        2급 응급구조사의 성인외상환자에 대한 처치의 적절성 평가

        In Korea, EMT have two classified subgroup of first and second degree : EMT-P, and EMT. For the beginning, authorization of the first EMT was made in this year through the EMT educational program. There are 8 items which EMT can perform the treatment without the supervision by a physician. But there was no previous report of pertinent assesment of treatment performed by the EMT. There upon authors have conducted review of assessment done by the EMT in EC setting, and proposed the following treatment application can be used by EMT at the field of accident. The study materials include adult trauma patient older than 15 years, and sustained trauma within 48 hours before visiting Ewha University DongDaeMoon Hospital Emergency Deparment. The duration of study was from Apr. 1 1995 to Aug. 31 1995. the study was conducted by 5 certified EMT. The EMT's performance was evaluated by the attending Emergency Medicine physician. The result were as followings ; 1. Among the total 5532 ED visiting patients during the study period, 1267 were adult trauma patients who recieved treatment within 48 hours of injury. 2. Among 1222 out of 1267 patients have completed performed survey. 3. Male to female ratio was 2.6 : 1, age distribution was from 15 to 87 years old and mean age was 34.6 years old. 4. The MOI showed : MVA (33.1%), blunt trauma (29.9%), slip down (16.8%), penetrating injury(15.4%), and miscellaneous (5.7 %), burn (3.1%), falling down injury(2.9 %), bite injury(1.1%) in order. 5. There were 437 cases of survey completed by the EMT. For assessment of mental status of 3 different method : AVPU, 5 scale mental status and GCS, were correlated with EM physician with 99.1%, 98.6%, 97.7 %, respectively. 6. For 320 out of 437 cases of survey completed, 319 patients recieved treatment by the EMT within 30 minute and 1 patient between 30 to 60 minute of an ED arrival. 7. For those 437 cases of survey completed, 266 patient (60.9%) had adequate treatment and 54 patients (12.4%) had inadequate treatment. 67 patient (15.3%) did not received treatment but it was adequate inway, and 50 patients (11.9%) did not received treatment even though patient required treatment. In cases of adult trauma patients, EMT's mental status assessment was correlated with EM physician's assessment. But treatment done by EMT without the supervision of EM physician was adequate only in 76.2 %. Therefore, continuous education and assessment of treatment performed by EMT in needed to improve field treatment by EMT future.

      • 목재 건조공정 배기가스의 특성에 관한 연구

        This study was performed to provides the characteristics of flue gas producted from wood drying process. Generally, the flue gas producted from wood drying process was elutriated with a large amount of moisture and odorous compounds such as BTX and ethanol. Therefore, it is very difficult to select and design of flue gas control devices. In order to optimum design and selection of control devices, have to investigate the various parameters such as temperature, moisture contents and flow rate of flue gas. When the wood is almost dry(<10% moisture), the moisture contents of flue gas were very high (about 18.2 V/V%). It is equivalent to 320 ton/day moisture as 1100 ton wood/day induce to the drying process. The temperature of flue gas ranges from 140℃ to 150℃ in each dryer stack with exception of the input site of wood(about 110℃). Thevelocity of flue gas In each stack ranges from 1.7 to 9.7 m/sec. About 40 VOCs in the flue gas were analyzed, and the concentrations of major VOCs(ppm) were as follows respectively; benzen: 0.054∼0.052, toluene: 1.011∼2.547, ethylbenzen: 0.472∼2.023, m,p-xylene: 0.504∼3.245, styrene: 0.015∼0.148, o-xylene : 0.271∼1.097, ethanol: 11.2∼32.5, α-pinene: 0.908∼10.578, β-pinene: 0.982∼14.278.

      • Rhizopus製劑 添加給與가 肉鷄의 血液像에 미치는 影響

        This study was conducted to observe the effect of four Rizopus levels C(0%), T_1(0.04%), T_2(0.08%) and T_3(0.12%) on Hematological valuse in male and female broiler chicks at 56days of age The results obtained were summarized as follows: 1. The mean Erythrocyts count was 3.50±0.19×10exp(6)/㎣ in female and 3.70±0.48×10exp(6)/㎣ in male and all treatments was higher than that of control, and especially Rhizopus at level of 0.04% were the highest among all treatments. 2. The mean hemoglobin value was 8.45±0.49gm/100㎖ in female and 8.36±0.74gm/100㎖ in male and it counts at each level of addition were high in order of T_3(8.27±0.59gm/100㎖) C and T_2(8.41±0.47gm/100㎖) and T_1(8.70±0.24gm/10㎖)in female and C(7.76±0.47gm/100㎖), T_3(8.30±0.74gm/100㎖) T_2(8.56±0.44gm/100㎖) and T_1(8.88±0.50gm/100㎖) in male. 3. The hematocrit value was 29.83±1.34% in female and 30.25±2.20% in male, and it have not tendency on treatment Rhizopus at the levels of addition. 4. The mean MCV value was 84.35±5.19μ^3 in female and 83.22±12.62μ^3 in male. 5. The mean MCH was 23.87±1.57μ㎍ in female and 22.91±2.96μ㎍ in male. 6. The mean MCHC value was 28.35±1.75% in female and 27.79±3.14% in male. 7. The mean Leukocyte count was 8.87±0.58×10exp(3)/㎣ in female and 9.24±1.24×10exp(3)/㎣ in male. 8. The mean differential count of Neutrophils, Basophils, Lymphocytes and Monocytes was 23.00±1.87%, 1.75±1.30%, 1.75±0.83%, 72.00±2.12% and 1.5±0.5% in female and 22.50±2.29%, 2.5±1.66%, 1.00±0.71%, 71.75±0.83% and 2.25±1.48% in male.

        The value of a structural system is dependent on the performance and the cost. To increase the value of a structural system, we should increase the performance and decrease the cost at the same time. When selecting an adequate alternative among various candidates, we should consider criteria both in performance and cost. In this paper, we propose an Analytic Hierarchy Process based methodology for selecting the optimal bridge alternative among various candidate bridges, which consider multiple evaluation criteria simultaneously. The proposed selection methodology can give the optimal alternative by using not only general qualitative criteria such as sociality, shape and constructibility, but also quantitative criteria such as life cycle cost, structural stability, usability, and environment. However, the proposed methodology is difficult to use manually since it requires much information and complex analysis models. To overcome the lack of practical usability, we also propose a framework for a decision support system which can help the decision maker by using information technology. We expect that the proposed selection method and the decision support system framework can be used as a useful tool for initial planning of structural systems, especially bridges.

      • 음식물 발효퇴비내 염분의 용출특성

        The objectives of food waste composting are to treat the wastes and to reclaim valuable substances present in the wastes for possible reuse. In the process of food waste composting, however, the salt in the compost can be retained within the soil for extended periods of time. In this study, therefore, a series of experiments were carried out to investigate the leaching characteristics of the salts in the compost of food waste. The resalts obtained from this study can be concluded as follows: 1. The composting process reduced the amount of waste to management because the 80% of water content in the food waste was lowered to 10% of that in the compost, and organics substances in the food waste were decomposed into CO₂, NH₃, water, and various inorganic substances. 2. It was found that the amount of toxic substances such as heavy metals in the food waste were very little, therefore, the metal concentration might not cause any adverse effect on the growth of the corps. 3. The concentrations of leachable cations in the compost were 10∼20% higher than those in the food waste of around 1000 meq/kg. And, SAR of the leachates of the food waste and compost was around 3. 4. The leaching rate of the salts in the compost was decreased with increasing the composting period. Also, the electric conductivities of initial leachate were increased with increassing the mixing ratio of the compost to the soil used. 5. The threshold dose of the compost per day per unit area of the soil was estimated with the following equation: mmho 12.0meq·cm kg 1.65L Threshold dose= 5______ × ___________ × _______ × _____ cm L·mmho 1200meq d·㎡ = 0.08kg/㎡/d

      • 70계열 알루미늄합금 용접부의 피로균열진전 거동과 피로수명 예측에 관한 연구

        It has been important to determine life cycle and stress range for the each welded metal under high cycle load control test conditions. In this study, fatigue crack propagation rate for 7017, 7020, 7039 and 5083 alluminum welded alloy was compared with stress intensity factor range linear elastic fracture mechanics. Compact tension specimens were used and crack growth rate da/dN and stress intensity factor range ΔK were analyzed concerning to the R value of 0.1 and 0.3. In order to find out the value of ΔK, ΔK-inceasing test method was applied by the standard fatigue testing method describes in ASTM E647-81. From the result, the stable crack propagation region of 7017, 7020, 7039 and 5083 alluminum welded alloy was applicable to the Paris equation da/dN = C(ΔK)m (C and m are material constants). Constant m decreases and constant C increases with increasing of stress ratio R. Without dependence on materials, the fatigue life and the propagation rate increase, while the gradients of crack propagation rate decrease, whith increasing stress ratio. Fatigue life was observed roughly in the order of 7020 > 5083 > 7017 > 7039, independently on crack propagation direction and stress ratio. The fatigue life is longer when the crack propagation direction of specimen and welded bead are parallel to each other than transverse. The crack propagation rate and the gradient of it are larger in the case of parallel specimen than transverse. The minimum crack length of fatigue life can be estimated through the knowledge of maximum equivalent crack length obtained by using nondestructive test.

        대한 응급의학회지 논문의 분석 및 고찰

        To raise the quality of research, medical articles which are feasible and objective must be continuously published and be continuously monitored. All articles published in the Journal of the Korean Society of Emergency from November, 1990 to November, 1995 were compared with a foreign emergency medical journal, the American Journal of Emergency Medicine(AJE). Classifying the articles in the Journal of the Korean Society of Emergency Medicine, there were 15 (8.4%) review articles, 145 (81.0%) original articles and 19 (10.6%) case reports. In the AJE, there were 103 (15.1%) review articles, 304 (44.4%) original articles and 277 (40.5%) case reports. Classifying the original articles in the Journal of the Korean Society of Emergency Medicine, There were 13 (9.0%) experimental articles, 88 (60.7%) descriptive articles and 44 (30.3%) analytic articles. In the AJE, there were 40 (13.2%) experimental articles, 59 (19.4%) descriptive articles and 205 (67.4%) analytic articles. The classification of original articles according to frequency in the Journal of the Korean Society of Emergency Medicine is trauma, resuscitation, toxicology and medical emergency, where as the AJE is medical emergency, trauma, toxicology and EMS. For statistical methods used in original articles, 90 (62.1%) case did not do any statistical analysis or where only descriptive statistics were used. In 6 (4.1%) cases the statistical level was described but statistical methods were nonmentioned. By simple calssificaton study, there were 44 (30.3%) cases of original articles in the Journal of the Korean Society of Emergency Medicine, and 6 (2.0%) cases in the AJE. Because the development of Emergency Medicine as a special department is based on logical articles, organization of articles have study design, make hypothesis, sample gathering, statistical analysis and feasibility of conclusion. For this, the emergency training course needs a course on research methodology.
