        Purpose: The grade of complexity in the diagnosis related group (DRG) payment system is influenced by the secondary diagnosis of specific complication and comorbidity level, in which moderate or severe malnutrition is included. This study examined an existing proportion of patients with malnutrition who were supposed to be qualified for the complexity level and devised quality improvement measures to increase the proportion of qualifying complexity payments. Methods: The goal of the activities was to increase the rate of complexity payment claims for patients with malnutrition (%). Cases ineligible for the DRG payment system and cases with no diagnosis of malnutrition were excluded. We established a collaborative system between the nutrition support team and departments related to each improvement factor (i.e., patient care, medical records, insurance review, and medical information). Results: Before implementing the activities, this study investigated the current level of complexity payment claims for malnutrition patients who were discharged within a specific period (June 1, 2015∼August 31, 2015). The results showed that complexity payment claims were filed in 10.00% (2 of the 20 malnutrition cases). After the activities, the rate of complexity payment claims for the patients with malnutrition within the study period (June 1, 2016∼August 31, 2016) was 46.43% (26 out of 56), showing an approximately 364% increase from the pre activity rate. This change was statistically significant according to the chi-square test on Microsoft Excel 2010 (P<0.01). Conclusion: Collaborative efforts by the related departments enabled the smooth implementation of each activity. In addition, moderate or severe malnutrition was revealed to be a variable in the complexity-specific payment system. In the future, hospital-wide awareness and effort are crucial to promot the steady practice of these activities and expand their implementation.

        화상으로 인한 소구증 환자의 치험례

        Deep burns of the face & lips often lead to scarring and contraction of the circum-oral tissues with a marked reduction in the size of the oral aperture. Such burns most commonly caused by electrical contact, for example, children sucking electric plugs, or as a result of chemical burns and the exposture to flame. Once having burns, burned tissue may contract and reduce considerably the size and mobility of the mouth ; therefore bring up the resultant functional disturbance, verbal difficulty, even disgestive difficulty, and poor appearance, which fall into difficulty in social acceptability, caused by burns. In our department, 2 patients who were complain of functional limitations and esthetic problem owing to scar contracture, were visited, and we treated this microstomia with scar excision, graft and flap technique, and postoperative intensive physical therapy. We obtained relatively favorable results, thus report this cases with literature review.

        구강악안면 영역에서 Tutoplast Dura의 임상적 적용

        Human Dura mater treated by various processes was used to restore small periodontal defects, large bony defects for improvement of new bone formation, and soft tissue defects and replace the disc of TMJ, etc.. Tutoplast Dura is the solvent-preserved Human Dura mater and sterilized by gamma radiation. In our department, Tutoplast Dura was implanted in 32 patients, from 1994, 6 to 1995, 7. We implanted the Tutoplast Dura at 11 various cysts, 6 implantations, 4 fractures, 3 clefts, 2 TMJ disease, 2 maxillary sinusitis, etc.. We performed the retrospective study about the purpose of Tutoplast Dura implantation, postoperative complication and histologic examination of biopsy specimen at implant second surgery.

        태도 및 태도변화 연구에 관한 고찰

        In this rewiew, Allport's definition of attitude was accepted and the structure of attitude was discussed by three components-cognition, affection, behavior. Factors affecting attitude change were analyzed in terms of Hovland's attitude change model. There were communication, message source, target, and surrounding situation factors in his model. Research trends of attitude and attitude change were reviewed and in were reviewed and it was concluded that traditional six areas of attitude research-mere exposure, mathmatical model, group polarization, judgemental effect, attributional process, and source effect have continued and three areas of this research-cognitive response analysis, the attitude-behavior relationship, and the consistancy model are emphasized in recent years. It was also concluded that distinction between central and peripheral routes is a important variabl in this areas.

      • 신라대학교 주변 백양산의 동물상

        To study fauna of Silla University located in Mt. Baek-yang, we conducted six surveys from 29 May 2011 to 28 April 2012. We found five phyla 41 orders 197 species of animals inhabiting in this area: four phyla 27 orders 163 species of invertebrates and in vertebrates one order six species of fishes, three orders nine species of amphibians and reptiles, six orders 14 species of aves, and four orders five species of mammals. Of these species, some Korean endemic species were included: a fish of Rhodeus uyekii in a pond near main gate of this university, a frog of Rana coreana in a creek near a dormitory, and a rabbit of Lepus coreanus and a water deer of Hydropotes inermis in a grass region near a main stadium. A kind of hawk, Falco tinnunculus as a natural monument (animal) of no. 323-8, was observed on a building of medical and life science. A blue frog of Hyla japonica as a variation was found near a spring resort. Therefore, this area is considered an ecologically important area having a high animal diversity.

        A decision support system is introduced for the efficient operation of port container terminals. It consists of ship planning subsystem, yard planning subsystem, resource management subsystem, real-time control subsystem, and simulation subsystem. The system is intended to support the planning process of the planners and the operation process of the administrators in the container terminals. It is assumed that the transfer crances and the yard tractor/trailers are used to handle containers in the marshaling yard. The system is explained in terms of the functions of each subsystem and the data flows among various subsystems.

      • 디지탈 서보 시스템을 위한 속도 관측기의 설계

        Several algorithms for estimating the instanteneous speed based on the incremental optic encoder are discussed in this paper. First and second order velocity observers are introduced along with simple polynomials. On comparing the performance of these techniques, Pulse-counting method and Period-counting method are robust in whole of operating range. First order velocity observer is shown less phase error than those of others. The derived algorithms are independent of the machine parameters and can be implemented easily based on microprocessor.

      • 셀룰로스계 혼화제를 사용한 수중불분리성 콘크리트의 최적 배합

        수중불분리성 콘크리트는 통상적인 수중 콘크리트와는 개념부터 다르다. 콘크리트에 수중불분리성 혼화제를 혼합하면 콘크리트의 점성이 증가되고 수중에서의 재료분리에 대한 저항성이 현저히 증가된다. 이 효과는 수중콘크리트 자체의 신뢰성을 높여줄 뿐만 아니라 타설지역의 환경보존에도 현저한 기여를 하게 된다. 본 연구에서는 셀룰로스계 수중불분리성 혼화제 (수용성 셀룰로스 에틸계)와 고유동화제 (고축합 트리아진계)의 첨가량 변화에 따른 수중불분리성 콘크리트의 제반 물성 변화를 검토하고 비교적 정온 상태의 해상 환경에 적용할 수 있는 수중불분리성 콘크리트의 치적 배합을 결정하였다. Antiwashout underwater concrete is quite different in concept from conventional underwater concrete. By mixing an antiwashout admixture with concrete, the viscosity of the concrete is increased and its resistance to segregation under the washing action of water is enhanced. The effect of this is not only to greatly improve the reliability of concrete placed under water, but it also has remarkable effects on environmental preservation in the construction area. In this study, property tests of antiwashovt underwater concrete have been carried out, as variable add of special admixture, i.e antiwashout underwater agent (Hydroxy Ethyl Cellulose), superplasticizer (High condensate Tri - Azine). We made the antiwashout underwater concrete have enough strength to endure with easy construction. And we have decided the optimal mix proportion far antiwashout underwater concrete under standard state.
