        억제대 적용지침 개발을 통한 불필요한 억제대 적용율 감소활동

        문제: 억제대 적용에 대한구체적 적용지침 없이 의례적으로 억제대를사용함으로 인해 환자들의 신체적 또는 정신적 안전을 위협할 수 있다. 목적: 완전 진정상태 이거나 근력이 약하여 발관능력이 없는 환자에 대한 예방적 적용, 바빠서 환자를볼수 없 는경우, 간호사본인의 판단이 없이 타인의 요구에 따르는등의 불필요한 억제대 적용이 증가하는 원인을 파악하여 중환자실의 특수성에 맞는 억제대 적용및 제거 지침을 마련하고자 한다. 의료기관: 인천광역시에 소재한 의과대학 소속병원의 집중치료실 개선효과: 억제대 적용에 대한중환자실 간호사에게 미치는 영향을 보면 태도 정도 p=0.09(p<0.1), 올바른 수행 능력은 p= 0.005 (p <0.0히로통계적으로유의한것으로 나타났다. 지식 정도 p=0.172(p<0.05) 통계적으로 유의 하지는 않지만 모든항목이 개선 활동전 에 비해 2,3차 개선 활동후에 향상한 것으로 나타났다. 개선활동전에 8.1%였던 간호 기록, 0.7%였던 간호중재, 0%였던 의사처방율이 3차 개선 활동 후 3가지 모두 100% 달성되었다. 이는 억제대 적용율 갑소 및 부적절한 억제대 적용율은 유의하게 감소하였다. 교훈: 중환자실 간호사의 억제대 적용에 대한태도, 지식, 올바른 간호 수행 능력은 지속적으로 재평가 되어지고, 교육내용은좀더 나은 간호제공을 위하여 개발 되어져야 할 것이다. 억제대 적용 기준과 제거 기준을 Check List 하여 객관성의 유지를 위한 개선 활동은계속적으로 Feedback 되어야 할 것이다.

      • 2000년 이후 주요 학술지에 발표된 떡 연구 고찰

        The purposes of the study were to review research on dduk (Korean traditional rice cake) published in two academic journals (Journal of the Korean Society of Food and Cookery Science and Journal of the Korean Society of Food Culture) for year 2000-2012 and to provide research suggestions for future research. A total of 171 articles were reviewed and analyzed by research purpose, research method, type of dduk, and ingredients. All of the research was conducted on dduks which were prepared by steaming or punching. In the same context, sulgidduk and jeungpyeon were the most frequently studied. The research purposes were quality characteristics evaluation and standardized recipe development, functionality of ingredients, customer’s perception on dduk, microbiological assessment, packaging and storage, and review of old literature. Most research used regular rice as a main ingredient and various secondary ingredients were used. Based on the result, several suggestions were provided for future research. The result will guide researchers to plan research projects on dduk and an infrastructure is needed that bridges between the needs of the researchers and practitioners in the dduk industry.

        부산시 일개구 지역주민의 위암선별검사 수검 및 반복수검 실태 및 관련요인조사

        The importance of repeat screening for stomach cancer is well known to decrease deaths from stomach cancer. This study was aimed at assessing practice behaviors and to identify related factors in the aspects of demographic factors, health status and cancer risk recognition, attitude to cancer screening, health behaviors, and inhibiting or facilitating factors to stomach cancer screening in an urban area. Data was collected through self-administered questionnaires from 403 people aged 40 to 69 years from April 23th to May 15th, 2002. Practice behaviors were classified as :"ver"r "ver"and "peat"r "t repeat"rouped based on a recent 5 year screening history. The results were as follows: 1. Among the 403 subjects: "e ever group"as 23.8% and "e repeat group"as 4.0%. The rates of screening were 20.5% for men, 27.8% for those women, and 27.3% for aged 40-49, 24.5% for those aged 50-59, 19.0% for those aged 60-69. The rates of repeat screening were 4.0% for men, 4.1% for women, and 2.9% for those aged 40-49, 6.3% for aged 50-59, 2.5% for those aged 60-69. 2. The main factors associated with adherence to gastric cancer screening were education(post high school vs below : OR=2.44), previous cancer screening(yes vs no : OR=2.61), belief in personal health(no vs yes : OR=2.72), health status(unhealthy vs healthy : OR=3.40), possibility of cancer compared to others(low vs not low : OR=2.56), and regular exercise(yes vs no : OR=2.94). The main factor associated with adherence to gastric cancer repeat screening was other cancer screening (yes vs no : OR=6.33). Consequently, there is a need to change the recognition of the importance and necessity of stomach cancer screening in healthy conditions through health education, and to perform multiple screening tests each visit.

        In China Men, Maxine Hong Kingston incorporates chinese myths, legends, and Western literary works into her writing. In dealing with her kinsmen's dreams, adventures, struggles and ordeals in America, she constitutes an intertextuality in between Chinese writings and American experiences. The Chinese-American experiences in the novel are interpreted in terms of Chinese ethos and the chinese myths are in return affected in an American context. Hence, the interpretative space in between the Chinese and the American is in constant change which works to transform America, Kingston's newly acquired home. In china Men, Kingston creates an alternative narrative which enables her to carry out her feministic task without weakening Asian American men's interests. For this, Kingston, in addition to unmasking the oppression of Chinese American women, extends her feministic concern to other groups that have been forcefully 'feminized" in the dominant culture. In other words, she borrows the stories of Tang -Ao,ghostmate, Tu Tzu-chun and Maui from the Chinese and Western myths transplants the myths of Tang-Ao and Tu Tzu-chun to North America to symbolize the emasculation of China men in the United States. IN addition, she turns Robinson Crusoe int a Chinese story to recreate the image of Chinese American men. By doing so, she not only revises the American history but transfigures the politics of ethnicity in America. Kingston's attempt in the novel is to decenter and subvert the male dominant master narratives of the Anglo-American history.

        예비수학교사의 신념에 따른 교수학적 내용지식(PCK)과의 관련성에 관한 연구

        This study investigated the relationship between PCK (Pedagogical Content Knowledge) and pre-service mathematics teachers' mathematical beliefs, and the effects of PCK on their teaching. The data were collected through pre & post interviews, a questionnaire, and classroom observations of six pre-service mathematics teachers who were taking their field practica in their senior year. The results indicate that the pre-service mathematics teachers' differing beliefs about the nature of mathematics affected their PCK. This study showed the relationship between their beliefs about teaching and the learning of mathematics and PCK. It is recommended that more opportunities for PCK development, as well as for modifications in of their beliefs have to be offered in education programs for pre-service mathematics teachers. 본 연구의 목적은 예비수학교사들의 수학적 신념과 수업에서 발현된 교수학적 내용지식(PCK)의 특징을 알아보고, 이를 통해 수학적 신념에 따른 PCK의 관련성에 대해 분석하는 것이다. 이 연구는 교육실습 중인 예비수학교사 6명을 대상으로 실시한 PCK 설문지, 인터뷰 자료, 수업관찰 자료를 분석하는 질적 연구 방법을 토대로 하였다. 연구결과, 예비수학교사들의 다양한 수학 본질에 대한 신념은 PCK에 서로 다른 영향을 주었으며, 수학 교수-학습에 대한 신념과 PCK사이에는 밀접한 관련성이 있음을 볼 수 있었다. 따라서 신념과 교수학적 내용지식(PCK)을 함께 발달시킬 수 있는 예비교사 연수 및 지원 프로그램의 확충이 필요하다.

        연쇄알구균 감염에 의한 급성 인후염 후 반응성 관절염 1예

        연쇄알구균에 의한 급성 인후염 후 발생한 반응성 관절염은 급성 류마티스열의 개정된 진단기준을 충족하지 못하면서, 지속적 혹은 재발성의 비이동성 관절염과 발열, 홍반성 결절, 일시적 간기능 이상 등을 보이면서 심장염과 무도증을 드물게 동반하는 질환이다. 따라서, 발열을 동반한 급성 관절염 환자에서 감별해야 할 질환 중의 하나이며, 심근염 예방을 위한 페니실린 요법의 적응증 및 기간에 대해서는 아직 이론이 많은 상황으로, 보다 광범위한 연구가 필요하다. 저자들은 연쇄알구균에 의한 급성 인후염 1주 후에 발생한 하지의 다발성 비이동성 관절염과 함께 결절홍반 및 일시적 간기능 이상을 보인 환자에서 PSRA로 진단하고 비스테로이드성 항염증제 및 스테로이드로 성공적인 치료를 하였기에 이를 문헌 고찰과 함께 보고하는 바이다. Poststreptococcal reactive arthritis (PSRA) is associated with recent streptococcal infections. However, PSRA is distinct from acute rheumatic fever by its clinical manifestations: non-migrating arthritis, erythema nodosum or erythema multiforme, and transient elevation of serum transaminases. We experienced a 33-year-old man who presented with fever, arthritis of both knees and ankles, and erythema nodosums on extensor surfaces of lower legs which developed 6 days after the onset of pharyngitis symptoms. Blood and urine cultures were negative. Throat culture was negative for group A β-hemolytic streptococcus. The ASO titers increased up to 2080 IU/mL in sequential monitoring. The result of bone scan was compatible to arthritis of both knees and ankles. There were no signs or symptom of carditis. He showed clinical improvement with anti-inflammatory drugs (naproxen 1,000 mg/day and prednisolone 7.5 mg/d). PSRA should be included in the differential diagnosis of patients presenting with arthritis combined with fever.

        녹차 및 결명자 추출물의 치주조직병 원인균에 대한 억제효과

        It is known that the periodontal disease is caused by infection of inner oral bacteria as a suffering disease to human mankind for a long time. It can make the various clinical symptoms such as gingiva, bleeding, formation of periodontal pocket, loss of attached gingiva, collapse of alveolar bone and halitosis, and is known to the major contributor of loss of teeth. This study investigated the growth restraint effect of Camellia sinensis and Casia tora. As a result of study, the minimum restraint concentration of standard microbe on F. nucleatum for extracts from C. sinensis showed 2.5, 0.63 and 1.25 mg/ml, respectively and minimum restraint concentration of clinical separation microbe on F. nucleatum for extracts from C. sinensis showed 2.5 and 5.0 mg/ml. The minimum restraint concentration of standard microbe and clinical separation microbe, respectively. Meanwhile, the minimum restraint concentration of standard microbe and clinical separation microbe on F. nucleatum for extracts from C. tora showed 2.5 and 2.5-5.0 mg/ml, respectively. The minimum restraint concentration of standard microbe and clinical separation microbe for P. intermedia, P. nigrescens, P. gingivalis, and P. endodontalis showed above 5.0 mg/ml for standard microbe and above 5.0 mg/ml for clinical separation microbe, respectively. As a result of investigating the cell toxicity for each extracts, extracts from C. sinensis does not showed the toxicity for the cell below the concentration of 2.5 mg/ml, but it showed toxicity for the cell above concentration of 2.5 mg/ml and it decreased the existence ratio for the cell. Meanwhile, in the case of C. tora, it increased the multiplication ratio below the concentration of 1.25 mg/ml compared to the subject group and it decreased gradually the multiplication ratio that the growth ratio for the cell is increased better than subject group. As a result of this study, extracts from C. sinensis was more effect on the growth restraint effect against periodontopathogens than extracts from C. tora and is proven that it is possible for use as a oral mouthrinsing liquids in the near future. On the basis of results of this study, it is judged that the study for reaction mechanism and discrete clinical test for implementation should be needed.

        발효 온도와 시간 조합을 달리한 총각김치의 품질 특성

        To determine the conditions of the fermentation and storage for Chonggak kimchi refrigerator, prepared Chonggak kimchi took into kimchi refrigerators which were controlled at four different modes of the fermented temperature and time, and fermented and kept for 16 weeks. The pH in Chonggak kimchi fermented at 20℃ for 24 hours/stored at 01℃ dropped greater than all of kimchi fermented at other combinations, and the changes of pH at any combinations were not greater than those in Baechu kimchi, because pH in Chonggak kimchi did not dropped below 4.5. Acidities in Chonggak kimchi were greatly increased at higher temperature. The acidity in Chonggak kimchi during the first week of fermentation was lower than that in Baechu kimchi and then it was rather higher becuase of the addition of waxy rice paste. In texture, puncture force of Chonggak kimchi was decreased slowly until 8 weeks of fermentation and then did not changed much and the highest values showed in Chonggak kimchi stored directly at -1℃ without any fermentation. In sensory evaluation, the score for the carbonated flavor and the sourness were the highest in Chonggak kimchi fermented at 20℃ for 24 hours/stored at -1℃, but the lowest in Chonggak kimchi stored directly at -1℃ without any fermentation because of some undesirable flavors. The lowest hardness showed in Chonggak kimchi fermented at highest temperature and the best hardness was in Chonggak kimchi fermented at 5℃ for 3 days or 6 days/ stored at -1℃. The appearance was the best in Chonggak kimchi fermented at 20℃ for 24 hours/stored at -1℃ and the worst was in Chonggak kimchi stored directly at -1℃ without any fermentation. The overall acceptability of Chonggak kimchi fermented at 20℃ for 24 hours/stored at -1℃ was good after 4 weeks of fermentation, but in Chonggak kimchi fermented at 5℃ for 3 days or 6 days/stored at -1℃ it was food after 6 weeks. Total microbial counts in most of Chonggak kimchi were reached to a maximum number within 7 days, and then decreased similarly at all modes. Leuconostoc spp. and Lactobacillus spp. increased to maximum number of 1.48 10^(9) and 5.62 0^(9), respecively, in Chonggak kimchi fermented for 7 days. Yeast counts showed a increasing trend not depends on fermenting temperature and they were lower counts than those in Baschu kimchi. Waxy rice paste which added to Chonggak kimchi resulted in increasement of glucose as a carbon source and stimulated to reproduce the microbes in Chonggak kimchi.

      • 병원급식 체계에 관한 조사 연구

        This study was conducted to evaluate the hospital foodservice management practices in Seoul and to suggest the guideline for the effective foodservice management. Questionnaire survey method were used in this study. Questionnaire were administered to dietitians in twenty hospitals. Data were analyzed for frequency, percentage, χ^(2)-test, and one-way ANOVA using SAS PC Package program. The results were summerized as follows : The number of dietition were less than the required number for efficient foodservice. Centralized foodservice method was adopted in most hospitals and conventional foodservice system was taken in all the hospitals. The space of a kitchen and a dining room were confined and sanitary facilities and working condition revealed problems. Equipped facilities in surveyed foodservice operations were assessed as the insufficient condition, especially, efficient facilities such as a cutting machine were scarecely make up.
