        접합면의 차이에 따른 두개골 외판의 생존

        Onlay graft of the calvarial bone has been popularized in craniofacial surgery because absorption of the calvarial bone is less than that of the endochondral bone. But the problems with using the calvarial bone are its rigidity and difficulty in setting a precise apposition. When the outer tables of the calvarial bone are used for augmentation and reconstruction of the convex zygoma, forehead, or chin, it is better to place the cancellous surface over the convex recipient bone to get a precise apposition. Whereas, it is better to place the cortical surface over the concave nose or orbital cavity to achieve a good apposition. Therfore, our study was designed to determine the differences of bone absorption and regeneration between cancellous and cortial bone contact to facial bone, and between preserved periosteum and detached periosteum in autograft of calvarial bone in dog models. Outer tables of the calvarial bone were placed in subperiosteal pockets of the upper and lower maxilla in four different ways : Group I ; The cancellous surface was placed in contact with the bare recipient bone, and the cortical surface attached with periosteum was accordingly contacted with the elevated periosteum of the recipient bone. Group Ⅱ ; The corical surface attached with periosteum was placed in contact with the bare recipient bone and the cancellous surface was contacted with the elevated periosteum of the recipient bone, Group Ⅲ ; The arrangenent was similar to Group Ⅰ except that the periosteum of the graft was deprived. Group Ⅳ ; The arrangement was similar to Group Ⅱ except that the periosteum of the graft was deprived. Volume measurements using a caliper technique and histological study were made 20 weeks postoperatively. The volume of maintenance is as follows ; Group Ⅰ, 84.2% ; Group Ⅱ, 77.6% ; Group Ⅲ, 77.0%, and Group Ⅳ, 69.5%. The histolgical contribution of living bone was assessed by a modified point counting technique : Group Ⅰ, 86.6%, Group Ⅱ, 83.8% ; Group Ⅲ, 79.6% and Group Ⅳ, 77.6%. Greater volume maintenance and histological contribution of living bone were found when cancelllous surface rather that the cortical were placed in contact with the recipient bone and the grafts from their periosteum were preserved. We concluded that in order to expect better survival of a grafted bone, the cancellous surface of the graft should contact with the recipient bone and that the periosteum of the graft should be preserved.

        급성췌장염을 동반한 특발성 양측성 횡격막 마비 1례

        Bilateral diaphragmatic paralysis(BDP) is a rare disorder, which can be secondary to spinal cord injury, motor neuron disease, myopathy, noninfectious polyneuropathy, infection, iced saline cardioplegia performed during cardiac surgery, or idiopathic causes. There may be typical presentations such as dyspnea, paradoxical respiratory movement, and hypercapnic respiratory failure. It needs to exclude above secondary causes to consider idiopathic bilateral diaphragmatic paralysis. We report a 51-years-old man who presented with idiopathic bilateral diaphragmatic paralysis combined with acute pancreatitis. We couldn't find out the association of these two clinical conditions. The patient was improved by mechanical ventilation.

        혈행차단에 따른 두개골외판 이식의 생존 차이

        Difficulties arise in prediction of maintenance of the graft volume and viability of graft over time when a bone graft used for facial reconstruction. A bone-to-bone contact between the graft and the recipient bone is imporant for creeping substitution and survial of the recipient bone is important for creeping substitution and of the grafted bone. An adequate blood supply is also essential to ensure the survival of any live cells of the surface of the graft. Our study was designed to determine which one is an important factor viability of the grafted bone in Korean adult dogs : a bone-to-bone contact or overlying soft tissue. Blocks of outer table of the parietal bone were placed at the maxillae subperiostially in 4 different ways : bone-to-bone contact groups(groups Ⅰand Ⅱ)with placing a silicone membrane over the grafted bone and soft tissue contact groups(groups Ⅲand Ⅳ)with placing a silicone sheet between the grafted bone and the recipient. In groups Ⅰand Ⅲ, the cancellous surfaces of the parietal bone was placed on the recipient and the cortical surfaces were placed on the recipient in groups Ⅱ and Ⅳ. Caliper techniques were used to study the rates of volume maintenance of the grafts at 6, 12, and 20 weeks after bone grafting. The volumes of the living bone were quantified microscopically using a modified point-countiong technique. The volume is reduced in a similar rate with time in all groups. At 6 week, living bone cells increased in soft tissue contact groups Ⅲ and Ⅳ however, and increased in bone to bone contact groups Ⅰand Ⅱ at 12 and 20 weeks. there were osteoblastic proliferation and laminated mature bones in group Ⅰand Ⅱ. But osteoclasts and their associated osteolytic changes were still seen near the silicone membrane in group Ⅲ and Ⅳ, which may imply a continuing resorptive process with time. In summary, revascularization from the overlying soft tissue is important for the graft survival in early stage of the bone grafting while bone-to-bone contact may be essential in a later stage.

      • 흰쥐 뇌파에 미치는 Halothane 마취 효과의 정량적 분석

        An objective quantification of the level of halothane anesthesia by spectrum analysis of EEG recorded from the rat scalp was studied. The rats were divided into four groups according to exposure concentration of 1.0, 1.5, 2.0 and 2.5% halothane, and then bipolar EEG recording through one lead from the rat scalp was followed by power spectrum analysis. The density of each band (delta 1-3.5 Hz, theta 3.5-8 Hz, alpha 8-13 Hz, betal 13-18 Hz, beta2 18-21 Hz, and beta3 21-30 Hz) and total density, and median power frequency were calculated from the spectrums. These 8 parameters were taken for a stepwise or direct discriminant analysis. Predicted groups by the discriminant analysis for the EEG parameters were in accordance with 94.12% of the actual groups. The results suggest that the discriminant analysis for the EEG parameters derived from power spectrum can be applied to determination on the level of halothane anesthesia.

        We have encountered a rare case of symmetrical peripheral gangrene complicating Escherichia coli sepsis in a 47-years-old male. He was successfully treated with antibiotics, anticoagulants, and vasodilator. To our knowledge, this is the first report on symmetrical peripheral gangrene complicating E. coli sepsis in Korea.

        오대산 월정사지역 전나무림의 생태학적 연구

        본 연구는 오대산 국립공원내 월정사지역에 분포하고 있는 전나무림의 식생구조를 분석하여 이 지역 전나무림의 관리에 대한 기초자료를 제공하기 위하여 수행되었다. 그 결과 전나무림의 층위구조는 교목층(2종류), 아교목층(20종류), 관목층(46종류) 그리고 초본층(87종류)으로 구성되어 있었다. 조사지역에 출현한 전체식물의 중요치는 전나무가 34.09%로 가장 높게 나타났고, 당단풍(23.07%), 청시닥나무(14.44%), 고광나무(10.72%), 오리방풀(8.49%), 복자기(7.80%), 고추나무(7.65%), 산거울(6.79%), 방아풀(5.76%), 고로쇠나무(5.47%), 개다래(5.41%)의 순으로 나타났다. 특히, 전나무의 치수가 전혀 없어 앞으로 아교목층과 관목층을 형성하고 있는 단풍나무류, 참나무류, 피나무류 등의 경쟁에 의하여 천이가 있을 것으로 사료된다. The vegetation structure of Abies holophylla forest was analyzed to provide the information for conservation of Abies holophylla at Wol-jong Temple in Mt. Odae national park, southern Korea. The layer structure of Abiesholophylla forest was consists of tree layer(2 species), subtree layer(20 species), shrub layer(46 species) and herb layer(87 species). The importance value of Abies holophylla was the highest one 34.09% , and those of Acer pseudo-siebodianum, Acer barbinerve, Philadelphus schrenckii, Isodon excisus, Acer triflorum, Staphylea bumalda, Carex humilis, Isodon japonicus, Acer monp and Actinidia polygama were 23.07%, 14.44%, 10.72%, 8.49%, 7.80%, 7.65%, 6.79%, 5.76%, 5.47% and 5.41%, respectivery. Especially, when predicting succession in Abies holophylla forest, such seedlings were not formed and succession was governed by competition of Acer species, Quercus species and Tilia species of subtree and shrub layer.       • Mg(OH)₂를 이용한 습식탈황에서 폐슬러리 처리특성연구

        Properties analyses and treatment characteristics of waste slurry discharged in the wetted flue gas desulfurization process using Mg(OH)₂as a absorbent were investigated. Aeration and coagulation test were carried out to evaluate variation of some factors such as COD, SS, T-N, etc. related to waste slurry treatment. The hydraulic retention time(HRT) and coagulant dosage rate were applied as operating variables in experiment. As experimental results, the reduction effects of COD, SS in aeration test were distinct, due to oxidation of SO₃ and floc, formation of suspended solids. It was also found that the reduction efficiencies of SS and T-P in coagulation test are higher than 90%.

      • Diving Pattern에 따른 체내 질소축적율의 검토

        To perform a study, the influence on nitrogen's percentage of body according to diving pattern, made male colegian sports diver(5) with 2 year diving experience dive by 4 diving patterns(deep-sea diving, long diving, repetitive diving, standard diving) and, after collecting statistics of the solution of nitrogen gas and analysed it by diving time(the depth of water), by the parts of the body, the conclusion are as following. 1. When they reached the water surface after 4 pattern of diving, the nitrogen gas dissolved in the tissue of human body did not form any bubbles even of a small quantity of nitrogen gases would remain dissolved in the tissue of human body not being excreted. Accordingly, When other diving similar to these diving patterns will be performed, there is little probability of decompression sickness, althouth there is some difference according to diver's physical constitution of condition. 2. In the solution of nitrogen gas in the tissue human body, the nervous system and the skin tissue were rapid in absorption and excretion, but, on the other hand, the muscular tissue and the bone tissue were not. 3. It is ideal that all sports divers should stay in the water or perform a standard under 18 meter depth of water and decompress safety in the water 5 meter deep for the purpose of safe as possible as they can. Throuth the above findings, I hope a study on the safe ascent process and one on the changes of heart best and the movement of blood will be preceded as a future researth task.

        A comparison of the BISAP score and serum procalcitonin for predicting the severity of acute pancreatitis

        Background/Aims: The bedside index of severity in acute pancreatitis (BISAP) is a new, convenient, prognostic multifactorial scoring system. As more data are needed before clinical application, we compared BISAP, the serum procalcitonin PCT), and other multifactorial scoring systems simultaneously. Methods: Fifty consecutive acute pancreatitis patients were enrolled prospectively. Blood samples were obtained at admission and after 48 hours and imaging studies were performed within 48 hours of admission. The BISAP score was compared with the serum PCT, Ranson`s score, and the acute physiology and chronic health examination (APACHE)-II, Glasgow, and Balthazar computed tomography severity index (BCTSI) scores. Acute pancreatitis was graded using the Atlanta criteria. The predictive accuracy of the scoring systems was measured using the area under the receiver-operating curve (AUC). Results: The accuracy of BISAP (≥ 2) at predicting severe acute pancreatitis was 84% and was superior to the serum PCT (≥ 3.29 ng/mL, 76%) which was similar to the APACHE-II score. The best cutoff value of BISAP was 2 (AUC, 0.873; 95% con- fidence interval, 0.770 to 0.976; p < 0.001). In logistic regression analysis, BISAP had greater statistical significance than serum PCT. Conclusions: BISAP is more accurate for predicting the severity of acute pancreatitis than the serum PCT, APACHE-II, Glasgow, and BCTSI scores.
