Purpose: The purpose of this study was to compare the effectiveness of case management in elderly medicaid beneficiaries according to geographic location. Methods: Data were collected from 23,633 elderly medicaid beneficiaries assigned from over users of medicaid. We used the need assessment tool developed by the government, which consists of 19 items with four subscales. Results: Among elderly medicaid beneficiaries, statistically significant differences in effectiveness of case management were observed for quality of life, self-care competency, medical care utility, and support system. Differences in case management effectiveness were higher in urban areas than in metropolitan or rural areas. Conclusion: The differentiated and tailored intervention model based on characteristics of participants, resource distribution, and geographic location may be needed for effective case management for elderly medicaid beneficiaries.

Objectives:The purpose of this study was to examine the association of exposures to heavy metals with positive diagnosis for attention-deficit hyperactivity disorder (ADHD) and Tourette’s syndrome (TS). Methods:Study participants included 27 children diagnosed with ADHD (9.9±2.9 years of age), 21 diagnosed with Tourette’s disorder (10.7±2.2 years of age), and 45 normal control children (9.6±0.5 years of age). A Perkin-Elmer mass spectrometer was used to measure the concentrations of 5 heavy metals (Pb, Cd, U, Be, Hg) in hair samples obtained from each participant. Each heavy metal concentration was compared among the groups by use of a Kruskal-Wallis test. Results:The levels of lead (p=.006) and cadmium (p=.037) observed in the hair of children diagnosed with ADHD were significantly higher than those found in the control subjects. There were no significant differences observed for heavy metal levelswhen comparing TS and control subjects. Conclusion:We confirmed that lead exposure is a risk factor for ADHD. We also identified that cadmium may be a new candidate risk factor for manifestation of ADHD. We did not find an association between heavy metals and manifestation of TS.

Experimental Dermatology 2010; <B>19</B>: e258–e264.</P><P>Abstract: </P><P>Vitamin C is used as an anti-ageing agent because of its collagen enhancing effects. The precise cellular signalling mechanism of vitamin C is not well known. Here, we investigate the profibrotic mechanism of vitamin C against LL-37. Antimicrobial peptide LL-37 decreases collagen expression at mRNA and protein levels in human dermal fibroblasts (HDFs). The ability of LL-37 to inhibit collagen expression is dependent on phosphorylation of extracellular signal-regulated kinase (ERK). HDFs and human keloid fibroblasts were treated with vitamin C followed by 2 h of LL-37 treatment. Collagen mRNA expression and total soluble collagen production inhibited by LL-37 was enhanced by treatment with 0.5 m<SMALL>M</SMALL> vitamin C. Vitamin C also decreased intracellular reactive oxygen intermediates (ROI) levels that were increased by LL-37. Furthermore, the phosphorylation of ERK was analysed by Western blot following treatment with vitamin C and LL-37. Vitamin C turned off phosphorylation of ERK that was induced by LL-37. Ets-1 transcriptional factor, which is involved in the regulation of collagen expression by LL-37, was also inhibited by vitamin C. This study shows that vitamin C enhances collagen production by inhibiting the ERK pathway induced by LL-37.</P>

The main purpose of this study is to identify nursing needs of parents who have hospitalized child. Research design is a descriptive survey. The subjects for the study were 79 parents who have hospitalized child at a pediatric ward of one of the general hospital attached to a university in J city. The data was collected by researchers using a structured questionnaire. The questionnaire was Likert type 5 point scale, composed of 5 categories with 61 items. The data was analyzed by SPSS/PC. The results of the study were as follows ; 1) Mean score of nursing needs of subjects were 3.93 at 2nd day of admission and 3.99 at 7th day of admission. 2) Among the categories, the highest nursing need at the End day and 7th day of admission was ‘dilect nursing’, in desending order, ‘facilities and environment’, ‘education and counseling’, ‘nursing assessment’. The lowest nursing need was ‘reference’. 3) Differences between the nursing needs of subjects at 2nd day and 7th day were as follows : (1) By categories, there was a significant difference only in the ‘reference’ categoly(t=2.74, P=.008). (2) By items, there were significant differences in items of ‘to check necessary materials(t=2.31, P=.024)’, ‘to understand family function and family relationship(t=2.12, P=.041)’, ‘to set up study room(t=2.22, P=.030)’, and ‘to mediate parent's meeting group(t=3.89, P=.000)’. The above result indicated that nursing needs of parents with hospitalized child were above average, especially very high in items about disease process, and items directly associated with treatment and nursing care. So, nurses have to focus on information about the patient's state of disease, treatment, test and procedure, and in efficiently giving direct nursing care to implement mure effective care for the hospitalized children and their parents. And some future researche is needed to identify the difference of degree of nursing needs of parents with hospitalized child according to admission duration using a different sample and a longer sampling interval.

Predicting energy expenditure (EE) is important to prevent work-related musculoskeletal disorders (WMSDs). The problem to predict EE is that the standard of EE is based on western data. The authors checked average EE by job categories to provide basic data for suggesting proper work intensity for Korean workers. This study was conducted from 2003 to 2005. Study subjects were recruited from 4 car parts assembly plant, 2 car assembly plant, 2 Heavy machine manufacturing plant and 2 shipyards. Total study subjects were 515 male workers. To estimate VO2max, sub-maximal test was conducted to measure VO275%max by bicycle ergometer (Combi Co, Aerobike 75XL II). Heartbeats were recorded with heartbeat recorder (Polar Electro Co, Finland, S810) during work. EE of work was calculated by recorded heartbeat and individual regression equation which was derived from sub-maximal test. Subjects were classified into 4 industry and 8 work posture, 23 job task categories. Mean EEs (S.D.) according to industry classification (kcal/min) were 4.9 (0.7), 4.8 (0.7), 4.9 (0.7), 5.0 (0.9), and 4.0 (0.5) for Car Part manufacture, Car Assembly, Ship Building, Heavy Machinery Manufacture, and Hospital Office, respectively. The results suggest that Korean male workers of exceeding to the NIOSH criteria will be needed to plan for job rescheduling to maintain worker°Øs health. Further study to establish Korean work intensity standard would be needed.
