감염근관에서의 주치근단 파일의 크기는 학자들마다 많은 논란이 되고 있지만 최근 여러 연구 결과들은 감염근관의 효과적인 세정을 위해서는 근단부 확대 크기가 증가해야 함을 보여주고 있다. 따라서 임상가는 감염근관의 치료 시 새로운 개념에 맞는 치료 전략을 가지고 근관치료에 임해야 한다. The final preparation (MAF) size in infected root canals is still controversial. Nonetheless, recent studies demonstrated that larger apical preparation sizes produces a greater reduction in remaining bacteria and dentinal debris as compared to smaller apical preparation sizes. Therefore, clinicians should be practiced with treatment strategies guided by evidence-based information, especially in infected/failed root canals.
모형레진 근관에서 엔진 구동용 니켈-타이타늄 파일의 경사도의 변화에 따른 근관성형력의 비교

The aim of this study was to compare the preparation efficiency of nickel-titanium rotary files with different taper in simulated resin canals which have 40o mean curvature. The instruments used in this study were GT and .04 taper ProFile with ISO-sized tip, .04 taper ProFile with ISO-sized tip alone, and .02 taper Naviflex file. Prevalence of canal aberrations, change in working length was measured. A total of 30 composite images were made from pre- and post-canal scanned images using Corel photopaint 8.0 and then, the amount of coronal substance the instruments removed was calculated two-dimensionally on digitized images with the Brain C software to compare the relative enlarging efficiency. The prepared internal canal form was also evaluated three-dimensionally with impression technique. The results were as follows; 1. Instrumentation time for canal preparation was not significantly different among the three instrumentation groups(ANOVA, P>0.05). 2. The change of working length in Naviflex system was significantly high compared to the other two instrumentation groups(ANOVA, P<0.05) . 3. The incidence of zipping was significantly high for Naviflex groups compared to the other two groups(Kruskal-Wallis test, P<0.05). 4. The amount of coronal substance the GT with .04 taper ProFile group removed was significantly larger than the other two groups(ANOVA, P<0.05). 5. The prepared internal canal form in the GT with .04 taper ProFile group exhibited better taper, flow, and smoothness than the other two groups. Under the conditions of this study, the combined use of larger tapered nickel-titanium rotary files prepared simulated canals safely, efficiently and with good three-dimensional form.
