        공허안 증후군을 동반한 양측성 거대결절성 부신피질 과증식에 의한 쿠싱 증후군 1 예

        ACTH-dependent bilateral adrenocortical hyperplasia, Known as Cushing's disease, accounts for 60-70% of the cases of Cushing's syndrome. In about 30-40% of patients, manifestations of Cushing's syndrome occur independently of pituitary ACTH secretion due to either ectopic ACTH production or an adrenocortical adenoma or carcinoma. Another rere cause of Cushing's syndrome, also independent of ACTH, is so-called primary adrenocortical nodular dysplasia, in which adjacent areas of the adrenal cortex are atropic. The term bilateral macronodular adrenocortical hyperplasia(MNH) is restricted to the presence of multiple nodules visible to the naked eye and often more than 0.4cm in diameter. Its pathogenesis is still unknown. This report describes a patient with Cushing's syndrome due to ACTH - independenilateral macronodular adrenocortical hyperplasia associated with empty sella syndrome. A 63-year- old woman presented with Cushingoid features, hypertension, diabetes mellitus, dyspnea and chest pain. Urinary free cortisol and 17-hydroxycorticosteroid excretion were 350ug/day and 10.9 mg/day, respectively. Urinary free cortisol didn't decrease after administration of dexamethasone (low and high dose). Plasma ACTH was 4.2pg/ml. Magnetic resonance imaging(MRI) of the sella turcica revealed complete cisternal hernation, suggesting empty sells syndrome. Bilateral total adrenalectomy was perfomed. The right adrenal gland weighed 11 gm and contained two nodules and their sizes are 2.5cm, and 1 cm. The left adrenal gland showed all the features of odular hyperlasia with a 1.5 cm nodule. The postoperative course was uneventful and the patient was discharged to be treated with replacement steroids. (J Kor Soc Endocrinol 8: 203~210, 1993)

        뇌하수체에 전이된 십이지장 팽대부주위 선암종 1 예

        Pituitary metastais is a rare condition of distant metastasis of systemic cancer. Pituitary metastasis may present headache, hypopituitarism, and diabetes insipidus resulting from metastatic site. We report a 42 year-old man with pituitary metastasis from periampullary adenocarcinoma who developed diplopea follwing headache and importence. He also had widespreak lymph node metastasis. Diagnosis was made by endoscopy with biopsy, abdominal ultrasonogram, abdominal CT scan, and brain CT scan. Brain CT scan showed tumor in the pituitary gland with invasion into sphenoid bone and cavernous sinus. Light microscopic fingdings revealed moderately differentiated adenocarcinoma in periampullary mass, and metastatic adenocarcinoma in the cevical lymph node. The combined pituitary stimulation test showed decreased reserve capacity of pituitary hormones. We report a case of pituitary metastasis from periampullary adenocarcinoma with the revies of the literature.(J Kor Endocrino 8:88~93, 1993)

        스마트 디바이스 제품속성이 제품수용에 미치는 영향

        본 연구는 UTAUT이론을 바탕으로 정보통신 기술수용연구에서 상대적으로 간과되어왔던 다양한 제품특성(제품기능성, 디자인, 사용편의성)이 기능적 동기(노력기대, 성과기대)와 쾌락적 동기(인지된 즐거움)에 미치는 영향과 사용동기가 스마트 디바이스 사용의도에 어떠한 영향을 미치는지 규명하고자 진행되었다. 서울을 포함하여 5대광역시의 스마트 디바이스 사용자 800명을 비례할당방식으로 조사한 결과, 기능적 동기(노력기대, 성과기대)와 쾌락적 동기(인지된 즐거움)는 스마트 디바이스 사용의도에 정(+)의 영향을 미치는 것으로 나타났으며, 스마트 디바이스의 디자인은 인지된 즐거움에, 사용용이성은 노력기대와 성과기대에 각각 정(+)의 영향을 미치는 것으로 나타났다. 그러나 제품기능성은 노력기대와 성과기대에 유의한 영향을 미치지 못하는 것으로 나타났다. The purpose of this study is to investigate the relationship between product attributes of smart device and user acceptance of technology. Moreover, the authors examined mediating effect of utilitarian and hedonic motivation between product attributes and user acceptance. Previous researches focused on user attitude and behavior intention of new technology based on Technology Acceptance Model(TAM). However, differently from previous researches, which are focused on user attitude like perceived usefulness and perceived ease of use, this research emphasized the relationship product attributes such as product functionality, design, and convenience and user motivation both utilitarian(effort expectancy and performance expectancy) and hedonic motivation(perceived enjoyment) according to UTAUT model. The results shows that 3 motivation variables(effort expectancy, performance expectancy, and perceived enjoyment) have positive impacts on user acceptance intention of smart device. product design has positive relationship to perceived enjoyment and product convenience has positive relationship to both effort and performance expectancy. Furthermore, 3 motivation variables play a full mediation between product attributes and user acceptance intention of new technology. However, product functionality has no relationship to both effort and performance expectancy. Research contribution, implications, and further research directions are discussed at the end of the paper.

        생체활성 유기물로 표면이 개질된 임플란트 개발 추이 분석 연구

        연구 목적: 기존의 기계절삭 가공된 티타늄 표면에 생체활성 유기물질이 코팅된 생체 활성 임플란트에 관한연구가 최근에 많이 이루어지고 있다. 이러한 생체 활성 임플란트는 임플란트 자체에 골치유 및 골형성 효과를 부여할 수 있다. 본 연구의 목적은 생체 활성 임플란트 표면에 대한 연구 동향을 분석함으로써, 표면처리 기술 개발의 현황과 향후 발전 전망에 대해서 알아보고자 하였다. 연구 재료 및 방법: 본 문헌 분석 연구는 'Web of Science (http://isiknowledge.com, Thomson Scientific)'를 통해 SCI (science citation index) 등재 논문들을 기반으로 분석하였다. 주제별로 연구비중을 비교하기 위해서, 임플란트 표면 코팅에 사용되는 생체활성 유기물들로 키워드를 선정하고 각 키워드로 대표되는 리서치 그룹을 선정하여 각 그룹들의 연구 동향을 분석하였다. 또한 키워드별로 동물실험 동향을 분석하여 상용화 가능성에 대해서도 고찰하였다. 결과: 연구의 비중은 콜라겐, 피브로넥틴, 골형성단백질, RGD (Arg-Gly-Asp) 순으로 나타났으며, 콜라겐을 이용한 연구가 40%로 가장 많은 연구가 이루어지고 있었다. 네가지 주제 모두 시간의 경과에 따라 연구횟수가 증가하는 양상을 보였으며, 특히 RGD에 관한 연구횟수가 가장 급격하게 증가하였다. 콜라겐 대표 리서치 그룹의 연구 동향을 분석하였을 때, 콜라겐은 단독 사용보다는 다른 생체활성 유기물 및 무기물과의 병용 처치에 관해 주로 연구가 되고 있었으며, 특히 세포외기질인 콘드로이틴 설패이트와의 병용 처치에 관한 연구가 가장 많이 이루어지고 있는 것으로 나타났다. 골형성단백질의 대표 리서치 그룹들에서는 rhBMP-2 (recombinant human bone morphogenetic protein-2)에 관한 연구가 주로 이루어지고 있었다. 동물실험에 관한 연구 동향 분석 결과 콜라겐은 차폐막 혹은 약물 전달 매체로서 주로 사용되고 있었고, 골형성단백질은 치조골이식술 혹은 임플란트 표면 코팅에 대한 효과가 연구되고 있었다. 결론: 연구 결과를 주제별로 분석하였을 때 주제별로 일관성 있는 결과를 얻기 어려웠으며, 보다 나은 결과를 얻기 위한 기계적 표면처리와 여러 세포외기질, 성장인자 등의 생체물질의 최상의 조합을 찾기 위한 노력이 진행중이었다. 연구가 초기단계에 머물고 있기 때문에 상용화에 이르기까지는 많은 시간과 노력이 필요할 것이다. Purpose: This study aims to evaluate and prospect for current research trend and developmental perspectives via analyzing recent biomaterial coated-implants study. Materials and methods: To investigate each subject respectively, several biomaterials that are using for implant surface coating were set as 'keywords'. By these keywords, major research groups in each subject were chosen, and research trend of them was analyzed. Trend of In vivo studies that examined selected biomaterials were analyzed to evaluate commercial potential. Results: The collagen research accounted for 40% of total implant study, which was the highest, and fibronectin, BMPs (bone morphogenetic proteins) and RGD (Arg-Gly-Asp) peptides followed, which were ranked in descending order. Furthermore, figures of all four research subjects were also increased with time, especially a sharp increase in RGD research. According to the results of major research group, collagen that was combined with other organic and inorganic biomaterials was mostly examined, rather than using collagen only. Major research groups investigating BMPs mostly focused on rhBMP-2. In animal studies, collagen was used as resorbable membrane in guided bone regeneration (GBR) or drug carrier, while BMPs were used with bone graft materials or coating material for titanium implant surface. Conclusion: There is not consistency of results even in identical subjects research field. Many studies are ongoing to optimize combination between mechanical surface treatment and biomaterials such as extracellular matrix component and growth factors.

        Warfarin skin necrosis mimicking calciphylaxis in a patient with secondary hyperparathyroidism undergoing peritoneal dialysis

        Warfarin skin necrosis (WSN) is an infrequent complication of warfarin treatment and is characterized by painful ulcerative skin lesions that appear a few days after the start of warfarin treatment. Calciphylaxis also appears as painful skin lesions caused by tissue injury resulting from localized ischemia caused by calcification of small- to medium-sized vessels in patients with end-stage renal disease. We report on a patient who presented with painful skin ulcers on the lower extremities after the administration of warfarin after a valve operation. Calciphylaxis was considered first because of the host factors; eventually, the skin lesions were diagnosed as WSN by biopsy. The skin lesions improved after warfarin discontinuation and short-term steroid therapy. Most patients with end-stage renal disease have some form of cardiovascular disease and some require temporary or continual warfarin treatment. It is important to differentiate between WSN and calciphylaxis in patients with painful skin lesions.

        We isolated a marine bacterium, Pseudomonas sp., which could produce the enzyme of alginate lyase, and cloned the alginate lyase gene in Escherichia coli. The cloned DNA was over expressed with approximately 50% amount of total proteins. In addition, the expressed proteins were not secreted into the medium, and most of them existed in the cytoplasm by the soluble form but not observed any inclusion body by TEM. For the optimum enzyme activity, temperature was 20℃, pH was 7.0, and Km and Vmax values of the enzyme were 0.4% and 625 units/㎎, respectively.
