        심장호흡물리치료의 비전

        Purpose : This study is designed to discuss the vision of the cardiorespiratory physical therapy for the future. The vision of the cardiorespiratory physcial therapy will be achieved with the enthusiasm and interests of physical therapists in Korea. Currently, academic conference for the cardiorespiratory physcial therapy has been received a great deal of attention from the medical societies. The vision of the cardiorespiratory physical therapy will be realized with various programs for the research and training in the cardiorespiratory physical therapy. Korean cardiorespiratory physical therapists’ mission is to improve public health by enhancing clinical excellence in physical therapy practice through credentialing clinical specialist. The Korean academy of cardiorespiratory physical therapy(KACRPT) needs to move toward the vision of cardiorespiratory physcial therapy for the public health promotion.

        뇌졸중 유병경험자와 정상 노인의 신체활동 강도에 따른 심혈관질환 위험요인 비교

        Purpose : This study aimed to analyze the difference in the cardiovascular risk factors between post-stroke elderly patients and healthy elderly patients, according to the intensity of physical activity, to determine the effect of physical activity on stroke-related cardiovascular risk factors. Methods : This study involved 4,956 elderly participants(aged >65 years) out of 24,269 subjects who participated in the 7th National Health and Nutrition Examination Survey(2016–2018). Variables that were risk factors for cardiovascular disease in the selected subjects were analyzed. Results : Total cholesterol was significantly higher in the healthy elderly group compared with the post-stroke elderly group(P < 0.05 ), and fasting blood glucose was significantly higher in the post-stroke elderly group compared with the healthy elderly group(P < 0.05 ). H DL c holesterol w as s ignificantly h igher in t he healthy e lderly group compared w ith the p ost-stroke elderly group(P < 0 .05 ). Conclusion: Comparison of the intensity of physical activity revealed the importance of blood sugar control through regular aerobic exercise and exercise intensity. It is suggested that this can serve as basic data for health promotion in stroke patients and the preventive management of cardiovascular disease in healthy elderly people.

        하지 보조기(Hands-Free Crutch)보행 시 20대 정상 성인의 산소 섭취량에 미치는 영향(case study)

        Purpose: This study aimed to investigate changes in oxygen intake during hands-free crutch gait compared with normal gait. Methods: This study was conducted on two adults—one male and one female, both in their 20. A 6-min walking test was conducted using normal gait and hands-free crutch gait performed on separate days. The peak oxygen intake(VO2peak) and lower limb muscle activity were compared using a portable metabolic analyzer and surface electromyography. Results: The results confirmed that hands-free crutch gait had a greater effect on the VO2peak and lower extremity muscle activity compared with normal gait. Conclusion: In the future, if patients with cardio and respiratory system diseases undergo lower extremity surgery or injury, not only lower extremity balance training but also cardio and respiratory system management and education will be required when using assistive devices.

        목빗근에 대한 수동 스트레칭 운동이 폐 기능에 미치는 영향

        Purpose : This study aimed to investigate the effect of passive stretching of the sternocleidomastoid(SCM) muscle on pulmonary function. Methods : The subjects were 20 female university students without neurological, muscular, or cardiopulmonary dysfunction. Pulmonary function was tested using a digital pulmonary function-measuring device(Pony FX, COSMED Inc., Italy). The experimental group performed three sets of passive stretching on the SCM muscles. One set included three sets of passive stretching. The control group did not perform any other exercises. After passive stretching, pulmonary function was remeasured. The statistics program was SPSSWIN(ver 25.0) package program. Results : The forced vital capacity(FVC), forced expiratory volume in one second(FEV1), and vital capacity(VC) were significantly improved in the experimental group after stretching exercise. However, there was no change in the control group. The changes in the FVC, FEV1, and maximal voluntary ventilation(MVV) were higher in the experimental group than in the control group. Conclusion : The results indicated that passive stretching exercise of the SCM muscle can improve pulmonary function. In particular, it was particularly effective for maximal effort vital capacity(MEVC) and MVV.

        The Comparison of Type of Music Tempo on Cardiopulmonary Function During Exercise

        Purpose: The study aimed to examine the effects of beats per minute (BPM) on the cardiopulmonary functions of adult men during exercise and to provide reference materials for future physical therapy studies of the respiratory system. Methods: This study recruited 18 healthy male adults who agreed to participate in this study and had no cardiopulmonary problems. The participants conducted a total of five experiments with no music, 30 BPM, 130 BPM, 180 BPM, and 230 BPM, and music education was conducted regarding the music experience before the experiment. The experiment was conducted twice a week for a total of 3 weeks, and one-way analysis of variance was used to compare the mean differences in VO₂max, time, respiratory rate (RR), metabolic equivalents (METs), and peak time between each BPM. Results: For each BPM, the VO2max, time, RR, METs, and peak time showed significant differences. With 180 BPM, the VO₂max, time, RR, METs, and peak time were high. For each BPM, the VO₂max, time, RR, METs, and peak time showed significant differences. With 180 BPM, the VO₂max, time, RR, METs, and peak time were high. Conclusion: The results of this study show that the most effective BPM for cardiopulmonary functions is 180 BPM, and 180 BPM will enable to perform exercises more efficiently.

      • 세라밴드운동과 노인의 심장 자율신경 활동에 대한 PSA 평가

        Purpose: The purpose of this study is to find out the effect of Thera-Band exercise on cardiovascular autonomic nerve system activity with the Heart Rate Variability Power Spectral Analysis(HRV PSA) by using “The Biopac and Program”. Metods: The old men were divided into two groups, Thera-Band exercise group(EG, n=13) and control group(CG, n=8).They participated in pre-test and after 8 weeks’ Thera-Band exercise. The Biopac System Test conducted for 5 minutes. Results: Heart Rate(HR) significantly effected after Thera-Band exercise. But no significant changes in CG. R-wave significantly effected after Thera-Band exercise. But no significant changes in CG. The present observations provide new information(PS form) on the contribution of the cardiovascular autonomic function from Power Spectral Analysis(PSA) to the exercise responses. High frequency(HF, 0.15-0.4Hz), as the marker of parasympathetic nerve activity tone, is the distributed power 10 -15 ~10 -17. Low frequency(LF, 0.04-0.15Hz), as the marker of sympathetic nerve activity tone, is distributed power 10 -12~10 -10. The common frequency doma- in(0.135-0.3Hz) of PSN․SN, as the marker of parasympathetic and sympathetic nerve activity tone. is the distributed power 10 -16~10 -11. Conclusion: From the discussions above results, we can conclude that Thera-Band exercise in old men induce the effect of autonomic nerve activity. These results are associated with increased and decreased HR and R-wave after Thera-Band exercise. Finally, in order to Normalized for the Power Spectrum, the further experimental work is needed to under-stand the possible mechanistic link between the cardiovascular autonomic function and the exercise responses.

        코로나 바이러스로 인한 폐 섬유증에 대한 호흡물리치료 임상사례

        Purpose : This case study aimed to investigate the effectiveness of respiratory physical therapy for pulmonary fibrosis caused by coronavirus disease 2019(COVID-19). Methods : A patient with pulmonary fibrosis caused by COVID-19 underwent respiratory physical therapy and education for diaphragmatic breathing patterns, ventilatory movement strategies, positioning for dyspnea relief, diaphragmatic controlled breathing, and airway clearance techniques for 3–4 months. Results : The patients with pulmonary fibrosis caused by COVID-19 who presented with dyspnea, fatigue, and general weakness showed improvement after 3–4 months of respiratory physical therapy. The oxygen supply was reduced from 3 L/min with a T-piece to 1 L/min using a nasal cannula, thereby stabilizing the patient’s breathing. In addition, functional mobility, such as bridging, rolling, sitting on a bed, and wheelchair transfer on a bed, became possible, and symptoms could be alleviated using a diaphragm breathing pattern for dyspnea. Conclusion : The results of the intervention indicate that respiratory physiotherapy is an essential interventional approach for pulmonary fibrosis caused by COVID-19.

        남성 심부전 환자의 넙다리 네갈래근 근력이 최대 유산소 능력과 재입원에 미치는 영향

        Purpose: The cardiopulmonary exercise test (CPET) is widely used to evaluate the clinical prognosis in patients with heart failure (HF). However, some patients with HF cannot perform CPET at maximal exercise intensity because of unstable medical condition or musculoskeletal problems. Thus, the study aimed to investigate the effects of quadriceps maximal voluntary isometric contraction (Q-MVIC) strength on aerobic capacity and re-hospitalization (RH). Methods: Patients with HF (n = 60, 59.03 ± 13.4 years, male 100%) were recruited in this study. We evaluated the Q-MVIC strength and aerobic capacity after discharge from hospitalization and followed up the RH rate during home-based self-care periods. Results: The RH (n = 15, 64.00 ± 14.69) group showed significantly lower Q-MVIC strength (p = .028) than the no RH(NRH, n = 45, 57.78 ± 13.15) group. In addition, the RH group showed significantly lower peak VO2 (p < .001) than the NRH group. Moreover, the Q-MVIC strength and peak VO2 demonstrated significant correlation (r = .414, p < .001). Conclusion: Our study showed that the knee extensor strength has strong predictive values on aerobic capacity and RH rate in patients with HF.

        둥근 어깨 자세 교정이 호흡 및 집중력에 미치는 효과

        Purpose: The study aimed to investigate the effect of round shoulder posture correction on respiration and concentration. Methods: A total of 34 college students were included in this study. The participants were divided into three groups: group 1, stretching and stability (n = 12); group 2, stretching and strengthening (n = 12), and group 3, control (10). The exercise interventions were conducted for 4 weeks. Group 1 performed warm-up, stretching, muscle strengthening, and cool down for 30 min. Group 2 performed warm-up, stretching, stabilization, and cool down for 30 min. Group 3 did not receive any intervention. All groups performed round shoulder posture tests, lung capacity tests to determine respiratory function, stroop test, and concentration ability test to determine the concentrations before and after intervention. Results: Significant differences were found in the round shoulder posture on the left between before and after interventions over time (p < 0.05) and between groups (p < 0.05). However, no interaction was observed. The post-hoc test showed that the interventions in group 2 were more effective than those in group 3. A significant difference was noted in the round shoulder posture on the right between before and after intervention over time (p < 0.05). No significant difference and interaction were found between groups. No significant difference was noted in lung capacity. A significant difference in the stroop test (time and number of wrong answers) was found between before and after intervention over time (p < 0.05). No significant difference and interaction were found between the groups. No significant difference was found in the concentration ability test. Conclusion: Our results reveal some evidence that stretching and strengthening exercise causes a decrease in round shoulder posture. However, the intervention period was short and did not allow investigation of the effects on respiratory function and concentration. Continued research on this subject could provide benefits to physiotherapy as it relates to cardiopulmonary treatment.

        심부전 환자의 신체적 수행 능력에 따른 도구적 일상생활 수준과 인지 수준의 연관성

        Purpose: This study aimed to investigate the level of physical functional ability in patients with heart failure and to suggest the correlation between cognitive level and instrumental daily activity level with physical function ability. Methods: The study included individuals who were diagnosed with heart failure and those who were not diagnosed with stroke. Among 66 patients who were tested for weakness at S Hospital located in Gyeonggi-do from June 2021 to April 2022, data from 56 patients were selected and analyzed. Results: In the correlation between MMSE-K and SPPB, balance (r = .381, p < .01), gait speed (r = .368, p < .01), chair stand (r = .245, p >.05), and total score (r = .387, p < .01) showed a positive correlation except for the chair stand. In the correlation between K-IADL and SPPB, balance (r = −.532, p < .01), gait speed (r = −.644, p < .01), chair stand (r = −.391, p < .01), and total score (r = −.600, p < .01) showed a negative correlation. The higher the simple physical function, the higher the cognitive level and the lower the level of need for help in the level of instrumental daily activities. Conclusion: A correlation was found between the level of physical function and the level of cognitive and instrumental daily activities in patients with heart failure; thus, its evaluation is essential.
