        의료서비스에서 환자 및 소비자 참여 – 영국적 상황에서의 비판적 분석

        연구배경: 선진국에서는 공공서비스 관리에 있어 시민참여가 일반적이다. 전국민을 대상으로 무상의료를 제공하는 영국의 경우에는, 그 예산이 일반세금에서 수립되기 때문에 의료소비자의 참여가 더욱 중요한 문제라 할 수 있다. 때문에 영국에서는 의료 정책 수립 및 서비스 제공에 있어 의료소비자들의 참여(Patient Public Involvement: PPI)가 증가하고 있으며 그 필요성 또한 더욱 강조되고 있다. 연구목적: 이 논문은 영국 내 의료환경에서 소비자들이 참여하게 된 정치적 맥락과 가정(assumption)과 한계를 ‘사회과학적’ 시각에서 비판적으로 논의하고자 하였다. 연구방법: 먼저 이 논문은 영국적 의료환경에서 소비자가 참여할 수 있게 된 정치적 맥락을 다루었다. 다음으로 의료서비스 발전을 위한 소비자 참여를 정치적 가정과 경제적 가정을 들어 분석하였다. 논의는 영국에서 발행된 정부정책 계획서 및 보고서들을 토대로 하였다. 연구결과: 이 논문은 의료서비스에 있어서 소비자 참여에 대한 맥락과 한계를 비판적으로 분석함으로써 한국에 비해 의료 정책 수립에 있어 소비자참여가 활발한 영국의 현 상황을 논의하였다. 이 논문에서 영국의 사례를 드는 이유는 환자 및 소비자 참여와 관련된 개념적 정의의 문제나 나타난 문제 및 그러한 문제의 해결방안을 구체적이고 축약적으로 제시함으로써 한국의 의료계나 관련단체가 참조할 수 있도록 하기 위함이다

        간호대학생과 타전공 일반대학생의 죽음에 대한 태도 및 죽음의 의미에 관한 비교

        The study was to investigate the attitude toward death and meaning of death of nursing students and non-nursing major students. This study was used a descriptive comparative designed. Data were collected universities in J city in from June. 1 to June. 30 in 2014. After receiving the agreement, 150 nursing students and 117 non-nursing major students were recruited. They were asked to fill in a self-reported questionnaire about the attitude toward death and meaning of death. The data were analyzed using descriptive statistics, t-test, ANOVA and pearson correlation. This study compared the other major university student study the results of the meaning of death than nursing students is much lower, but no significant differences in the attitude toward death. There was a significant correlation between the a death anxiety (death of others & dying of self) and meanings of death (negative) in nursing student and the other major university student. On the other hand, there was a significant negative correlation between the a death anxiety (death of self & dying of others) and meanings of death (positive) in the other major university student. The finding of the study provide the basis for expanding education related to death for the other major university student. A training course may help nursing student develop their view on the meaning of death, which in turn would enhance their performance in caring dying patients.

      • 의과대학의 의료커뮤니케이션 교육과정의 문제점

        The purpose of this study is to identify the problems of the current curriculum for improving medicalcommunication skills in Korean medical schools. After investigation of the state of education of medicalcommunication in curriculum of each medical school some general problems were identified which couldmake education of medical communication difficult. The concrete problems of method and content ofmedical communication curriculum were described. Some suggestions were made to solve the problems.

      • 의과대학 교육과정에서의 의료커뮤니케이션 교과목 구성

        Background: The need of development of medical communication subject is increasing. Recently manymedical schools in Korea are newly developing the curriculum to teach medical students aboutcommunication. The purpose of this study is to develop a standard medical communication subject in amedical school to teach medical students about the knowledge, skills and attitude for effective medicalcommunication. Methods: 5 professionals including 1 professor in family medicine, 1 professor in familymedicine and medical education, 1 professor in linguistics, 1 professor in German, and 1 professor incomputer science collaborated to review the references and survey the need of medical students, medicalteachers, patients, and community people for medical communication education. We developed a standardmedical communication subject by mutual consensus. Results: We developed the purposes of medicalcommunication education, and developed learning subjects of medical communication. We also developedthe tools of teaching medical communication and evaluation method to assess students' performances. Conclusion: The medical communication subject should be tailored to medical students' needs. Experientiallearning for medical communication will be effective. The effect of "hidden curriculum" in a medical schoolshould be considered, and the medical school faculties should be encouraged to continue showing good rolemodels to the medical students.

        의학저널리즘의 의료커뮤니케이션적 기능과 역할, 향후 과제

        As a patient has recently been recognized as not a passive object but a subject of medical services, various attempts are being made to strengthen the status of a patient. Medical communication which has been led by a doctor so far is being made with a focus on a patient due to sovereignty of a consumer and activation of medical information. The purpose of this paper is to investigate the process to strengthen the status of a patient as a consumer of health care service through a patient association as medical information becomes public. In order to achieve this goal, we would like to deal with the roles of media which may affect the communication between a doctor and a patient. Accuracy and objectivity are vital in the medical news requiring high professionalism. Journalists who collect news in the medical field dealing with people’s lives can prevent the side effects caused by the articles which should accurately identify the fact and meaning. The Media environment is being eaten into by the commercial viability, but the medical field can restore the trust through self reflection thinking of the basis by itself.

        국내 의과대학 의료인문학 교육현황 분석 및 교육체계 제언

        The educational development in Korean Medical School has shown remarkable results in about 20 years. These institutes has an excellent evaluation base and educational independence. And the curriculum, the method, and the evaluation in medical education have been innovated over recent 20 years. Those revolutionary outcome of the innovation of medical education affirms possibilities and potentialities of innovation and reformation in the future. In response, this article attempted to reflect on the deficiencies of medical education through the analysis of the educational status of medical humanities in 40 medical schools for better future medical humanities education under such a strong environment. Therefore, the purpose of this article is to present the direction in which medical humanities literature based on value education. In this process, in order to find the essence of medical education through educational philosophy discourse, the blue print of evidence based medicine is presented, and the role and direction of medical humanities are suggested through reflection on strengthening medical education capabilities.

        응급의료센터 이용설명서를 포함한 시스템 구축이 환자의 응급의료서비스 만족도에 미치는 효과 연구

        Background : The purpose of this study was to determine the effect of emergency medical service system development including service and process manual on patients satisfaction with emergency medical service. Methods : This study used simulated control group pre-posttest design and included 322 of pre- and 227 of post-construction for system in emergency centers (EC) of university hospitals located in Daegu. Data were collected with a self-administered questionnaire for one month pre-post construction for the system. Construction for the system included development of emergency medical center manual, computerization of EC patient-bed number, compartmentalization and assignment of Nurse System in the EC, providing education for compassion care for all EC staff, improving direction signs for the department of administration, and emergency pharmacies. Data analysis was conducted using descriptive statistics. Results : There was a statistically significant difference in nurse kindness between the experimental group of 12.38 and the control group of 11.82 (p = .011). Satisfaction was also improved in the facility environment, work procedures, doctor friendliness, medical technology and costs, including overall satisfaction, and waiting time was slightly higher in the experimental group (10.92±1.87) than the control group (10.76±2.03), but there were no statistically significant differences. Conclusion : The result of this study was statistically significant only in the nurse kindness. While the difference was not statistically significant, the experimental group showed improved satisfaction in all areas except latency. This resulted in a limitation in testing the effectiveness of the emergency medical center system because the waiting time was higher in the experimental group due to the strike and construction going on at two nearby university hospitals. Therefore, since waiting time is an important factor in the satisfaction of EC service, interventional studies are needed to reduce waiting time.

        응급구조사 양성 교육과정에서 토론을 바탕으로 한 팀기반학습의 만족도

        Background: Team-based learning is known for its effective and satisfying education methods in the study of various medical schools. This study was prepared to confirm the satisfaction of applying this team-based learning to the training course of emergency medical technician. Methods: A total of 165 new members of the course of second grade National Emergency medical technician at 2019 were finally enrolled in the study. Data were collected with a self-administered questionnaire. The team-based learning method was organized with eight students per group to conduct pre-learning and then proceed to open book to encourage active discussion among individuals.The questionnaire consisted of questions about the satisfaction of the discussion process, such as whether the knowledge gained from the discussion was appropriate, and whether collaboration between colleagues was successful during the discussion, and about the benefits of team-based learning, how well knowledge transfer compared to existing lectures, and the replacement of existing lectures. The questionnaire used a Likert 5 point scale, and the data obtained were analyzed using the SPSS version 22.0. Results: The overall rating of team-based learning was 7.8 ± 1.5 out of 10. Many students answered positively (88.5%) for benefits, while 87.9% responded positively to whether the knowledge transfer effect was better compared to existing lectures. In addition, the satisfaction of team-based learning was generally high, with more than 90 percent of education students giving positive answers to the replacement of other lectures with team-based learning. Conclusion: The learners gave positive answers to the satisfaction, usefulness and learning effects of team-based learning. Team-based learning is a discussion-based study, so it is a good way to improve communication skills. Since communication and teamwork are important in first aid activities, the characteristics of team-based learning could lead to improvements in first aid services, leading to high-quality emergency medical services. Back ground: 팀기반학습은 여러 의과대학의 연구에서 효과적이고 만족도가 높은 교육 방식으로 알려져 있다. 이러한 토론 위주의 팀기반학습을 응급구조사 양성과정에 적용하였을 때의 그 만족도를 확인하기 위해 이 연구는 계획되었다. Methods: 이 연구는 2019년 신규 임용된 소방공무원의 2급 구조사 교육과정에 있는 교육생을 대상으로 하였다. 수업 시작 전에 연구 목적을 설명하였고, 자발적인 서면 동의를 받았으며 수업 종료 후 설문조사지를 배부하였고 충분한 응답을 한 165명의 교육생 설문을 최종 연구대상으로 선정하였다. 팀기반학습 방법은 그룹당 8명의 교육생이 편성되어 사전학습을 시행한 후 개인 간의 활발한 토론을 유도할 수 있게 오픈북으로 진행하였다. 설문은 토론을 통해 얻게 된 지식이 적절했는지, 토론 도중 동료 간의 협동은 잘 이루어졌는지 등의 토론과정의 만족도를 묻는 질문과 팀기반학습의 유익성, 기존 강의와 비교했을 때 지식전달이 잘 되는지, 기존 강의를 대체하는 것에 대한 질문 등 팀기반학습의 만족도를 묻는 질문으로 구성되었다. 각 문항 측정도구는 Likert 5점 척도로 하였고, 수집된 자료는 SPSS version 22.0를 이용하여 분석하였다. Result: 팀기반학습의 전체적인 평점은 10점 만점에 7.8±1.5점이었다. 유익성에 대해서는 88.5%가 긍정적인 답변을 하였고, 기존 강의와 비교했을 때 지식전달 효과에 대한 응답에서는 87.9%가 긍정적인 답변을 하였다. 다른 강의를 팀기반학습으로 대체하는 것에 대해서도 90% 이상의 교육생이 긍정적인 답변을 하는 등 대체적으로 팀기반학습의 만족도가 높았다. Conclusion: 교육생들은 팀기반학습에 대해 만족도, 유용성, 교육 효과등의 부문에서 긍정적인 평가를 하였다. 팀기반학습은 토론을 기반으로 한 학습이므로 의사소통 기술을 향상시킬 수 있는 좋은 교육방법이다. 구급활동에서 의사소통과 팀워크가 중요하기 때문에 팀기반학습의 특징은 응급구조서비스의 향상으로 이어져 고품질의 응급의료 서비스로 이어질 수 있을 것이다.

Methods:A totalof184nursingstudents(grade1and2)completedthe26-item communicationskillsattitudescale(CSAS)satisfactorilyandatotalof212nursingstudentsansweredtheopenquestionnaireforstudent'sattitudestowardscommunicationskillslearning.Factoranalysiswasconductedtodeterminethefactorsunderpinningthescale.Theinternalconsistencyofthesubscaleswasdeterminedusingcronbach'sα coefficients.SPSS12.0program wasusedforstatisticanalysis. Results:3 factors(「The usefulness& importance ofcommunication skillslearning」,「The negativerecognition ofcommunication skillslearning」,「Theexcessiverecognition ofcommunication capability」)wereextractedasaresultofCSASanalysis,whichwerereflectedwellintheresultofopenquestionnaire. Nursingstudentswereevaluatedtohavehighlypositiveattitudestowardcommunicationskillslearning,especiallyinthefactorof「Theusefulness& importanceofcommunicationskillslearning」. Conclusion:Highly positiveattitudesofnursing studentstowardscommunication skillslearning areexpectedtobecontributedtothedevelopmentofcommunicationskillseducationintheschoolofnursing.
