        Effect of an Inpatient Rehabilitation Program for Recovery of Deconditioning in Hematologic Cancer Patients After Chemotherapy

        Objective To investigate the effect of a rehabilitation program in terms of De Morton Mobility Index (DEMMI) score, in hematologic cancer patients after chemotherapy. Methods Hematologic cancer patients admitted for chemotherapy were reviewed. They received a rehabilitation program during their hospital stay. DEMMI score measurement was performed, before and after rehabilitation. Demographics, diagnosis, chemotherapy information, rehabilitation program duration, mortality, body mass index (BMI), and laboratory test results were collected. For analysis, patients were classified according to diagnosis (multiple myeloma, leukemia, and others), mortality, and additional chemotherapy. Results There was statistically significant improvement in DEMMI score of 10.1 points (95% confidence interval, 5.9–14.3) after rehabilitation. It was more evident in the multiple myeloma group, and they revealed less mortality. When patients were divided according to mortality, survivors received the program earlier, and in a shorter period than in mortality cases. Although survivors revealed higher initial DEMMI score, improvement after rehabilitation did not differ significantly. Conclusion In hematologic cancer patients, rehabilitation program was effective for recovery from deconditioning, revealing significant increase in DEMMI score. Multiple myeloma patients may be good candidates for rehabilitation. Rehabilitation could be sustained during chemotherapy and for high-risk patients.

        공초점 레이저 주사 현미경을 이용한 법랑질 초기 우식 재광화의 정량적 분석

        Dental Caries which has high prevalence rate, accounts for majority of dental diseases. Many treatment and preventive treatment has been developed, thereby reducing the prevalence rate, but in our country, flouridization has not spread widely yet, so prevention has not been done satisfactorily. When dental caries progresses, irreversible damage of tooth structure occurs. In initial dental caries. Demiteralized tooth structure can be remineralized, so restorative treatment is unnecessary. In the study, 20 teeth restored with composite resin without fluoride release were used and divided into two groups. Incipient dental caries were artificially made and remineralization procedure was done for 1 and 2 weeks, for each group. Changes in mineral contents aroud the margins were analysed with confocal laser scanning microscope. The results were as follow. 1.Both total fluorescence of the lesion and average fluorescence of the lesion of remineralized samples decreased compared to demineralized state. (p<0.01) 2.Confocal laser scanning microscopy can be used in quantitative analysis of mineral change. In result, confocal laser scanning microscopy can be used in quantitative analysis of mineral change and it could be used in many different fields of dentistry in the future.         Enterobacter Asburiae Pneumonia with Cavitation

        Enterobacter species have increasingly been identified as pathogens over the past several decades. These bacterial species have become more important because most are resistant to cephalothin and cefoxitin, and can produce extended-spectrum β-lactamase. Enterobacter asburiae (E. asburiae) is a gram-negative rod of the family Enterobacteriaceae, named in 1986. Since then, there has been only one clinical report of E. asburiae pneumonia. We report a case of E. asburiae pneumonia with cavitation and compare it with the previous case.

        제어코일 전류변화 패턴을 활용한 가스밀폐형 직류 접촉기 동작상태 판정 기법

        As electric vehicles and LVDC distribution systems expand, demands for DC contactor are increasing. DC Contactor is a device that acts as a switch in the system, and the most important performance is the stability of operation. To check if the on-off operation is normal or not, the pickup voltage applied to the control coil is measured and judged until now. However, it is difficult to confirm the complete operating on-off state of DC contactor by it. In order to solve this problem, this paper presents a method to determine whether DC contactor is completely on or off through some changes in the control coil current. A theoretical basis is presented to explain the change in electromotive force, and it is verified that the method is able to accurately determine the on-off state through experiments.

        Pure Intrasellar Meningioma Located Under the Pituitary Gland: Case Report

        Most intrasellar meningiomas are located in the subdiaphragmatic and supraglandular region because they originate from the diaphragma sellae. Subglandular meningiomas located under the pituitary gland are extremely rare.. Intrasellar meningiomas in the subdiaphragmatic and subglandular region probably originate from the dura in the sellar floor. We report a case of a subglandular meningioma along with a review of the literature.

        Reliable measurements of physiologic ankle syndesmosis widening using dynamic 3D ultrasonography: a preliminary study

        Purpose: The purpose of this study was to present a technique for measuring physiologic distal tibiofibular syndesmosis widening using 3-dimensional ultrasonography (3D-US) with an evaluation of its reliability, and to determine whether there were differences in the measurements between different dynamic stress tests. Methods: We retrospectively evaluated 3D-US of 20 subjects with normal ankle syndesmosis. 3D-US was performed in neutral (N), dorsiflexion with external rotation (DFER), and weightbearing standing (WB) positions at the anterior inferior tibiofibular ligament level in both ankles for comparison. Using 3D-US volume data, axial images were reconstructed at the level of the lateral prominence of the anterior tibial tubercle to ensure consistent measurements of the tibiofibular clear space (TFCS) by two radiologists. Results: There was a wide range of TFCS values among the subjects (N, 1.2 to 4.2 mm; DFER, 2.3 to 4.8 mm; WB, 1.7 to 4.6 mm). When both ankles of each subject were evaluated, the side-toside differences were less than 1 mm in all positions, with high intraclass correlation coefficient (ICC) values between both ankles (ICC, 0.85 to 0.93). The inter-rater agreement for all TFCS measurements between the two radiologists was excellent (ICC, 0.81 to 0.96). In comparisons between the two dynamic stress tests, the TFCS was significantly wider in the DFER position than in the WB position (DFER vs. WB, 3.3 mm vs. 2.9 mm; P<0.001). Conclusion: Using 3D-US, we were able to consistently evaluate the TFCS with good reliability. In a comparison of the two dynamic tests, there was more significant widening of the TFCS in the DFER position than in the WB position.

        2-spool 선박용 가스터빈 엔진의 동적 시뮬레이션 수학모델 개발

        In this study, a dynamic behavior simulation mathematical model of 2-spool marine gas turbine engine is developed. Specifically, we develop an engine performance model using data provided by an engine manufacturer for ease of integration with other component performance models of ship propulsion systems and for fast calculation processing speed. SIMULINK is used for dynamic behavior modeling, and performance data and steady-state convergence interval data are compared to verify the mathematical model. In addition, performance analysis of the transient mode, which increases from engine stop to maximum output, is performed and compared with manufacturer data. A simulation of the engine behaviors in terms of acceleration, deceleration, and stopping to stop, idling, and maximum output is performed using the verified model. 본 연구에서는 2-Spool 선박용 가스터빈 엔진의 동적거동 모사 수학 모델을 개발하였다. 선박 추진시스템의 다른구성품 성능 모델과의 통합의 용이함, 빠른 계산처리 속도를 위해 엔진 제작사로부터 제공받은 데이터를 이용하여 엔진성능 모델을 개발하였다. 동적 거동 모델링에는 SIMULINK를 사용하였으며, 수학적 모델의 검증을 위해 정상상태 수렴구간에서의 성능데이터와 제작사 제공 데이터를 비교하였다. 또한 엔진의 정지 상태에서 최대 출력까지 증가하는 천이모드의 성능해석을 수행하여 제작사 데이터와 비교하였다. 검증된 모델을 이용하여 정지, 공회전까지의 가속, 최대 출력까지의 가속, 감속 및 정지의 동작 거동 시뮬레이션을 수행하였다.

      • VDE 기반 클라우드 컴퓨팅 서버를 위한 클라이언트 단위 네트워크 성능 고립화

        클라우드 데이터 센터 내의 각 서버는 서로 다른 클라이언트에 속하는 복수의 가상 머신을 수행한다. 이들은 네트워크 자원을 공유하기 때문에 과도한 트래픽을 발생시키는 일부 클라이언트가 다른 클라이언트들의 서비스 품질을 급격히 저하시키는 문제가 발생할 수 있다. 이를 해결하기 위한 기존의 네트워크 고립화 기술들은 클라이언트에 대한 고려가 없거나 클라우드와 같은 대규모 네트워크에 적용하기 어렵다는 한계가 있다. 이 논문에서는 VDE(Virtual Distributed Ethernet)를 기본 프레임워크로 사용한 클라이언트 단위의 네트워크 성능 고립화 기법을 제안한다. 제안된 기법은 각 클라이언트에 가중치를 부여하고 이에 비례하여 네트워크 대역폭을 할당하기 위해 fair share 스케줄링과 송수신 패킷 dispatch를 수행한다. 제안된 기법은 기존 시스템에 최소한의 변경만을 가하여 구현 가능하므로 대규모 네트워크에 도입이 용이하다. 실험 결과 각 클라이언트는 가중치로부터 도출된 대역폭의 99.4% 이상을 할당 받는 것으로 나타나 제안된 기법의 효용성이 입증되었다.

      • VDE 기반 클라우드 데이터 센터를 위한 클라이언트 단위 네트워크 성능 고립화 : FS-VDE

        클라우드 데이터 센터는 계층적 네트워크로 연결된 수천 대의 서버로 구성되고 각 서버는 서로 다른 클라이언트에게 속하는 복수의 가상 머신을 수행한다. 이들은 서버의 네트워크 링크를 공유하기 때문에 서비스 품질 보장을 위해서는 클라이언트 단위로 네트워크 성능을 고립시키는 것이 매우 중요하다. 그런데 기존의 네트워크 성능 고립화 기술은 클라이언트라는 개념 없이 TCP 흐름 단위의 고립만을 지원하기 때문에 클라우드 데이터 센터에 효과적이지 못하다. 뿐만 아니라 기존 네트워크 설비와 게스트 OS에 수정을 요구하여 대규모 도입에 어려움이 있다. 이 논문에서는 이 같은 문제를 해결하기 위해 각 클라이언트에게 가중치를 부여하고 이에 비례하여 네트워크 대역폭을 할당해 주는 FS-VDE를 제안한다. 제안된 기법은 송신 트래픽에 대해 쉐이핑 메커니즘을 수행하고 수신 트래픽에 대해 폴리싱 메커니즘을 사용한다. 이 기법은 기존 시스템에 최소한의 변경만을 요구하므로 대규모 도입에 효과적이다. 우리는 FS-VDE를 Linux 상에 구현하고 실험을 통해 성능을 측정하였다. 실험 결과 각 클라이언트는 가중치로부터 도출된 송신 및 수신 대역폭에 비해 각각 99.4%, 96.5% 이상을 보장 받았으며 이는 단 5%의 성능 저하로 달성될 수 있었다. A cloud data center consists of thousands of servers connected by a hierarchical network. Each server runs multiple virtual machines owned by different clients. Since several virtual machines share network links, it is important to provide for per-client network performance isolation for quality of service guarantees. Unfortunately, existing network performance isolation techniques provide only per-flow isolation without the notion of clients and thus are not effective for a cloud data center. Moreover, these techniques lack scalability as they require non-trivial modification to legacy network equipment and the protocol stack of a guest OS. In this paper, we propose FS-VDE, a mechanism that assigns each client a weight and allocates it network bandwidth proportional to the weight. To do so, our approach performs outgoing traffic shaping and incoming traffic policing. FS-VDE can be widely adopted since it only requires limited modification to the host OS. We have implemented the proposed mechanism in Linux and measured the throughput of clients. Experimental results show that each client receives 99.4% and 96.5% of desired outgoing and incoming bandwidth, as specified by its weight, respectively. This result was achieved with only 5% of performance overhead.
