of soft palate was correlated with mandibular setback amount after orthognathic surgery. On the vowel squre diagram, the author found that the place of articulation after operation moved downward, backward, upward. 6.In assessing speech abnormalities, dental occlusion should be considered as a contributing factor. The vast majority of subjects with preoperactive misarticulations eliminated or reduced their errors following orthognathic surgery. There was significant difference in speech impovement between pre- and post-operation.

        This study is regarding Social Network Analysis(SNA) effect that is becoming more and more important in knowledge management from knowledge ecology's point of view. And this study first began to see if there is any possibility for substituting for Knowledge Performance improvement. The purposes of the study are as follows; First, clarifying the definition of Social network analysis by re-verifying how central positions in personal social network affect individual performance. Second, clarifying how two different personalities, absorptive capacity from individual capability point of view and interdependence, affect in network performance. To accomplish the study results, classified network types to two different types - friendship relationship and Advice relationship, in the first place. Grasp the result of network centrality, absorptive capacity, interdependence and empirical studies afterwards. According to the result of this study, closeness centrality and individual absorptive capacities of the advice network affects significantly in individual performance. Interdependence affects both in friendship network of individual network and advice network. However, not considering variables in results and individual characters variously, we faced limitation for clarifying independence and absorptive ability interaction effect.

      • 제주형 작물재배용 비닐하우스의 모델개발 : 풍하중에 대한 수치해석

        An elastic analysis under wind load is performed for the double layered plastic greenhouse model developed particularly for minimizing damages under typhoons at Cheju Citrus Research Institute in Chejudo. General EVA frlm is used for the inner covering and for the outer covering the developed special film whch would break the wind pressure down is used. Based on the wind tunnel test. this special film will reduce effectively the wind speed by up to 86 to 98% under very controlled situation. Many more tests need to be performed to adopt the results to greenhouse analysis. Based on the elastic analysis performed in the study, the behavior of the greenhouse is changed dramatically due to the boundary conditions. Not like other researchers before we applied dead load of the concrete support at the ground pipe and fixed support boundary conditions at the 4 corner pipes. The analysis shows that the greenhouse is lifted and pulled the pipe out of the ground due to the sucking wind pressure. The behavior of the greenhouse is very much similar to that one real greenhouse failure. Therefore, not only we need to find the realistic boundary conditions for the supports, but also need to perform many more researches to find how to rest the pipe supports on the ground without losing economy.

        중등 컴퓨터과학교육을 위한 객체지향형 EPL '두리틀'의 적용 및 평가

        제 7차 컴퓨터교과 교육과정은 소프트웨어 활용 부분에 치중되어 컴퓨터과학에 관한 원리교육이 이루어지지 못하고 있다. 한편, ACM K-12 컴퓨터과학 교육과정 모델에 관한 보고서에서는 현재의 컴퓨터교육이 그릇된 방향이며 컴퓨터과학 중심의 컴퓨터교육을 위해서 프로그래밍언어 교육을 통한 원리교육의 중요성을 강조하고 있다. 본 논문에서는 프로그래밍 체험학습을 위한 객체지향 기반의 새로운 교육용프로그래밍언어인 두리틀(Dolittle)을 소개한다. 두리틀은 간결한 구문과 한국어와의 대응성, 인크리멘탈 방식의 프로그래밍, 수치, 문자열, 배열과 같은 알고리즘과 구조화, 텍스트 위주의 프로그래밍 등의 설계방침을 적용한 언어이다. 중학생을 대상으로 실제 수업에 적용해 보고 학습효과를 분석한 결과, 객체지향기반이지만 클래스나 상속 등의 개념을 모르고도 활용 가능한 교육용프로그래밍언어라는 것을 확인할 수 있었다. 또한 객체와 메소드 활용을 통하여 프로그래밍 결과를 쉽게 확인할 수 있고 오류 수정이 용이하므로, 학생들의 높은 흥미도를 유지하면서 학업성취도를 향상시키고 차후 학습으로의 연계가 수월하였다. Current computer education is difficult to educate basic concepts and principals of the computer science because the 7th curriculum of computer education is focused on the application of software. According to the ACM K-12 report about the computer science education model, current computer education is taking the wrong way and we should put the highly priority on the education of the fundamentals through programming languages for a better computer education oriented to the computer science. This paper introduces a new object-oriented educational programming language "Dolittle'". The design principals of Dolittle are simple syntax of Korean, incremental programming, text based programming, aliasing of function, and object-oriented programming. Being applied to middle school classes, we can confirm that Dolittle is easy to learn, and gives rise to high interest and keeps interest through a course,and also is of great practical use in class for programming novice.
