중풍장애노인의 생활스트레스와 우울간의 관계 -사회참여와 가족지지의 매개효과 검증-

본 연구는 중풍노인의 우울완화를 위한 개입방안을 모색하기 위하여 서울시 지역사회거주 중풍지체장애노인 385명을 대상으로 생활스트레스와 우울간의 관계에서의 사회참여와 가족지지의 매개효과를 검증하였다. 구조방정식모형을 이용하여 우울에 영향을 주는 생활스트레스의 직접효과를 파악하고, 사회참여와 가족지지의 매개효과를 검증한 연구의 결과는 다음과 같다. 첫째, 독립변인인 생활스트레스가 우울에 미치는 직접효과는 유의미하게 나타난 반면, 생활스트레스가 사회참여에 미치는 직접효과의 경로는 유의미하지 않게 나타나서 사회참여는 매개효과가 없는 것으로 나타났다. 둘째, 매개효과 검증에서 가족지지는 생활스트레스와 우울간 직접효과와 간접효과가 유의미하게 나타나 매개효과가 있는 것으로 나타났다. 셋째, 가족지지의 매개모형을 검증하기 위해 X2[수식] 값 비교와 Sobel 검증을 실시한 결과 부분매개모형으로 확인되었다. 이 결과를 바탕으로 중풍장애노인의 우울완화를 위한 개입 방안으로서 다양한 가족기능강화프로그램의 연구·개발의 필요성이 요구된다. The purpose of this paper was to find successful interventions to reduce depression in the Elderly with stroke disabilities. This study examined the mediation effect of social participation and family support in the relationship between daily hassles and depression among 385 Elderly with stroke disabilities living in Seoul city. Structural Equation Modeling was used to examine the direct effects of daily hassles to depression. Then, the mediation effect of social participation and family support were examined. The major findings were as followed: First, the direct effect of daily hassles on depression was statistically significant. The direct effect of daily hassle to social participation was not significant while social participation to depression was statistically significant. Therefore, social participation was not proved as a mediator. Second, the direct effect of daily hassles and family support was statistically significant and the indirect effect of family support to depression was also statistically significant. As a result, family support was shown to be a mediator between daily hassles and depression. After X2 difference and the Sobel test were performed, a partial mediator model was proved. Based on this result, it is necessary to research and develop the various strengthening family programs as a successful intervention that can reduce depression for stroke disabled elderly.

Actinomyces species are anaerobic or microaerophilic non-spore-forming gram-positive rods that may reach, occasionally, the normal female genital tract. Actinomycosis is uncommon and often presents as a complication of an intrauterine device (IUD). IUD and pessaries facilitate the access of the microorganisms to the pelvis. Almost 85% of cases occur in women who have had an IUD in place for 3 or more years. In fact, the detection rate of Actinomyces in patients with pelvic actinomycosis is as low as 2%. The diseases caused by Actinomyces spp. is often of difficult diagonsis. The diagnosis of actinomycosis can be confirmed by culture. However, it is often difficult to culture Actinomyces. Therefore a diagnosis of actinomycosis can be made from the finding of sulfur granules within inflammatory exudate on histologic examination after surgery. However, it may be possible to diagnose actinomycosis before surgery by the finding of Actinomyces-like organisms on Papanicolaou smears. The endometrial involvement is extremely rare. The authors report an unusual case of endometrial infection by Actinomyces in a 21 year-old female without intercourse, admitted into the institute with menorrhagia. She had received oral pills. Abdominal ultrasonography showed a endometrial thickening. The pre-diagnosis was endometrial hyperplasia. The endometrial biopsy was done. The sample from the endometrium developed actinomycosis. After procedure and treatment, the menorrhagia was disappeared.
