        Cisplatin의 투여 후 사구체여과율 및 신혈류량의 변화

        N/A While cisplatin has been widely used in the treatment of a variety of cancers, nephrotoxicity is one of the major problems which frequently limit clinical usefulness of cisplatin. This study has been conducted to investigate nephrotoxicity of cisplatin in terms of changes in glomerular filtration rate (GFR) and effective renal plasma flow (EFPF) measured by the simultaneous use of Tc-99m-DTPP and-131I-OIH, before and after administration of cisplatin, in 12 patients with lung cancer and four patients with esophageal cancer. Cisplatin was administrated at total doses of 75∼100 mg/㎡ with two hour hydration and diuresis method. GFR determined by the use of Tc- 99m-DTPA had a good correlation with 24-hour creatinine clearance rate (r=0.77, p〈0.001). GFR and filtration fraction decreased immediately after administration of cisplatin, however, they showed a tendency to be in completely recovered four weeks after administration. ERPF was not changed immediately after and four weeks after administration of cisplatin. GFR before and immediately after administration of cisplatin were analyzed with regard to age, sex, performance status, previous adminstration of cisplatin and method of administration. None of these factors had any influence on the rate of decrease in GFR except method of administration. Administration of cisplatin as a single dose lowered GFR more compared with that as divided doses. In this study, we have also demonstrated that the simultaneous use of Tc-99m-DTPA and 131I-OIH was a useful tool for the measurement of GFR and ERPF respectively.

        Liposomes의 음성염색에 의한 전자현미경적 관찰

        To investigate the research of the tumor seeker using liposomes with negatively stained. The fine structure and size in liposomes composed of PC, DSPC, DAPC and SM phospholipid were observed. And the yield of vesicle affected with sonication by mechanical force were determined. The results were as follows. 1. The stain of 2% UA are obtained a good resolution from electron microscopic observation to compare the negatively liposomes with PTA, AM and UA solution. 2. The fine structure of liposomes except DAPC, PC and CH alone samples could be observed from EM experiment of liposomes composed of PC, DSPC, DAPC and SM phospholipid. 3. The results of experiment from 10, 20, 40 and 60 times sonication with 30sec, prepared 0.1mM NTA-SM-liposomes were obtained unilamella vesicles from groups of multilamella vesicles of phospholipid. 4. About 50nm diameter liposomes obtained through membration filtration step to prepare homogenized liposomes.

        단일광자방출단층촬영 영상에서 산란 및 감쇠 보정에 의한 절대방사능 측정

        Purpose: The pvrpose of this study was to evaluate the accuracy of radioactivity quantitation in Tc-99m SPECT by using combined scatter and attenuation correction. Materials and Methods: A cylindrical phantom which simulates tumors (T) and normal tissue (B) was filled with varying activity ratios of Tc-99m. We acquired emission scans of the phantom using a three-headed SPECT system (Trionix, Inc.) witb two energy windows (photopeak window: 126 154 keV and scatter window: 101 123 keV). We performed the scatter correction with dual-energy window subtraction method (k=0.4) and Chang attenuation correction. Three sets of SPECT images were reconstructed using combined scatter and attenuation correction (SC+AC', attenuation correction (AC) and without any correction (NONE). We compared T/B ratio, irnage contrast [(T-B)/(T+B)) and absolute radioactivity with true values. Results: SC+AC images had the highest mean values of T/B ratios. Image contrast was 0.92 in SC+AC, which was close to the true value of 1, and higher than AC (0.77) or NONE (0.80). Errors of true activity by SPECT images ranged from 1 to 11% for SC+AC, 22-47% for AC, and 2 16% for NONE in a phantom which was located 2.4cm from the phantom surface. In a phantom located 10,0cm from the surface, SC+AC underestimated by ?4%, NON.E 40%. However, AC overestimated by 10%. Conclusion: We conclude that accurate SPECT activity quantitation of Tc-99m distribution can be achieved by dual window scatter correc.tion combined with attenuation correction.         림포신티그래피용 99mTc 표지황화안티몬 콜로이드 및 전분의 제조에 관한 연구

        N/A For the development of Tc-99m-labelled antimony sulfide colloid and hydroxyethyl starch, various experiments such as preparation of colloid, control of the distribution of particle size, establishment of labelling conditions, determination of labelling yield and radiochemical purity, examination of stability, and organ imagings of rabbits etc. were carried out. 1) Antimony sulfide colloid was readily prepared by the reaction of aqueous solution of antimony potassium tartrate with hydrogen sulfide generated by treating ferrous sulfide with dilute sulfuric acid. The colloid could be stabilized by adding small amount of polyvinylpyrrolidone. 2) Electron microscopy analysis exhibited the distribution of colloid size in the range of 1∼15 nm with a major portion of 9 nm. The colloid solution was sterilized by membrane filtration (0.2 μm) and then stored at 4℃. This sterilized colloid was so stable that it was usable at least for one year. 3) The antimony sulfide colloid was labelled by adding sodium pertechnetate-Tc-99m solution to the reaction vial, followed by adding hydrochloric acid and then boiled for 30 min. The optimal pH of the reaction mixture was found to be in the range of 1.3∼1.4. Instant thin layer chromatography (ITLC) analysis showed high labelling yield of above 99.5%. This labelled colloid maintained high radiochemical purity of above 99% until 10 hours after labelling. 4) Animal studies showed high uptake of Tc-99m-Sb2S3 colloid at lymph vessels and nodes indicating a suitable agent for lymphoscintigraphy. Satisfactory results were also abtained in other clinical studies. 5) Hydroxyethyl starch (HES 0.6∼1.0%) was labelled with Na99mTO4, in the presence of SnCl2 with high labelling yield of above 99.5%. The optimal pH of the reaction mixture was in the range of 1.8∼2.0. Tc-99m-HES maintained high radiochemical purity of above 99% until 10 hours after labelling. 6) Animal studies showed that Tc-99m-HES migrated more rapidly from the injection sites into the lymph vessels than Tc-99m-Sb2S3 colloid while less amount of the former was uptaken at lymph nodes than that of the latter. Similar phenomenon was also observed in other clinical studies. As a result, Tc-99m-Sb2S3 colloid was found to be more effective lymphoscintigraphic agent than Tc-99m-HES.

        99mTc-HIDA 를 이용한 원발성간세포암전이 병소의 진단

        Tc-99m-Sulfur Colloid is concentrated in Kupffer cells of the liver, whereas the new biliary agents such as Tc-99m-HIDA are processed by hepatic parenchymal cells. The distant metastatic lesions in skull and lung of the primary hepatoceullar carcinoma in 38-year old Korean male were detected with Tc-99m-HIDA scintigraphy. The chest PA, skull bone X-ray and radionuclide scintigraphic studies are illustrated. This observation suggests that Tc-99m-HIDA scintigraphy is useful for detection of distant metastases of primary hepatocellular carcinoma.

        Rat Insulinoma 세포에서 Interferon-γ와 Tumor Necrosis Factor 에 의한 Major Histocompatibillity Complex 항원 발현의 조절

        인슐린 의존형 당뇨병 환자의 췌장 B 세포에서는 정상적인 상태에서는 없던 class Ⅱ Major Histocompatibility Complex(MHC) 항원이 발현 유도되어 있고 class Ⅰ MHC 항원의 발현 역시 증가되어 있으며, 이는 interferon-y과 밀접한 관련이 있음이 알려져있다. 그러나 이 현상에 대한 기전이나 면역학적 의미는 확실하게 규명되어 있지 않으며, 이에 관한 연구의 일환으로 immortalized rat insulinoma cell등을 대상으로 MHC항원 발현 조절에 대한 연구가 시도되고 있으나 연구자들간에 일치된 결과를 보이지 못하고 있다. 연자는 immunocytochemistry 법과 flow cytometry 법을 이용하여 RINmSF세포에서 recombinant rat interfron-y (IFN-y)와 recombinant human tumor necorsis factor(TNF)에의한 MHC항원의 발현 조절에 관한 연구를 시행하여 다음의 결과를 얻었다. 1) IFN-r를 가하지 않은 상태에서도 8.4%의 세포에서 rat의 class I MHC항원의 하나인 RT1.A항원이 양성이었으며 1, 10, 100 그리고 1000U/ml의 IFN-r를 가한 경우에는 3일 후에 각각 20.7, 80.7, 86.7%의 세포에서 RT1.A항원이 양성이었다. 2) IFN-r를 가하지 않은 상태에서 rat의 class Ⅱ MHC 항원의 하나인 RT1.B항원 양성인 세포는 없었으며, 1~1000U/ml의 IFN-r를 가하여도 그 항원은 발현되지 않았다. 3) 1, 10 그리고 100 ng/ml의 TNF를 가한 경우에는 각각 7.2, 8.4 그리고 23.1%의 세포에서 RT1.A 항원이 양성이었다. 4) 1~100 ng/ml의 TNF를 단독하여 가하거나 100U/ml의 IFN-r와 병합 투여하여도 RT1.B항원은 발현되지 않았다. 이상의 결과에서 IFN-r는 RINm5F세포에서 존재하지 않던 class Ⅱ MJC 항원을 발현시키지 못하나 기존의 class Ⅰ MHC항원의 발현을 증가시키며, TNF는 고농도에서 class Ⅰ MHC항원의 발현을 증가시킴을 알 수 있었다.

      • 四鹽化炭素投與후 白鼠肝에서 (67)^Ga 攝取率과 DNA 合成과의 關係에 關한 硏究

        The ability of ^(67)Ga, administered carrier free as the citrate complex, to localize in human and animal tumors to an extent sufficient to permit visualization of the lesion by scanning is well established. However, neither the mechanism of ^(67)Ga uptake by tumors or inflammatory cells nor its relationship to cell type or to the biochemical status of the cell is yet understood. Author investigated the uptake of ^(67)Ga-citrate using subcellular tissue fractionation of rat livers treated with ^(67)Ga associated with the ³H-thymidine incorporation rate to detect subcellular localization of ^(67)Ga and it's relationship in DNA synthesis. Large amounts of ^(67)Ga asso챠ated viith the soluble portion of tissue homogenate rather than with isolated cell organelles and not related nuclei residue in the regenerating period after hepatocellular injury caused by CCl₄. The elevated uptake of ^(67)Ga in the livers of CCl₄ treated rats was also inhibited when protein synthesis was stopped by cyclohexamide. Thus protein and the soluble portion of tissue homogenates seems to play an important role in the eievared uptake of ^(67)Ga in liver injury induced by CCl₄ treated rats.

        各種惡性腫瘍患者에서의 血液凝固 및 出血傾向

        A comprehensive studies of the hemostatic and coagulation mechanisms in 93 patients with disseminated malignancy was carried out. Significant laboratory abnormalities were found in the patients. Some of patients was the "hypercoagulable state". This included high level of fibrinogen and thrombocytosis. But 25% of patients was hypocoagulable state. Fibrinolysis was confirmed 15 patients by partial thromboplastin time, euglobulin-lysis time, thrombin time and protamine-gelation test. The data suggest that patients with cancer have an alteration in the hemostatic level of control with increased of both clotting factors and their inhibitors resulting in a more easily disturbed balance between opposing hemostatic forces. Clinical thrombosis or bleeding may follow.
