頭鍼이 中風患者의 단기간 NIHSS와 MBI에 미치는 영향

Objective ; The aim of this study was to investigate the effect of Scalp acupuncture on recovery of motor disorders in stroke patients. Methods ; Twenty two patients with poststroke-hemiplegia were randomized into two groups. Ten patients(test group) treated by 2 methods-Scalp acupuncture and body acupuncture. The other twelve patients(control group) treated only by body acupuncture. The activity of daily living was measured with a National Institutes of Health stroke scale(NIHSS) and Modified Barthel Index(MBI). The therapy was performed one a day for 2 weeks. Results ; In terms of score of NIHSS, the test group showed statistically meaningful decrease after 2 week treatment, but the control group showed statistically meaningful decrease after 1 week(p<0.05). And in terms of score of MBI, the test group did not show statistically meaningful increase but the control group showed statistically meaningful increase after 1 week(p<O.05). There was no statistically meaningful difference after 1 and 2 week treatment between the groups. Conclusions ; These results support that test group has almost same effectness compared with control group in improvement of the activity of daily living of poststroke-hemiplegic patients.

The paper describes the development of a new automatic wrapping machine using paper films as it has not been used so far. The most of the automatic wrapping machine have used the vinyl film causing environmental pollution, the paper films, however, have a weak point in the aspect of high temperature adhesion. Therefore, the mechanism of the existing wrapping machine should be altered to prevent it from curling effect through the technical know-how. A new method and technology reducing the curling effect in the automnatic wrapping machine are demonstrated by using developed blower. Thus it is exqection and will decrease environmental pollution.

Performance Evaluation of end mills for high speed cutting has been performed in a view of dynamic characteristics and noise-vibration under operation. The tools tested in this research consist of three foreign country made and one korean made. In addition, numerical models using finite element method are established, which are confirmed by experimental results. The evaluation results has been feedback for developing high performance end mills for high speed cutting tools.
Helicobacter pylori와 대장균의 Shuttle Vector 개발

In this study, a vehicle vector using cryptic plasmids was constructed for gene transfer in Helicobacter pylori. pHP51(3.9 kb) and pHP489(1.2 kb) were selected for constructing vectors from cryptic plasmid of H. pylori isolates in Korea. The HindⅢ-digestedDNA fragment(1.2kb) of pHP489 and 1.6kb DNA fragment of pHP51 were ligated with a kanamycin resistance gene(aph3'-Ⅲ) from C. jejuni to produce the recombinant plasmids pHP489K and pHP51K, respectively. Transformation frequency of pHP51K by electroporation was low. But pHP489K could be effectively transformed into various H. pylori strains. In order to design an intermdiate vehicle vector for gene transfer into H. pylori, pBlueHP489K was prepared by recloning pHP489K DNA into pBluescript and pTZ19R vector. This vector permitted the DNA fragment containing pHP489 sequence, aph3'-Ⅲ, and cloned DNA to be cut and self-ligated in the SacⅠ site after cloning. ureA and ureB gene were inserted into pBlueHP489K, resulting in pBlueHP489K/AB. The DNA fragment containing pHP489, kanamycin resistance gene(aph3'-Ⅲ), and urease structural gene was cut away from pBlueHP489K/AB and self-ligated to generate pBlueHP489K/AB. pBlueHP489K/AB made urease-negative H. pylori strains restore their urease activity. By this experiment, pBlueHP489K was confirmed to be the vehicle system for transferring H. pylori genes.

There was not the evident analysis about the cutting process of CNC machining, and wouldn't be difficult to estimate the result of machining for the various cutting conditions. Therefore they were not founded the systemic technology about the optimum cutting conditions and selection of cutting tools. So this study have investigated the common facts for needs through the end-mill cutting machining by Machining-Centers or High-speed cutting machines, and developed the user-centered intelligent decision system to selection of the methodology about cutting conditions to improve the machining efficiency of end-mill cutting process.

Background A heterogenous radiological response is frequently observed in cancer patients and could reflect tumor heterogeneity. We investigated the prognostic impact of heterogeneous radiological responses in patients with advanced non-small cell lung cancer (NSCLC) who received platinum-based chemotherapy. Methods The treatment response according to RECIST 1.1 (Response Evaluation Criteria in Solid Tumors) criteria were evaluated in 212 patients with advanced NSCLC who received platinum-based chemotherapy. Patients with partial response (PR) or stable disease (SD) were classified into “PR homo,” “PR hetero,” “SD homo,” and “SD hetero” by the presence of a heterogenous radiological response, and survival was compared between groups. We also compared survival based on the presence of metabolic responses in lesions with heterogeneous radiological responses. Results Fifty-two patients (24.5%) were classified as PR, 112 patients (52.8%) as SD, and 48 patients (22.7%) as progressive disease (PD). There was no significant difference in progression-free survival (PFS) and overall survival (OS) between the PR homo and PR hetero groups. The SD homo group had a longer PFS and OS than the SD hetero group (Figure 1). In the SD hetero group, patients with increased maximum standardized uptake value (SUVmax) in lesions with heterogenous radiological responses had a shorter PFS than those with a stable SUVmax (Figure 2). Conclusions The presence of lesions with radiological heterogeneity was associated with disease progression and poor prognosis in the SD group. Patients with heterogeneous radiological responses therefore required careful monitoring.

Purpose Inflammation plays a critical role in tumorigenesis and is now characterized a hallmark of cancer. Accumulating evidence have demonstrated that the serum neutrophil to lymphocyte ratios (sNLR) among systemic inflammatory markers have prognostic significance in various cancers. However, no study on the prognostic impact of NLR of BAL fluids (bNLR), which may be provided worthwhile information of local inflammation of tumor origin, in lung cancer patients have been reported thus far. Method We performed a retrospective review of the prospectively collected BAL fluids. The optimal cutoff values for NLRs were determined using the maximally selected rank statistics of R software. The patients were categorized into high bNLR and low bNLR groups, and clinical variables were investigated for their correlation with survival. Result The study included 48 lung cancer patients who met the inclusion criteria. High bNLR and sNLR were significantly associated with worse overall survival (OS) (p = 0.020 and p < 0.001, respectively, figure 1 and 2). The following variables were associated with worse OS: performance status 2-4 (p < 0.001), former or current smokers (p = 0.028), advanced TNM stage (p = 0.001), high c-reactive protein (CRP) (p < 0.001), and hypoalbuminemia (p = 0.014). A multivariable analysis confirmed that CRP (p = 0.029), sNLR (p = 0.009), bNLR (p = 0.008), and TNM stage (P=0.002) were independent predictors of OS. Conclusion The present study indicates that high bNLR, like sNLR, is a significant independent prognostic factor for adverse OS of patients with lung cancer. To the best of our knowledge, this is the first study that has investigated the prognostic value of the bNLR in patients with lung cancer. Furthermore, this result may serve as the cornerstone of further studies using BAL fluids obtained by liquid biopsy.
