      • 미세알부민뇨를 동반한 인슐린 비의존성 당뇨병 환자에서 혈장 Endothelin의 변화

        목적 : 혈관내피세포에서 생성되는 endothelin(ET)는 강력한 혈관수축작용을 가진 펩타이드로 당뇨병 환자에서 혈관내피세포의 손상여부에 따라 혈중 농도가 증가할 수 있으며 혈관합병증과 관련이 있다는 보고가 있다. 그러나. 혈중 ET치의 상승과 당뇨병성 합병증의 상관성에 대해서는 논란이 많다. 이에 저자들은 인슐린 비의존성 당뇨병 환자에서 미세알부민뇨의 동반유무에 따른 혈중 ET치의 변화 및 혈장 ET치와 요중 알부민 배설률과의 상관성을 검토하였다. 대상 및 방법 : 인슐린 비의존성 당뇨병 환자중 미세알부민뇨를 보인 22명과 정상알부민뇨를 보인 43명을 대상으로 하였으며 혈장 ET를 측정하여 요중 알부민 배설률, 당뇨병성 망막증 및 당뇨병 관련인자들과의 관계를 비교하였다. 결과 : 당뇨병성 망막증이 합병된 군에서 망막증이 합병되지 않은 군에 비하여 혈장 ET치가 유의하게 높았던 반면 (각각 1.9±0.66, 1.5±0.76pg/㎖, p<0.05) 미세알부민뇨를 보인 군과 정상알부민뇨를 보인 군사이에 혈장 ET치는 통계학적으로 유의한 차이가 없었다(각각 1.9±1.14, 1.7±0.69pg/㎖). 또한 요중 알부민 배설률과 혈장 ET치 사이에 유의한 상관관계는 없었으며 미세알부민뇨를 동반한 인슐린 비의존성 당뇨병 환자군에서 혈장 ET치는 당뇨병의 관련인자들과도 유의한 상관관계가 없었다. 결론 : 인슐린 비의존 당뇨병 환자에서 혈장 ET치가 초기 당뇨병성 신증의 합병여부나 미세알부민뇨의 정도를 반영하기는 어려울 것임을 시사한다. Background : Endothelin(ET), a potent vasoconstritor peptide released from vascular endothelial cells, is known to be increased in diabetic patients with vascular damage and related with diabetic angiopathy. However, whether plasma ET levels may be related to diabetic complications is controversial. We studied the change of ET levels in non-insulin dependent diabetics with microalbuminuria and relation between ET and urinary albumin excretion rate. Methods : Plasma ET levels of 22 non-insulin dependent diabetics with microalbuminuria and 43 patients with normoalbuminuria were measured and relations of plasma ET to diabetic complications such as urinary albumin excretion rate, retinopathy and other associated factors were studied. Results : Plasma ET levels were significantly higher in patients with diabetic retinopathy than those without retinopathy(1.9 ± 0.66 vs 1.5 ± 0.76 pg/㎖. p<0.05). The difference, however, between diabetics with and without microalbuminuria was not significant(1.9 ± 1.14 vs 1.7 ± 0.69 pg/㎖). There were no correlations of plasma ET to urinary albumin excretion rate and to the other associated factors in non-insulin dependent diabetics. Conclusion : These results suggest that plasma ET levels may not seem to act as a predictor of diabetic nephropathy or microalbuminuria in non-insulin dependent diabetics.

      • Geotextile 利用效果에 관한 硏究

        In construction of embankment over soft strata in road and river, Geotextile has been increasingly used recently after identified that it was very economical and effective as seperater, reinforceing material, and filter of soft ground. This study deals with the effects of bearing capacity, permeability and stability by computing using STABR program of Modified Simplified Bishop method. The STABR program is applied to Hyunsan River and Hakkyo River embankments for the stability analysis in cases of with and without Geotextile. The conclusions are as follows; 1) The increase of safety of river embankment is not so much in case of with Geotextile. 2) From the results of permeability varying procedure analysis for the embankment construction of Sapkyo dyke. it reveals about 100 times as decreased as initial permeability coefficient in one year.

      • 정상 성인에 경구 투여한 Cefixime 두 정제의 생물학적 동등성 비교

        목적 : 식품의약안전청 고시 제 1998-86호(1998. 8.26) "생물학적 동등성 시험 기준"에 의하여 섹심 캅셀(시험약, 한국유나이트제약)과 슈프락스 캅셀(대조약, 동아제약)의 생물학적 동등성을 비교 검토하였다. 방법 : 2×2 라틴 방격법에 따라 건강한 성인 남성 지원자 20명에게 cefixime 100㎎씩 경구투여하였다. 약제투여 후 12시간까지 일정간격으로 채혈하였고 각 피험자들의 혈중약물농도 데이터로부터 구한 Area under the concentration (AUC), C_(max), T_(max) 등의 생체이용률 파라미터에 대해 통계학적으로 고찰하여 두 제제간의 생물학적 동등성을 평가하였다. 결과 : AUC, C_(max), T_(max)에 대한 평균값의 차가 각각 1.26%, 3.73%, 6.21%로서 모두 대조약의 20% 이내에 들어 생물학적 동등성 판정기준의 전제조건을 만족하였다. 분산분석 결과 AUC, C_(max), T_(max)의 각 비교 항목에 있어서 모두 유의수준 α=0.05의 군/순서효과 검정에서 F비가 F 분석표의 한계값보다 작아 교차시험이 제대로 이루어졌음을 확인할 수 있었으며, AUC, C_(max), T_(max) 각 비교항목에 있어서 유의수준 α=0.05에서 대조약에 대한 최소검출차(Δ)가 각각 17.74%, 17.05%, 15.67%로 모두 20% 이내이었으며, 검출력(1-β)도 모두 0.8 이상으로 나타나 생물학적 동등성 판정기준을 만족시켰다. 또 AUC, C_(max), T_(max) 각 비교항목에 있어서 대조약에 대한 생체이용률 차이의 신뢰한계(δ)가 ±20% 이내이어야 한다는 생물학적 동등성 판정기준을 모두 만족시켰다. 결론 : 이상의 실험결과로 시험약은 대조약에 대하여 생물학적 동등성 평가항목인 AUC, C_(max), T_(max)에서 모두 판정기준을 만족시켜 시험약은 대조약과 생물학적으로 동등하다고 사료된다. Background : The study was designed to compare the bioeqivalence between the test (CEXIME®. United Korea. Co., Ltd) and control (SUPRAX®. Dong-a Pharm. Co.. Ltd) capsules of cefixime. Methods : Twenty. healthy, male volunteers completed the study Each subject ingested single dose (100 ㎍) of the control and test drugs in 2?2 crossover design with a week drug-free interval between the doses. Blood samples were collected serially up to 12 hours and plasma concentrations of cefixime were measured by high-performance liquid chromatography with UV detection. A non-compartmental approach was used for analysis of the concentration-time data. Results : The area under the curve (AUC), the maximum concentration of drug (C_(max)). and time to reach C_(max)(T_(max)) did not show any significant difference between the two preparations by ANOVA. The mean differences of AUC, C_(max), and T_(max) were within 20% of the control drug, those were 1.26%. 3.73%. and 6.21%. respectively. The confidence limits of three parameters were satisfied the bioeqivalence criteria. Conclusion : These results suggest that the test drug was bioequivalent with the control drug.

        잠재적 간성혼수 환자의 인지기능과 양성자 자기공명분광 소견과의 상관관계

        연구목적 : 잠재성 간성혼수 환자들의 신경인지기능 장애 정도와 기저 핵이나 두정엽 백질부에서의 자기공명분광 (proton MRS)으로 측정한 양성자 함유 화합물의 변화와의 상관관계를 조사하고자 하였다. 방법: 한국판 웩슬러 지능 검사(KWIS) 중 토막짜기나 기호 바꾸기, 그리고 번호잇기 검사를 적용하여 잠재성 간성혼수로 정의된 18명의 간경변 환자를 대상군으로 하고 20명의 정상인을 대조군으로 하였다. 복합주의력과 시각 정보 분석능력 측정을 위해 Vienna 전산화 신경인지기능 검사 중, Cognitrone 검사. 미세 협응 운동능력을 위해 Grooved pegboard 검사를 적용하였단 1.5 Tesla 임상용 자기공명 촬영기를 이용해 자기공련명 분광을 얻었으며 두정엽 백질부와 기저핵의 각각 2×2×2cm3 vocel에서의 myoinositol(ml), N-acetyl-L-aspartate(NAA). creative(Cr), choline(Cho) 등의 농도를 측정하고 Cr 농도에 대한 상대적 농도비율을 구하여 분석하였다. 결과: 1) 잠재성 간성혼수 환자들의 기저핵과 두정엽 백질부에서 Cho/cr 및 ml/Cr 비가 감소하였다. 2) 미세 협응 운동능력을 반영하는 Grooved pegboard 검사의 수행능력이 기저핵의 ml 및 두정엽 백질부의 Cho농도와 유의미한 역상관관계를 보였다(r= -0.59. p<.05). 3) Cognitrone 검사 중 정반응하는데 걸리는 평균시간은 두정엽 백질부의 NAA와 역 상관관계를 보였다(r= -0.55, p<.05) . 결론 : 잠재성 간성혼수 환자들의 특정 신경인지기능의 장애는 양성자 자기공명분광법으로 측정한 기저핵 및 두정엽 백질부의 신경화학적 변화와 상관관계를 보인다. 하지만 이러한 신경화학적 변화가 잠재성 간성 혼수의 근본적 병리 생리인지 2차적 변화인지를 규명하기 위해서는 질병의 경과에 따른 신경인지기능 및 양성자 자기공명분광 소견의 추적연구가 필요하며 비슷한 인지기능의 장애를 보이는 다른 신경퇴행성 질환과의 비교 연구가 필요하다. 0bjectives To investigate the relationship of neurochemical changes of brain in SCHE patients and the level of cognitive function, the authors measured hydrogen containing neurochemicals in two brain regions using 1H MRS, and compared those changes with the level of cognitive performances such as attention, visual analysis, or fine motor function. Methods ; A total of patients with liver cirrohosis were defined as SCHE, for they performed poorly(out of 1 SD of normative data) in more than one neuropsychological tests conventionally used(digit symbol and block design tests in KWIS, trail making test A and B). They were further evaluated in attentional ability and efficacy of visual analysis using Cognitrone subtest in Vienna Neurocognitive Test battery. Fine motor coordination were also measured by Grooved Pegboard test. Patients and 20 nodal controls underwent proton MRS study. Proton containg neurochemirals , such as myoinositol(mI) , N-acetrl-L-aspartate(NAA) , creating (Cr) , choline(Cho) were measured from 2*2*2cm3 voxel of basal ganglia and parietal white matter using 1.5 tesla dinical MRI/MRS system. The ratios of above metabolites to Cr were analyzed. Results : 1) Patients with SCHE showed reductions in Cho/Cr and ml/Cr in both basal ganglia and parietal white matter compared to normal subjects. 2) Performance of Grooved Pegboard test were negatively correlated with ml level of basal gang1ia and with Cho level of parietal white matter(r= -0.59, p<.05). 3) Mean time of correct responses in Cognitrone test skewed negative correlation with NAA level of parietal white matter(r= -0.55, p<.05). Conclusion : Certain neurocognitive disturbances in SCHE patients seemed to be related with neurochemical changes in basal ganglia or parietal white matter. To further elucidate the relationship of focal biochemical changes and neurocognitive deficits in SCHE patients, however, fo11ow-up study according to the illness stage must be performed. Studies on other disorders showing similar cognitive deficit patterns would be helpful.

      • 전침 자극에 의한 오디 괄약근 운동성 변화에 관한 연구

        Background/Aims: This study was designed to evaluated the effects of electroacupuncture on the sphincter of Oddi (SO) motility in humans and to correlate the manometric findings with plasma cholecystokinin (CCK) levels. Methods: Eleven patients (M: F= 5: 6) who had various kinds of biliary disorders were enrolled. SO motility was monitored with a conventional low-compliant continuously perfused technique using ERCP (n=9) or percutaneous transhepatic cholangioscope (n=2). After baseline monitoring for phasic wave contractions of SO, electroacupuncture was applied at a specific acupoint, GB 34, in these 11 patients. A nonspecific acupoint 5 cm away from GB 34 was selected as a control. Manometric parameters of SO were also checked in 6 subjects during stimulation of the control acupoint. CCK plasma levels were measured during the time course of electroacupuncture stimulation. Result: All the manometric parameters including the basal pressure of SO, amplitude, frequency and duration of phasic wave contraction of SO were significantly decreased (p< 0.05) during electroacupuncture stimulation. The inhibition of SO contractility was accompanied by increased CCK plasma levels. After removal of electroacupuncture stimulation, restoration of amplitude and duration to basal condition were noted. A tendency towards the return of SO contractility was also observed in basal pressure and frequency. Stimulation of the control acupoint, however, did not affect the SO contractility and plasma CCK levels. Conclusions: Electroacupuncture stimulation of acupoint GB 34 showed reversible inhibition of SO contraction in humans. We speculate that the response of SO to electroacupuncture stimulation may be mediated by some neurohormonal mechanisms including CCK release.         The Internet of Things, the core of the Fourth Industrial Revolution, is growing by incorporating various technologies into various fields. Among them, smart home systems that apply IoT technology to home environments are being actively studied. The home hub market, a means of communication for smart homes, is also developing at the same time. As the home hub market develops, security threats are also increasing accordingly. In this paper, 'Smart Home Mirror' is implemented by combining mirror and IoT technologies so that users can use them to communicate with smart home applications and other devices. To meet security issues, users in it are recognized by using face recognition technology and AES between all data communications in it is applied. The proposed device’s usefulness is approved by implementing it in smart home environments.

        혈액종양병동 입원환자에서 Vancomycin 내성 장구균의 직장내 감시 배양 연구

        목적 : Vancomycin 내성장구균(VRE)은 병원감염의 중요한 원인균으로 VRE 보균상태의 환자들을 파악하여 감염을 예방할 수 있는 감시체계가 필요하다. 본 연구에서는 혈액종양병동 입원 환자를 대상으로 VRE의 직장내 감시배양을 실시하여 그 빈도 및 분포를 알아보고, 집락화된 균주와 임상검체에서 분리된 VRE와의 관계를 알아보고자 하였다. 방법 : 가톨릭대학교 성모병원 혈액종양병동에 입원해 있는 환자를 대상으로 14개월간 직장 감시배양을 시행하였다. 혈액한천배지에서 장구균이 의심되는 집락을 따서 vancomycin 6㎍/㎖이 포함된 Brain-heart infusion agar에 10^(6) CFU를 접종하여 vancomycin 내성선별검사를 하였다. 이 기간 동안 4병실에서 환경감시배양을 시행하였다. 분리된 균주는 Microscan을 이용하여 균종을 동정하였고 vancomycin에 대한 최소억제농도를 측정하였다. Multiplex PCR로 균종 확인 및 동시에 vancomycin 내성 유전형을 알아보았다. 결과 : 대상이 된 452명의 환자 중 230명(50.9%)이 직장내 VRE 보균자로 확인되었다. 균종의 분포는 E. faecium (72.4%), E. faecalis(24.1%) 순으로 vancomycin에 대한 MIC가 128㎍/㎖ 이상의 고도 내성이었으며, vanA형이 대부분이었다. 환경 감시배양에서는 2개 병실에서 VRE 4주가 분리되었다(E. faecium 3주, E. faecalis 1주). 감시기간동안 32명의 환자의 임상검체에서 33주의 VRE가 동정되었고 이 중 29명(90.6%)이 직장내 VRE 보균자였다. 결론: 가톨릭의대 성모병원 혈액종양병동의 VRE의 직장 내 집락화율이 높음을 알 수 있었다. 특히 임상검체에서 VRE가 분리된 환자의 경우 VRE 직장내 집락화율이 유의하게 높게 나타나 보균상태의 VRE가 환자의 감염에 직접적인 연관이 있을 수 있으므로 이를 예방하고 관리하는 지침이 필요하다고 생각한다. Background: Vancomycin-resistant enterococci (VRE) have emerged as important pathogens among hospitalized patients. We evaluated the prevalence of rectal VRE colonization at hematology-on-cology unit and the correlation between VRE colonization and infection. Materials & Methods: All VRE isolates were obtained from rectal swabs and clinical samples of the patients hospitalized over 14-month period at hematology-oncology unit in St. Mary's Hospital. The agar screening test was performed by using 6 ug/ml of vancomycin in brain heart infusion agar with 10^(6) CFU inoculum. During this period, the environmental surveillance culture was performed in 4 hospital rooms. Species identification of VRE was done by Microscan and MICs to vancomycin. Identification and genotyping of VRE pattern were confirmed by multiplex PCR. Results: Rectal colonization rate was 50.9% (230 among 452 patients). The most common isolate was E. faecium (72.4%) followed by E. faecalis (24.1%), and all had vanA genotype. Four VREs (three E. faecium and one E. faecalis) were isolated from 2 hospital rooms. During this period, 33 clinical VRE strains from 32 patients were isolated (infected or colonized). Twenty nine (90.6%) of these clinical isolates came from VRE carriers. Conclusion: This study showed a high rectal colonization rate of VRE in patients hospitalized at the hematology-oncology unit in St. Mary's Hospital. The colonization rate of VRE in the rectum was higher among the patients who had clinical infection by VRE. Therefore, the strategies for prevention and control of VRE infection should be deveoloped and monitored.

        The role of nitric oxide (NO) in the regulation of exocrine secretion was investigated in chicken pancreatic tissues. Nitric oxide synthase (NOS) in the pancreatic tissues were Ca^2+-dependent form. Treatment of tissue with L-arginine (10 mM) or carbachol (10^-5 M) showed the increase of pancreatic exocrine secretion, but that of D-arginine (10 mM) did not. ^3H-citrulline formation, cyclic GMP level and methyl arginine contents in the homogenates of pancreatic tissues treated with carbachol were not affected. Although the action mechanism of NO in the pancreatic tissues remains unknown, NO apparently participates significantly in the overall control of pancreatic secretory function.

        결핵성 및 암성 흉수의 감별진단에서 흉수내 SC5b - 9 농도 측정의 진단적 의의

        N/A To evaluate the diagnotic value of pleural SCSb-9 in the differential diagnosis of tuberculous and malignant pleural effusion, we measured pleural ADA activity and SCSb-9 concentration in 71 cases of pleural effusions due to tuberculosis (n=35) and malignancy (n=36). And we also measured pleural C4d fragment and Bb fragment concentration to determine by which pathway the complement activation occurs in pleural space. The results were as follows: 1) The median value of pleural SC5b-9 concentration of tuberculous effusion was 6.4 ug/ml (interquartile range 2.6~11.7) and was significantly higher than that of malignant effusion (p<0.005). 2) The median values of pleural C4d fragment concentration were 15.5 mg/ml (9.9~18.9) in tuberculous effusion and 12.8 mg/ml (7.3~19.6) in malignant effusion. There were no significant difference between two groups. The median value of pleural Bb fragment concentration of tuberculous effusion was 7.8 mg/ml (5.5~12.8) and was significantly higher than that of malignant effusion (p<0.01). 3) The SC5b-9 concentration showed no significant correlation with C4d fragment concentration both in tuberculous effusion and malignant effusion. But the SC5b-9 concentration showed significant correlation with Bb fragment concentration both in tuberculous effusion (r=0.48, p<0.005) and malignant effusion (r=0.72, p<0.001). 4) The median value of pleural ADA activity of tuberculous effusion was 64.3 U/L (40.9~103.5) and significantly higher than that of malignant effusion (p<0.001). 5) Pleural ADA activity showed significant correlation with pleural SC5b-9 concentration in tuberculous effusion (r=0.53, p<0.005). 6) According to the receiver-operating characteristic curve analysis, the measurement of pleural ADA activity is superior to that of pleural SC5b-9 concentration in the differential diagnosis of tuberculous and malignant effusion. In conclusion, the measurement of pleural SC5b-9 concentration can be used as an adjunctive test in the differential diagnosis of tuberculous and malignant effusion but pleural ADA activity measurement was superior to pleural SC5b-9 concentration measurement. And pleural SC5b-9 concentration of tuberculous effusion was higher than that of malignant effusion and it seems to be due to the difference in activation of alternative pathway activation of complement in two groups.
