Comparison of Melanoma Subtypes among Korean Patients by Morphologic Features and Ultraviolet Exposure

Background: Genetic alterations have been identified in melanomas according to different levels of sun exposure. Whereas the conventional morphology-based classification provides a clue for tumor growth and prognosis, the new classification by genetic alterations offers a basis for targeted therapy. Objective: The purpose of this study is to demonstrate the biological behavior of melanoma subtypes and compare the two classifications in the Korean population. Methods: A retrospective chart review was performed on patients found to have malignant melanoma in Severance Hospital from 2005 to 2012. Age, sex, location of the tumor, histologic subtype, tumor depth, ulceration, lymph node invasion, visceral organ metastasis, and overall survival were evaluated. Results: Of the 206 cases, the most common type was acral melanoma (n=94, 45.6%), followed by nonchronic sun damage-induced melanoma (n=43, 20.9%), and mucosal melanoma (n=40, 19.4%). Twenty-one patients (10.2%) had the chronic sun-damaged type, whereas eight patients (3.9%) had tumors of unknown primary origin. Lentigo maligna melanoma was newly classified as the chronic sun-damaged type, and acral lentiginous melanoma as the acral type. More than half of the superficial spreading melanomas were newly grouped as nonchronic sun-damaged melanomas, whereas nodular melanoma was rather evenly distributed. Conclusion: The distribution of melanomas was largely similar in both the morphology-based and sun exposure-based classifications, and in both classifications, mucosal melanoma had the worst 5-year survival owing to its tumor thickness and advanced stage at the time of diagnosis. Biodegradable retinal fixation devices obtain mechnical fixation of the retina with desirable chorioretinal scarring and with the potential for local, sustained release of antimetabolites and steroids to inhibit proliferative vitreoretinopathy. We manufactured a biodegradable retinal tack with barb that was designed in order to prevent intrusion from implantation of retinal tacks. This study was carried to evaluate the efficacy for retinal fixation and the capability for sustained release of drugs with a newly designed biodegradable retinal tack Biodegradable retinal tacks were made of polymers of glycolic acids and were designed with barbs in a shape to prevent the disinsertion. Biodegradale retinal tacks are divided into 3 parts, a conical portion that is inserted into the sclera, a cylinder portion that remains in the vitreous, and a neck portion between the pin and the cylinder. The tapered conical end was manufactured to allow easy insertion through the retina and choroid into the sclera. A cylinder portion was manufactured with a tapered angle that fixes firmly into the orifice of 19 gauge spinal needle. A neck portion, 0.4 mm in diameter, was designed to prevent disinsertion from following implantation of retinal tack. The applicator was a 19 gauge spinal needle and its orifice was prepared to 15°angle to accept the tapered cylinder portion of the retinal tack. The retinal tacks, secured in the needles, were passed through the formed vitreous and inserted into the retina, choroid, and sclera and were released by pushing the internal needle, usually within 2-3mm of the medullary ray of the posterior rabbit retina A retinal tack was placed in each of 8 pigmented rabbit eyes. Slit-lamp biomicroscopy, indirect ophthalmoscopy and fundus photography were performed periodically from 1 day to 8 weeks after surgery. Eight eyes were enucleated and studied by light microscopy at 8 weeks. Biomicroscopic evaluation of the animals revealed edemas adjacent to the retinal surfaces immediately after insertion of the biodegradable retinal tacks in all the animals. These edemas disappeared after 1 week. The first noticeable change in the size of retinal tacks was shown after 2weeks. The size of the retinal tacks gradually got smaller, decreasing to about one-half at 4 weeks and about one-third at 8 weeks. All retinal tacks remained in inserted places without any movement for an 8 week period. On light microscopy, epiretinal proliferations were seen to extend into the vitreous cavity. Cellular capsules that lined the inner aspect of the scleral defect caused by tack insertion were found. However the adjacent retina had a normal cytologic appearance and architecture in all specimens. We manufactured a biodegradable retinal tack that is designed to prevent intrusion from implantation of retinal tacks. All biodegradable retinal tacks reduce in size with time, but no retinal tacks extruded from the inserted place. The newly designed biodegradable retinal tack can be used for retinal fixation and may be used as a vehicle for the introduction of pharmacologic agents to prevent the cellular events that promote proliferative vitreoretinopathy.

In recent years, gonorrhea has been pandemic and remains one of the most common STDs in the world, especially in developing countries. For the detection of a more effective therapeutic regimen and assessing the prevalence of Penicillinase Producing Neisseria gonorrhoeae(PPNG), we have been trying to study the patients who have visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul since 1980 by menas of the chromogenic cephalosporin method. In 1998, 93 strians of N. genorrhoeae were isolated, among which 60(64.5%) were PPNG. The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 64.5% in 1998.

생체분해성 고분자를 이용한 사시수술용 제형개발 및 효용성에 대한 연구

A new device, muscle clamping system was developed to facilitate exact quatifying technique A strabismus surgery, and reduce the risk of complications. The device is composed of a lower fixing body with three jaws and an upper supporting body. They are used to clip an extraocular muscle and fix it to the sclera with a single bite. Superior rectus recession on 16 rabbit eyes were performed with this new device. Conjunctival injection, muscle adhesion strength, and light microscopic findings were examined at 1, 2, 4 and 8 weeks postoperatively. The Conjunctival injection were minimal, adhesion power ranged from 420 to 600 gram gravity, which is sufficient in withstand the normal pull of human extraocular muscle. In microscopic exam, some Inflammatory cells and fibrosis were found. The new device was technically easy, fast, and accurate, so it may be useful in stabismus surgery.

Flexural Responses of Prestressed Hybrid Wide Flange Composite Girders

In this study, prestressed hybrid wide flange (PHWF) composite girders were proposed, and full-scale flexural tests were conducted to evaluate their structural performances. This new proposed girder system was developed and designed to effectively resist external loads considering the actual construction sequences. Two specimens with and without shear connectors were fabricated and tested to examine the effect of the shear connectors for achieving the fully-composite behaviors between a cast-in-place (CIP) concrete and the prefabricated prestressed steel–concrete composite girder. The test results showed that sufficient flexural strengths and deformation capacities can be obtained in both types of PHWF composite girders with and without shear connectors. To reflect the actual construction stages of the proposed PHWF composite girder, nonlinear flexural analyses were proposed considering the prestress effect and segmental effect before and after composite with the CIP concrete, respectively. The observed and analysis results of strain behaviors of the PHWF girder specimens were also compared and discussed in detail.

Purpose : To find the cause of root curvature by use of panoramic and lateral cephalometric radiograph. Materials and Methods : Twenty six 1st graders whose mandibular 1st molars just emerged into the mouth were selected. Panoramic and lateral cephalometric radiograph were taken at grade 1 and 6, longitudinally. In cephalometric radio graph, mandibular plane angle, ramus-occlusal plane angle, gonial angle, and gonion-gnathion distance (Go-Gn distance) were measured. In panoramic radio graph, elongated root length and root angle were measured by means of digital subtraction radiography. Occlusal plane-tooth axis angle was measured, too. Pearson correlations were used to evaluate the relationships between root curvature and elongated length and longitudinal variations of all variables. Multiple regression equation using related variables was computed. Results : The Pearson correlation coefficient between curved angle and longitudinal variations of occlusal plane-tooth axis angle and ramus-occlusal plane angle was 0.350 and 0.401, respectively (p<0.05). There was no significant correlation between elongated root length and longitudinal variations of all variables. The resulting regression equation was Y=10.209+0.208X_(1)+0.745X_(2) (Y: root angle, X_(1): variation of occlusal plane-tooth axis angle, X_(2): variation of ramus-occlusal plane angle). Conclusion: It was suspected that the reasons of root curvature were change of tooth axis caused by contact with 2nd deciduous tooth and amount of mesial and superior movement related to change of occlusal plane. The aging properties of cellular plastic insulations(EPS,PUF and UFF) were investigated by varying temperature, relative humidity and time. The time aging properties were changed with in 10 - 12% of initial properties at high temperature(above 318K) and relative humidity (above 90%). The amount of HCHO gas release from UFF was increased as temperature and relative humidity increased, and HCHO release rate was decreased exponentially with time.

입원환자를 대상으로 한 대집단 싸이코드라마의 치료요인 연구

연구목적 : 본 연구는 개인정신치료와 집단정신치료의 훈련을 받은 정신과 의사라면 누구든지 쉽게 이용할 수 있는 우리나라 입원환자에게 적합한 대집단 싸이코드라마의 모델을 개발하고, 이를 입원환자에게 적용한 후 치료요인을 조사하여, 입원환자 치료에 도움이 되고자 하는 목적으로 본 연구를 시행하였다. 방 법 : 본 연구는 1998년 3월부터 1998년 9월까지 계요병원 입원환자 싸이코드라마에 참가한 모든 환자들과, 보조자아로 참석한 모든 치료팀을 대상으로 하였다. 대상자는 환자가 346명(남자 152명, 여자 194명)이었고, 치료팀은 총 60명이었다. 환자에게는 13가지 치료요인 척도, 싸이코드라마에 대한 일반적인 태도 척도, 도움이 되었던 점에 대해 기술하도록 한 설문지를 사용하였으며, 치료팀에게는 13가지 치료요인과 환자에게 도움이 되었던 점에 대해 기술하도록 한 설문지를 사용하였다. 환자집단은 관객, 보조자아의 역할을 한 환자, 공유(sharing)시간에 참여한 환자, 주인공으로 나누어 치료요인을 비교하였다. 결 과 : 먼저, 환자의 관점에서 본 치료요인은 다음과 같다. 1) 싸이코드라마에서의 역할에 관계없이 거의 대부분의 환자집단에서 실존적 인지와 일차가족집단의 교정적 감정경험을 매우 중시하였다. 2) 주인공, 보조자아의 역할을 한 환자, 공유시간에 참여한 환자와 같이 어떤 형태로든 싸이코드라마에 직접적으로 참여한 환자들은 사회화 기술의 발달을 중시하였으며, 보다 직접적인 형태인 행동으로 참가한 주인공이나 보조자아의 경우 카타르시스를 중시하였다. 3) 주인공을 제외한 모든 환자들은 희망의 고취를 중시하였다. 4) 관객의 경우 치료자와 환자에 대한 동일시와 보편성을 중시하였다. 환자와의 동일시는 공유에 참가한 환자의 경우 특히 중시하였고, 보편성은 보조자아로 참가한 환자의 경우 또한 중시하였다. 다음으로 치료팀이 평가한 치료요인을 환자들이 평가한 치료요인과 비교하면, 치료팀은 환자들보다 집단응집력을 더 중시하였으며, 반면 환자들이 중시한 사회화 기술의 발달과 치료자나 환자에 대한 동일시는 매우 낮게 평가하였다. 결 론 : 환자들의 역할에 따라 세분화하여 조사한 치료요인을 통해 입원환자 대집단 싸이코드라마의 특성과 환자의 역할에 따른 치료요인의 특성을 파악할 수 있었다. 본 모델은 입원환자 치료에 쉽게 적용될 수 있었으며, 전체적으로 많은 환자들에게 도움이 되었다. Objectives : This study was performed to make a model of large group psychodrama suitable for Korean inpatients and easy for psychiatrists trained in the individual psychotherapy and the group psychotherapy to use, to investigate the therapeutic factors after applying this model to inpatients, and then to contribute to the treatment of inpatients. Methods : The study subjects were all the inpatients and the therapists(the auxiliary ego's) who participated in psychodrama from March 1998 to September 1998. The subjects were composed of 346 inpatients (152 males, 194 female) and 60 therapists. To the inpatients 13-Therapeutic Factors Scale, Scale of General Attitude to Psychodrama, and the Questionnaire describing helpful things in psychodrama were administerd. The inpatient group were divided into 4 groups(the audience, the auxiliary ego's, the participants in sharing, the protagonists). The therapeutic factors among 4 groups were compared. Results : First, the therapeutic factors described by the inpatients group were as follows : 1) Most inpatients made great account of ‘existential factor’ and ‘the corrective recapitulation of the primary family group’ regardless of their roles in psychodrama. 2) The inpatients who participated in psychodrama directly as the protagonists, the auxiliary ego's, or the participants in sharing made much account of ‘development of socializing techniques’. The inpatients who participated in psychodrama more directly as the protagonists or the auxiliary ego's made much account of ‘catharsis’. 3) All the inpatients except the protagonists made much account of ‘instillation of hope’. 4) The audience made much account of 'identification' and 'universality'. Especially,'identification with members' was much accounted of by the participants in sharing. ‘Universality’ also was much accounted of by the auxiliary ego's. Next, when we compared the therapeutic factors in the inpatient group with those in the therapist group, the therapist group made much account of ‘group cohesiveness’ which the inpatient group made little account of. And the therapist group made no account of ‘development of socializing techniques’which the inpatient group made much account of. Conclusion : By dividing the inpatient group according to their roles, we found the characteristics of the inpatient large group psychodrama and of the therapeutic factors according to the roles of the inpatients. This model could be applied to the treatment for the inpatients easily and was helpful to many inpatients on the whole.
