        Hepatocellular carcinoma(HCC)is one of common causes of cancer-related death in Korea where the majority of HCC patients were Hepaitc B virus(HBV)carriers and have cirrhosis. Transarterial chemoembolization(TACE)is commonly applied to the treatment of multinodular HCC in Korea and careful selection of candidate is important for the risk of various side effects. Besides common side effects as fever, nausea, abdominal pain and elevation of liver enzyme, TACE may predispose to hepatic failure, ischemic cholecystitis, pulmonary embolism, cerebral embolism and pneumonitis. In previous studies, some cases of pulmonary and cerebral embolism cases were reported but lipiodol pneumonitis after TACE was rarely reported. A 65-year-old woman with a multinodular HCC associated with HBV infection, was treated with TACE. Seven days after the procedure, nonspecific respiratory symptoms such as dyspnea and dry cough developed. Chest X-ray and chest computed tomography showed diffuse ground glass opacities in whole lung fields, suggestive of lipiodol pneumonitis. After several days of supportive care with steroid administration, radiologic abnormalities and subjective symptoms were much improved, considered that the disease was compatible with lipiodol pneumonitis.

        The Stability Evaluation of Plasma and Urine Metanephrines for Diagnosing Pheochromocytoma and Paraganglioma by LC-MS/MS

        The measurement of metanephrine (MN) and normetanephrine (NMN) in plasma and urine using liquid chromatography-tandem mass spectrometry (LC-MS/MS) is the gold standard for the initial biochemical testing for pheochromocytomas and paragangliomas (PPGLs). Despite their high diagnostic value, there is a lack of consensus on the stability of MN and NMN in clinical specimens. This study aimed to analyze the stability of MN and NMN in both plasma and urine using LC-MS/MS under various conditions. Plasma and urine samples were prepared by spiking them with standard materials of MN and NMN. The samples were pretreated under solid phase extraction and analyzed using the LC-MS/MS system (Agilent Technologies, USA). In both plasma and urine specimens, MN and NMN were stable under the following conditions: (1) room temperature for 24 hours, (2) 4℃ for 8 days, (3) –20℃ for 8 days, and (4) –80℃ for 7 months. MN and NMN were stable after at least three cycles of the freeze and thaw process, and stable for 7 days in a container with dry ice. In this study, we comprehensively evaluated the stability of MN and NMN in plasma and urine and made recommendations to improve specimen handling to ensure reliable testing for PPGLs.

        Neuroprotective Effects of a Butanol Fraction of Rosa hybrida Petals in a Middle Cerebral Artery Occlusion Model

        The neuroprotective effects of a butanol fraction of white rose petal extract (WRPE-BF) were investigated in a middle cerebral artery occlusion (MCAO) model. Seven week-old male rats were orally administered WRPE-BF for 2 weeks and subjected to MCAO for 2 h, followed by reperfusion. Twenty-four h later, MCAO-induced behavioral dysfunctions were markedly improved in a dose-dependent manner by pretreatment with WRPE-BF. Moreover, higher dose of WRPE-BF not only decreased infarction area but also effectively reduced astrogliosis. The expression of inducible nitric oxide synthase, cyclooxygenase-2, and glial fi brillary acidic protein in MCAO model were markedly inhibited by WRPE-BF treatment. Notably, WRPE-BF decreased nitric oxide and malondialdehyde levels in the striatum and subventricular zone of stroke-challenged brains. These data suggested that WRPE-BF may exert its neuroprotective effects via anti-oxidative and anti-infl ammatory activities against ischemia-reperfusion brain injury and could be a good candidate as a therapeutic target for ischemic stroke.

        표준화된 의사 소통 도구와 시뮬레이션 방식을 사용한 구급대원 의사 소통 교육과정의 효과

        Background : Since primary emergency treatment should be performed appropriately and promptly, efficient and accurate communication between paramedics and medical staff is paramount to a successful primary emergency treatment and patient handover. The problem of the training program in Korea is that it concentrates more on in-class lectures, often delivered by non-medical specialists, who may lack in practical experience and without proper communication training. To solve this problem, we have devised a simulation based training that focuses on event debriefings and two-way communication. Methods : 62 paramedics from 3 stations enrolled in the study. 4 different courses with different emergency situations were created and each course was taken twice resulting in a total of 8 classes. All courses were based on actual cases. The curriculum consisted of subject lectures with guidelines, skill practice courses, and simulation courses based on hands-on method. In simulation courses, paramedics use standardized check list to communicate with medical specialists. All curriculums except subject lectures include debriefing, which allows free talking with educators comprised of medical specialists. In order to measure the educational impact, all students performed self-assessment through a structured questionnaire before and after the training. Results : Regardless different situations and paramedics’ education level, their performance and communication skills have improved after simulation training course. Paramedics mentioned learning skills in simulation course through communication with medical staffs as the biggest advantage. Conclusion : Receiving the simulation training with standardized communication tools is effective at enhancing the communication between the paramedics and medical staff.

        빵효모 저장성 향상을 위한 동결보호제로서의 투라노스 활용 연구

        본 연구에서는 새로운 자당대체제인 투라노스의 동결건조 과정에서 빵효모로 널리 활용되고 있는 S. cerevisiae에 대한 동결보호 효과를 평가하였다. 빵효모 세포의 생존율은 동결보호제로서 투라노스를 첨가함에 따라 증가되는 것을 확인하였으며, 세포표면의 손상을 동결로부터 보호해주는 것을 규명하였다. 또한, 탈지유가 포함된 투라노스는 냉동보관 시(-20℃), 빵효모의 생존력이 크게 향상되는 것으로 확인되었으며, 이는 빵효모의 유통 과정에서 우수한 보존 효과를 기대할 수 있다. 따라서, 투라노스는 동결보호 능력이 강화된 신개념의 소재로서 활용이 기대된다. In this study, the protective effects of turanose on Saccharomyces cerevisiae (Baker’s yeast) were examined during the freeze-drying process to evaluate the feasibility of utilizing turanose as a novel cryoprotectant. The survival rate of the Baker’s yeast cells improved substantially with a cryoprotective medium containing turanose in a substitution-dependent manner. In accordance with these survival rates, the yeast cell surfaces became smoother as the turanose content increased. Turanose with skim milk maintained the viability of the Baker’s yeast, which improved substantially upon storage at -20℃. Thus, it is thought that turanose will exhibit excellent preservation effects during the distribution of Baker’s yeast. Finally, these results suggest that turanose has the potential to be used as a novel cryoprotectant against various microorganisms.

        Effects of anti-obesity drugs, phentermine and mahuang, on the behavioral patterns in Sprague-Dawley rat model

        According to WHO global estimates from 2008, more than 1.4 billion adults were overweight and among them, over 200 million men and 300 million women were obese. Although the main treatment modalities for overweight and obese individuals remain dieting and physical exercise, the synthetic anti-obesity medications have been increasingly used due to their perceived convenience. Generally, anti-obesity medications are classified as appetite suppressants or fat absorption blockers. In the present study, we examined the adverse side-effects in respect of behavior changes of phentermine and Ephedra sinica (mahuang) that are anti-obesity drugs currently distributed to domestic consumers. Phentermine is mainly classified as an anorexing agent and mahuang a thermogenic agent. Because phentermine and mahuang are considered to display effectiveness through the regulation of nerve system, their potential influences of on behavioral changes were examined employing animal experiments. From the results of experiments testing locomotor activity through the use of treadmill, rota-rod, and open field system, phentermine and mahuang were commonly revealed to induce behavioral changes of rats by reducing a motor ability, an ability to cope with an external stimulus, and a sense of balance or by augmenting wariness or excitement. These adverse effects of phenternime and mahuang in behavioral changes need to be identified in humans and anti-obesity medications such as phentermine and mahuang should be prescribed for only obesity where it is anticipated that the benefits of the treatment outweigh their potential risks.

      • 토양수분, 초본밀도, 동물섭식이 초기 소나무 갱신에 미치는 영향

        본 연구는 2008년 모수림 작업이 시행된 이후 소나무 천연갱신이 잘 이루어지지 않고 있는 강원도 평창 가리왕산 163임반에서 야생동물침입방지용 토양수분, 초본밀도, 야생동물의 섭식이 소나무종자의 발아 및 유묘의 생존에 미치는 영향을 밝히는데 그 목적이 있다. 연구 대상지 내에 중소형포유류 침입 방지 울타리 설치, 배수 처리를 달리한 후 종자를 직파, 이후 제초 빈도(1, 2, 3 회/년)를 달리하여 소나무 종자의 발아율을 조사한 결과, 대부분 6월에 발아하였으며, 제초빈도에 따른 시기별발아율과 울타리 유무에 따른 발아율의 차이가 없었다. 배수처리에 따른 고사율 차이가 없었으나, 시기별 제초 작업에 따른 생존율에는 차이가 있었다. 특히 7월 제초작업을 수행한 이후 고사율이 감소했다. 또한 중소형포유류의 침입방지를 위해 울타리를 설치한 구역에서 고사율이 증가했는데, 이는 울타리 내에 그늘로 증가한 초본류가 영향을 미친 것으로 판단된다. 갱신 초기 소나무 유묘의 고사원인은 병(모잘록병 등), 곤충의 섭식, 설치류 굴, 초본과의 경쟁 등이 주 요인으로 판단된다. This study was conducted to determine the effect of soil moisture, vegetation density, and animal feeding on germination and survival of red pine seedling in Mt. Gariwang, stand #163 where red pine has not been well-regenerated naturally since seed-tree cutting method was implemented in 2008. After sawing red pine seeds in the study site, three treatments including fence, drainage, and weeding frequency (once, twice, and thrice /yr) were applied and germination ratio and survival ratio were monitored. As a result, most of seeds were germinated on June and there was no significant differences monthly among neither weeding frequency nor fence treatments. While there was no differences of survival ratio under drainage treatments, significant difference were shown along the different weeding frequency. Particularly, after weeding on July, mortality of red pine seedlings was decreased. The reason why seedling mortality increased under fencing is expected that herbs increased under shadow affected seedlings. At the initial stage of regeneration, disease, insect attack, rodents holes, and competition with other vegetation were investigated as limiting factors of red pine seedling survival.

        치매 환자의 매선 요법 치료에 관한 치험 3예

        Objectives : The purpose of this study is to evaluate the effects of Needle-Embedding Therapy on dementia patients. Methods : We recruited 3 dementia patients who have been experiencing memory disorder and orientation disorder. The patients had been evaluated with Mini-Mental State Examination-K (MMSE-K), Global Deterioration Scale (GDS) and Clinical Dementia Rating (CDR). The patients were treated with herbal medication (Sunghyangjunggi-san) and acupuncture. After 15 days, the patients were added to Needle-Embedding Therapy. We measured MMSE-K, GDS and CDR for every 15 days. The effects of additional Needle-Embedding Therapy were compared with the effects of acupuncture and herbal medicine. Results : After Needle-Embedding Therapy was added, Patients` memory and orientation have been improved and the score of MMSE-K ascended. The grade of GDS and CDR were maintained or decreased. Conclusions : This study suggests that Needle-Embedding Therapy is significantly effective on Dementia patients.
