        아동기 과체중 위험 인자로서의 TV시청시간, 사회계층요인, 부모의 과체중 및 부모의 활동수준

        This study was done to determine the factors associated with childhood overweight in 721 sixth grade elementary school students, in Busan. The students' heights, weights, waist circumferences and triceps-skinfold thicknesses were measured using standard techniques. Other data were collected using a questionnaire that included information about physical activity, television watching, and the amount of exorcise taken during leisure times, family history of diseases related to obesity: social data including family income, parents'education and occupations, eating behaviors; parental weights and heights; and parental activity levels. Childhood overweight was defined as a body mass index at or above the 85th percentile for age and sex. The prevalence of overweight revealed no significant difference between sexes, (24.2% in boys and 22.03% in girls). The risk of childhood overweight was significantly greater if either the mother or the father were overweight. The odds ratio for childhood overweight associated with maternal overweight was 5.045 (94% CI : 3.262-7.801), and 2.727 (95% CI : 1.764-4.218) was the case for parental overweight. Children having a history of hear diseases had higher odds ratios than those who did not. The odds ratios for overweight associated with income were not different. However, a higher odds ratio for overweight was observed in children whose fathers had only an elementary or middle school education than those whore fathers had a high school or college education. Children whose fathers' occupations were service workers or shopkeepers (OR : 3.314, 95% C = 1.851-5.934) or had no occupation (OR = 3.756, 95% CI : 1.898-7.430) had a treater risk of overweight than those whose fathers'were professionals or once workers. The risk of overweight increased in children having more irregular meal times and faster eating times, rather than those having an intake pattern of high energy and sugar containing floods. The amount of exercise taken during leisure times, and daily physical activity showed no difference between overweight and non-overweight children. However, television watching time, especially on weekends, was greater in overweight children than in non-overweight children. Television watching time was positively correlated with BMI, triceps-skin(31d thickness, waist circumference and waist/height ratio. Therefore, television watching was found to be a useful predictor of overweight in children. Television watching in children was negatively related to paternal activity levels, and positively related to parental television watching time. In fact, fathers whose children were overweight were physically less active than fathers whose children were non-overweight. Parents appeared to be a strong influence on their children's physical activity levels. In conclusion, a low family social class, defined on the basis of the father's occupation or education, parental overweight, increased television watching, and unhealthy physical activity levels in parents were all considered risk factors for childhood overweight. Among these, television watching time and lack of physical activity were considered to be the most important risk factors that could be easily modified for the prevention of and intervention in, overweight in children.

        Assessment of Relationship between Fyn-related Kinase Gene Polymorphisms and Overweight/Obesity in Korean Population

        The fyn-related kinase (FRK) belongs to the tyrosine kinase family of protein kinases. Recent studies have shown that Frk affects pancreatic beta cell number during embryogenesis and promotes beta cell cytotoxic signals in response to streptozotocin. To investigate the genetic association between FRK polymorphisms and the risk of obesity in Korean population, single nucleotide polymorphisms (SNPs) in the FRK gene region were selected and analyzed. The body mass index (BMI) was calculated, and biochemical data (systolic blood pressure, diastolic blood pressure, hemoglobin A1C, triglyceride, total cholesterol, high density lipoprotein, and low density lipoprotein) of blood sample from each subject were also measured. One hundred fifty five healthy control and 204 overweight/obesity subjects were recruited. Genotype frequencies of six SNPs [rs6568920 (+8391G>A), rs3756772 (+56780A>G), rs3798234 (+75687C>T), rs9384970 (+68506G>A), rs1933739 (+72978G>A), and rs9400883 (+ 75809A>G)] in the FRK gene were determined by Affymetrix Targeted Genotyping Chip data. According to the classification of Korean Society for the Study of Obesity, control (BMI 18 to <23) and overweight/obesity (BMI≥23) subjects were recruited. For the analysis of genetic data, EM algorithm, SNPStats, Haploview, HapAnalyzer, SNPAnalyzer, and Helixtree programs were used. Multiple logistic regression analysis (codominant, dominant, and recessive models) was performed. Age and gender as covariates were adjusted. For biochemical data, Student's t test was used. The mean value of BMI in the control and overweigh/obesity groups was 21.1±1.2 (mean±SD) and 25.6±2.0, respectively. All biochemical data of the overweight/obesity group were statistically significance, compared with the control group. Among six SNPs, two linkage disequilibrium (LD) blocks were discovered. One block consisted of rs1933739 and rs9400883, and the other comprised rs3756772 and rs3798234. One SNP (rs9384970, +68506G>A) showed an association with overweight/obesity in the codominant model (p=0.03). Interestingly, the AA genotype distribution in the overweight/obesity group (n=7, 3.5%) was higher than those in the control group (n=1, 0.6%), which is not found in either Japanese or Chinese subjects. Therefore, the AA genotype of rs9384970 may be a risk factor for development of obesity in Korean population.         서울 시내 일부 학령 전 아동들의 비만 유병률과 이에 영향을 주는 요인

        The purpose of this study was to estimate the prevalence and to identify risk factors of overweight among preschool children in Korea. The study subjects were 750 children, aged 2-6, attending child care centers in Korea. A measurement of the height and weight of the children, as well as collection of wide range of variables including general characteristics, and potential factors related with dietary habit for children and their parents, were conducted. Overweight was defined based on more than 110% of ideal body weight. A logistic regression analysis was adopted to identify the factors associated with overweight. Subjects were classified into three categories according to the obesity index : underweight(PIBW〈90%, n=34), normal(90%$\leq$PIBW〈110%, n=577), and overweight(PIBW$\geq$110%, n=139). The overall prevalence of overweight and underweight of the subjects were 21.3% and 4.0% of the boys and 15.5% and 5.1% of the girls respectively. Parent's obesity was associated with a higher risk of overweight on girls. Subjects o the third quartile(girls) and fourth quartile(boys) of income level had a substantially higher risk of overweight than did those in the first quartile. Fast eating, overeating, and food prejudice were also associated with an increased risk of overweight. The results of a logistic regression analysis showed that the eating habits and food preferences of the children were the most influencial factors on overweight. These finding may imply the importance of early stage nutrition education on rational dietary habit to prevent prevailing obesity of preschool in Korea.

        Distribution and social determinants of overweight and obesity: a cross-sectional study of non-pregnant adult women from the Malawi Demographic and Health Survey (2015-2016)

        OBJECTIVES: Hitherto regarded as a public health issue of well-heeled nations, overweight and obesity have emerged as a problem of concern in developing nations. Although social and demographic factors are equally important as proximal lifestyle factors affecting health, their role is neither well researched nor well understood. We conducted a novel study to determine the distribution, prevalence, and social and demographic determinants of overweight/obesity in Malawi. METHODS: A population-based, quantitative cross-sectional study using data from the Malawi Demographic and Health Survey (2015-2016) was conducted among non-pregnant women aged 18-49 years. A total of 6,443 women were included in the analysis. Overweight/obesity, defined as a body mass index (BMI) ≥25.0 kg/m2, was the main outcome variable. The analysis was done in SPSS version 20.0; after calculating descriptive statistics, bivariate and multivariate logistic regression was conducted to evaluate associations and determine odds. RESULTS: In total, 16.8% and 6.3% of women were overweight and obese, respectively (p<0.001). Overweight and obesity were more prevalent in urban than in rural areas. The BMI distribution among women varied across different background characteristics. Women from the Ngoni ethnicity were more likely to be overweight/obese than others (adjusted odds ratio [aOR], 1.54; 95% confidence interval [CI], 1.14 to 2.08). Socioeconomic status (SES) and the age of the respondent were highly significant determinants that were strongly associated with being overweight/obese. The richest women were 3 times more likely to be overweight/obese than the poorest (aOR, 3.30; 95% CI, 2.46 to 4.43). CONCLUSIONS: Overweight and obesity were highly prevalent and significantly associated with increasing SES, age, and being from the Ngoni ethnicity. Holistic interventions should also focus on improving social determinants in order to entirely curb the epidemic.

        The double burden of malnutrition among adults in India: evidence from the National Family Health Survey-4 (2015-16)

        OBJECTIVES: India still faces the burden of undernutrition and communicable diseases, and the prevalence of overweight/obesity is steadily increasing. The discourse regarding the dual burden of underweight and overweight/obesity has not yet been widely explored in both men and women. The present study assessed the determinants of underweight and overweight/obesity in India among adult men and women aged 15-49. METHODS: Population-based cross-sectional and nationally representative data from the National Family Health Survey-4 (2015-16), consisting of a sample of men and women, were analyzed. Stratified 2-stage sampling was used in the NFHS-4 study protocol. In the present study, bivariate and adjusted multinomial logistic regression analyses were performed to determine the correlates of underweight and overweight/obesity. RESULTS: The results suggested a persistently high prevalence of underweight coexisting with an increased prevalence of overweight/obesity in India. The risk of underweight was highest in the central and western regions and was also relatively high among those who used either smoking or smokeless tobacco. Overweight/obesity was more prevalent in urban areas, in the southern region, and among adults aged 35-49. Furthermore, level of education and wealth index were positively associated with overweight/obesity. More educated and wealthier adults were less likely to be underweight. CONCLUSIONS: In India, underweight has been prevalent, and the prevalence of overweight/obesity is increasing rapidly, particularly among men. The dual burden of underweight and overweight/obesity is alarming and needs to be considered; public health measures to address this situation must also be adopted through policy initiatives.

        한국 청소년에서 과체중과 사회 경제적 요인과의 관계: 2007년 청소년 건강행태 온라인조사 자료를 이용하여

        Background: Overweight in Korean adolescents has continuously increased over the past few years and it is recognized as a public health problem. We investigated the relationship between socioeconomic status and overweight in Korean adolescents. Methods: The data was obtained from the 2007 Korean Youth’s Risk Behavior Web-based Study. We included adolescents aged 12 to 18 yearswho were ≥ 5th percentile of the BMI cut-off point based on the Korean child growth curve. Logistic regression analysis was performed to examine the association between socioeconomic status and overweight. Results: The adjusted odds ratios for overweight were 1.116 (95% confidence interval [CI],1.021 to 1.220), 1.390 (95% CI, 1.242to 1.555) in girls having middle and low grade family affluence scale (FAS), respectively, compared in girls having high grade FAS (P < 0.05). However, in boys, there was no significant relationship between FAS and overweight. In both gender,perceived household economic status and parental education level were negatively related to overweight in adolescents (P < 0.05). Conclusion: Lower socioeconomic status increased the risk of overweight in Korean adolescents. Efforts to reduce socioeconomic disparities in adolescents should be initiated for overweight prevention. 연구배경: 청소년기 과체중이 지속적으로 증가하고 있으며이는 청소년 개인의 건강 문제뿐 아니라 사회 전체적인 문제가 되고 있다. 본 연구에서는 한국 청소년에서의 과체중과 사회 경제적 요인간의 관계를 살펴 보고자 하였다. 방법: 2007년 시행된 청소년 건강행태 온라인조사 자료에서12세 이상 18세 이하의 청소년 중 2007년 소아 청소년 표준 성장 도표를 참고하여 체질량지수 5백분위수 이상인 학생을 대상으로 하였고 연령과 중등도 신체 활동을 보정하여 사회경제적 요인들에 따른 과체중의 교차비를 구하여 분석하였다. 결과: 연령과 중등도 신체 활동을 보정한 과체중의 교차비는여아의 경우 가구 풍요도를 상위 계층, 중산층, 하위 계층으로나누었을 때 상위 계층을 기준으로 중간층과 하위 계층에서교차비가 각각 1.116 (95% 신뢰구간, 1.021-1.220), 1.390 (95%신뢰구간, 1.242-1.555)로 증가되는 경향을 보였다(P<0.05). 하지만 남아의 경우 가구 풍요도와 과체중의 교차비가 유의한 차이가 발견되지 않았다. 또한 남, 녀 모두에서 주관적으로인지하는 경제 상태와 부모의 교육 정도가 낮을수록 과체중의 교차비가 유의하게 증가되었다. 결론: 남아의 경우 가구 풍요도와 과체중간의 연관성이 발견되지 않았으나 그 외의 다른 사회 경제적 요인들에 있어서 사회경제적 상태가 낮을수록 남아, 여아에서 과체중의 위험이증가하는 경향을 보였다. 청소년 과체중을 예방하는데 있어서 개인의 문제로 국한시키기 보다는 국가적 차원에서의 해결 방안을 모색할 필요가 있을 것으로 생각된다.

        학령전기 소아비만에 영향을 주는 영유아기 인자

        Purpose:Childhood obesity is a problem that places a child at great risk for becoming an obese adult. To prevent obesity, it is important to focus on early life risk factors that may contribute to childhood obesity. The aim of this study is to find obesity-causing infantile risk factors in preschool children. Methods:A total of 223 children aged 3 to 5 years old from Busan were the subjects of this study. We calculated their body mass index (BMI) and classified them into two groups (normal weight and overweight/obese children). Information regarding parental weight and height, mother's educational level and employment status, birth weight, feeding patterns, start time of weaning, duration of night feeding, and fosterer was obtained by using questionnaires. Results:There were 68 (30.5%) overweight/obese children and 155 (69.5%) normal weight children. Overweight/obese children were significantly associated with formula milk feeding, rapid weight gain during the first 6 months, and maternal obesity (P<0.05). There were no statistical differences in paternal obesity, mothers educational level and employment status, birth weight, start time of weaning, duration of night feeding, and fosterer between the normal and overweight/obese children. Conclusion:Although healthy diet and regular exercise will remain the cornerstones of weight management in obese children, our data supports the view that education about maintaining a normal weight could be introduced much earlier in families with high-risk children.         Eating away from home is associated with overweight and obesity among Ugandan adults: the 2014 Uganda non-communicable disease risk factor survey

        BACKGROUND/OBJECTIVES: We investigated the associations between eating away from home (EAFH) and overweight and obesity among Ugandan adults using the 2014 Uganda non-communicable disease risk factor survey. SUBJECTS/METHODS: In total, 3,025 participants aged 18–69 years were included in the analysis. The frequency of EAFH was assessed by asking participants the number of meals eaten per week that were not prepared at a home. EAFH frequency was categorized as; less than once/week, 1-2 times/week, or ≥ 3 times/week. Logistic regression was used to evaluate the associations between overweight, obesity, and EAFH. We also tested whether sex and age modified these associations. RESULTS: Participants that ate away from home ≥ 3 times/week were 2.13 times more likely to be obese than those that ate away from home less than once/week (odds ratio [OR], 2.13; 95% confidence interval [CI], 1.28–3.54). However, when the analysis was stratified by sex, women that ate away from home ≥ 3 times/week were 42% less likely to be overweight than those that ate away from home less than once/week (OR, 0.58; 95% CI, 0.36–0.94). Men that ate away from home ≥ 3 times a week were 3.89 times and 2.23 times more likely to be obese and overweight, respectively, than those that ate away from home less than once/week (obesity: OR, 3.89; 95% CI, 1.50–10.09; overweight: OR, 2.23; 95% CI, 1.42–3.51). Age-stratified analysis showed that among participants aged 31–50 years, those that ate away from home ≥ 3 times a week were 3.53 times more likely to be obese than those that ate away from home less than once/week (OR, 3.53; 95% CI, 1.69–7.37). CONCLUSIONS: Frequent EAFH was positively associated with overweight and obesity among men, and obesity among young/middle-aged adults, but negatively associated with overweight in women. Nutritional interventions for obesity reduction in Uganda should include strategies aimed at reducing the frequency of eating meals prepared away from home, and specifically target men and young/middle-aged adults.

        Is inadequate play area in schools associated with overweight among students in Addis Ababa, Ethiopia? A comparative cross-sectional study

        OBJECTIVES: The prevalence of childhood obesity has more than doubled since it was formally recognized as a global epidemic in 1997. With the increasingly dwindling space for private schools in Ethiopia, unresolved concerns exist among the public regarding the possible effect of limited play areas in schools on overweight/obesity. This study intended to determine and compare the levels of overweight/obesity among adolescents in private schools with and without adequate play area in Addis Ababa, Ethiopia. METHODS: A school-based comparative cross-sectional study was conducted among 1,276 adolescents. Twenty private schools were grouped into 2 groups based on the size of the play area. Data were collected using a pre-tested questionnaire and anthropometric measurements and analyzed using descriptive statistical tests and logistic regression. RESULTS: The magnitude of overweight/obesity was significantly higher in schools with inadequate play area (19.4%; 95% confidence interval [CI], 16.4 to 22.7) than in schools with adequate play area (14.6%; 95% CI, 11.9 to17.5). Inadequacy of the play area was also positively associated with overweight/obesity in the multiple logistic regression analysis (odds ratio [OR], 1.62; 95% CI, 1.05 to 2.51). Using private car transportation to and from school (OR, 2.27; 95% CI, 1.13 to 4.57), father’s educational status (secondary school and above: OR, 2.54; 95% CI, 1.14 to 5.62), and middle wealth quintile (OR, 2.54; 95% CI, 1.50 to 4.33) were other factors significantly associated with overweight/obesity. CONCLUSIONS: Inadequate play area in schools was an important contributor to overweight/obesity. Sedentary behavior was also significantly associated with overweight/obesity

      • Clinical characteristics of overweight asthma patients in systemic inflammation and airflow obstruction

        Background: The association between systemic inflammation, lipid profile and airflow obstruction has not been fully evaluated. Methods: Bronchial asthma patients proven by pulmonary function test with lipid profile results were retrospectively reviewed from March 2019 to July 2019. Patients who had body mass index (BMI) 25 and above were defined as overweight. Total white blood cell (WBC), eosinophil, neutrophil, lipid profile, apo-lipoprotein A1 & B and lung function test results were analyzed according to the BMI. Results: Thirty-three (42.0%) were overweight and 6 (7.4%) were obese. Most common underlying diseases were allergic rhinitis (38%) and 23% of patients had no underlying diseases, 19.8% had dyslipidemia, 9.9% diabetes mellitus. Forty-four percentages of the patients were atopic asthma. Treatment steps by GINA guidelines were analyzed, 58% were step3 and 30.8% were step4 or 5. In overweight asthma patients, total WBC and eosinophil counts were higher (WBC; 7,794 vs 6,625 X 10<sup>6</sup>/L, P = 0.02, eosinophil count 377 vs, 211 X 10<sup>6</sup>/L, P = 0.03) and HDL/apo-lipoprotein B ratio was smaller (0.62 vs 0.76, P = 0.03) than patients with normal BMI. Among the patients, 27.2% had higher WBC over 7,000 X 10<sup>6</sup>/L and 22.2% had higher eosinophil counts over 200 X 106/L. When we divided patients to four groups according to the BMI 25, WBC and eosinophil counts, pre-bronchodilator FEV1/FVC ratio, FEV1 percentage and FEF 25-75% were statistically lower in high BMI and high WBC (WBC ≥7000) groups. Patients with high BMI and high eosinophil counts showed statistically no differences compared to normal BMI and low eosinophil counts in airflow obstruction. Discussion: Overweight resulted in mild systemic inflammation in bronchial asthma patients represented by increased peripheral WBC and eosinophil counts. Although prospective follow up results are needed, patients who had overweight and elevated WBC showed increased airflow obstruction and decreased FEV1.
