      • 혈액 투석 환자에서 중심정맥 협착에 대한 스텐트 삽입술 : Wallstent Placement

        Stenosis of central vein is a common complication arising after percutaneous subclavian vein catheter insertion performed for temporary vascular access in chronic renal failure patients undergoing hemodialysis. There are several treatment methods for the condition like percutaneous angioplasty(PTA), stent insertion, and surgery, but recent trend is toward PTA and stents. Among the patients diagnosed with chronic renal failure from March 1993 to May 2002 and undergoing hemodialysis through AV fistula, the 14 Patients in whom central vein stenosis arose were selected for the study. A total of 28 percutaneous interventions(5 PTA and 23 stent placement) were performed, and restenosis rate and the time taken till the restenosis in de novo lesions and instant lesions were compared. All 28 cases were operated successfully. The 14 cases that received both anigioplasty and stent placement initially. (de novo lesion : 14 cases), Among the 10 cases with de novo lesion that followed up more than 1 year, 3 cases are currently undergoing hemodialysis without restenosis, and the remaining 7 cases have recurred stenosis with the mean time to restenosis of 10.9 months. In the 7 cases in whom stenosis recurred, 11 interventions were done(instent lesion: 11 cases). 4 of these were using only ballon angioplasty with 100% restenosis rate and the mean time of 3 months until restenosis. The remaining 7 cases were using both balloon angioplasty and stent placement, also with 100% restenosis rate but with the mean time of 12 months until restenosis, which was later than the group receiving only balloon angioplasty. In treating the patients with central vein stenosis, stent placement seems to be more advantageous over PTA in terms of restenosis rate and the mean duration of patency. In the case of instent lesion, inserting the stent for the second time after stenosis recurred lengthened the duration of patency compared to performing balloon angioplasty alone.

        뇌졸중 환자에게 적용된 흡기근 훈련에 관한 국내 연구 동향 분석 : 체계적 고찰

        Purpose : This study aimed to analyze the effectiveness of inspiratory muscle training(IMT) studies for stroke patients conducted over the past 10 years(2011–2020) in Korea. Methods : Previous literature was electronically searched using the RISS, KISS, KCI, and NDSL databases. Tree databases were searched for stroke IMT articles. A total of 12 studies were selected according to the PRISMA guidelines, and the Jeong’g qualitative evaluation criteria were used to refer to the checklist presented by the Scottish Intercollegiate Guideline Network. Results : Twelve studies met the criteria. In a qualitative evaluation on the papers, one study scored 8 out of 10, seven scored between 6 and 7, and four scored 5. Intervention sessions were conducted between 20 and 30 min, each at a frequency of three to five sessions per week, for a total of 4–8 weeks. The results indicated that IMT effectively improved respiratory function, physical activity, and respiratory muscle function in stroke patients. Conclusion : IMT was found to have a positive effect on respiratory function, physical activity, and respiratory muscle function in stroke patients.

      • 남성화를 보이는 여성에서 발견된 난소의 Steroid Cell Tumor 1예

        Steroid cell tumor is a rare ovarian sex cord-stromal tumor which accounts for 0.1% of all ovarian tumors. Until now, only 4 cases have been reported in domestic literatures. Steroid cell tumor often secrets testosterone and presents virilization in adult women or precocious puberty in children. Treatment is often performed by surgical removal, adjuvant chemotherapy and radiation, but completely accepted treatment was not existed. We experienced a case of steroid cell tumor, which was manifested by typical virilization in a 43-year old patient, who was previously performed hysterectomy and unilateral oophorectomy. So, we present with a brief review of the literatures.

        With the widespread use of fertility medications and techniques of in vitro fertilization, multiple gestation has become epidemic in modern obstetric practice. Delivery of the initial fetus in a multiple gestation usually is followed by delivery of the subsequent fetus or fetuses shortly thereafter. However in rare circumstances, the delivery is delayed for days due to disappearance of uterine contraction after delivery of the first fetus. When uterine activity ceases after the delivery of the first neonate, a policy of nonintervention may be considered in case of an immature or very premature delivery, in order to achieve a gestational age for the remaining fetus(es) more compatible with neonatal survival. We report a case of twin gestation with prolongation of the delivery interval between the twins for 46 days.

        This study examined the nonpersonal sales activities such as international advertising, international sales promotion and international publicity which are the main vehicles of international promotion activities. With the keen competitions increasing in the world markets today, an enterprise must carry out the effective promotion activities to compete with the rival enterprises and to increase its own proceeds. Especially, international advertising is a powerful means of appeal for overseas sales promotion. It is an essential factor of the international promotion mix and one of the nonpersonal sales activities. And also, international sales promotion activities is an important means for increasing the personal sales activities and international advertising. Lastly, international publicity can be an effective means of promotion in that it can supply overseas consumers with information on the commodities and services. In conclusion, an enterprise will only be able to continue prosperity by overcoming the narrow home markets and expanding its sales to overseas markets. Accordingly, an international enterprise will have to actively perform the nonpersonal sales activities such as international advertising, international sales promotion and international publicity in order to accomplish the mass sales to overseas markets.

        This paper addresses a MR(Magneto-Resistive) sensor based vehicle speed detector which has been developed as an alternative to the conventional loop detector most available on the road. Consisting of two MR sensors and voltage amplifiers, microprocessor, RF module for wireless data transferring, the proposed detector can offer essential traffic data such as vehicle volume and speed. At first, basic speed detecting performance of the detector was verified with a simple car moving simulator in laboratory, and then real world field test was performed to show the practical usefulness as an alternative to the loop detector

        There are three methods in "Manuscripts". They are 1.positive analysis, 2.metaphysical method, 3.universal dialectics. They are conflicted with each other in "Manuscripts". In "Capital" there are two methods. They are 1.scientific method, 2.special dialectics. They are harmony with each other in "Capital". "Manuscripts" was the first note in which Marx had studied economy. At time Marx did'nt have any his economy catagory of his own. For this reason Marx criticized the position of the political economy from his philosophical view. This is the reason why there were different methods and they were conflicted with each other in "Manuscripts". At that time Marx had the view of Feuerbach's humunism. From this the problem came out. Because this humunism did'nt correspond with the positive analysis on the political economy. For this Marx introduced 'the dialectics of the labor'. In the dialectics Marx discribed human alienation and its sublation.

      • 객체 추출 알고리즘을 이용한 내용기반검색

        Recently there have been many efforts to support searching and browsing based on the visual content of image and multimedia data. Most existing approaches to content-based image retrieval rely on query by example or user based low-level features such as color, shape, texture. But these methods of query are not easy to use and restrict. In this paper we propose a method for automatic color object extraction of content-based image retrieval system. Theses approaches simplify the regions within images using similar colorizing algorithm and extract color object using proposed Color and Spatial based Binary tree map(CSB tree map). This allows very fast indexing of the image by color contents of the images and spatial attributes. Futhermore, information about the labelled regions, such as the color set, size, and location, enables variable multiple queries that combine both color content and spatial relationships of regions. We proved our proposed system to be high performance through experiment comparable with another algorithm using "Washington" image database.

        The controversy over the standardization of global advertising centers around the appropriateness of variation within advertising content from country to country. First discussed in 1961 by Elinder, the technique has generated a heated and lively debate among advocates on both sides and has been both praised and condemned. As a result, three schools of thought regarding advertising standardization can be identified: 1) standardization 2) adaptation 3) compromise. Advertisers who use the standardized approach argue that consumers anywhere in the world have the same basic needs and desires and can therefore be persuaded by universal appeals. On the other hand, advertisers who follow the localized approach assert that consumers differ from country to country and must accordingly be persuaded by universal appeals. As the debate over standardized advertising continued, a middle ground began to emerge. Several writers began to examine the factors that could influence the decision to standardize and, later, others considered the degree to which standardization could occur. These writers argued that in certain circumstances it may be desirable to standardize and in other situations it may not. While the desirability of advertising adaptation vs. standardization has long been debated within both academic and business circles, empirical studies investigating the conditions under which each strategy becomes appropriate have been rare. More empirical evidence is needed to resolve the standardization-adaptation controversy.

        We live in the commodity-society now. It means that our life is defined by the action of exchange which is mediated by commodities. The way of our life based on it is the mode of production by commodities, for commodities, of commodities. If my action and its result were not to become a commodity, I wouldn't enter into the relation of exchange. In commodity-society it signifies a death. Even though a commodity is only a thing which exists outside men, the being of men is determined by it. Thus in the commodity-society, paraphrasing Nietzsche's word, the will to commodity is universalized. This is the fetishism of the commodity. The fetishism is caused by the commodity-form itself. And the commodity-form presupposes the private property. It is contrary to the communal property. In the world based on the private property we remain alienated with each other, because we are nothing but the moment of the exchange of the commodity in the society dominated by the commodity-form. Accordingly the fetishism of the commodity is the utopia of private property. This utopia is the illusion of the commodity-society.
