        한국인에서 심바스타틴의 효과와 부작용 분석을 위한 다기관 공동 임상 연구

        The aim of this study was to investigate the efficacy of simvastatin to improved lipid profiles in hypercholesterolemic Korean patients. Methods : From 25 hospitals in Korea, 478 hypercholesterolemic patients were enrolled from November 1996 to April 1998. The inclusion criteria was hypercholesterolemia over 240 mg/dl after diet therapy for 1 month or hypercholesterolemia over 220 mg/dl in patients with definite evidence of ischemic heart disease. Simvastatin 10mg was started and doubled up to 40mg if total cholesterol level remained higher than 200 mg/dl at monthly check. Of 478 subjects, 344 patients in whom study protocol was not violated were analyzed. Results : Male to female ratio was 27:73 and 47% of the subjects were in 6th decade. Hypertension, coronary artery disease, and diabetes mellitus were present in 30, 10, and 4% of the subjects. Baseline lipid profile (mean of total cholesterol-LDL-HDL-triglyceride mg/dl) was 274-185-52-188. The dose of simvastatin for 3 months was 10/10/10mg in 61% of subjects, 10/20/20mg in 21%, 10/10/20mg in 7%, and 10/20/40mg in 12%. The change of total cholesterol level(before-4wk-8wk-12wk-withdrawal 4wk) was 274-209- 205-198-250, and the maximal reduction rate was 27%. The change of LDL-cholesterol was 185-123-116-110-159, with maximal reduction rate 39%. The change of HDL-cholesterol was 52-54-56-55-54, with maximal increase rate 9%. The change of tryglyceride was 188-161- 164-162-189, with maximal reduction rate 15%. The value before/after treatment of ApoA1, ApoB, and Lp(a) was 129/129, 138/83, and 9.3/10.7, respectively. The level of LDL-cholesterol at the end of treatment was below 100mg/dl in 36% of subjects, 100-130 in 45%, 130-160 in 16%, and over 160mg/dl in 4%. The reduction rate of LDL-cholesterol was different between subjects whose LDL decreased below 100 and those whose LDL did not decrease below 130mg/dl, which suggests the existence of the individual difference of responsiveness to simvastatin. There were only 3 subjects (0.9%) who showed increase of liver enzyme over 3 times as the upper normal limit. Conclusion: Simvastatin is effective in improving lipid profiles in hypercholesterolemic Korean patients without serious side effects. (Korean. J. Med 57:906-915, 1999)

        디프드로잉용 열연강판의 균열발생현상 연구 (1) : 결정입계 탄화물이 균열발생에 미치는 영향 Part Ⅰ. The Effect of grain Boundary Carbides on the Crack Formation

        An investigation was conducted to examine the effects of microstructural parameters on the cracking behavior, often occurred in the top parts of the deep drawn cups of hot rolled formable steel. Particular emphasis was placed on the role of grain boundary carbides in the carcking phenomenon. Detailed microstructural analyses of the cracked region showed that a number of voids initiated at grain boundary carbides were observed to form intergranular cracks. These grain boundary carbides were identified as Mn-containing Fe₃C carbides which might be associated with the formation of the band structure. In the microstucture of the hot rolled steel, film or bulk-type carbides along grain boundaries, together with banded ferrite-pearlite structures, were commonly observed, and were thought to initiate cracks during deep drawing process. Thus the formation of grain boundary carbides and band structures must be minimized to prevent cracking. A1 killing process, homogenization treatment and low temperature coiling treatment are suggested as possible methods for minimizing the amount of grain boundary carbides.

      • 골형성부전증 10예의 임상적 특징

        연구배경 및 방법: 골형성부전증은 비교적 희귀한 유전병으로 교원질 대사 장애로 인한 골의 취약성과 다발성 골절 및 척추측만증 등을 특징으로 한다. 유전방식과 표현형의 발현 정도에 따라 다양한 임상 양상을 보이며, 임상적 중증도에 따라 4가지 형태로 분류된다. 지금까지 국내 보고는 분만과정이나 태아 진찰시 골격이상으로 발견된 증례보고가 주였다. 저자들은 비교적 경미한 임상 양상을 보이는 예를 포함한 다수의 골형성부전증 환자들의 전반적인 임상적 특징에 대해 보고하는 바이다. 결과: 2001년 6월부터 2003년 2월까지 골형성부전증으로 진단받은 6 가계, 10예를 대상으로 하였다. 평균 나이는 27.3(5∼56)세였고 소아가 2예였다. 모두 상염색체 우성으로 유전되었으며, 제 I형이 4예, 제 III형이 4예, 그리고 제 IV형이 2예였다. 전 예에서 다수의 골절 경험이 있었고, 골밀도 저하와 골피질 두께 감소 소견이 관찰되었다. 전신의 평균 골밀도는 0.690(0.421∼1.039) g/cm²였다. 골형성지표로 측정된 sAlk는 소아의 경우만 증가되어 있었고, 골흡수지표로 측정된 uDPD의 평균치는 12.9(4.4∼36.3) nM/mM Cr으로 증가된 소견을 보였다. mobility score는 대부분 3,4단계에 속해 있었다. 중증형일수록 진단 시의 mobility score가 낮은 경향을 보였다. 결론: 한국인 골형성부전증 환자의 임상적 특징을 살펴본 결과 기존의 보고된 II형 외에도 I, III, IV형이 다양하게 존재함을 알 수 있었으며, 모든 예에서 상염색체 우성으로 유전됨을 확인할 수 있었다. 또한 모든 예에서 증가된 골흡수로 인한 골밀도 저하와 골절을 확인할 수 있었다. Osteogenesis Imperfecta (OI) is a relatively rare hereditary disease, which is characterized by multiple bone fractures and spine scoliosis, due to the fragility of bone, and is often associated with blue sclerae, deafness and dentinogenesis imperfecta. Four types of OI can be distinguished, according to the clinical findings. Although mutations affecting type I collagen are responsible for the disease in most patients, the mechanism by which the genetic defects cause abnormal bone development remains to be fully understood. Here, the clinical characteristics of 10 OI patient cases are reported, with a review of the literature. All the cases, including 4 type I, 4 type III and 2 type IV, inherited OI as an autosomal dominant trait. All the subjects had multiple old fractures and decreased bone densities. In this study, the biochemical marker of bone formation, serum alkaline phosphatase, was found to be increased only in the pediatric OI patients, while the biochemical marker of bone resorption, urinary deoxypyridinoline, was increased in all cases. The mobility score was found to correlate with the severity of the type on diagnosis (J Kor Soc Endocrinol 18:496∼503, 2003).

        방광의 MALT 림프종 1예

        Primary lymphoma of the urinary bladder is a rare non-epithelial bladder tumor accounting for less than 1% of all bladder tumors. Approximately 17 cases of MALT lymphomas of bladder have been reported in the literature. Most reported MALT lymphomas of bladder have a female sexual preponderance with a mean age of 58 years with common presenting symptoms of hematuria, dysuria and urinary frequency. The reported prognosis of MALT lymphoma of the urinary bladder is excellent. We report a case of MALT lymphoma of urinary bladder in a 57-year-old woman patient who presented with a two-year history of persistent dysuria and urinary frequency. An intravenous pyelogram and cystoscopy revealed a 1 cm focal elevated lesion at the base of urinary bladder. The tissue obtained by transurethral resection (TUR) showed plasma cell infiltration consistent with low grade marginal zone B cell lymphoma. The immunohistochemical studies showed an immunoglobulin restriction to lambda light chain while the nested polymerase chain reaction analysis of the tissue showed a monoclonal Ig heavy-chain gene rearrangement. The clinical staging protocol revealed that the tumor was primarily arising from the urinary bladder with no evidence of other site involvements. The patient received radiation therapy of 3060 cGy in 17 fractions.

        Isolation and Characterization of a Rhodococcus Species Strain Able to Grow on ortho- and para-Xylene

        Rhodococcus sp. strain YU6 was isolated from soil for the ability to grow on o-xylene as the sole carbon and energy source. Unlike most other o-xylene-degrading bacteria, YU6 is able to grow on p-xylene. Numerous growth substrate range experiments, in addition to the ring-cleavage enzyme assay data, suggest that YU6 initially metabolizes 0- and p-xylene by direct aromatic ring oxidation. This leads to the formation of dimethylcatechols, which was further degraded largely through meta-cleavage path-way. The gene encoding meta-cleavage dioxygenase enzyme was PCR cloned from genomic YU6 DNA using previously known gene sequence data from the o-xylene-degrading Rhodococcus sp. strain DK17. Subsequent sequencing of the 918-bp PCR product revealed a $98\%$ identity to the gene, encoding meth-ylcatechol 2,3-dioxygenase from DK17. PFGE analysis followed by Southern hybridization with the catechol 2,3-dioxygenase gene demonstrated that the gene is located on an approximately 560-kb megaplasmid, designated pJY J1

      • Snail1 is Stabilized by O-GlcNAc Modification in Hyperglycemic Condition and Causes Epithelial-Mesenchymal Transition

        Protein O-phosphorylation can occur reciprocal with O-GlcNAc modification and represents a regulatory principle for proteins. O-phosphorylation of serine by GSK-3β on Snail1, a transcriptional repressor of E-cadherin and a key regulator of the epithelial-mesenchymal transition (EMT) program, results in its proteasomal degradation. We show that Snail1 carries O-GlcNAc at serine112 that stabilizes it by suppressing O-phosphorylation-mediated degradation. Stabilization by O-GlcNAc of Snail1 results in attenuation of E-cadherin mRNA expression. Enhanced O-GlcNAc modification occurred under hyperglycemic conditions and initiated EMT by transcriptional suppression of E-cadherin through Snail1. Thus, a molecular link exists between cellular glucose metabolism and the control of EMT by dynamic reciprocal O-phosphorylation and O-GlcNAc modification of Snail1.

        다발성 원격전이와 SIADH를 동반한 원발성 충수돌기 악성유암종 (Carcinoid Tumor) 1예

        Carcinoid tumors are the most trequently occurring neoplasms of the appendix and small bowel. Most carcinoid tumors of the appendix are incidentally discovered during other surgical procedures. Its natural course is usually benign and the size of the tumor is closely related with the liability of regional or distant metastasis, so this affects the mode of therapy. Lesions less than 2㎝ in greatest dimension are never shown to have metastatic lesions and those larger than 2㎝ are likely to metastasize to regional lymph nodes and occasionally to distant organs such as liver, lung and bones. We experienced a malignant carcinoid tumor of the appendix with metastasis to regional lymph nodes, lung, ribs and thoracic vertebrae, cervical lymphnodes, and the right adrenal gland without liver metastasis. The patient had no signs of carcinoid syndrome but showed hyponatremia which was corrected by fluid restriction, suggesting SIADH. The patient was treated with a combination regimen of 5-fluorouracil and doxorubicin after surgical removal of the appendiceal mass, and a palliative radiotherapy for the osteolytic lesion of the right 12th rib. A review of our experience suggests a more extensive program in disseminated malignant carcinoid tumors is required.

        실제 네트워크 모니터링 환경에서의 ML 알고리즘을 이용한 트래픽 분류

        Traffic classification의 방법은 동적으로 변하는 application의 변화에 대처하기 위하여 페이로드나 port를 기반으로 하는 것에서 ML 알고리즘을 기반으로 하는 것으로 변하여 가고 있다. 그러나 현재의 ML 알고리즘을 이용한 traffic classification 연구는 offline 환경에 맞추어 진행되고 있다. 특히, 현재의 기존 연구들은 testing 방법으로 cross validation을 이용하여 traffic classification을 수행하고 있으며, traffic flow를 기반으로 classification 결과를 제시하고 있다. 본 논문에서는 testing방법으로 cross validation과 split validation을 이용했을 때, traffic classification의 정확도 결과를 비교한다. 또한 바이트를 기반으로 한 classification의 결과와 flow를 기반으로 한 classification의 결과를 비교해 본다. 본 논문에서는 J48, REPTree, RBFNetwork, Multilayer perceptron, BayesNet, NaiveBayes와 같은 ML 알고리즘과 다양한 feature set을 이용하여 트래픽을 분류한다. 그리고 split validation을 이용한 traffic classification에 적합한 최적의 ML 알고리즘과 feature set을 제시한다. The methodology of classifying traffics is changing from payload based or port based to machine learning based in order to overcome the dynamic changes of application's characteristics. However, current state of traffic classification using machine learning (ML) algorithms is ongoing under the offline environment. Specifically, most of the current works provide results of traffic classification using cross?validation as a test method. Also, they show classification results based on traffic flows. However, these traffic classification results are not useful for practical environments of the network traffic monitoring. This paper compares the classification results using cross validation with those of using split validation as the test method. Also, this paper compares the classification results based on flow to those based on bytes. We classify network traffics by using various feature sets and machine learning algorithms such as J48, REPTree, RBFNetwork, Multilayer perceptron, BayesNet, and NaiveBayes. In this paper, we find the best feature sets and the best ML algorithm for classifying traffics using the split validation.

        전,의경에서 발생한 실신의 원인과 임상적 특징

        Background/Aims: The most common type of syncope in young adults is neurocardiogenic in origin, which is not related to organic problems and has a benign nature. Therefore, there have been few reports regarding syncope in young Korean adults. Here, we examined the causes of syncope and its clinical characteristics in young combat and auxiliary police in Korea. Methods: We performed a retrospective study of the medical records of 193 combat and auxiliary police in Korea admitted to the National Police Hospital for syncope or presyncope between January 2004 and December 2007. Results: The subjects` mean age was 21±1.2 years, and there were 2.8±2.9 episodes of syncope (mean±SEM). The first syncope occurred after enlistment in the police in 102 patients (52.8%). Basic diagnostic studies showed two cases of elevated creatine phosphokinase and one case of anemia to be related to syncope. Syncope-related traumatic injuries occurred in 38 patients (19.7%), and syncope was more prevalent in the summer. Head-up tilt test was performed in 175 of the 193 patients, and 123 showed a positive response. Other evaluations, including brain magnetic resonance imaging, electroencephalography, and echocardiography, were not helpful for diagnosis. Syncope was neurocardiogenic in origin in the majority of cases. Other causes of syncope were exhaustion (n=2), hyperventilation syndrome (n=2), Meniere`s disease (n=1), anemia (n=1), and psychiatric problems (n=5). The causes of syncope could not be identified in 59 patients (30.5%). Conclusions: Neurocardiogenic syncope was the most common type in our study population. More than half of our patients experienced their first episode after enlisting with the police. Further studies in these groups are necessary. (Korean J Med 78:198-206, 2010)
