        Changes in the Diagnostic Efficiency of Thyroid Fine-Needle Aspiration Biopsy during the Era of Increased Thyroid Cancer Screening in Korea

        Purpose The volume of thyroid cancer screening and subsequent thyroid fine-needle aspiration (FNA) have rapidly increased in South Korea. We analyzed the thyroid cancer diagnoses/thyroid FNA ratio according to the annual number of FNA to evaluate changes in the diagnostic efficiency of FNA. Materials and Methods This was a nationwide population-based retrospective cohort study. The overall thyroid cancer diagnoses/thyroid FNA ratio and annual incremental thyroid cancer diagnoses/incremental thyroid FNA ratio were indirectly calculated using data obtained from the Korea Central Cancer Registry database and the Korean National Health Insurance Service claims database from 2004 to 2012. Pearson correlation analyses were performed to evaluate the strength of linear associations between variables. Results The number of thyroid FNA increased from 28,596 to 177,805 (6.2-fold increase) from 2004 to 2012. The overall thyroid cancer diagnoses/thyroid FNA ratio decreased from 36.5% in 2004 to 25.1% in 2012 and was negatively correlated to the number of FNA (R=-0.977, p < 0.001). The annual incremental thyroid cancer diagnoses/incremental thyroid FNA ratios (range, 15.3% to 30.7%) were always lower than the overall thyroid cancer diagnoses/thyroid FNA ratio in each year and also worsened according to the increase in the number of FNA (R=-0.853, p=0.007). Conclusion The diagnostic performance of both overall and annual incremental thyroid FNA worsened, whereas the number of thyroid FNA procedures increased. More sophisticated indications for FNA are required to improve its diagnostic efficiency, considering the increased burden of screening-detected thyroid nodules.

      • 政治的 選擧가 韓國經濟의 巨視變數에 미치는 影響 分析

        This paper attempts to analyze the impact of elections on the Korean macroeconomy. It measures the shock to the Korean economy caused by political elections by election dummies from 8 major elections held in the 1980s-1990s. The four variables' Bayesian Vector Autoregression Model which included the monetary aggregates, consumer price, industrial production and interest rates was based on the monthly time series data. Before the construction of the VAR Model, the Dickey - Fuller test was performed in order to test the stationarity of variables. The stationary test shows every variable has statistical properties which is stationary after first - differencing. The empirical results of this study show that the political elections, in general, affected negatively on the Korean economy. Especially the consumer price increased in the previous month before election, reflecting the expectational effect and the relaxation of government price control before election. M2 decreased in the month before election but the currency and base money increased during the election periods and the increment was partly recalled in the nest month. Industrial production significantly declined during the election month because workers turned into campaigners. But it was tend to increase after the election because of the endeavours to offset the decrease of production during election periods. Due to the increased liquidity interest rates measured by corporated bond yields declined before the month in which the elections were held and significantly declined after election month when the uncertainties of financial market disappeared, which was somewhat contrary to the concern that interest rate would increase during election periods. The result of simulation implies that it is very difficult to maintain stable general price in Korean economy during the election in 1997. Therefore economic manegement in 1997 should place its first priority on stabilizing the growth of the monetary base for the manegement of general price.

        흰쥐 자궁에서 스테로이드호르몬에 의한 c-Fos, CREB, ATF 및 HSP70의 발현에 관한 연구

        Steroid hormone is known to cause the dynamic changes of mammalian uterus during reproductive cycle. However there is little information about the effect of estrogen (E) and progesterone (P) on the expression of various transcription factors involved in gene expression. Thus the present study was designed to demonstrate E and/or P-induced expression of c-Fos, CREB, ATF and HSP70 in rat uterus. Rats, ovariectomized (OVX) for two weeks, were divided into 6 experimental groups, 1) OVX, 2) OVX+V, 3) OVX+E, 4) OVX+P, 5) OVX+E+V, 6) OVX+E+P, and western blotting assay for nuclear extract and immunohistochemical staining were carried out for each experimental group. Treatment of E $(10{\mu}g)$ showed to increase the expression of c-Fos, CREB, ATF, and HSP70, and maximal expression was occured at $3\sim6$ hr after E administration. P (1mg) also increased, but much less than E, the expression of c-Fos, ATF, and HSP70. However, P did not reveal any effect on the expression CREE. P treatment 4 hr after E injection decreased c-Fos, CREB, and ATF expression, but did not show any change in the E-induced HSP70 expression. In immunohistochemical study c-Fos-, CREB-, and ATF-immunoreactivities were confined to the cells of luminal epithelium of uterine endometrium. These results suggest that proliferation and differentiation of rat uterus during reproductive cycle may mediated via expression of transcription factors, such as c-Fos, CREB, ATF, and HSP70.

        치과용 금합금에 대한 금속 프라이머 처리와 열순환 처리가 교정용 브라켓의 전단결합강도에 미치는 영향

        치과용 금합금에 교정용 브라켓 접착 시 금속 프라이머와 열순환 처리가 전단결합강도에 미치는 영향을 알아보기 위하여 80개의 치과용 금합금 시편을 샌드블라스팅만 처리한 군과 3종류의 금속 프라이머(Alloy Primer, Metaltite, V-Primer)처리를 시행한 군으로 분류하고, 이를 열순환 처리 시행 여부에 따라 모두 8개 군으로 분류하였다. 만능물성 시험기를 사용하여 브라켓의 전단결합강도를 측정하고, modified ARI (Adhesive Remnant Index) scores를 통해 브라켓 접착면의 파절 양상을 평가하였다. 열순환 처리 미시행 시에는 금속 프라이머의 적용 시 샌드블라스팅만 단독으로 처리한 경우에 비하여 통계적으로 유의성 있는 전단결합강도의 증가가 있었다 (p < 0.05). 열순환 처리를 시행한군에는 금속 프라이머의 처리에 의한 전단결합강도의 변화는 유의한 차이가 없었다 (p > 0.05). 금속 프라이머를 적용한 경우 열순환 처리 미시행 시에는 파절 양상이 브라켓과 접착제 계면에서의 파절 발생 빈도가 높았으나, 열순환 처리 시행 시에는 각 군 간 파절 양상에 유의한 차이가 없었다. 이상의 결과로 금속 프라이머의 적용이 치과용 금합금에 대한 교정용 브라켓 접착 시 초기 접착 강도에서는 유의성 있는 결합력의 증가를 보이나, 열순환 처리 시행 후에는 결합 강도에서는 유의한 차이가 없는 것으로 나타나 금속 프라이머에 의한 결합력 증가가 감소하는 것으로 생각한다. Objective: The aim of this study was to evaluate the effect of metal primers and thermocycling on shear bond strength between the orthodontic bracket and gold alloy. Methods: For this study, 80 specimens made of dental gold alloy were divided into 8 groups based on the combination of metal primers (none, Alloy primer, Metaltite, V-primer) and thermocycling (with and without thermocycling). Shear bond strength testing was performed with a universal testing machine. Bond failure sites were classified by a modified ARI (Adhesive Remnant Index) score. Results: All metal primer treated groups showed a significantly higher shear bond strength than the only sandblasting treated group without thermocycling (p < 0.05). There were no significant differences on shear bond strength in the groups with thermocycling (p > 0.05). Bond failure sites of the metal primer treated group without thermocycling occurred at gold alloy/adhesive interface, whereas there were no differences on bonding failure sites in the groups with thermocycling. Conclusions: These findings suggest that using metal primer on gold alloy enhances the initial bracket bond strength. But, this effect was not shown with thermocycling.

      • (100)Si 기판상에 2원증착법으로 성장된 Al-Y 박막의 특성

        본 논문에서는 2원증착법으로 성장된 Al-Y 2원 합금 박막의 특성을 비저항(resistivity)과 박막 표면상의 평활도(surface roughness)의 관점에서 고찰하였다. Al-Y 박막의 비저항은 Y 농도가 증가함에 따라 거의 직선적으로 증가(1.7 μΩcm/wt.%)하였으나, 진공열처리에 의하여 급격하게 감소하였다. 특히, 300℃ 이상에서 열처리한 Al-0.7wt.%Y 박막의 비저항은 순수 Al 박막(2.7 μΩcm)과 거의 동일한 2.9 μΩcm이었고, 표면 평활도 또한 0.7wt.% Y 첨가의 결과로 결정립이 미세화되어 박막의 항복강도가 증가하여 현저하게 개선되었다. In this paper, the annealing characteristics of Al-Y alloy films formed by double-source evaporation method were studied with respect to electrical resistivity and surface roughness. Electrical resistivity increases linearly with increasing Y concentration, that is, about 1.7 μΩcm/wt.%, but it decreases by annealing. Particularly, in Al-0.7 wt.% Y alloy film annealed above 300 ℃, it decreases to a similar to that of pure Al film, that is, 2.9 μΩcm. The same concentration of Y is also sufficient to minimize the generation and growth of annealing hillocks.
