      • Diltazem에 의한 계단형상에 관한 연구

        Effects of ditiazem on staircase phenomenon were investigated in rabbit papillary mucles partially depolarized by 19mM K-Tyrode solution. The following results were obtained: 1. Contractile forces were increased in paralle with the increase of stimulation frequencies from 0.2㎐ to 2㎐ in the muscle preparation partially depolarized around -50㎷ by 19mM K-Tyrode solution. 2. Contractile forces induced by various frequencies of electrical stimulation were markedly changed in high Ca^2+ solution, while those were not remarkable in low Ca^2+ solution. 3. The relationship between developed tension and stimulation frequency were negatively inverted in low concentration of diltiazem(0.05 and 0.1㎎/l). 4. Amplitudes of action potentials were reduced in administration of low calcium and ditiazem which affected calcium flux during action potential. 5. Durations of action potential were prolonged in low calcium solution and those were shortened in low concentration of ditiazem. 6. Amplitudes of action potential in ditiazem-treated 19mM K-Tyrode solution were continuously reduced by change in stimulation frequencies from low to high rate. Above results suggests that amplitudes of action potential closely related with contractility of rabbit papillary muscle in 19mM K-Tyrode solution and decreased influx of calcium during action potential could be related with nagatives staircase phenomenon.

        내시경을 이용한 관골궁골절 정복술

        Although the zygomatic arch can be employed as a key landmark to the accurate alignment of a displaced zygoma fracture, it has been traditionally avoided because of the need for a bicoronal incision. Exposure of the zygomatic arch by means of the conventional bicoronal incision has several possible disadvantages, including an increased risk of blood loss, alopecia, loss of sensation posterior to the incision, and traction palsy of the facial nerve. Endoscopic-assisted exposure of a zygomatic arch can largely obviate the disadvantages of a bicoronal incision and yield this site accessible to reduction and internal fixation in the routine treatment of displaced zygoma fractures. 9 cases of endoscope-assisted open reduction of zygomatic arch fracture and 3 cases of internal fixation of a moderately displaced zygoma fracture are presented. To accomplish this technique, a rigid 4 mm, 30 degree down-angled endoscope, trocar and cannula, endoscopic forehead lift instrument were used. The postoperative courses were satisfactory with few complications. The use of endoscope in the treatment of zygoma reduction provided an expanded field of vision, direct manipulation of lesions, minimal postoperative scar. In particular, exposure and fixation of the zygomatic arch were performed without the need for a bicoronal incision.

        Dermatomyositis is characterized by progressive, symmetric, Proximal muscle weakness and a nonsuppurative inflammatory myopathy of unknown etiology involving predominantly skeletal muscles. It is also characterized by typical skin lesions. Interstitial lung disease has a poor prognosis when it is associated with dermatomyositis. Organizing pneumonia is a disease in which granulation tissue fills the lumina of terminal and respiratory bronchioles and extends into the distal airspaces. The cryptogenic nature of the process is appreciated in that organizing pneumonia patterns of injury can be seen in secondary forms of the disease (secondary organizing pneumonia). Organizin pneumonia has been reported to occur in 5~10% in dermatomyositis-polymyositis patients Anti-histidyl tRNA synthetase antibody (anti-Jo-1) is a Predictive disease marker that is reported to occur in up to 70% of patients. We describe a 49-year-old male dermatomyosits patient who presented with organizing pneumonia and was found to have negative anti-Jo-1 antibody.

        The twisting and extrusion process of the product with involute helical fin from the round billet is developed by the upper bound analysis. The twisting of extruded product is caused by the twisted die surface connecting the die entrance section and the die exit section linearly. In the analysis, the internal shear surface is defined as the curved twisted plane from the taisting of die surface and the shear work is calculated by the consumption of shear energy The increase rate of angular velocity is determined by the minimization of plastic work. The results of the analysis. It is shown that the angular velocity of the extruded product increases with the die twisting angle, the reduction of area, and decreases with the die length, the friction condition.

      • 신생아에 심한 심부전증을 일으킨 선천성 뇌동정맥 기형: 풍선 색전 요법을 시도한 1례

        Cerebral arteriovenous malformation is a rare cause of congestive heart failure in neonates and is known to carry a very poor prognosis even wish prompt recognition and aggressive treatment. We recently encountered a 2-day-old male neonate with huge cerebral arteriovenous malformation who presented with intractable heart failure. Despite aggressive medical treatment and partial embolization with detachable balloon, baby died of severe hearts failure. Since there is no report of percutaneous balloon embolization in a neonate with this lesion in Korean literature, we wish to present our experience in one such neonate. Also we believe this is the first report of color Doppler findings in cerebral arteriovenous malformation in Korean literature.

        사비의 교정에 있어서 내시경의 이용

        Owing to the complexity of the deviated structures and the septal deformities accompanying the external nasal deformities, the complete correction of the deviated nose is regarded as a difficult one. The approach to the nasal bone and osteotomy in classic corrective rhinoplasty is almost blind technique, where the results depends on the feeling by surgeon's hand. To overcome these drawback, endoscopic-assisted corrective rhinoplasty and septoplasty performed for 8 cases of deviated nose between January 1996 and May 1997. Average follow-up period was 10 months. All patients were evaluated by symmetrical nasal pyramid, recurrence of the bony deflection and septal deviation. The postoperative courses were satisfactory in most of cases with few complications. It appears that endoscopic control during corrective rhinoplasty and septoplasty is a big step toward obtaining better results in bony and cartilage resection with extreme precision under monitor control and magnification. This technique is not an open approach but permits one to see more of the nasal skeleton and bony septum what is causing deformity, and the immediate effect of the corrective measures used. The use of endoscope in corrective rhinoplasty will provide expanded field of vision, direct manipulation of lesions, and better aesthetic an6 functional results.

      • 활성탄소섬유의 제조에 관한 연구

        Polyacrylonitrile(PAN) 섬유를 이용하여 활성탄소섬유(Activated Carbon Fibre: ACF)를 제조하였다. 원료를 먼저 탄화시킨 후에, 이산화탄소 분위기에서 수행되는 물리적인 활성화 방법을 사용하였다. 활성화 온도는 850∼l,100℃로 선택하였다. 활성화 반응의 반응성은 Burn-off율을 계산하여 구하였으며, 생성된 ACF의 기공 특성은 액체 질소 흡착/탈착 실험을 수행하여 측정하였다. 활성화 온도가 1000℃ 이하인 경우에, 제조된 활성탄소섬유의 평균 기공 크기는 약 23 Å이었으며, Burn-off와 비표면적은 활성화 온도 및 활성화 시간에 따라서 각각 선형적으로 증가하였다. 이에 반하여, 활성화 온도가 1,000℃ 이상인 경우에는, 활성화 온도 및 활성화 시간에 따라서 Burn-off는 증가하지만 비표면적은 오히려 감소하였다. Polyacrylonitrile(PAN) fibre was used to prepare the activated carbon fibre(ACF). The PAN fibre was carbonized first, then the fibre was activated physically by using carbon dioxide at 850∼l,100℃. The reactivity of the fibre was determined in terms of the burn-off of the fibre and the pore characteristics of the prepared ACF were determined by the liquid nitrogen adsorption/desorption experiment. When the activation temperature was lower than 1,000℃, the mean pore size of the prepared ACF was 23 Å and the burn-off and the specific surface area of the prepared ACF were increased linearly with the activation temperature. On the contrary, when the activation temperature was higher than 1,000 ℃. the burn-off was increased linearly, but the specific surface was rather decreased with the activation temperature.

      • 광촉매반응에서 본 메탄의 전환반응

        가장 효율적인 담체는 티타니아(TiO_2)이므로 아나타제 타입(anatase type)의 담체로된 광촉매만으로 실험결과를 비교하였다. 500℃에서 2시간 동안 부분산화시킨 촉매계의 경우, Pd, Pt, Cu, 및 Ag 금속만이 0.2%의 전환율을 보였고, 각각의 반응에서 나타난 결과를 비교한 바, Pd 금속이 가장 효과적임을 알았다. 또, 300℃와 500℃에서 2시간 동안 수소기류에서 환원시킨 촉매계의 경우, 에틸렌의 수율과 선택도는 부분산화촉매계와 비교하여 감소되었다. 이는 에틸렌의 수율과 선택도가 금속의 하전상태(metal valence state)에 의존함을뜻한다고 생각된다. 그리고 500℃에서 공기기류로 완전산화시킨 촉매계의 경우, Pt는 C_3생성에서 상대적이었고, 에틸렌의 생성에서는 Pd가 가장 효과적이었다. 결론적으로, 완전산화상태로 소성된 Pd/TiO_2 광촉매계가 광반응에 의한 메탈전환반응에서 적합함을 알 수 있었다.
