        조혈모세포이식 환자에서 침습성 진균 감염에 대한 Micafungin의 예방 효과 및 안전성

        Background: Micafungin, a potent inhibitor of 1,3-β-D-glucan synthase, is a novel antifungal agent of the echinocandin class. In vitro study showed that micafungin was effective against Aspergillus species as well as Candida species, but clinical data on the prophylactic efficacy against invasive fungal infections (IFIs) other than candidiasis are still lacking. Materials and Methods: We identified 60 consecutive adult hematopoietic stem cell transplantation (HSCT) recipients who received at least 3 doses of micafungin during neutropenic period. Micafungin was started as an alternative in patients who were intolerant or had adverse events (AEs) to primary prophylactic antifungal agents. We retrospectively reviewed the medical records and analyzed the efficacy and safety of micafungin for prophylaxis against IFIs. Results: The patients either had autologous (n=9) or allogeneic (n=51: 1 syngeneic, 24 sibling, 26 unrelated donor) HSCT. Itraconazole oral solution (n=58) was the most frequently used first line antifungal agent for prophylaxis and was administered for median 11 days. The most frequent cause of switch to micafungin was vomiting (n=42). The duration of neutropenia and micafungin administration was median 13 and 12 days, respectively. A successful outcome was achieved in 45 (75%) patients. Empirical antifungal therapy was initiated in 13 (22%) patients. There were 2 cases (3.3%) of breakthrough fungal infections which comprised a probable invasive pulmonary aspergillosis and a possible invasive fungal sinusitis. There was no case of invasive candidiasis. A total of 53 (88%) patients experienced at least one AE regardless of causality during micafungin administration. The most frequent AEs were hypokalemia, vomiting, diarrhea, and elevated serum aspartate aminotransferase or alanine aminotransferase. Among the aforementioned AEs, only 1 case of diarrhea could be classified as a probable relation with micafungin when causality was assessed. There was no AEs that caused discontinuation of micafungin. Conclusions: Micafungin seems to be a safe and effective agent for prophylaxis of IFIs including aspergillosis as well as candidiasis in HSCT recipients. However, further large, prospective, and randomized comparative studies are warranted for aspergillosis.

        An Ac-type Plasma Display Panel (PDP) operating with Ne-Ar(0.1%) Penning mixture gas is fabicated. The characterics of the panel with electrodes covered with thin and thick dielectric layers are studied. The brightness of the Neon-orange light emitted by the panel measured as function of applied voltage and frequency. As an application, a graphic display system equipped with PDP showing still and moving pictures is made.

        Graft-versus-host disease (GVHD) is a frequent complication after bone marrow transplantation. Infectious complications are common in GVHD patients due to defect in cell-mediated immunity. A rare case of S. pneumoniae meningoencephalitis occured in a patient with extensive form of chronic GVHD after allogeneic bone marrow transplantation. He was immediately treated with full dosage of ceftriaxone and ampicillin. He suffered from various complications such as sepsis, acute renal failure, atelectasis, and seizure. Despite of aggressive treatment, he died probably due to renal shutdown and massive subacute cerebral infarction of left cerebral hemisphere. This report showed two unusual and rare features. First, the infection site was CNS rather than respiratory system. Second, the causative organism was S. pneumoniae, which is rare cause of CNS infection in immunocompromised patients.

      • CD34+ 조혈 모세포 이식 2례

        Background: In most solid tumors, the CD34 antigen has not been detected, so positive selection of CD34+ cells may reduce tumor cell contamination and the CD34+ cells are capable of reconstituting hematopoiesis. We tried CD34+ cell transplantation in two patients. Method: CD34+ cells from chemotherapy + G-CSF mobilized PBPCs or bone marrow were positively selected with an avidin-biotin immunoadsorption column (CEPRATE SC system). Case 1. One course of chemotherapy using cyclophosphamide(200㎎/㎡) and etoposide (4.2g/㎡), combined with G-CSF(5㎍/㎏) S.C. was used in a relapsed lymphoma patient. This patient responded to the induction chemotherapy. CD34+ cells from harvested bone marrow were selected by the CellPro immunoadsorption column. The total number of mononuclear cells loaded onto the CellPro was 2.4×10^(8)/㎏, with 1.1% CD34+ cells. After column separation, the total number of positively selected cells was 5.16×10^(6)/㎏. The number of CFU-GM was 76.8×10⁴/㎏. This patient was treated with melphalan (140㎎/㎡) and TBI (1200cGy) and the positively selected CD34+ cells were infused. The time to neutrophil recovery greater than 0.5×10^(9)/L was 19 days and the time to platelet recovery greater than 50×10^(9)/L was 21 days. Case 2. Two courses of mobilizing chemotherapy were given 4 weeks apart using taxol(210㎎/m2) and adriamycin(60㎎/m2), combined with G-CSF(5㎍/㎏) S.C. in a breast cancer patients with 7 axillary node metastasis. CD34+ cells from each single leukapheresis product were selected by the CellPro immunoadsorption column. In the first collection, the total number of nucleated cells was 4.4×10^(8)/kg, with 0.42% CD34+ cells. In the second collection, the total number of nucleated cell was 2.8× 10^(8)/㎏ with 0.43% CD34+ cells. After colum separation, the total numbers of collected cells were 4.0×106/kg and 4.8×10^(6)/kg, the total number of CD34+ cells were 1.2×10^(6)/㎏ and 0.82×10^(6)/㎏. Colonogenic assays of positively selected CD34+ cells gave rise to myeloid erythroid, and multilineage colonies, with a median of 190 CFU-GM, 190 BFU-E, and 164 CFU-GEMM per 1×10³ adsorbed cells, respectively. High-dose chemotherapy with cumulative doses of 40mg/㎡ mitoxantrone, 750mg/㎡ thioptepa, and 1000mg/㎡ carboplatin was administered. Positively selected CD34+ cells were rapidly infused 24 hours after the end of high-dose chemotherapy. The time to neutrophil recovery greater than 0.5×10^(9)/L was 16 days and the time to platelet recovery greater than 50×10^(9)/L was 20 days.

      • Sol-gel법으로 제조된 PEO Electrolytes의 물성 연구

        The field of polymer electrolytes has gone through a sort of three stages : polymer gels, solid drying system, and polymer composites. In spite of high ionic conductivity, the polymer gels suffer from poor mechanical properties and high reactivity with lithium metal anode. To solve these problems, the solid drying system and polymer composites have been intensively studied, due to their good mechanical, thermal, chemical, and electrochemical stability. The objectives of this experiment are to improve ionic conductivity and mechanical properties of the solid polymer electrolytes based on PEO-LiClO4. To obtain higher ionic conductivity and better mechanical properties, ceramic or rubber phase was added in the PEO-LiClO4 matrix. The results showed that ionic conductivity and mechanical properties were improved. The ionic conductivity of the samples was as high as 10-5 Scm-1. This value is similar to the best ionic conductivity ever reported in the solid drying system. To obtain better results, we used PEO with various molecular weight and changed the salt contents. By using DSC, we found that the addition of salt reduced the crystallinity of PEO. And the molecular structure dependence on salt contents was examined the use of FT-lR.

        In this paper aluminium-doped zinc oxide(ZnO:Al) conducting layer was deposited on polyethylene terephthalate(PET) substrate by r. f. magnetron sputtering method PET substrate was modified by a DBD(dielectric barrier discharge) plasma before sputtering to improve the electric and optical properties of ZnO:Al film and to increase also the deposition rate The effect of heat treatment time on the dectrical, optical, chemical and structural properties of ZnO:Al thin film were investigated experimentally The PET films modified with the DBD plasma showed a significant decrease in water contact angle The deposition rate of ZnO:Al film in the case of the plasma-treated substrate was also increased by around 2 times compared to the untreated substrate The minimum resistivity of about 28×10-3 Ω-cm and transmittance of about 83.3% were obtained under the experimental condition of 6min of heat treatment time.

      • 실리카지지 니켈-구리 합금에서 일산화탄소의 흡착에 관한 FTIRS 연구

        실리카지지 니켈, 실리카지지 구리, 실리카지지 니켈과 구리의 합금에서 일산화탄소의 압력을 변화시키면서 적외선 스펙트럼을 관찰했다. 또한 시료를 실온에서 진공탈착시키면서 적외선 스펙트럼을 관찰했다. 실리카지지 니켈에 일산화탄소를 흡착시켰을 때 2019.1~2036.5 cm^(-1), 1862.9~1868.7 cm¹의 두 흡수띠가 관찰되었고, 실리카지지 구리에 일산화탄소를 흡착시켰을 때 2109.8~2115.5 cm^(-1)의 두 흡수띠가 관찰되었으며, 실리카지지 니켈과 구리의 합금에서는 2113.6~2123.2 cm¹, 2009.5~2036.4 cm^(-1), 1876.4 cm^(-1)~1899.5 cm^(-1)의 세 흡수띠가 관찰되었다. 실리카지지 구리, 실리카지지 니켈, 실리카지지 구리-니켈에서 관찰된 흡수스펙트럼은 이전의 보고와 근사적으로 일치했다. 실리카지지 구리-니켈에서 조성에 따른 흡수띠의 이동은 Ni과 Cu의 원자의 크기의 차이로 인한 표면 스트레인의 변화, 표면에서 금속의 전자밀도 변화, 표면 구조의 변화의 복합작용으로 설명이 가능하다. We have investigated the infrared spectra for CO adsorbed on silica supported nickel, copper, Ni-Cu alloys of several compositions at various CO partial pressures and on pumping to vacumn at room temperature within the frequency range of 1500 cm^(-1)~2500 cm^(-1). Two bands(2019.1-2036.5 cm^(-1), 1862.5- 1868.7 cm^(-1)) were observed for the 4 % Ni-SiO₂sample, a band(2109.8~2115.5 cm^(-1)) was observed for the 4 % Cu-SiO₂sample, and three bands(2113.6~2123.2 cm^(-1), 2009.5~2036.5 cm^(-1), 1876.4~1888.5 cm^(-1)) were observed for the 4 % Ni/Cu-SiO₂samples. These absorption bands corresponded with those of the previous reports. With the 4 % Ni/Cu-SiO₂ samples the absorption band shifts may be ascribed to interaction of the strain changes due to atomic size differerence, the changes of electronic densities, and the changes of surface geometries.

        군 훈련병의 스트레스, 성인 주의력결핍 과잉행동장애 증상과 자살사고의 연관성에 관한 연구

        Objectives:This study is conducted to investigate the effect of Attention Deficit and Hyperactivity Disorder(ADHD) symptoms on stress and suicide idea among 224 conscripts. Methods:Total number of subject is 224 conscripts. We evaluate symptoms of adult ADHD with Korean-Wender Utah rating scale(K-WURS) and Korean adult attention-deficit/hyperactivity disorder scale(K-AADHDS), stress with Brief Encounter Psychosocial Instrument-K(BEPSI-K), suicide idea with Scale for suicide ideation(SSI) after basic military training. Multivariate logistic analysis with backward stepwise selection is performed to evaluate risk factors of stress. Multiple linear regression analysis with stepwise selection is performed to evaluate association of suicide idea with adult ADHD symptoms. Results:Stress is significantly associated with K-WURS score, K-AADHDS score and SSI score(p<0.01, p<0.05, p<0.01). K-WURS score are significant risk factor of stress after basic military training(p<0.05, O.R : 1.034, 95% CI : 1.005-1.065). And K-WURS score influence suicide idea after basic military training(p<0.05, β : 0.031). Conclusion:The result suggests that K-WURS score might be a risk factor of stress and could affect suicide idea.

        셀레콕시브 및 그 합성유도체들의 항암활성 스크리닝

        Selective COX (cyclooxygenase)-2 inhibitors including celecoxib have been shown to induce apoptosis and cell cycle changes in various tumor cells. New inhibitors are recently being developed as chemomodulating agents. We evaluated celecoxib and screened 150 synthetic compounds for anti-proliferative activities in vitro. Effects of celecoxib on COX activity, cell growth, cell cycle distribution, and apoptosis induction were determined in A549 COX-2 overexpressing human non-small cell lung cancer (NSCLC) cells. The COX inhibition of celecoxib increased with concentration up to 82% at 1μM after 24 hr exposure. Forty μM and 50μM of celecoxib induce G_1 arrest, and TUNEL-positive apoptotic cells, respectively. Among 150 compounds, several compounds were selected for having greater COX-2 inhibitory activity and higher selectivity than celecoxib with growth inhibitory activity. Celecoxib showed concentration-dependent COX inhibitory activity, and ability to induce cell cycle arrest and apoptosis in human NSCLC cells in vitro. Among synthetic analogues screened, several compounds showed promising in vitro activity as COX-2 inhibitory anticancer agents, which warrant further evaluation in vitro and in vivo.
